        Comparison of Clinical and Radiographic Outcomes Between Transforaminal Endoscopic Lumbar Discectomy and Microdiscectomy: A Follow-up Exceeding 5 Years

        Objective: To compare the long-term clinical and radiographic outcomes of transforaminal endoscopic lumbar discectomy (TELD) versus microdiscectomy (MD). Methods: The data of 154 patients with lumbar disc herniation (LDH) who underwent TELD (n = 89) or MD (n = 65) were retrospectively analyzed. The patients’ clinical outcomes were evaluated using visual analogue scales for leg and low back pain, the Japanese Orthopaedic Association (JOA) score, and the Oswestry Disability Index (ODI). The evolution of radiographic manifestations was observed during follow-up. Potential risk factors for a poor clinical outcome were investigated. Results: During a mean follow-up of 5.5 years (range, 5–7 years), the recurrence rate was 4.49% in the TELD group and 1.54% in the MD group. All scores significantly improved from preoperatively to postoperatively in both groups (p < 0.01). The improvement in the ODI and JOA scores was significantly greater in the TELD than MD group (p < 0.05). Forty-seven patients (52.8%) in the TELD group and 32 (49.2%) in the MD group had Modic changes before surgery, most of which showed no changes at the last follow-up. The degeneration grades of 292 discs (71.0%) were unchanged at the last follow-up, while 86 (20.9%) showed improvement, mostly at the upper adjacent segment. No significant difference was observed in the intervertebral height index or paraspinal muscle-disc ratio. Conclusion: Both TELD and MD provide generally satisfactory long-term clinical outcomes for patients with LDH. TELD can be used as a reliable alternative to MD with less surgical trauma. Modic type II changes, decreased preoperative intervertebral height, and a high body mass index are predictors of a poor prognosis.       • Low Level Segmentation of Motion Capture Data based on Hierarchical Clustering with Cosine Distance

        3D motion capture is to track and record human movements. In recent years, it has been applied into many fields, such as human computer interaction, animation, etc. Low-level segmentation of motion capture data is of significance to the various applications of 3D motion capture; however, due to the high dimensionality of motion capture data, traditional low-level segmentation methods can hardly work out a suitable segmentation for motion capture data. In order to solve this problem, a low-level temporal segmentation algorithm based on cosine distance is proposed, hierarchical clustering is explored so that similar velocity vectors are clustered together according to the cosine distance in a progressive way, the center of each cluster is updated as the vector derived with linear regression, the segment boundaries are determined as the point when the cosine distance between adjacent velocity vectors is greater than 1 (angle>90 degrees). We have conducted experiments on the motion capture database provided by Carnegie Mellon University (CMU), the experiment results show that the performance of the proposed method is optimistic.

        Outcomes of Anti-CD19 CAR-T Treatment of Pediatric B-ALL with Bone Marrow and Extramedullary Relapse

        PurposeAnti-CD19 chimeric antigen receptor T-cell immunotherapy (19CAR-T) has achieved impressive clinical results in adult and pediatric relapsed/refractory (r/r) B-lineage acute lymphoblastic leukemia (B-ALL). However, the application and effect of CAR-T therapy in B-ALL patients with extramedullary relapse are rarely issued even disqualified in some clinical trials. Here, we examined the efficacy of 19CAR-T in patients with both bone marrow and extramedullary involvement.Materials and MethodsCAR-T cells were generated by transfection of primary human T lymphocytes with a lentiviral vector expressing anti-CD19 single chain antibody fragments (scFvs) with the cytoplasmic domains of 4-1BB and CD3ζ, and used to infuse patients diagnosed as having r/r B-ALL with extramedullary origination. Clinical responses were evaluated by the use of bone marrow aspiration, imaging, and flow cytometry. ResultsEight patients received 19CAR-T infusion and all attained complete remission (CR). Only one patient was bridged to hematopoietic stem cell transplantation (HSCT). Although three patients relapsed after infusion, they received 19/22CAR-T infusion sequentially and attained a second remission. To date, five patients are in continuous CR and all eight patients are still alive. The mean follow-up time was 21.9 months, while the 24-month estimated event-free survival is 51.4%. Conclusion19CAR-T therapy can lead to clinical remission for extramedullary relapsed pediatric B-ALL patients. However, the problem of CD19+ relapses after CAR-T remained to be solved. For patients relapsing after CAR-T, a second CAR-T therapy creates another opportunity for remission for subsequent HSCT.

