      • 수화동영상 전송 시스템의 개발 및 구현

        본 논문에서는 JAVA를 이용한 수화 동영상 전송 시스템을 설계및 구현한다. 일반적인 형식의 동영상 전송기법은 서버 측의 동영상 데이터가 사용자에게 노출될수 있거나 고가의 인코더를 이용해야 하는 등 보안과 유지에 적지 않은 문제점을 가지고 있다. 본 논문에서 구현하는 시스템은 100% 순수 JAVA로 구현되어, JAVA환경만 갖추어져 있으면 일반 Web Browser 로도 데이터의 전송이 가능하며, 서버측의 멀티미디어 데이터 파일이 사용자로부터 보호될수 있는 특징을 가지고 있다. 따라서 이 시스템은 수화를 비롯한 각종 멀티미디어 교육 시스템에서 데이터 전송 모듈로 적합하며 손쉽게 다른 서버 시스템으로 이식이 가능하다.

        오대산 월정사지역 전나무림의 생태학적 연구

        본 연구는 오대산 국립공원내 월정사지역에 분포하고 있는 전나무림의 식생구조를 분석하여 이 지역 전나무림의 관리에 대한 기초자료를 제공하기 위하여 수행되었다. 그 결과 전나무림의 층위구조는 교목층(2종류), 아교목층(20종류), 관목층(46종류) 그리고 초본층(87종류)으로 구성되어 있었다. 조사지역에 출현한 전체식물의 중요치는 전나무가 34.09%로 가장 높게 나타났고, 당단풍(23.07%), 청시닥나무(14.44%), 고광나무(10.72%), 오리방풀(8.49%), 복자기(7.80%), 고추나무(7.65%), 산거울(6.79%), 방아풀(5.76%), 고로쇠나무(5.47%), 개다래(5.41%)의 순으로 나타났다. 특히, 전나무의 치수가 전혀 없어 앞으로 아교목층과 관목층을 형성하고 있는 단풍나무류, 참나무류, 피나무류 등의 경쟁에 의하여 천이가 있을 것으로 사료된다. The vegetation structure of Abies holophylla forest was analyzed to provide the information for conservation of Abies holophylla at Wol-jong Temple in Mt. Odae national park, southern Korea. The layer structure of Abiesholophylla forest was consists of tree layer(2 species), subtree layer(20 species), shrub layer(46 species) and herb layer(87 species). The importance value of Abies holophylla was the highest one 34.09% , and those of Acer pseudo-siebodianum, Acer barbinerve, Philadelphus schrenckii, Isodon excisus, Acer triflorum, Staphylea bumalda, Carex humilis, Isodon japonicus, Acer monp and Actinidia polygama were 23.07%, 14.44%, 10.72%, 8.49%, 7.80%, 7.65%, 6.79%, 5.76%, 5.47% and 5.41%, respectivery. Especially, when predicting succession in Abies holophylla forest, such seedlings were not formed and succession was governed by competition of Acer species, Quercus species and Tilia species of subtree and shrub layer.         When embankment is constructed on soft ground, lateral displacements and consolidation settlements develops. When load work on the soft clay ground, we can earn the following result through model test that is the lateral displacement by the piles embedded distance(L) and the piles spacing rate(s/D). 1. As increasing L and s/D, the maximum heaving height(HH Max) of the ground was also increasing, and the maximum heaving distance(HL Max) became far from the end of the load. 2. As L and s/D was increasing, the maximum of the lateral displacement(SL Max) became increasing, and The location of the maximum lateral displacement(SL Max) occurred in the -0.17~-0.28B spot regardless L and s/D. 3.         Relationship Between Plasma Adiponectin, Retinol-Binding Protein 4 and Uric Acid in Hypertensive Patients With Metabolic Syndrome

