        Preoperative dexmedetomidine and intraoperative bradycardia in laparoscopic cholecystectomy: a meta-analysis with trial sequential analysis

        Background: While laparoscopic surgical procedures have various advantages over traditional open techniques, artificial pneumoperitoneum is associated with severe bradycardia and cardiac arrest. Dexmedetomidine, an imidazole derivative that selectively binds to α2-receptors and has sedative and analgesic properties, can cause hypotension and bradycardia. Our primary aim was to assess the association between dexmedetomidine use and intraoperative bradycardia during laparoscopic cholecystectomy. Methods: We performed a systematic review with a meta-analysis and trial sequential analysis using the following PICOS: adult patients undergoing endotracheal intubation for laparoscopic cholecystectomy (P); intravenous dexmedetomidine before tracheal intubation (I); no intervention or placebo administration (C); intraoperative bradycardia (primary outcome), intraoperative hypotension, hemodynamics at intubation (systolic blood pressure, mean arterial pressure, heart rate), dose needed for induction of anesthesia, total anesthesia requirements (both hypnotics and opioids) throughout the procedure, and percentage of patients requiring postoperative analgesics and experiencing postoperative nausea and vomiting and/or shivering (O); randomized controlled trials (S). Results: Fifteen studies were included in the meta-analysis (980 patients). Compared to patients that did not receive dexmedetomidine, those who did had a higher risk of developing intraoperative bradycardia (RR: 2.81, 95% CI [1.34, 5.91]) and hypotension (1.66 [0.92,2.98]); however, they required a lower dose of intraoperative anesthetics and had a lower incidence of postoperative nausea and vomiting. In the trial sequential analysis for bradycardia, the cumulative z-score crossed the monitoring boundary for harm at the tenth trial.Conclusions: Patients undergoing laparoscopic cholecystectomy who receive dexmedetomidine during tracheal intubation are more likely to develop intraoperative bradycardia and hypotension.

        Anti-inflammatory effects of methyl ursolate obtained from a chemically derived crude extract of apple peels: potential use in rheumatoid arthritis

        Ursolic acid (UA), a pentacyclic triterpene acidfound in apple peels (Malus domestica, Borkh, Rosaceae),has a large spectrum of pharmacological effects. However,the vegetal matrix usually produces highly viscous andpoorly soluble extracts that hamper the isolation of thiscompound. To overcome this problem, the crude EtOH–AcOEt extract of commercial apple peels was exhaustivelytreated with diazomethane, after which methyl ursolate(MU) was purified by column chromatography and characterizedspectrometrically. The anti-inflammatory effectsof UA and MU (50 mg/kg) were analyzed by zymosaninducedpaw edema, pleurisy and in an experimentalarthritis model. After 4 h of treatment with UA and MU,paw edema was reduced by 46 and 44 %, respectively. Both UA and MU inhibited protein extravasation into thethoracic cavity; tibio-femoral edema by 40 and 48 %,respectively; and leukocyte influx into the synovial cavityafter 6 h by 52 and 73 %, respectively. Additionally, bothUA and MU decreased the levels of mediators related tosynovial inflammation, such as KC/CXCL-1 levels by 95and 90 %, TNF-a levels by 76 and 71 %, and IL-1b levelsby 57 and 53 %, respectively. Both the compounds wereequally effective when assayed in different inflammatorymodels, including experimental arthritis. Hence, MU maybe considered to be a useful anti-inflammatory derivative toovercome the inherent poor solubility of UA for formulatingpharmaceutical products.

        복합 한약재 추출물 첨가가 돈육 패티의 저온저장 중 항산화 및 항균성에 미치는 영향

        The antioxidant and antimicrobial activities of pork patties supplemented with medicinal herb extract mix were investigated. The medicinal herb extract powder was mixed at a ratio of 48.5:48.5:3.0 for Morus alba L, Lonicera flos, and Coptis chinensis, respectively. The pork was prepared and supplemented with medicinal herb extract mix (0, 0.5, 1, or 2%) and the antioxidant and antimicrobial activities were tested during storage at $4^{\circ}C$. The pH value decreased during cold storage at $4^{\circ}C$ for allsamples, however the pH of samples supplemented with herb extract mix rapidly decreased by days 5 and 10 ($p{\leq}0.05$). The total phenol content in the pork patties with herb extract mix was higher than in the control patties. The ABTS+ radical scavenging activities increased with increasing concentrations of herb extract mix from 0.5% to 2%. In addition, pork patties supplemented with herb extract mix showed an approximately 1 decimal reduction in total aerobic counts. Therefore, the addition of herb extract mix into pork patties increased the antioxidant activity and slightly improved the antimicrobial activity of pork patties during cold storage. However, the levels of added medicinal herb extract mix should be considered prior to its use in order to maintain proper sensory acceptance.

      • THICK DISKS OF EDGE-ON GALAXIES SEEN THROUGH THE<i>SPITZER</i>SURVEY OF STELLAR STRUCTURE IN GALAXIES (S<sup>4</sup>G): LAIR OF MISSING BARYONS?

        <P>Most, if not all, disk galaxies have a thin (classical) disk and a thick disk. In most models thick disks are thought to be a necessary consequence of the disk formation and/or evolution of the galaxy. We present the results of a study of the thick disk properties in a sample of carefully selected edge-on galaxies with types ranging from T = 3 to T = 8. We fitted one-dimensional luminosity profiles with physically motivated functions-the solutions of two stellar and one gaseous isothermal coupled disks in equilibrium-which are likely to yield more accurate results than other functions used in previous studies. The images used for the fits come from the Spitzer Survey of Stellar Structure in Galaxies (S(4)G). We found that thick disks are on average more massive than previously reported, mostly due to the selected fitting function. Typically, the thin and thick disks have similar masses. We also found that thick disks do not flare significantly within the observed range in galactocentric radii and that the ratio of thick-to-thin disk scale heights is higher for galaxies of earlier types. Our results tend to favor an in situ origin for most of the stars in the thick disk. In addition, the thick disk may contain a significant amount of stars coming from satellites accreted after the initial buildup of the galaxy and an extra fraction of stars coming from the secular heating of the thin disk by its own overdensities. Assigning thick disk light to the thin disk component may lead to an underestimate of the overall stellar mass in galaxies because of different mass-to-light ratios in the two disk components. On the basis of our new results, we estimate that disk stellar masses are between 10% and 50% higher than previously thought and we suggest that thick disks are a reservoir of 'local missing baryons.'</P>
