<P><B>Introduction</B></P><P>Estrogen receptor (ER) β is predicted to play an important role in prevention of breast cancer development and metastasis. We have shown previously that ERβ inhibits hypoxia inducible factor (HIF)-1α mediated transcription, but the mechanism by which ERβ works to exert this effect is not understood.</P><P><B>Methods</B></P><P>Vascular endothelial growth factor (VEGF) was measured in conditioned medium by enzyme-linked immunosorbent assays. Reverse transcription polymerase chain reaction (RT-PCR), Western blotting, immunoprecipitation, luciferase assays and chromatin immunoprecipitation (ChIP) assays were used to ascertain the implication of ERβ on HIF-1 function.</P><P><B>Results</B></P><P>In this study, we found that the inhibition of HIF-1 activity by ERβ expression was correlated with ERβ's ability to degrade aryl hydrocarbon receptor nuclear translocator (ARNT) via ubiquitination processes leading to the reduction of active HIF-1α/ARNT complexes. HIF-1 repression by ERβ was rescued by overexpression of ARNT as examined by hypoxia-responsive element (HRE)-driven luciferase assays. We show further that ERβ attenuated the hypoxic induction of VEGF mRNA by directly decreasing HIF-1α binding to the VEGF gene promoter.</P><P><B>Conclusions</B></P><P>These results show that ERβ suppresses HIF-1α-mediated transcription via ARNT down-regulation, which may account for the tumour suppressive function of ERβ.</P>
[P252] P252A case of subungual exostosis located in the left fifth toe

Subungual exostosis is a benign acquired osteochondral tumor causing pain and nail deformity. A 21-year-old man visited to our department with complaint of a flesh-colored, tender, hyperkeratotic nodule on left fifth toenail which occurred a year ago. It developed after traumatic injury. We removed the toenail for incisional biopsy and found a hyperproliferative lesion as rigid as cartilage. The radiograph revealed a calcifying projection at the distal edge of the left fifth toe connected with the phalangeal bone. Based upon these findings, we diagnosed him as subungual exostosis. He was referred to an orthopedic surgeon for complete surgical resection. The exact pathogenesis of subungual exostosis is unclear but mostly considered to be a reactive metaplasia resulting from microtrauma. Because subungual exostosis commonly appears on the hallux, it is easy to be misdiagnosed when it arises in uncommon areas. Here, we demonstrate the case of subungual exostosis involving the fifth toe, a rare location. Dermatologists should be aware of this disease to avoid misdiagnosis and to manage it properly.

Morgellons disease is a rare disease with unknown etiology. Herein, we report the first case of Morgellons disease in Korea. A 30-year-old woman presented with a 2-month his-tory of pruritic erythematous patches and erosions on the arms, hands, and chin. She insisted that she had fiber-like ma-terials under her skin, which she had observed through a magnifying device. We performed skin biopsy, and observed a fiber extruding from the dermal side of the specimen. Histopathological examination showed only mild lympho-cytic infiltration, and failed to reveal evidence of any microorganism. The polymerase chain reaction for Borrelia burgdorferi was negative in her serum. (Ann Dermatol 29(2) 223∼225, 2017)
Acral Lentiginous Melanoma, Indolent Subtype Diagnosed by En Bloc Excision: A Case Report

Nail unit melanoma is a type of acral lentiginous melanoma and requires histopathologic examination for a confirmed diagnosis. However, inadequate biopsy techniques make definitive diagnosis difficult. A 61-year-old man presented with progressive nail pigmentation for 15 years, which was clinically highly suspicious for malignancy. Acral lentiginous melanoma was not detected in punch and longitudinal biopsy specimens, but en bloc excision tissue revealed melanoma. Acral lentiginous melanoma is known to have a heterogeneous pathologic manifestation depending on the pigmented region and the time it takes to progress. In this regard, en bloc excision can be considered as a first-line biopsy technique to diagnose acral lentiginous melanoma, indolent subtype. (Ann Dermatol 29(3) 327∼330, 2017)
Robotic surgery in Gynecology: the present and the future

Since its introduction, laparoscopic surgery has been often preferred over open surgery in obstetrics and gynecology due to its advantages, such as less bleeding, lower incidence of adhesions, reduced postoperative pain, short hospital stay, and quick return to daily life. However, in the case of complex surgeries, laparoscopy presented some limitations. Nonetheless, since the 1980s, medical robots have been introduced to overcome the technical limitations of laparoscopy and start a new age for minimally invasive surgery. In this review, we explore the indications and advantages and disadvantages of robotic surgery in the field of gynecology, and try to assess the recent trend of robotic surgery.
Development of therapeutic technology through analysis of growth factor of myoma

