        Novel and Highly Diverse Fungal Endophytes in Soybean Revealed by the Consortium of Two Different Techniques

        Fungal endophytes were isolated from the leaves of soybean cultivars in Brazil using two different isolation techniques – fragment plating and the innovative dilution-to-extinction culturing – to increase the species richness, frequency of isolates and diversity. A total of 241 morphospecies were obtained corresponding to 62 taxa that were identified by analysis of the internal transcribed spacer (ITS) of the ribosomal DNA (rDNA). The Phylum Ascomycota predominated, representing 99% and 95.2% of isolates in the Monsoy and Conquista cultivars, respectively, whereas the Phylum Basidiomycota represented 1% and 4.8% of isolates, respectively. The genera Ampelomyces, Annulohypoxylon, Guignardia, Leptospora, Magnaporthe, Ophiognomonia, Paraconiothyrium, Phaeosphaeriopsis, Rhodotorula, Sporobolomyces, and Xylaria for the first time were isolated from soybean; this suggests that soybean harbours novel and highly diverse fungi. The yeasts genera Rhodotorula and Sporobolomyces (subphylum Pucciniomycotina) represent the Phylum Basidiomycota. The species richness was greater when both isolation techniques were used. The diversity of fungal endophytes was similar in both cultivars when the same isolation technique was used except for Hill’s index, N1. The use of ITS region sequences allowed the isolates to be grouped according to Order, Class and Phylum. Ampelomyces, Chaetomium, and Phoma glomerata are endophytic species that may play potential roles in the biological control of soybean pathogens. This study is one of the first to apply extinction-culturing to isolate fungal endophytes in plant leaves, thus contributing to the development and improvement of this technique for future studies.

        Systolic blood pressure variability within 120 hours of admission predicts the functional outcomes at discharge of patients with acute ischemic stroke

        Background: Blood pressure variability (BPV) is a predictor of short- and long-term disability in patients with acute ischemic stroke (AIS). Its effect on more immediate functional outcomes has been seldom studied, and the results are inconsistent. We aimed to determine the role of BPV during the first 5 days of hospitalization in functional status at the time of discharge of patients with AIS.Methods: We enrolled 134 patients diagnosed with AIS and BPV using standard deviation and coefficient of variation (CV %). These were associated with the dichotomized modified Rankin Scale at discharge using logistic regression. Results: Patients with unfavorable outcomes were significantly older (P=0.014), had a lower body mass index (P=0.001), were less likely to present with dyslipidemia (P=0.001), had lower serum triglyceride levels (P=0.012), had a longer hospitalization period (P<0.001), and had a higher mean National Institutes of Health Stroke Scale score at admission (P<0.001). After adjusting for multiple confounders, the CV % of systolic blood pressure (SBP) in the first 120 hours after admission had a significant effect on functional disability at discharge. Conclusion: Variability in SBP in the first 5 days of hospitalization had a deleterious effect on the functional outcomes at discharge of patients with AIS. The role of diabolic blood pressure variability seems to be significant only in the first 24 hours of admission; however, further research is required.

        Ocimum basilicum belongs to the family Lamiaceae and can be found in tropical Asia, Africa, Central America and South America. It is widely cultivated commercially as a condiment or for the production of essential oil. In this review, this plant is accredited with important effects on health, in addition to being a condiment. In folk medicine it is used to treat vomiting, intestinal colic and diarrhea, menstrual pains and to improve kidney function. Some studies have demonstrated that it can be used for the treatment of hyperlipidemia and also that it has antioxidant, neuroprotective, anti-inflammatory and vasodilator properties as well as hepatoprotective effects. In view of its broad range of effects, further studies should be conducted to ascertain the possible use of this plant in clinical trials, not only for prevention but also as an adjuvant in the treatment of different diseases.

        Fractal dimension analysis as an easy computational approach to improve breast cancer histopathological diagnosis

        Histopathology is a well-established standard diagnosis employed for the majority of malignancies, including breast cancer. Nevertheless, despite training and standardization, it is considered operator-dependent and errors are still a concern. Fractal dimension analysis is a computational image processing technique that allows assessing the degree of complexity in patterns. We aimed here at providing a robust and easily attainable method for introducing computer-assisted techniques to histopathology laboratories. Slides from two databases were used: A) Breast Cancer Histopathological; and B) Grand Challenge on Breast Cancer Histology. Set A contained 2480 images from 24 patients with benign alterations, and 5429 images from 58 patients with breast cancer. Set B comprised 100 images of each type: normal tissue, benign alterations, in situ carcinoma, and invasive carcinoma. All images were analyzed with the FracLac algorithm in the ImageJ computational environment to yield the box count fractal dimension (Db) results. Images on set A on 40x magnification were statistically different ( p = 0.0003), whereas images on 400x did not present differences in their means. On set B, the mean Db values presented promissing statistical differences when comparing. Normal and/or benign images to in situ and/or invasive carcinoma (all p < 0.0001). Interestingly, there was no difference when comparing normal tissue to benign alterations. These data corroborate with previous work in which fractal analysis allowed differentiating malignancies. Computer-aided diagnosis algorithms may beneficiate from using Db data; specific Db cut-off values may yield ~ 99% specificity in diagnosing breast cancer. Furthermore, the fact that it allows assessing tissue complexity, this tool may be used to understand the progression of the histological alterations in cancer.

        Background: Tuberculosis is a known cause of fever of unknown origin. Nowadays most authors consider mycobacterium tuberculosis isolation essential prior to treatment. Case Report: A 63 year-old women was admitted to our department with vespertine fever, night sweats, anorexia and weight loss in the past 4 months. She had a history of childhood pulmonary tuberculosis, with sequelar bronchiectasis. Besides paleness and low BMI no other relevant physical fi ndings were observed. Laboratory evaluation revealed anemia and elevation of CRP and ESR. Thorax-CT showed bronchiectasis on the upper and inferior right lobe. Bronchoalveolar lavage cultures were negative to bacteria, mycobacteria and fungus as well as M.tuberculosis complex DNA. Despite multiple antibiotic courses she maintained intermittent pyrexia. After excluding most relevant causes of fever of unknown origin, a PET scan confi rmed exclusive involvement of the right superior pulmonary lobe. In the second bronchofi broscopy signifi cant bronchial hemorrhage was visible, so catheter embolization of the right bronchial artery was performed. An arterio-arterial subclavian-pulmonary fi stula was detected. Two days later multiple infra and supratentorial intracerebral hemorrhages were diagnosed. In the following week she had two ischemic strokes affecting the right and left middle cerebral artery territory. Transcranial Doppler showed moderate right-to-left shunt. All those events were suggestive of embolic etiology. Despite all negative cultural and DNA results, attending to typical manifestations, exclusion of other foci and to this rapid deterioration, empiric treatment was started with isoniazid, rifampin, ethambutol and pyrazinamide. One month after treatment initiation, persistent apyrexia was obtained with normalization of all infi ammatory parameters. In the last 3 months good evolution of pulmonary lesions and signifi cant improvement of her neurologic state were observed. Conclusions: We present a bizarre case of tuberculosis with catastrophic consequences, which demonstrates that empiric treatment still has its place in modern medicine.
