        급성 림프구성 백혈병 환아에서 발생한 거대세포바이러스망막염 1예

        Purpose: To report a case of cytomegalovirus (CMV) retinitis in a child who had been managed by immunosuppressive chemotherapy for acute lymphoblastic leukemia (ALL). Methods: An 8-year-old boy who had been managed by immunosuppressive chemotherapy for ALL was referred to the ophthalmologic clinic because of blurred vision in his right eye. Fundus examination showed hard exudates and white retinal leaion in the macular area of the right eye, white retinal lesion accompanied with retinal hemorrhage and exudates in the midperipheral retina of the left eye. Serologic tests demonstrated a positive response of CMV antibody IgG and a negative response of CMV antibody IgM. CMV was isolated upon the culture of bone marrow and peripheral blood. Immunosuppressive chemotherapy was discontinued and ganciclovir was administered. Results: Administration of ganciclovir led to clinical stabilization of the retinitis lesion that was accompanied by scarring. We were able to discontinue the administration of ganciclovir after the retinal lesions were resolved, and no recurrence of CMV retinitis was noted. Conclusions: Ganciclovir treatment was clinically effective in improving CMV retinitis in an immunocompromised child with ALL. In addition, the improved systemic immune function achieved by discontinuing immunosuppressive chemotherapy permitted discontinuation of anticytomegalovirus therapy.

      • 정서유발단어에 대한 주의이동시 뇌신경망의 활성 양상

        본 연구는 fMRI를 사용하여 정서유발단어에 의한 주의이동과 관련한 뇌신경망을 규명하기 위하여 12명의 오른손잡이 여대생을 대상으로 수행하였다. fMRI 실험을 위한 뇌활성화 과제는 예비실험을 통하여 선정된 행복, 슬픔, 위협에 해당하는 각각 36개씩의 단어와 각 단어의 짝을 이루기 위한 중성단어를 이용하여 변형된 Posner 패러다임을 블록디자인으로 고안하였다. fMRI 영상은 1.5T Simens Vision scanner를 사용하여 실시하였으며, single-shot EPI 기법으로(TR/TE:3840/40ms, flip angle: 90, FOV: 220, 64x64 matrix, Slice thickness: 6mm) 이미지를 얻었고 피험자의 반응시간을 기록하였다. 자료분석은 SPM-99 소프트웨어를 사용하였다. 정서유발단어는 중성단어에 비하여 반응시간이 의미있게 빨라 효과적인 주의이동이 일어났음을 알 수 있었다. 정서유발단어와 중성단어에 의한 뇌활성화 상태를 감산분석한 결과, 좌측 상두정엽, 양측 후두엽 및 소뇌에서 뇌활성화를 보였고, 이들 영역은 정서유발단어에 의한 주의이동시 특징적으로 활성이 일어나는 뇌영역임을 보여주었다. 또한 여러 가지 정서가를 가진 단어에 의해 유발되는 주의의 정도가 특징적인 신경망의 활성과 관련되어 있음을 알 수 있었다. The purpose of this study is to delineate the neural network related to the shift of attention and its modulation by emotion-provoking words using functional MRI (fMRI). Twelve right-handed normal female volunteers participated in fMRI study. Their mean age was 24 years and the mean modified Edinburgh score was +90. Emotion-provoking words representing happy, sad, threatened, and neutral emotional valences respectively were chosen by the assessment of 60 normal volunteers using semantic differentiation and Mannequin scales. The activation tasks were designed using modified Posner's paradigm to have 4 blocks of each emotional and control periods, Imaging was conducted on a 1.5T Siemens Vision scanner. Single-shot echoplanar images (TR/TE = 3840/40 ms, flip angle = 90, FOV = 220, matrix = 62 ? 64, slice thickness = 6㎜) were acquired in 20 contiguous slices parallel to the AC_PC line as well as conventional T1-weighted images. Response times were obtained during the scanning, Imaging data were realigned, coregistered, normalized, smoothed, and statistically analyzed in group using SPM-99 software. The results showed that emotional valence of word stimuli produced the effect of attentional engagement. The left superior parietal, inferior frontal, bilateral occipital lobes and cerebellum were main area of modulation of attentional network by emotional words stimuli. The different extent of brain activation induced by different emotion-provoking words were determined.

