Malignant Melanoma of the Nipple: A Case Report

Malignant melanoma rarely originates from the female nipple. Tumors that develop on the skin of the breast are often subject to a delayed diagnosis. Cytologic examination provides excellent diagnostic capabilities and is a safe procedure with a lower risk of local implantation, compared to needle or incisional biopsy. We herein report a patient who underwent surgical resection of a primary malignant melanoma of the nipple. An elastic soft nodule was observed on the left nipple, and no abnormal lesions were identified in the breast. Eventually, a malignant melanoma was diagnosed from the clinical and cytological evaluation findings. This bulky tumor was classified as a stage IIIC nodular melanoma, with a thickness of 12 mm. The patient received adjuvant chemotherapy and exhibits no evidence of recurrence 7 years after surgery.
Indoor Air Quality Monitoring via IT Network

A simple and rapid monitoring system of indoor air pollution is proposed by integrating a colorimetric detection method of formaldehyde (HCHO) and a function of image transmission of a mobile phone via IT network.The colorimetric detector employs a solid phase colorimetric reagent made from AHMT, ZnO, KIO4 and agar, and discolors from white to purple when exposed to a certain amount of HCHO gas. Although the color degree responds to the HCHO concentration levels, visual judgment often involves personal bias that leads to uncertainty. In this feasibilitystudy, variation of the acquired photo images expressed in RGB model was investigated by changing a type of mobilephone and conditions of photography. As a result, the calibration of the measured color degree with a color standardreduced the variation of the results and gave a significant color degree when the images were taken under the consistentcondition. Since the light source was one of the significant elements for image analysis, the light condition should be decided for practical applications.

The Sick House/Building Syndrome has been a serious problem since 1996, due to low level exposure to hazardous chemicals such as formaldehyde and volatile organic compounds (VOCs) emitted in airtight houses. This paper aims to show current status of the syndrome in Japan and practical researches to promote prevention of, and improvement to indoor air pollution due to chemicals.

The Influence of Laterality during Gait Cycle on Manual Pushing

The objective of this study was to investigate the influence of laterality in various speeds of gait cycle on manual pushing which has been given less attention. The participants of this study were 17 healthy males, 6 of them were left-handers. They performed trials by pushing a wall force plate during walking on a treadmill in three speeds: 1.5, 3, and 4 km/h. The participants were instructed to push at no less than 50% of their maximum static pushing force. Electromyogram (EMG) data was collected bilaterally from tibialis anterior, soleus, lumbar erector spinae and triceps brachii muscles. Time series analysis was normalized into gait cycle which was described as time between heel strike and just before the next heel strike of the same foot and it consisted of 100 data points. To show symmetry, cross-correlation function analysis from the left and right gait cycle variables were compared. Asymmetrical gait cycle period was only observed among right-handers at 3 km/h speed where right gait cycle had longer time than left gait cycle. Pushing force symmetry index was not changed in all of the three speeds in both right-handers and left-handers. Pushing force exertion showed asymmetry in between left and right gait cycle in 4 km/h pushing. The activation pattern of tibialis anterior, soleus and lumbar erector spinae muscles did not show any significant change due to different speeds of manual pushing. However, triceps brachii showed differences due to speeds among left-handers. The better understanding of laterality on manual pushing will help in not only better design of manual pushing tools and tasks but also on its application beyond manual materials handling.

Objective: Highly effective chemotherapy for patients with low-risk gestational trophoblastic neoplasia (GTN) is associated with almost a 100% cure rate. However, 20%–30% of patients treated with chemotherapy need to change their regimens due to severe adverse events (SAEs) or drug resistance. We examined the treatment outcomes of second-line chemotherapy for patients with low-risk GTN. Methods: Between 1980 and 2015, 281 patients with low-risk GTN were treated. Of these 281 patients, 178 patients were primarily treated with 5-day intramuscular methotrexate (MTX; n=114) or 5-day drip infusion etoposide (ETP; n=64). We examined the remission rates, the drug change rates, and the outcomes of second-line chemotherapy. Results: The primary remission rates and drug resistant rates of 5-day ETP were significantly higher (p<0.001) and significantly lower (p=0.002) than those of 5-day MTX, respectively. Forty-seven patients (26.4%) required a change in their chemotherapy regimen due to the SAEs (n=16) and drug resistance (n=31), respectively. Of these 47 patients failed the first-line regimen, 39 patients (39/47, 82.9%) were re-treated with single-agent chemotherapy, and 35 patients (35/39, 89.7%) achieved remission. Four patients failed second-line, single-agent chemotherapy and eight patients (17.0%) who failed first-line regimens were treated with combined or multi-agent chemotherapy and achieved remission. Conclusions: Patients with low-risk GTN were usually treated with single-agent chemotherapy, while 20%–30% patients had to change their chemotherapy regimen due to SAEs or drug resistance. The second-line regimens of single-agent chemotherapy were effective; however, there were several patients who needed multiple agents and combined chemotherapy to achieve remission.