        <P><B>Background and Objectives</B></P><P>Adipokines have been suggested for their potential use in tracking the clinical progress in the subjects with metabolic syndrome (MS). To investigate the relationship between the serum levels of adipokines {adiponectin and retinol-binding protein 4 (RBP4)} and the serum level of uric acid in hypertensive (HTN) patients with MS.</P><P><B>Subjects and Methods</B></P><P>In this study, 38 totally untreated HTN patients were enrolled. Anthropometric measurements, blood pressure (BP) were taken in the 12 HTN patients without MS and the 26 HTN patients with MS. Fasting blood samples were collected for measurement of adiponectin, RBP4, nitric oxide (NO), glucose, creatinine, uric acid, lipid profile and insulin.</P><P><B>Results</B></P><P>The HTN with MS group had significant higher values of body mass index, waist length, serum uric acid and triglyceride levels than the HTN without MS group. Compared to the HTN without MS group, the HTN with MS group showed significantly lower adiponectin (p=0.030), NO (p=0.003) and high density lipoprotein levels (p<0.001). Serum adiponectin levels negatively correlated with insulin level (R=-0.453, p=0.026) and uric acid level (R=-0.413, p=0.036), and serum RBP4 levels positively correlated with uric acid level (R=0.527, p=0.006) in the HTN with MS group. Multiple linear regression analysis using RBP4 and adiponectin levels as the dependent variables showed that uric acid level correlated with serum RBP4 level (p=0.046) and adiponectin level (p=0.044).</P><P><B>Conclusion</B></P><P>The HTN with MS group showed a correlation with two types of adipokines (adiponectin, RBP4) and uric acid. Adiponectin, RBP4 and uric acid may be important components associated with MS, especially when associated with hypertension.</P>

      • Contrast Volume/Raw eGFR Ratio for Predicting Contrast-Induced Acute Kidney Injury in Patients Undergoing Percutaneous Coronary Intervention for Myocardial Infarction

        <P>Abstract</P><P><B><I>Background:</I></B> Considering that contrast medium is excreted through the whole kidney in a similar manner to drug excretion, the use of raw estimated glomerular filtration rate (eGFR) rather than body surface area (BSA)-normalized eGFR is thought to be more appropriate for evaluating the risk of contrast-induced acute kidney injury (CI-AKI). <B><I>Methods:</I></B> This study included 2,189 myocardial infarction patients treated with percutaneous coronary intervention. Logistic regression analysis was performed to identify the independent risk factors. We used receiver-operating characteristic (ROC) curves to compare the ratios of contrast volume (CV) to eGFR with and without BSA normalization in predicting CI-AKI. <B><I>Results:</I></B> The area under the curve (AUC) of the ROC curve for the model including all the significant variables such as diabetes mellitus, left ventricular ejection fraction, preprocedural glucose, and the CV/raw modification of diet in renal disease (MDRD) eGFR ratio was 0.768 [95% confidence interval (CI), 0.720-0.816; p < 0.001]. When the CV/raw MDRD eGFR ratio was used as a single risk value, the AUC of the ROC curve was 0.650 (95% CI, 0.590-0.711; p < 0.001). When the CV/MDRD eGFR ratio with BSA normalization ratio was used, the AUC of the ROC curve further decreased to 0.635 (95% CI, 0.574-0.696; p < 0.001). The difference between the two AUCs was significant (p = 0.002). <B><I>Conclusions:</I></B> Raw eGFR is a better predictor for CI-AKI than BSA-normalized eGFR.</P><P>© 2015 S. Karger AG, Basel</P>

        Volumetric change of the latissimus dorsi muscle after postoperative radiotherapy in immediate breast reconstruction with an extended latissimus dorsi musculocutaneous flap

        Background This study aimed to determine the magnitude of volume reduction of the latissimus dorsi (LD) muscle after treatment using only postoperative radiotherapy (PORTx) in patients who underwent immediate breast reconstruction using an extended LD musculocutaneous (eLDMC) flap after partial mastectomy. Methods We retrospectively reviewed 28 patients who underwent partial mastectomy and an eLDMC flap, received only PORTx, and underwent chest computed tomography (CT) 7 to 10 days after surgery and 18±4 months after the end of radiotherapy, from March 2011 to June 2016. The motor nerve to the LD was resected in all patients. One plastic surgeon performed the procedures, and the follow-up period was at least 36 months (mean, 46.6 months). The author obtained LD measurements from axial CT views, and the measurements were verified by an experienced radiologist. The threshold for statistical significance was set at P<0.05. Results A statistically significant decrease in the LD volume was found after the end of PORTx (range, 61.19%-80.82%; mean, 69.04%) in comparison to the measurements obtained 7 to 10 days postoperatively (P<0.05). All cases were observed clinically for over 3 years. Conclusions The size of an eLDMC flap should be determined considering an average LD reduction of 69% after PORTx. Particular care should be taken in determining the size of an eLDMC flap if the LD is thick or if it occupies a large portion of the flap.

