돼지 대동맥판막을 이용한 이첨판 대동맥판막 후류 유동 분석

Aortic valvular stenosis where calcium accumulates in the valve and the valve hardens is the most common heart valve disease in developed countries. The type of aortic valves is generally divided into tricuspid aortic valve (TAV) and congenital malformation bicuspid aortic valve (BAV) stenosis. Although flow characteristics of TAV have been well established, that of BAV is still not fully unelucidated. In this study, we proposed an ex vivo flow visualization platform by using a pig heart. Normal TAV and BAV models were prepared by aortic valve plastic surgery. Valve wake flow characteristics of normal TAV and BAV were compared by using 4D flow magnetic resonance imaging. Normal TAV exhibited that the valve was fully opened during systole, while the valve of BAV was partially opened. The Stroke volume of the normal TAV and BAV were similar. However, the peak velocity of BAV was higher than that of the TAV. The effect of malfunction of BAV on hemodynamics was systematically analyzed. This study would be helpful for diagnosing aortic stenosis and understanding prognosis prediction especially for aortic stenosis patients with BAV.

Recent advancement in the MR imaging technology have enabled direct three-dimensional flow quantification in-vivo. However, the spatial and temporal resolution of MRI is still limited compared to 2D echocardiography, which has been suggested as a clinical guideline for various cardiovascular diseases, including mitral regurgitation. In this study, we have directly compared the accuracy of echocardiography and MRI in estimating the regurgitant flow rate using in-vitro mitral valve regurgitation model. The in-vitro mitral valve model was fabricated using the flexible silicon, and was able to mimic the movement of the mitral valve during the systolic phase using the pulsatile flow generated by the piston pump.
소규모 사업장에서의 안전보건관리담당자 제도 도입 효과 고찰

Objective: This study evaluated the effect of legislating the appointment of safety and health management officers in Korea and suggested recommendations to improve its effectiveness. Background: Though the appointment of safety and health management officers was legislated in 2016, the effect of this legislation has not been evaluated yet at this point of settlement. Method: We devised a scorecard system to assess the level of safety and health management system of enterprises. The effectiveness of the legislation was validated by pairwise comparison on the scores gathered from 34 enterprises before and after appointing the officers. The differences in 10 individual scores consisting the scorecard system were analyzed for finding least effective areas of the management and education. Result: The appointment of officer by the legislation was found to be effective in improving all the individual scores of safety and health management system. There were differences however in the amount of improvement among the scores, which means the necessity for complementing the legislation. Conclusion: It was recommended to strengthen quality assurance of education and training of the officers as well as financial support of the government for the complement. Application: The scorecard system suggested in this study would be utilized in the further studies as a basis for assessing the level and effectiveness of safety and health management system.
