Epinephrine 의 선택적 신동맥 주입에 의한 신동맥 및 신실질 변화에 관한 실험적 연구

Selective infusion of the epinephrine into the renal artery has been used in the field of the diagnostic and the therapeutic raidology for correct diagnosis and effective treatment, respectively. However, administration of overdose of epiniphrine may cause serious complication, renal infarction. The study was undertaken to evaluated the sequetial change of renal arterial constrictive effect of selective infusion of epiniphrine into renal artery and to determine the critical doses of epiniphrine producting irreversible renal infarct, A total of 25 rabbits are used, which are divided into 5 groups. Under the general anesthesia is made the selective infusion of various doses of epiniphrine into the right renal artery of the rabbits. At the various time interval during and after the epinephrine infusion, renal angiography was done, and 24 hrs. later, gross and microscopic findings of the kidney were observed. The results are as follows : 1. Vasoconstriction of renal artery occur ed within 2 mins, infusion, and maxmum effect within 5 mins. 2. It seems that there is correlation between the amount of infused epinephrine and the time takned to recover form constriction of renal artery. 3. When epinephrine infused into the renal artery in the rate of 1 ug/min ,, renal infarct is not noticed below the level of 10 mins. But correlation between the amount of infused epiniphrine and the frequency of renal infarct occurs above 20 mins, infusion.

Radiologic manifestations were analysed in chest injured patients who were admitted and treated at the Department of Thoracic and Cardiovascular Surgery during the period of 5 years from Aug. 1978 to Aug. 1983. The patients were 253 in number and the results were as follows; 1. Nonpenetrating trauma is more frequent (83.8%) than penetrating trauma. 2. The most frequent oause of trauma is traffic accident(60.1%). 3. The frequent radiologic findings in nonpenetrating chest injury were rib fracture, hemothorax and subcutanneous emphysema, and in poetrating chest injury, subcutaneous emphysema, hemothorax and pneumothorx in order of frequency. 4. The most frequently fractured rib was 6th rib, and frequently fractured ribs were from 3rd to 8th ribs(more than 40% of patients). 5. In all cases of hemopneumothorax were associated with rib fracture in nonpenetrating injury. 6. Delayed apperaance of hemothorax, pneumothorax and hemopeumothorax was seen in 9.0% of cases(14 cases).

To elucidate whether or not innervation of purinergic nerve and functional action of $P_{2X}$-, $P_{2Y}$-purinoceptor, the neurogenic effects of perivascular nerve stimulation were investigated using by physiograph recording system in isolated coronary artery of pig. 1. The contractile responses induced by perivascular nerve stimulation (20V, 0.5msec, 20sec) were the frequency (2~64Hz) dependent manner. 2. The neurogenic contractile responses induced by perivascular nerve stimulation were inhibited by the pretreatment with either ATP or adenosine ($10^{-7}{\sim}10^{-4}M$). 3. The neurogenic contractile responses induced by perivascular nerve stimulation (20V, 16Hz, 0.5msec, 20sec) were increased by the pretreatment with reactive blue 2, but were not affected by the pretreatment with 8-phenyltheopylline ($10^{-5}M$). 4. The neurogenic contractile responses induced by perivascular nerve stimulation (20V, 16Hz, 0.5msec, 20sec) were inhibited by the desensitization of the P2X-purinoceptor using by treatment of $10^{-5}M$, -methylene ATP as 3 times over again. The accomplished present study on isolated coronary artery of pig suggest that purinergic nerve is innervated and that the neurogenic contractile response was mediated by activation of $P_{2X}$-purinoceptor and the neurogenic relaxative response was mediated by activation of both $P_1$ and $P_{2Y}$-purinoceptor.

13 cases of atypical meningioma were analysed, which were proven surgically in S.N.U.H. for 3 years since March, 1979. C.T. findings of atypical meningioma were central low density in 8 cases, large cyst in 4 cases, calcified mass in 1 cases and non-specific scalp mass in 1 case. All the CT findings of atypical meningioma were pathologically proven as follows. Central low density was tissue necrosis in 5 cases, multiple cysts in 2 cases and hemorrhage in a case. Large cyst was arachnoid cyst in all 4 cases. Calcified mass was massive calcium deposition on tumor. Non-specific scalp mass on temporal area was meningioma involving soft tissue, bone and dura.