        A novel hydrothermal releasing synthesis of modified SiO2 material and its application in phenol removal process

        A modified SiO2 material (MSM) was successfully synthesized by using Na2SiO3 and a novel CO2SM in the presence of CTAB through an innovative hydrothermal releasing treatment protocol. The MSM was systemically characterized using several techniques, including N2 adsorption-desorption isotherm, thermogravimetric analysis (TGA), Fourier transform infrared spectroscopy (FTIR), scanning electron microscopy (SEM), transmission electron microscopy (TEM) and X-ray diffraction (XRD). The CO2SM as a platform for CO2 capturing and releasing can be reversibly used for multiple cycles without significant loss of capabilities. The optimum conditions for the preparation of MSM were identified as follows: Na2SiO3 concentration, 0.2mol/L; hydrothermal temperature, 120 oC; CO2SM dosage, 6 g; CTAB concentration, 24 g/L, reaction time, 12 h. Additionally, the MSM exhibited high efficiency for the removal of phenol from aqueous solution: it showed a phenol adsorption capacity of 91.29mg/g when shaken with the aqueous phenol solution at 180 rpm for 1 h at 25 oC.

        Formation of Protein Bodies and the Response to Nitrogen in Different Positions During Wheat Endosperm Development

        The development of protein bodies (PBs) determinesthe processing properties of wheat (Triticum aestivum L.). Ithas been known that nitrogen uptake has a strong impact ongrain protein concentration. However, the differences of theformation of PB in different developmental stages anddifferent positions in wheat endosperm are still controversial. To solve these issues, PBs formation in different wheatendosperm parts and the response to nitrogen were investigatedusing light, transmission electron microscopes in presentstudy. The main results were as follows. (1) Proteins mainlyformed PBs via the Golgi apparatus in the vacuole at theearly stage of wheat endosperm development. After 7 dayspost anthesis, most proteins were derived from the roughendoplasmic reticulum. (2) The morphology of PBs was diverseamong different endosperm parts. Compared with the centralendosperm, the PBs in sub-aleurone cells were abundant andlarge. (3) More abundant endoplasmic reticulum, Golgi andmitochondrion were observed at the early stage after nitrogentreatment. Nitrogen also increased the accumulation of PBsat the later stage. (4) The sub-aleurone region cells of theendosperm presented more significant responses to nitrogenthan the central endosperm.

        Meta-analysis of the effect of melatonin application on abiotic stress tolerance in plants

        Melatonin is a hormone-like substance that promotes plant growth and development and alleviates stress levels. Although the physiological roles of melatonin and the underlying mechanisms have been qualitatively reviewed in plants, we do not fully understand when and how to apply melatonin to maximize its benefits. Here, we performed a meta-analysis to quantitatively evaluate the effect of melatonin on abiotic stress tolerance in plants and to determine the number of parameters modulated by melatonin. Melatonin significantly alleviated the growth inhibition induced by drought stress compared with other abiotic stresses, including salt, cold, heat, nitrogen deficit, and heavy metal toxicity, mainly owing to higher photosynthesis efficiency and antioxidant enzyme activity. Furthermore, melatonin modulated plant growth in a concentration-dependent manner and was more effective when applied to plants under moderate drought stress at an early stage via root irrigation. In addition, the impact of melatonin was greater in monocots than in dicots. Moreover, endogenous melatonin levels could be significantly increased via transgenic strategies. Among melatonin biosynthesis-related gene members, ASMT has tended to have the most influence on melatonin content in plants. In light of the rapidly developing genome editing technology, quantitatively increasing endogenous melatonin level in plant would be quite useful for moderating climatic conditions and combating desertification. Taken together, our results provide guidelines for melatonin application in crops plants for improving productivity under ongoing climate change.

