        Lumbosacral Sagittal Alignment in Association to Intervertebral Disc Diseases

        Study Design: A cross-sectional case-control study was designed to compare the sagittal alignment of lumbosacral regions in two groups of patients suffering from low back pain, one with intervertebral disc pathologies and one without. Purpose: To evaluate the correlation between lumbosacral sagittal alignment and disc degeneration. Overview of Literature: Changes in lumbar lordosis and pelvic parameters in degenerative disc lesions have been assessed in few studies. Overall, patients with discopathy were shown to have lower lumbar lordosis and more vertical sacral profiles. Methods: From patients with intractable low back pain undergoing lumbosacral magnetic resonance imaging, 50 subjects with disc degeneration and 50 controls with normal scans were consecutively enrolled. A method was defined with anterior tangent-lines going through anterior bodies of L1 and S1 to measure global lumbosacral angle, incorporating both lumbar lordosis and sacral slope. Global lumbosacral angle using the proposed method and lumbar lordosis using Cobb’s method were measured in both groups. Results: Lumbar lordosis based on Cobb’s method was lower in group with discopathy (20°–67°; mean, 40.48°±9.89°) than control group (30°–62°; mean, 44.96°±7.68°), although it was not statistically significant. The proposed global lumbosacral angle in subject group (53°–103°; mean, 76.5°±11.018°) was less than control group (52°–101°; mean, 80.18°±9.95°), with the difference being statistically significant (p =0.002). Conclusions: Patients with intervertebral disc lesions seem to have more straightened lumbosacral profiles, but it has not been proven which comes first: disc degeneration or changes in sagittal alignment. Finding an answer to this dilemma demands more comprehensive long-term prospective studies.

        Interaction Effects of Chitosan, Benzyladenine, and Gibberellic Acid on in Vitro Proliferation of M26 Apple Rootstock

        The present study was conducted to evaluate the proliferation ability of M26 apple rootstock on the MS medium supplemented with various concentrations of crab chitosan, gibberellic acid (GA3), and N6-benzyladenine (BA). Combination of 120 mg·L-1 chitosan and 0.5 mg·L-1 BA in the absence of GA3 was most suitable for the proliferation after 12 weeks. Without chitosan, total number of shoots increased by increasing BA concentration, but the shoots led to rosette and dense growth, developing tiny yellowish leaves with soft and watery tissues. Shoots taller than 3 cm were obtained by 0.1 or 0.3 mg·L-1 GA3 in combination with 120 mg·L-1 chitosan, but leaves were small with yellowish green color. These results indicated that chitosan can be used as a growth stimulator in combination with a minor amount of plant growth regulators to improve the proliferation rate of in vitro cultured M26 apple rootstock.

        Contralateral Superior Cerebellar Artery Syndrome : A Consequence of Brain Herniation

        Vascular compromise is a well-known consequence of brain herniation syndromes. Transtentorial brain herniation most often involves posterior cerebral arteries. However, isolated involvement of contralateral superior cerebellar artery (SCA) during unilateral impending brain herniation is reported only once and we present another case of this exceedingly rare entity. A 24-year-old man was referred to us with impending herniation due to a multiloculated hydrocephalus, and during the course of illness, he developed an isolated SCA ischemia in the opposite side of the most dilated entrapped horn. In the current article we discuss the probable pathophysiologic mechanisms of this phenomenon, as well as recommending more inclusive brain studies in cases suspected of Kernohan-Woltman notch phenomenon in unilateral brain herniation. The rationale for this commentary is that contralateral SCA transient ischemia or infarct might be the underdiagnosed underlying pathomechanism of ipsilateral hemiparesis occurring in many cases of this somehow vague phenomenon.

        On the generalized principally injective modules

        Some results are generalized from principally injective rings to principally injective modules. Moreover, it is proved that the results are valid to some other extended injectivity conditions which may be defined over modules. The influence of such injectivity conditions are studied for both the trace and the reject submodules of some modules over commutative rings. Finally, a correction is given to a paper related to the subject.

        Acute Necrotizing Encephalopathy in Adult Patients With COVID-19: A Systematic Review of Case Reports and Case Series

        Background and Purpose Acute necrotizing encephalopathy (ANE) is a rare neurological disorder that is often associated with viral infections. Since the emergence of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), a few COVID-19-associated ANE cases have been reported. Since very little is known about ANE, the present study aimed to determine the clinical, biochemical, and radiological characteristics of affected patients. Methods A search was conducted on PubMed, Scopus, Embase, and Web of Science databases for articles published up to August 30, 2022 using relevant keywords. Case reports and series in the English language that reported ANE in adult patients with COVID-19 confirmed by reverse transcription polymerase chain reaction were included in this study. Data on the demographic, clinical, laboratory, and radiological characteristics of patients were extracted and analyzed using the SPSS software (version 26). Results The study included 30 patients (18 males) with COVID-19 and ANE who were aged 49.87±18.68 years (mean±standard deviation). Fever was the most-prevalent symptom at presentation (66.7%). Elevated C-reactive protein was observed in the laboratory assessments of 13 patients. Computed tomography and magnetic resonance imaging were the most-common radiological modalities used for brain assessments. The most commonly prescribed medications were methylprednisolone (30%) and remdesivir (26.7%). Sixteen patients died prior to discharge. Conclusions The diagnosis of COVID-19-associated ANE requires a thorough knowledge of the disease. Since the clinical presentations of ANE are neither sensitive nor specific, further laboratory and brain radiological evaluations will be needed to confirm the diagnosis. The suspicion of ANE should be raised among patients with COVID-19 who present with progressive neurological symptoms.