        Evaluation of the Relationship between Circadian Blood Pressure Variation and Left Atrial Function Using Strain Imaging

        <P><B>Background</B></P><P>Non-dippers were reported as showing different left atrial function, compared to dippers, but no study to date investigated the changes in the left atrial function according to the diurnal blood pressure pattern, using tissue Doppler and strain imaging.</P><P><B>Methods</B></P><P>Forty never treated hypertensive patients between 30 and 80 years of age were enrolled in this study. Patients were classified as non-dippers when, during night time, they had a blood pressure decrease of less than 10%. Strain of the left atrium was measured during late systole, and peak strain rates of the left atrium were measured during systole, early and late diastolic periods.</P><P><B>Results</B></P><P>The left atrial expansion index, left atrial active emptying volume and left atrial active emptying fraction were all significantly increased in non-dippers. They also had increased values of mean peak left atrial strain (dippers = 21.26 ± 4.23% vs. non-dippers = 24.91 ± 5.20%, <I>p</I> = 0.02), strain rate during reservoir (dippers = 1.29 ± 0.23 s<SUP>-1</SUP> vs. non-dippers =1.52 ± 0.27 s<SUP>-1</SUP>, <I>p</I> = 0.01) and contractile period (dippers = -1.38 ± 0.24 s<SUP>-1</SUP> vs. non-dippers = -1.68 ± 0.32 s<SUP>-1</SUP>, <I>p</I> < 0.01).</P><P><B>Conclusion</B></P><P>Strain and strain rate acquired from color Doppler tissue imaging demonstrate exaggerated reservoir and booster pump function in never-treated, non-dipper hypertensive patients. These methods are simple and sensitive for the early detection of subtle changes in the left atrial function.</P>

        Effects of total body irradiation-based conditioning on allogeneic stem cell transplantation for pediatric acute leukemia: a single-institution study

        Purpose: To evaluate the effects of total body irradiation (TBI), as a conditioning regimen prior to allogeneic stem cell transplantation (allo-SCT), in pediatric acute leukemia patients. Materials and Methods: From January 2001 to December 2011, 28 patients, aged less than 18 years, were treated with TBI-based conditioning for allo-SCT in our institution. Of the 28 patients, 21 patients were diagnosed with acute lymphoblastic leukemia (ALL, 75%) and 7 were diagnosed with acute myeloid leukemia (AML, 25%). TBI was completed 4 days or 1 day before stem cell infusion. Patients underwent radiation therapy with bilateral parallel opposing fields and 6-MV X-rays. The Kaplan-Meier method was used to calculate survival outcomes. Results: The 2-year event-free survival and overall survival rates were 66% and 56%, respectively (71.4% and 60.0% in AML patients vs. 64.3% and 52.4% in ALL patients, respectively). Treatment related mortality rate were 25%. Acute and chronic graft-versus-host disease was a major complication; other complications included endocrine dysfunction and pulmonary complications. Common complications from TBI were nausea (89%) and cataracts (7.1%). Conclusion: The efficacy and toxicity data in this study of TBI-based conditioning to pediatric acute leukemia patients were comparable with previous studies. However, clinicians need to focus on the acute and chronic complications related to allo-SCT.

        Observation of the fast response of a magnetic resonance signal to neuronal activity: a snail ganglia study

        <P>We observed the response of a magnetic resonance signal to neuronal activity in dissected snail ganglia to investigate faster responding components than the blood oxygenation level dependent (BOLD) component. To eliminate the BOLD component completely from the magnetic resonance signal, we used dissected snail ganglia which have non-magnetic hemocyanin, other than hemoglobin, as an oxygen carrying protein. To activate the snail ganglia in synchronization with the magnetic resonance signal measurement, we applied 30 Hz electrical stimulation with a pulse width of 2 s to the ganglia just before the 90? RF pulse. The increase of neuronal activity in the snail ganglia after the electrical stimulation was confirmed with extracellular potential recording. The magnetic resonance signal intensity, measured with the volume selection pulse sequence in a 3.0 T MRI, decreased by about 2.97 ? 1.10% when the ganglia were activated by the electrical stimulation. We think that the MRI signal decrease is partly due to the magnetic field produced by the neuronal current and partly due to the Lorentz force exerted on the neurons when they were firing.</P>