Objective: The purpose of this study was to compare the level of expression of various growth factors in myoma and to investigate the effect of growth factor inhibitor and to compare the effect of conventional regimens. Methods: The expression levels of growth factors in normal myometrium and myoma were compared by immunohistochemical staining. After primary culture of normal myometrial cells and myoma cells, cell viability was confirmed by treatment with 100 nM Ulipristal acetate (UPA) and Mifepristone (RU486) for 48 hours. Western blot analysis was performed to determine the protein expression of each of these growth factors. Cell viability was determined by treatment with 10uM TGF inhibitor(LY364947) and 5uM VEGF inhibitor(Axitinib) for 24 hours in cultured normal myometrial cells and myoma cells. Results: Immunohistochemical staining revealed that TGF expression was 20% in normal myometrium and 60% in myoma. VEGF expression was 1% in normal myometrium and 16% in myoma respectively. Cell viability was the most significant decrease with 58.59% mifepristone treatment in myoma tissue, and 57.20% in normal myometrium with UPA treatment. In the protein expression, EGF was significantly increased by 80% and IGF was significantly increased by 250% when UPA treated to normal myometrial cells. The expression of VEGF was decreased by 70% when Mifepristone treated to myoma cells. In normal myometrial cell, cell viability was significantly decreased by 60% of the TGF inhibitor treated cells and by 70% of the VEGF inhibitor treated cells. Cell viability of myoma cells was significantly decreased by 27.69% treated with TGF inhibitor and 27.34% treated with VEGF inhibitor. Conclusion: In myoma, TGF was the most expressed among the various growth factors, and VEGF was next increased to be expressed. The administration of inhibitor significantly inhibited the growth of myoma cells, and the suppressive effect was significantly greater than that of conventional drug therapy.
Acne conglobata induced by anabolic androgenic steroids: A case report

A 29-year-old man visited to our department with complaint of sudden onset multiple inflammatory papules and nodules on the face, chest, and back which appeared three months ago. There was no specific trigger without fact that he had been injected with testosterone enanthate (one of the anabolic androgenic steroids) 250mg intramuscularly 16 times every week to gain muscular strength. Based upon the clinical manifestation and history, we diagnosed him as acne conglobata induced by anabolic androgenic steroids. Anabolic androgenic steroids are well known for exerting direct and indirect influence on sebaceous gland and Propionibacterium acnes, consequently resulting in acne. They now have very limited medical indications, such as endocrinopathy, due to their systemic side effect like libido change, mood swings, and testicular atrophy, as well as dermatologic complication-acne.

셀룰로오스는 지구상에 널리 분포하는 대표적인 친환경 고분자 물질로 재생 가능하며 생분해성, 무독성 등의 장점이 있다. 여기에 cellulose로부터 유래된 nanofibrils는 비표면적이 매우 크고 강도적 성질, 열안정성, 표면개질 특성 등이 우수한 천연 고분자 물질로서 다양한 소재로의 적용 가능성이 높다. 특히 길이가 긴 cellulose nanofibrils는 web-like entangled structure를 이루기 때문에 복합체의 강성을 개선시킬 수 있어 연구가 활발히 진행 중이다. 이에 본 연구에서는 길이가 긴 cellulose nanofibrils를 제조하기 위해 공시재료로서 섬유장이 긴 paper mulberry bast fiber를 사용하였으며 비교적 약한 강도로 nanofibrillation을 시키기 위해 초음파 처리를 실시하였다. 보다 효율적인 nanofibrillation을 위해 전처리로서 TEMPO 산화를 실시하였으며 TEMPO 산화된 섬유의 pH변화에 따른 nanofibrillation 수율을 분석하고 제조된 cellulose nanofibrils의 특성을 분석하였다. 그 결과 pH가 높을수록 TEMPO 산화된 섬유의 표면 zeta-potential이 음의 값으로 증가하였다. 이에 따라 정전기적 반발력이 증가하면서 nanofibrillation 수율이 증가하였으며 섬유장이 긴 섬유의 단점인 꼬임 현상의 발생을 억제할 수 있었다. 결정화도 지수는 83.50%로 침엽수에 비해 높게 나타났으며 인장강도는 110.18 MPa로 같은 조건의 침엽수 대비 약 30% 증가된 결과를 나타냈다.
