Pancreatic panniculitis in alcoholic pancreatitis with multiple pseudocysts

Pancreatic panniculitis is a rare complication of pancreatic disease appearing in approximately 2-3%. Its pathogenesis is still not well understood, but the release of pancreatic enzymes may play role, leading to subcutaneous fat necrosis. Tender, inflammatory nodules typically develop on the lower legs. Pancreatic panniculitis is usually confirmed by skin biopsy. Clinicians with less experiences, may be unable to identify pancreatic panniculitis. We report the case of a pancreatic panniculitis associated with pancreatitis. A 46-year-old man was admitted to our hospital with a 2 weeks history of abdominal pain after heavy drinking. He also presented with loss of appetite and fever. Physical examination on admission did not inspect any skin lesions. The results of laboratory tests were as follows: white blood cell count 20.9 × 109/L, lipase 2,144 U/L, pancreatic amylase 1,787 U/L and C-reactive protein 27 mg/dL. Liver function tests were normal. Computed tomography revealed multiple pseudocysts, fluid collection and underlying chronic pancreatitis (Fig. A). He was treated supportively for acute alcoholic pancreatitis and administered empirical antibiotics. Abdominal pain and fever relievedafter 5 days. On hospital day ten, multiple tender nodules on both legs (Fig. B). Histopathological examination showed lobular panniculitis without vasculitis, with fat necrosis, ghost adipocytes (anucleate adipocytes) and saponification (Fig. C). The nodules resolved over the course of 2 weeks.

이 글은 『혼불』을 여성을 중심으로 하는 역사서사라는 사실에 착안하여 ‘여성’과 ‘공간’을 살피고자 하였다. 이를 위해 먼저 ‘장소’와 ‘공간’이라는 개념을 정리한다. 다음으로는 남성과의 관계맺음을 중심으로 하여 여성의 ‘장소상실’과 ‘공간’이라는 변화를 분석한다. 마지막으로 이러한 과정을 통하여 『혼불』의 여성인물들의 ‘장소 · 공간 분화’에 대해 살핀다. 이는 매안이씨 종가와 종가의 여성들의 관계에 주목하여 ‘장소’로서의 ‘집’이 공간으로 확장되고 축소되고 있음을 밝히는 과정이다. 『혼불』에서는 양반사회에 대한 천민의 반란이 두드러진다. ‘변동천하’로 요약되는 그러한 반란의 기운을 ‘매안이씨 종가’의 여인들과 ‘장소 · 공간 분화’와의 관계 속에서 살폈다는 점에 이 글의 의의가 있다고 하겠다. This study aims to examine ‘Women’ and ‘Space’ as a process of approaching the fact that Honbul is a history book focusing on women. Prior to approaching ‘Women’ and ‘Space’, we will examine the concept of ‘Place’ and ‘Space’. Then, we will examine changes in ‘Loss of Place’ and ‘Space’ of women, while focusing on their making relationships with men. Lastly, based on the contents examined earlier, we will examine ‘Place & Space Differentiation’ of the female characters in Honbul. It appears remarkable that the lowest class’s revolt against the aristocratic society in Honbul. This study has significance, in that such a spirit of ‘Byeondongcheonha (meaning a great upheaval all over the world)’ can examine changes from the upper, not a revolt from the lower, by examining the women of ‘the Head Family of Maeahn’ and ‘Place & Space Differentiation’.

