Chylorrhea develops by various causes of (1) congenital anomalies such as atresia of the thoracic duct, (2) trauma, (3) neoplasms, (4) infections, and (5) thrombosis of left subclavian and jugular vein, or superior vena cava,' among which trauma and neoplasms are most frequent. Thoracic duct injury appears as chylothorax, chyloma, or chylorrhea. Trauma in thoracic duct is caused by (1) blunt trauma, (2) penetrating injury, (3) cervical, thoracic or abdominal operation, and (4) various diagnostic procedures such as subclavian vein catheterization, left heart catheterization, or translumbar arteriography. Trauma in thoracic duct be managed, at first, by conservative measures such as NPO & TPN, change to medium-chain for long-chain triglyceride, compression dressing on wound, effective thoracotomy tube drainage or pleurodesis with sclerosing agent in chylothorax. Should leakge of chyle not be controlled despite conservative measures, operative intervention be undertaken. Among operative precedures, right supradiaphragmatic ligation of thoracic duct between azygos vein and aorta is most popular. In many occasions, direct repair of injured thoracic duct is difficult because of difficulty in finding the site and friability of thoracic duct wall. When massive leakage of chyle continues, mortality is high because of metabolic, nutritional and immunological disorders. Authors have experienced one case of the thoracic duct injury due to continuous negative pressure suction with Hemovac through operation site after left radical cervical lymphoareolar dissection because of extensive, conglomerated tuberculous lymphadenitisfrom left upper deep cervical throughout left subclavicular region despite one year's antituberculous therapy, and treated successfully by primary suture with 6-0 prolene. As very rare cause of thoracic duct injury, contiuous negative pressure suction through operation site seems to be the first report, being not found in the literatures as far as we looked for.