        뇌졸중 환자에서 시각적 바이오피드백을 이용한 균형 훈련 효과

        Objective: To investigate the effect of dynamic balance training with center of pressure (COP) games using visual biofeedback of COP movement in stroke patients. Method: Thirty-eight stroke patients were randomly assigned to the static or dynamic balance training group, or control group. Static group received weight-shift training with visual biofeedback of weight distribution bar. Dynamic group received dynamic balance training using COP games. Each training session lasted 30 minutes, 3 times in a week during 3 weeks. Control group received traditional rehabilitation only. The ability of balance control was assessed using posturography by total path distance, frequency of sway, and COP weight-spectrums during standing, and during presentation of virtual moving surround. Total path error was also assessed during sine curve and dynamic circle trace test. Results: In static group, there were significant improvement in the total path distance during comfortable standing and path error during sine curve tracing. In dynamic group, significant improvements were found in the path error during sine curve and dynamic circle tracing. Conclusion: Dynamic training group showed more improvement in dynamic balance ability, whereas static group showed more changes in the static balance ability. The visual biofeedback training is more specified for balance rehabilitation in stroke patients.

        효모의 구성적 Promoter들에 의한 Inulinase 유전자의 발현

        S. cerevisiae의 대표적인 구성적 promoter로 GAPDH, ADH1 및 ENO1를 사용하고 이들 promoter 하류에 INUl의 ORF를 in frame으로 연결한 각각의 plasmid pYIGP, pADHl-lNU 및 pENO-INU를 구축하였다. 이들 plasmid 를 함유한 재조합 S. cerevisiae SEY2102 균주들을 sucrose 함유 평판배지에서 선별한 후, 초기 포도당 농도가 2$\%$ 또는 4$\%$인 배지에서 배양했을 때, 모든 균주들은 12시간 이후부터 정지기에 들어갔으며, 정지기에서도 느리지만 균체증식과 inulinase 발현은 계속되었다. 4% 포도당 배지에서 inulinase 총발현량은 ADH1 promoter 계를 제외하고 GAPDH와 ENO1 promoter의 경우 2$\%$ 포도당 배지 때 보다 약 2배 증가한 2.0 unit/mL과 1.4 unit/mL를 각각 보였다. 단위균체농도당 inulinase 활성 즉, 비활성은 GAPDH와 ENOl promoter계의 경우 포도당 농도가 4$\%$일 때 2$\%$때보다 약 63$\%$ 정도의 비활성 증가를 나타내었다. 하지만 ADH1 promoter의 경우는 오히려 비활성이 약 40$\%$ 감소하였다. 그러나, plasmid 안정성 측면에서는 ADH1과 ENO1 promoter 발현계가 GAPDH promoter 경우의 34$\%$보다 훨씬 뛰어난 80$\%$이상을 보였다. 결론적으로 높은 포도당 농도에서 구성적 promoter의 활성 (발현능)은 GAPDH, ENO1, ADH1 promoter 순으로 나타났지만, 초기 포도당 농도가 높을 때나 에탄을 생산이 심각한 유가식 배양에서는 ENO1 promoter가 inulinase의 구성적 발현ㆍ생산에 더 적합할 것으로 사료된다. To express constitutively the inulinase gene (INUl) of Kluyveromyces marxianus in Saccharomyces cerevisiae, three yeast promoters such as GAPDH, ADH1 and ENO1 were connected upstream of INUl. The resulting plasmids, pYIGP, pADHl-INU, and pENO-INU were introduced to S. cerevisiae SEY2102 host strain, respectively, and then each transformants were selected by staining of colonies on sucrose-agar plate. When the yeast transformants were cultivated on 2$\%$ dextrose media, the total expression levels of inulinase reached to 1.11 unit/mL, 0.88 unit/mL, and 0.69 unit/mL for respective GAPDH, ADH1, and ENO1 promoter systems. On 4% dextrose media, however, the inulinase activities were observed at 2.00 unit/mL for pYIGP, 0.71 unit/mL for pADH1-INU, and 1.40 unit/mL for pENO-INU. This result indicates that the constitutive expression of INUl was significantly affected by the initial concentration of dextrose and the promoter strength was in the order GAPDH, ENO1, and ADH1 promoter at high dextrose concentration. Taking into account the plasmid stability, however, it is suggested that the ENO1 promoter system is more suitable for the INU1 expression on high dextrose medium or in the fed-batch cultivation accumulating ethanol at high level.
