N/A To evaluate the in viva effect of hyperglycemia per se on plasma free fatty acid (FFA) levels, euglycemic and hyperglycemic clamp targeted at 80㎎/dl and 250㎎/dl respectively, were performed in 6 overnight fasted mongrel dogs while plasma insulin and glucagon levels were suppressed by somatostatin infusion. During somatostatin infusion, plasma FFA levels rose from 0.32±0.05mmol/l at basal state to 0.76±0.04mmol/l at the steady-state (60-120 min) in euglycemic clamp (p[t]<0.0001) and from 0.26±0.04mmol/l to 0.43±0.02mmol/l in hyperglycemic clamp (p[t]<0.0001). The steady-state plasma FFA levels in hyperglycemic clamp were significantly lower than those in euglycemic clamp (p[t]<0.0001). This findings suggest that hyperglycemia per se can significantly decrease plasma FFA levels and thus support that hyperglycemia per se may play a compensatory role in the circumstances of severe insulin deficiency.

N/A Five cases of differentiated carcinoma of thyroid associated with brain metastasis are reported. All of the patients had distant metastatic diseases or massive invasion of the surrounding tissue at the initial diagnosis. Metastatic lesions in the brain were not detected by conventional radioiodine whole body scan but by computerized axial tomography. TSH suppression, external radiotherapy, and radioactive iodine were tried but no consistently satisfactory effects were achieved. The prognosis of the patients with brain metastasis was poor, four patient diedat two to four months after detection of brain metastasis and only one patient is alive two months after detection, at the time of writing this paper.

N/A Several differences in phospholipid fatty acid composition of the erythrocyte membrane have been reported in diabetes mellitus, and these remain controversial. To evaluate the dietary fat intake pattern and the fatty acid metabolism in Korean diabetic patients, fatty acids in the erythrocyte membrane were determined by gas-liquid chromatography in 10 subjects with IDDM, 36 subjects with NIDDM, and 37 control subjects. Significant increases in palmitic acid (p<0.01) and oleic acid (p <005) were observed in erythrocytes from IDDM patients. Significant increases in myristric acid (p<0. 05), palmitic acid (p<0.01), and oleic acid (p<0.05), along with significant decreases in docosahexaenoic acid (p<0.01), were observed in erythrocytes from nonobese NIDDM patients, Similar fatty acid compositions in erythrocytes were noticed in obese NIDDM patients, except for eicosadienoic acid. The P/S ratios in the erythrocyte membrane fatty acids were significantly lower in diabetic patients than in the controls (0.92±0.03 vs 1.01±0.02, p<0.05 in IDDM; 0.85±0.03 vs 0.99±0.02, p<0.01 in nonobese NIDDM; 0.81±0.07 vs 0.99±0.02, p<0.05 in obese NIDDM). The arachidonic: linoleic acid ratio and oleic: stearic acid ratio in the erythrocyte membrane were not different between the diabetic patients and the controls, but the eicosapentaenoic: docosahexaenoic acid ratio was lower in the NIDDM patients than in the controls. These results suggest that an increase in the peroxidation of fatty acids in the erythrocyte membrane might result in a decrease of polyunsaturated fatty acids, especially DHA, and since membrane fatty acids are modified by receut dietary fat intake, we should be concerned about diet therapy, especially fat intake in diabetes. Fish oil supplements in diabetic patients should also be used with caution.

