의과대학생의 진료수행 자기효능감 척도 개발 및 타당화 연구

국문 초록 (Abstract)

본 연구에서는 의과대학생들이 의사가 되어 직무를 성공적으로 수행하기 위해 요구되는 능력에 대해 자신이 얼마나 성공적으로 수행할 수 있을 지에 대한 개인의 신념이나 기대를 측정하는 진료수행 자기효능감 척도를 개발하고 타당화하며, 척도의 특성을 규명하고자 하였다. 진료수행 자기효능감을 측정하는 문항을 구성하기 위해 3차에 걸친 델파이 조사를 실시하였으며, 요인구조를 파악하기 위한 예비조사에서 3학년과 4학년 총 224명을 최종 분석대상으로 하였다. 척도의 신뢰도와 타당도를 검증하고, 척도의 특성을 규명하기 위해 3학년과 4학년 학생에게 공감, 불안, 성격, 의학교육환경 지각, 임상실습교육 지각 척도를 포함한 본 조사를 실시하고, 총 674명을 최종 분석대상으로 하였다.
델파이 조사결과 분석을 위해 빈도분석, 평균과 표준편차, 내용타당도 지수를 구하였고, 예비조사 문항의 요인구조 탐색을 위해 탐색적 요인분석을 수행하고, 신뢰도를 구하였다. 본 조사에서도 신뢰도를 구한 후, 진료수행 자기효능감의 타당도를 검증하기 위해 Pearson의 적률상관계수를 구하였고, 진료수행 자기효능감 상·하 수준에 따른 집단 간 관련 변인의 차이를 알아보기 위해 다변량 분산분석을 수행하였다. 또한 본 연구에서 개발한 진료수행 자기효능감의 특성을 규명하기 위해 척도의 점수 분포를 제시하였고, 인구통계학적 변인에 다른 차이와 척도의 상·하 수준에 따른 집단 간 학업성취도의 차이를 알아보기 위해 t-검증을 실시하였다. 마지막으로 진료수행 자기효능감을 설명하는 변인이 무엇인지 알아보기 위해 다중회귀분석을 수행하였다.
3차에 걸친 델파이 조사에서 전문가 패널의 개방형 질문과 내용타당도비율 지수를 통해 59문항의 예비조사 문항을 확정하였다. 예비조사에서 탐색적 요인분석을 통해 7개 요인 36문항을 추출하였으며, 각 하위요인은 의학지식 습득 및 적용, 임상추론, 임상술기, 환자의사소통, 타 의료직과의 관계, 의료윤리의식, 자기계발 자기효능감으로 명명하였다. 7개 하위요인은 유의미한 상호 관련성이 있었으며, 하위요인의 신뢰도도 양호하였다.
진료수행 자기효능감 척도의 공인타당도를 검증한 결과 정서적 공감, 외향성, 성실성, 경험에의 개방성, 교수 지각, 학업분위기 지각, 임상실습 교육 지각과 정적 상관을 보였으며, 불안, 신경증과는 부적 상관을 보였다. 진료수행 자기효능감 상위 집단이 하위집단보다 공감이 높고, 외향적이고 성실하며, 개방적인 성격을 지니며, 의학교육환경 및 임상실습교육 지각에 대해 긍정적으로 지각하였다. 반면, 상위집단은 신경증적인 성격을 덜 보이며, 불안수준도 낮았다. 이러한 결과는 진료수행 자기효능감 하위척도와 관련 척도의 이론적 근거를 지지하고 있어, 척도의 타당성을 입증하는 것이라 할 수 있다.
본 연구에서 개발한 척도의 특성을 규명한 결과, 의학전문대학원생이 의과대학생보다 타 의료직과의 관계 및 자기계발 효능감이 높았으며, 3학년 학생보다는 4학년 학생이 의학지식 습득 및 적용, 임상추론, 임상술기 자기효능감이 높았다. 또한 학업성취는 척도의 하위요인 중 의학지식 습득 및 적용, 임상추론, 자기계발 요인과 정적상관을 보였으나, 임상술기, 환자의사소통, 타 의료직과의 관계, 의료윤리의식은 상호상관을 보이지 않았다.
마지막으로 진료수행 자기효능감을 예측하는 변인을 탐색한 결과, 정서적 공감, 성실성, 경험에의 개방성, 학업분위기 지각, 임상실습교육 지각은 긍정적 영향을 미쳤으며, 불안, 교수 지각은 부정적 영향을 미쳤다. 이 변인이 척도 전체 변량의 30%를 설명해 주었다.
결론적으로 진료수행 자기효능감 척도는 의과대학생들의 진료수행 자기효능감을 측정하는 요인으로 구성되어 있으며, 타당하고 신뢰로운 척도임을 알 수 있다. 따라서 이 척도는 학생의 진료수행 자기효능감 수준을 파악하고, 이를 증진시키는 교육과정이나 교수법, 학생지원 프로그램을 개발하는 데 기초자료로 활용할 수 있을 것이다. 이 결과에 기초하여 학생들의 공감능력, 성격이나 불안 수준과 같은 개인적 특성을 파악하도록 돕는 개별상담 프로그램을 개발하거나 진료수행 자기효능감 증진을 저해하는 불안감을 줄여 주고 자신의 성격 특성을 이해하고 개선할 수 있도록 돕는 다양한 지원 프로그램을 개발하고 적용하여야 할 것이다. 또한 의학교육 환경과 임상실습 교육환경을 긍정적이고 대리경험 및 성공경험을 많이 할 수 있는 교육환경으로 조성해야 할 것이다.

다국어 초록 (Multilingual Abstract)

The purpose of this study was to develop, to validate the self-efficacy scale on clinical performance for medical students, and to examine the characteristics of it.
This study was carried out in four steps.
First, the scale was made of 59 items on the basis of the delphi-studies three times from the medical school professionals.
Second, the CPSES was conducted to 224 medical students of 3rd and 4th grades as the pilot-test and the 7 factors with final 36 items were extracted: Knowledge Acquisition and Application, Clinical Reasoning, Clinical Skills, Communication with Patients, Relationships with other Health Professionals, Medical Ethics, and Self-Development. This scale was named as "The Clinical Performance Self-Efficacy Scale(CPSES)".
Third, the CPSES was validated through the concurrent validity analysis, which was to examine the relationship between CPSES and Korean version of Jefferson Scale of Empathy-S version, Big-Five Inventory, Trait Anxiety, Dundee Ready Education Environment Measure, and Clinical Practice Perceptions.
Fourth, the characteristics of the CPSES was analysed according to socio-demographic variables(types of medical educational institution, grade and gender), and academic achievement of medical students. The graduate medical students showed higher scores in the Relationships with Other Health Professionals and in Self-Development in the CPSES. The 4th grade students showed higher scores in Knowledge Acquisition and Application, Clinical Reasoning, and Clinical Skills than the 3rd grade students. The academic achievement had a positive relationship with the scores in Knowledge Acquisition and Application, Clinical Reasoning and Self-Development.
In conclusion, the CPSES consists of the valid and reliable factors to measure the clinical self-efficacy of medical students.
