        자궁경부종양에 있어 원추절제술의 역할

        Objective : The purpose of this study is to evaluate the role of cold knife conization in the diagnosis and treatment of cervical neoplasia. Methods : Sixty patients were divided into diagnostic and therapeutic conization group and then indication of conization, PAP smear, colposcopy directed biopsy, cone margin and residual lesion of each group were compared respectively. Results : If the difference of PAP and colposcopy directed biopsy was 2 grades or more, upper limit of the lesion was invisible, squamocolumnar junction was not seen, PAP Ⅴ or invasive cancer was suspected, diagnostic conization was performed in 22 patients. Six cases of follow-up group had cone margin (-) and no recurrence. Sixteen cases of immediate TAH (total abdominal hysterectomy) group had 2 cases of cone margin (+) with residual disease. There were 4 cases of cone margin (-) with residual lesion. If the difference of PAP and colposcopy directed biopsy was 1 grade or less, upper limit of the lesion was visible, squamocolumnar junction was seen and invasive cancer was ruled out, therapeutic conization was performed in 38 cases. All of therapeutic conization group had cone margin (-). Thirty two cases were follow up group and six cases were immediate TAH group. Three of follow up group had recurrences and delayed TAH was performed. There was no residual lesion in the specimen of immediate TAH group. Conclusion : The precise dignosis and treatment of cervical neoplasia was capable with cold knife conization. The more aggressive lesion or the more cases of cone margin (+) was diagnosed, the more residual lesion was found. Thorough follow up should be done after treatment of cervical neoplasia because of the possibility of residual disease even after documentation of cone margin (-).

        학령기 다문화 가정 아동의 이야기 글쓰기 특성

        Purpose: Writing is a means of communication using written language and the essential ability required in academicachievement. Thus, the purpose of this study was to investigate the writing skills of school-aged children frommulticultural families. Methods: The subjects included 10 children from multicultural families and 13 children fromnon-multicultural families in the third and fourth grades of elementary school. The children were asked to write afterlistening to a familiar narrative story; their writings were analyzed in several aspects: total number of words, total number of story units, total of number of complete episodes, syntactic complexity, sentence errors, and spelling errors. Results: Children from multicultural families wrote significantly fewer words and story units than children from non-multicultural families; on the other hand, children from multicultural families conveyed as many complete episodes as children from non-multicultural families. The syntactic complexity showed no significant differences between the two groups, but children from multicultural families produced more sentences with syntactic errors, especially missing main components of sentences. In addition, children from multicultural families made more spelling errors. Conclusions: Even though children from multicultural families produced less content when writing narrative stories, they had no problems to retrieve important information from long-term memory and reconstruct the content. However, they had difficulties to organize sentences without missing important components and in using correct spelling. These difficulties may result from deficit of monitoring skills.

        Methotrexate (MTX) is indicated for patients with moderate to severe rheumatoid arthritis (RA), but it sometimes causes severe hematologic complications in patients with impaired renal function. We experienced three cases of pancytopenia due to low-dose MTX treatment in RA patients with impaired renal function. Two patients were dialysis patients, and the other had stage 4 chronic kidney disease. The mean cumulative MTX dose and treatment duration for the three patients was 25 mg and 10 days, respectively. All cases experienced severe stomatitis and pancytopenia and recovered with conservative care. In conclusion, MTX should be used cautiously in patients with impaired renal function.         사회 보호 시설 아동과 일반 가정 아동 간 담화 이해 비교

        Purpose: Children are exposed to various environments from birth and develop language through interacting with the people around them. However, children in institutional care could experience environmental deprivation compared to children at home because they have been separated from their parents since their childhood and have lived with peers in a limited environment. The purpose of this study was to investigate discourse comprehension, especially inference ability, in children who have different external environments that could affect their language development. Methods: Fifteen children reared at home, 15 children in institutional care, and 15 children with SLI in institutional care participated in the study. They were asked literal and inferential questions after listening to short discourses. To compare the performances of the three groups, a repeated 2-way ANOVA was conducted. Results: The children in institutional care showed significantly better performance compared to the children with SLI in institutional care regardless of the questions’ condition. On the other hand, they performed similar to the children reared at home in the literal questions but significantly poorer than the children reared at home in the inferential questions. Conclusions: These results suggest that the institutional environment that cannot provide individualized experiences or opportunities would influence children’s inference ability.