        Prediction of Prostate Cancer Risk Stratification Based on A Nonlinear Transformation Stacking Learning Strategy

        Purpose: Prostate cancer (PCa) is an epithelial malignancy that originates in the prostate gland and is generally categorized into low, intermediate, and high-risk groups. The primary diagnostic indicator for PCa is the measurement of serum prostate-specific antigen (PSA) values. However, reliance on PSA levels can result in false positives, leading to unnecessary biopsies and an increased risk of invasive injuries. Therefore, it is imperative to develop an efficient and accurate method for PCa risk stratification. Many recent studies on PCa risk stratification based on clinical data have employed a binary classification, distinguishing between low to intermediate and high risk. In this paper, we propose a novel machine learning (ML) approach utilizing a stacking learning strategy for predicting the tripartite risk stratification of PCa. Methods: Clinical records, featuring attributes selected using the lasso method, were utilized with 5 ML classifiers. The outputs of these classifiers underwent transformation by various nonlinear transformers and were then concatenated with the lasso-selected features, resulting in a set of new features. A stacking learning strategy, integrating different ML classifiers, was developed based on these new features. Results: Our proposed approach demonstrated superior performance, achieving an accuracy of 0.83 and an area under the receiver operating characteristic curve value of 0.88 in a dataset comprising 197 PCa patients with 42 clinical characteristics. Conclusions: This study aimed to improve clinicians’ ability to rapidly assess PCa risk stratification while reducing the burden on patients. This was achieved by using artificial intelligence-related technologies as an auxiliary method for diagnosing PCa.

        Preparation of decorative slag glass-ceramics and research on the solidification of heavy metals

        Glass-ceramics with different CuO additions were prepared with Fe2O3 and Cr2O3 as composite nucleating agents, rare earthcontainingblast furnace slag (REBFS) as the main raw material. The existence state and stability of heavy metals Cu, Mn andCr in glass-ceramics were investigated. The results showed that Cr ions contributed to the generation of spinel phase, whileCu ions and Mn ions can enter the spinel crystal and exist stably, which indicated that there was a synergistic solidificationof Cu, Mn and Cr. The depolymerization effect of the copper ions contributed to the transformation of the crystal morphologyfrom dendrites to spherulites. Moreover, the increase in CuO contents promoted glass-ceramics from green to copper red. Based on the leaching experimental data analysis of heavy metals in glass-ceramics, the leaching concentration of heavy metalsCu, Cr and Mn were much lower than the standard leaching toxicity limit of hazardous waste (GB5085.3–2007, China). Theresults showed that the conversion of REBFS into environmentally friendly glass-ceramics can realize solid waste resourceutilization.

        Combined estrogen-progestin pill is a safe and effective option for endometrial hyperplasia without atypia: a three-year single center experience

        Objective: To evaluate the effectiveness of oral contraceptive pill (OCP) as therapy for endometrial hyperplasia (EH) without atypia in reproductive-aged women compared with oral progestin. Methods: A retrospective cohort study was carried out in our reproductive center. Consecutive patients diagnosed with infertility and non-atypical EH identified through electronic database who met inclusion criteria (n=309). Patients were assigned to two treatment groups: OCP (n=216) and oral progestin (n=93); clinical and reproductive outcomes were recorded. Results: Reversal of EH to normal endometrium, clinical pregnancy, live birth and miscarriage rate. Women in OCP group were younger, had higher prevalence of Polycystic Ovary Syndrome and other uterine pathology and longer duration of infertility than women in progestin group. Reversal of EH was observed in 93.52% women on OCP and in 86.02% women on progestin (p=0.032; adjusted odds ratio [aOR]= 2.35; 95% confidence interval [CI]=1.06-5.21) after the initial course of treatment for 2 to 6 months. Cyclic OCP (n=184) resulted in better response to treatment compared to continuous OCP (n=32) (95.11% vs. 84.38%; p=0.039; aOR =3.60; 95% CI =1.12-11.55). Clinical pregnancy rate in OCP group was marginally higher than progestin group (87/208, 41.83% vs. 27/90, 30.00%; p=0.054). Miscarriage (25.29% vs. 29.63%; p=0.654) and live birth rate (31.25% vs. 21.11%; p=0.074) were comparable between the groups. Conclusion: For the first time we demonstrate that OCP is an effective therapy for nonatypical EH and is associated with higher remission rate compared with oral progestin. Reproductive outcomes are reassuring and comparable between the two groups.