Small-molecule inhibitors are widely used to treat a variety of inflammatory diseases. In this study, we found a novel antiinflammatory compound, 1-[(2R,4S)-2-methyl-4-(phenylamino)-1,2,3,4-tetrahydroquinolin-1-yl]prop-2-en-1-one (MPQP). It showed strong anti-inflammatory effects in lipopolysaccharide (LPS)-stimulated RAW 264.7 macrophages. These effects were exerted through the inhibition of the production of NO and pro-inflammatory cytokines, such as interleukin (IL)-6, IL-1β, and tumor necrosis factor-α (TNF-α). Furthermore, MPQP decreased the expression levels of inducible NO synthase (iNOS) and cyclooxygenase 2 (COX-2). Additionally, it mediated the inhibition of the phosphorylation of p38, c-Jun N-terminal kinase (JNK), the inhibitor of κBα (IκBα), and their upstream kinases, IκB kinase (IKK) α/β, mitogen-activated protein kinase kinase (MKK) 3/6, and MKK4. Furthermore, the expression of IL-1 receptor-associated kinase 1 (IRAK1) that regulates NF-κB, p38, and the JNK signaling pathways, was also increased by MPQP. These results indicate that MPQP regulates the IRAK1-mediated inflammatory signaling pathways by targeting IRAK1 or its upstream factors. [BMB Reports 2018; 51(6): 308-313]
Skin Necrosis after High Dose Vasopressor Infusion in Septic Shock -Two Case Reports-

Survival sepsis campaign recommends that vasopressor therapy is required to maintain mean arterial pressure (MAP) ≥ 65 mmHg. However, the absolute maximum dose of vasopressor is difficult to determine. Herein, we report 2 cases of severe skin necrosis after high dose vasopressor infusion to maintain the recommended MAP in septic shock. In our first case, norepinephrine 1.0-2.0 μg/kg/min and vasopressin 0.03-0.1 U/min were infused for 5 days; in the second case, dopamine 10-20 μg/kg/min and norepinephrine 0.25-2.5 μg/kg/min were infused for 7 days. Severe ischemic skin lesions, which required amputations, developed in both cases. The clinical appearance of the skin lesions in the 2 cases was different because of the unique distribution of target receptors for different vasopressors. Thus, when high dose vasopressors are required to achieve recommended MAP, extra vigilance is required. Further studies for dose adjustment are needed.

Intercellular adhesion molecule-1 (ICAM-1), which is induced by tumor necrosis factor (TNF)-α, contributes to the entry of immune cells into the site of inflammation in the skin. Here, we show that protein tyrosine phosphatase non-receptor type 21 (PTPN21) negatively regulates ICAM-1 expression in human keratinocytes. PTPN21 expression was transiently induced after stimulation with TNF-α. When overexpressed, PTPN21 inhibited the expression of ICAM-1 in HaCaT cells but PTPN21 C1108S, a phosphatase activity-inactive mutant, failed to inhibit ICAM-1 expression. Nuclear factor-κB (NF-κB), a key transcription factor of ICAM-1 gene expression, was inhibited by PTPN21, but not by PTPN21 C1108S. PTPN21 directly dephosphorylated phospho-inhibitor of κB (IκB)-kinase β (IKKβ) at Ser177/181. This dephosphorylation led to the stabilization of IκBα and inhibition of NF-κB activity. Taken together, our results suggest that PTPN21 could be a valuable molecular target for regulation of inflammation in the skin by dephosphorylating p-IKKβ and inhibiting NF-κB signaling. Background/Aims: Pyogenic liver abscess (PLA) is a life-threatening condition, despite advances in diagnostic technology and strategies for treatment. A strong predictor of mortality in this condition is septic shock. This study describes clinical, biochemical, and radiologic features in patients with PLA with or without septic shock, with the intent of describing risk factors for septic shock. Methods: Of 358 patients with PLA enrolled, 30 suffered septic shock and the remaining 328 did not. We reviewed the medical records including etiologies, underlying diseases, laboratory, radiologic and microbiologic findings, methods of treatment and treatment outcomes. Results: The case fatality rate was 6.1%. In univariate analysis, the presence of general weakness, mental change, low platelet level, prolonged PT, high BUN level, high creatinine level, low albumin level, high AST level, high CRP level, abscess size >6 cm, the presence of gas forming abscess, APACHE II score ≥20, and the presence of Klebsiella pneumoniae infection were significantly associated with septic shock. Multivariate analysis showed the presence of mental change (p=0.004), gas-forming abscess (p=0.012), and K. pneumoniae infection (p=0.027) were independent predictors for septic shock. Conclusions: The presence of mental change, gas-forming abscess, and K. pneumoniae infection were independent predictors for septic shock in patients with PLA. (Korean J Gastroenterol 2016;67:245-252)