        파닉스를 적용한 집단 읽기 중재가 지적장애 학생의 해독 능력에 미치는 영향

        연구목적: 선행연구에서 읽기에 어려움을 보이는 학생들을 대상으로 파닉스 중재의 효과가 입증되고 있으나 대부분의 중재가 개별적으로 이루어졌다. 실제 학교 현장에서는 개별 교수보다는 집단 교수가 일반적이기 때문에 집단을 대상으로 파닉스 중재를 실시하였을 때도 효과가 있는지를 알아볼 필요가 있다. 이에 본 연구에서는 파닉스를 통한 집단 읽기 중재가 지적장애 학생들의 해독 능력에 긍정적인 영향을 주는지를 알아보고자 하였다. 연구방법: 연구 대상은 A광역시 특수학교에 재학 중인 초등학교 1학년 지적장애 학생 3명이며, AB설계를 적용하여 연구를 진행하였다. 3명의 아동에게 기초선 확인 후, 파닉스를 적용한 집단 읽기 중재를 총 24회기 진행하였고, 단어 해독 능력을 회기별로 평가하였다. 또한 중재 종료 3주 후에 유지 검사를 실시하였다. 연구결과: 파닉스를 통한 집단 읽기 중재로 모든 참여 학생들의 단어 해독 능력이 향상되었으며, 이는 중재가 종료된 후에도 유지되었다. 결론: 이러한 결과는 지적장애 학생들에게 파닉스 중재를 집단으로 실시했을 때도 효과가 있다는 것을 의미하며, 더 나아가 실제 학교의 집단 수업 과정에서 파닉스를 활용할 수 있다는 사례를 보여주었다는 점에서 의미가 있다. Purpose: Previous studies have demonstrated the effectiveness of phonics interventions for students struggling with reading, yet these interventions were predominantly conducted on an individual basis. Given that group instruction is more commonplace in actual school environments, it is crucial to assess whether phonics interventions would be equally effective when administered to groups. This study aimed to explore the impact of group reading interventions via phonics on the decoding abilities of students with intellectual disabilities. Method: The subjects of this study were three first-grade students with intellectual disabilities enrolled at a special school in A Metropolitan City. The research was conducted using an AB design. After establishing a baseline, a total of 24 sessions of group reading intervention using phonics were carried out with these children, and their word decoding abilities were evaluated in each session. Additionally, a maintenance test was conducted three weeks after the end of the intervention. Result: Group reading intervention using phonics improved word decoding skills among all participating students, and these improvements were sustained even after the intervention had ended. Conclusion: These results indicate that phonics intervention can be effective for students with intellectual disabilities even when implemented in group settings. Furthermore, this demonstrates that phonics can be successfully incorporated into real school group lesson plans.

        학령기 아스퍼거 아동의 관용어 이해 특성

        Purpose: Children with Asperger's syndrome seem to have the same linguistic and cognitive development as typical developmental children. But AS is a disorder that is characterized by significant difficulties in the pragmatic domain. The purpose of this study was to find the characteristics of idiom comprehension in children with AS, depending on the idiom's condition. Methods: The participants were 10 AS and 10 TD children in the second to third grade of elementary school. The children completed 20 idiom-related tasks that were composed of action meaning condition and emotion meaning condition idoms. We compared the difference between the groups and the conditions. Results: Participants in the AS group generally showed significantly lower levels of idiom understanding. In the comprehension of action idioms, both AS and TD groups had no significant difference in understanding the idioms, however in the comprehension of emotion idioms, AS participants showed a much lower ability to understand the idioms. Conclusion: According to the results of this study, people with AS can infer the meanings of some idioms but they have difficulty in comprehending, especially the meanings of emotion idioms because they have to connect the speaker's intention and emotion. Therefore, we have studied that this characteristic of AS could be related to a deficit of the theory of mind.

        쌍태임신 149 예에 대한 임상통계학적 고찰

        Objective : To evaluate the clinical aspects of twin pregnancy and its results Methods : We reviewed the medical records of the 149 cases of twin births among 18,333 deliveries, from Jan. 1, 1990 to Dec. 31, 1999 at Pohang St. Mary's Hospital. Results : The incidence of twin births was one in 123 births. The rate of twin pregnancies by assisted reproduction technique was increased from 5.3% between 1990 and 1994 to 25.4% between 1995 and 1999. The predominant age group was 26-30(41.6%) and mean age was 27.5±4.1 years old. According to parity, nullipara(71.8%) was the most frequent. The predominant gestational age of twin births was 37-38 weeks(43.6%) and mean gestational weeks of twin births was 35 weeks and 5 days. The cephalic-cephalic combination(42.3%) was predominant presentation. The most common mode of twin delivery was cesarean section(77.2%) and its main indication was elective. The mean birth weights of twins was 2,315±610 grams. Low one minute Apgar scores occured more often in second twins than in first ones. The most common type of placental membrane was diamniotic-monochorionic(54.4%). Both male group(44.9%) was predominant. The most frequent maternal complication during pregnancy was anemia(41.6%). The perinatal mortality rate was 97 per 1000 newborns and its main cause was prematurity(64.0%). Maternal deaths were not encountered. Conclusion : Recently, although the incidence of twin pregnancy has been increased, it has greater risks of obstetrical complications and higher perinatal mortality than singleton pregnancy. Therefore, further studies are needed for effective management of twin pregnancy.

        SNS 대화에서 뇌성마비 아동의 이모티콘 사용 능력

        Purpose : With the rapid spread of smart devices, SNS has become a necessary means to maintain and strengthen relationships with people. Therefore, the purpose of this study was to investigate whether children with cerebral palsy were different from typical children in using appropriate emoticons in real-time SNS conversation contexts. Methods : The subjects for this study were 13 children with cerebral palsy and 13 typical children. An emoticon expression task was conducted in which the children read real-time conversations in the Kakaotalk context and selected an emoticon that matched with the last conversation. We investigated whether there was a difference in emoticon choice between children with cerebral palsy and typical children depending on whether the speaker’s emotions were expressed directly or indirectly and depending on the speaker’s emotion (happy, sad, angry. surprised). Results : Children with cerebral palsy performed significantly poorly on the emoticon expression task overall compared to typical children, but did not show any more difficulty in selecting emoticons by inferring the speaker’s emotions. Second, children with cerebral palsy showed similar performance to typical children when selecting emoticons to express the speaker’s joy and sadness, but showed significantly lower performance when selecting emoticons to express anger and surprise. Conclusions : Based on the above results, even if children with cerebral palsy do not show any noticeable problems in understanding the context of a conversation, they can have difficulties using texts and emoticons together properly in SNS conversation situations. Thus, it would be meaningful to implement an intervention related to the use of social media for children with cerebral palsy.