Background : Although it is generally accepted that high serum glucocorticoid levels causes osteoporosis by suppressing the bone formation, conflicting results have reported on bone resorption. But, previous studies have been carried out in patients with glucocorticoid excess secondary to exogenous glucocorticoid treatment of disorders, which also affect bone turnover and mass by themselves. The purpose of this study were to assess the effect of glucocorticoid excess on bone mass and turnover not influenced by other diseases known to affect skeleton and by different gonadal status and sex and to study the reversibility of osteopenia after cure of Cushing's syndrome. Methods : We measured bone mineral density using dual energy X-ray absorptiometry(DEXA) in 28 patients with Cushing's syndrome before and after surgical cure. In addition, in the patients with Cushing's syndrome and 21 healthy premenopausal women matched for age, we measured biochemical bone markers. Results : 1) Marked osteopenia was present in most patients with active Cushing's syndrome. Lumbar spine BMD and Ward's triangle BMD were significantly lower than femoral neck BMD and femoral great trochanter BMD.(Z-score: lumbar spine -2.22±1.17, femoral neck -0.71±1.08, Ward's triangle -1.77±0.97, femoral great trochanter -0.64±0.71; mean±SD, p<0.05). In patients with active Cushing's syndrome, serum osteocalcin, a marker of osteoblastic function, was reduced(p<0.05), and bone resorption was increased, as indicated by increased urinary N-telopeptide(p<0.05). 2) Osteopenia was still in patients after surgical cure.(Z-score: lumbar spine -1.59±1.01, femoral neck -0.47±0.58, Ward's triangle -1.44±0.77, femoral great trochanter -0.42±0.61). In Cushing's syndrome patients after surgical cure, serum osteocalcin and urinary N-telopeptide were higher compared to controls but not stastistically significant(p>0.05). 3) When compared with pretreatment values, BMD after surgical cure of Cushing's syndrome was increased significantly(p<0.05). Serum osteocalcin was increased and urinary N-telopeptide was decreased significantly(p<0.05). Conclusion : These results show that compared to age matched control, premenopausal Cushing's syndrome patients have reduced bone formation, increased bone resorption, and reduced BMD, especially trabecular bone and that these abnormalities are improved after surgical cure.(Korean J Med 58:560-567, 2000)

A 29-year-old man with generalized sarcoidosis is described. Symptomatic panhypopituitarism, diabetes insipidus, hypercalcemia and renal insufficiency as well as pulmonary involvement were noted. Tests of hypothalamio-pituitary function documented a deficiency of all anterior pituitary hormones and increase of prolactin. Water deprivation test revealed central diabetes insipidus. A contrast-enhanced thickened lesion of pituitary stalk was demonstrated by Magnetic Resonace Images(MRI). Histologically, sarcoidosis was proven by the biopsy of mediastinal lymph nodes. this is the first reported case of sarcoidosis with hypopituitarism, diabetes insipidus and renal insufficiency in Korea. The literature pertaining to those items is reviewed.

N/A There is increasing evidence that free fatty acids (FFA) inhibit the activity of plasma membrane Na+, K+-ATPase in vitro and thereby could be a control agent of metabolism. However, in several recent studies, physiological conclusion were drawn from the effect of FFA in systems in which albumin was absent. To explore the role of albumin in modulating the effect of FFA on the activity of this enzyme, the activities plasma membrane Na+, K+-ATPase were measured in the presence of various concentrations of FFA and/or albumin. 1) The activities of Na+, K+-ATPase in the presence of 4% bovine serum albumin (BSA) were inhibited by oleic and linoleic acid at concentrations ranging from 0.05 to 2.0mM. 2) The inhibitory effects of oleic acid at each concentration on the activities of Na+, K+-ATPase were increased as the concentration of BSA was decreased from 4% to 0%. 3) The activities of Na+, K+-ATPase were not inhibited when the FFA to albumin molar ratio was less than 1.0 but were inhibited to less than 50% when the ratio exceeded 3.5. The above results shows that, in vitro, the activities of plasma membrane Na+, K+-ATPase are inhibited by unsaturated FFA and albumin has a protective role on this inhibition. This suggests that in vivo unsaturated FFA could be a regulator of cellular metabolism by inhibition of Na+, K+-ATPase under the pathologic condition in which the FFA to albumin molar ratio is increased markedly.

Adrenal rest tumor is an aberrantly located collection of adrenocortical tissue which has been most commonly described in abdominal and pelvic sites. Occasionally adrenal rest tumors have been reported to occur in the liver. We report a case of a large hypervasular hepatic tumor in 32-year-old female patient presented with virilization and inferility, Serum testosterone and dehy-droepiandrosterone sulfate (DHEA-S) levels were elevated and there was an increase in urinary 17-ketosteroid and free cortisol. Abdominal CT scan showed a huge mBss in posterior segment of right lobe of liver. Celiac angiography and abdominal aortography showed a hypervascular tumor in right lobe of liver, which was mainly supplied by right hepatic artery and parasitically supplied by right adrenal and inferior phrenic arteries. Venous drainage of tumor was via right hepatic vein. But selective venous sampling by inferior vena cava catheterization failed to localize the site of hypersecretion of stenoid hormone levels. In operative findings, both adrenal glands were intact. Tumor was found in posterior surface of right lobe of liver. After right lobectomy of liver, the serum DHEA-S level was decreased to normal range by post-op 4th day. To our knowledge, this is the first case of an adrenal rest tumor of the liver proved to be functionally active in Korea.
