        신경전도검사를 이용하여 경추 6-7 신경공협착에 의한 경추 포착 신경병증과 감각신경 활동전위의 연관성 연구

        Objective: To compare the sensory nerve action potentials (SNAPs) between patients with cervical 6-7 foraminal stenosis (FS) and those without. Methods: We enrolled patients who underwent a bilateral nerve conduction study (NCS) between January 2016 and December 2017. The FS group (n=35) included patients whose lesion was located at C6-7 FS. The non-FS group (n=75) had no lesions at C6-7 foramen. The amplitudes and latencies of SNAPs on the affected side were compared between the two groups. The proportion of those with abnormal SNAP responses was also evaluated. Results: Compared with the non-FS group, the FS group had a significantly lower amplitude (30.5 vs. 40.2 ÄV; age-adjusted p<0.001) and a longer latency (3.31 vs. 3.17 ms; age-adjusted p=0.001) of the median nerve. Abnormal amplitude was observed in 20.0% of the FS group and 4.0% of the non-FS group (age-adjusted p=0.007), and abnormal latency was observed in 45.7% of the FS group and 26.7% of the non-FS group (age-adjusted p=0.010). The median SNAP ratio between the affected side and the unaffected side was not significantly different between the two groups. Conclusion: Patients with C6-7 FS had a lower amplitude and a longer latency of the median nerve than did those without. Abnormal SNAP amplitude and latency were significantly more common in patients with FS. SNAP amplitude may be used as a predictor of cervical FS.

        중환자 말초신경근육병증 환자에서 일측 하부 뇌신경 침범: 증례 보고

        Critical illness neuromyopathy (CINM) manifests as weakness of the limbs and failure to wean from the ventilator as a complication of critical illness. It increases hospital mortality and causes chronic disability. Electrodiagnostic studies for CINM show both distal axonal sensory-motor polyneuropathy and myopathic motor unit action potentials. Invasion of the cranial nerves is unusual. We report the case of a patient who had CINM with left 11 and 12 cranial nerve palsies. A 62-year-old male patient received intensive care unit care for 21 days due to acute decompensated heart failure. His clinical features were weakness of the upper and lower extremities, decreased deep tendon reflex, tongue deviation, and atrophy of the left sternocleidomastoid and trapezius muscles. On electrophysiological studies, the compound motor action potentials of the left trapezius muscles were lower than those of the right trapezius muscles. The left upper trapezius and genioglossus muscles also clearly showed abnormal spontaneous activity.

        Clinical Factors Associated with Deep Vein Thrombosis in Rehabilitation Patients Suspected of Thromboembolism after Cerebral Infarction

        The aim of this study is to investigate the association between characteristics of cerebral infarction lesion (vascular territory, etiology, and size), functional status and the occurrence of thromboembolism in patients suspected of having thromboembolism in a rehabilitation setting after cerebral infarction. Cerebral infarction patients who were suspected of having thromboembolism and who had undergone deep vein thrombosis (DVT) evaluation were included in analyses. Of the total 916 cerebral infarction patients, 65 patients were suspected of having DVT; 27 patients belonged to the DVT group and 38 patients belonged to the non-DVT group. The DVT (+) group was more likely to have a higher ratio of female, previous DVT history, middle cerebral artery (MCA) infarction, large arterial disease, modified Rankin Scale (mRS) score 5, abnormal speech and higher D-dimer. In multivariate logistic regression analysis, female sex, MCA infarction and mRS score 5 were significantly associated with the occurrence of thromboembolism in patients suspected of having thromboembolism. In contrast, other functional status, cerebral infarction etiology (Trial of ORG 10172 in Acute Stroke Treatment [TOAST] classification), and infarct volume were not associated with the occurrence of thromboembolism. In this study, female gender, MCA infarction, and mRS score 5 could be potential risk factors for thromboembolism in rehabilitation patients after cerebral infarction.

        언어네트워크를 이용한 과학전람회 지구과학 부문 탐구주제 분석 : 최근 21년(2000-2020년)을 중심으로

        The purpose of this study is to analyze the field of Earth science at a science fair. For this purpose, 566 piecesof data spanning 21 years (2000 to 2020), acquired from entries in the Earth Science section on the science fair website,were analyzed using the semantic network method. As a result, geoscience topics have been actively explored in workssubmitted for the Earth Science section of the science fair. Fossils from the Cretaceous period of the Mesozoic Era wereparticularly predominant. Together with these, keywords corresponding to astronomy, space science, and atmosphericscience formed a small-scale network. Astronomy and space science mainly dealt with the dynamic characteristics ofasteroids, Venus, and Jupiter. Other subjects included the solar system, sunspots, and lunar phases. Atmospheric sciencehas focused on atmospheric physics, atmospheric observation and analysis technology, atmospheric dynamics, air qualitymonitoring, while marine science has been limited to physical oceanography and geologic oceanography. This study, isexpected to help select Earth Science topics and conduct inquiry activities in schools.
