        Switching to Aflibercept in Diabetic Macular Edema after Unsatisfactory Response to Other Anti-vascular Endothelial Growth Factor Drugs

        Purpose: To evaluate the efficacy of switching to aflibercept in diabetic macular edema (DME) with suboptimalresponse to previous anti-vascular endothelial growth factor (anti-VEGF) injections. Methods: A prospective interventional case series study recruited patients from a single center diagnosedwith DME with suboptimal response to anti-VEGF injections. Three consecutive monthly injections of afliberceptwere performed. The primary outcome measure was mean change in visual acuity after switching toaflibercept. Results: Forty-two patients (42 eyes) were included. Baseline logarithm of the minimum angle of resolution(logMAR) visual acuity was 0.87 ± 0.23 and improved significantly to 0.62 ± 0.29, 0.56 ± 0.34, and 0.46 ±0.35 at 1, 2, and 3 months, respectively, after the first injection. Mean baseline retinal thickness was 451.57± 107.09 μm and decreased significantly at 1, 2, and 3 months after switching to aflibercept (346.52 ± 79.03,328.24 ± 81.98, and 313.71 ± 85.79 μm, respectively). Both visual improvement and mean change in retinalthickness were significant in patients with pre-aflibercept best-corrected visual acuity less than 1.0 logMARbut were not significant in patients with best-corrected visual acuity more than 1.0 logMAR. Conclusions: Switching to aflibercept in DME patients with an unsatisfactory response to previous anti-VEGFinjections provided acceptable short-term visual and retinal architectural improvement.

        Blast Furnace Slag Addition Effects on Delayed Ettringite Formation in Heat-cured Mortars

        Reducing expansion due to Delayed Ettringite Formation (DEF) is becoming an important issue that must be taken into account, when producing heat-cured concrete. Mineral additives used as partial cement replacement should help in achieving the desired aspect. Based on an ongoing experimental program, this research emphasizes on the effect of Blast Furnace Slag (BFS) on the expansion of cement mortars caused by DEF. For this purpose, mortar prisms in which Portland cement was replaced by BFS, with three different replacement levels (10,20 and 40%) and two finenesses degree (400 and 500 m2/kg) were tested in length change, dynamic elasticity modulus and compressive strength over more than 750 days. Results showed that the efficiency of blast furnace slag in controlling expansion related to DEF was conditioned by its levels of replacement.

      • Bond behavior between concrete and prefabricated Ultra High-Performance Fiber-Reinforced Concrete (UHPFRC) plates

        Externally bonded ultrahigh performance fiber-reinforced concrete (UHPFRC) is commonly used as a strengthening material for reinforced concrete (RC) structures. This study reports the results of an experimental program investigating the bonding behavior between concrete and prefabricated UHPFRC plates. The overall experimental program is consisting of five RC specimens, which are strengthened using the different lengths and widths of prefabricated UHPFRC plates. These specimens were analyzed using the pull-pull double-shear test. The performance of each strengthened specimen is presented, discussed and compared in terms of failure mode, maximum load, load-slip relationship, fracture energy and strain distribution. Specimen C- 25-160-300 which bonded along the whole width of 160 mm recorded the highest maximum load (109.2 kN) among all the analysed specimens. Moreover, a 3D numerical finite element model (FEM) is proposed to simulate the bond behavior between concrete and UHPFRC plates. Moreover, this study reviews the analytical models that can predict the relationship between the maximum bond stress and slip for strengthened concrete elements. The proposed FEM is verified against the experimental program and then used to test 36 RC specimens strengthened with prefabricated UHPFRC plates with different concrete grades and UHPFRC plate widths. The obtained results together with the review of analytical models helped in the formation of a design equation for estimating the bond stress between concrete and prefabricated UHPFRC plates.

      • Experimental and numerical analysis of the punching behavior of RC isolated footings

        In the current study, punching behavior of Reinforced concrete (RC) isolated footings was experimentally and numerically investigated. The experimental program consisted of four half-scale RC isolated footing specimens. The test matrix was proposed to show effect of footing area, reinforcement mesh ratio, adding internal longitudinal reinforcement bars and stirrups on the punching response of RC isolated footings. Footings area varied from 1200x1200 mm2 to 1500x1500 mm2 while the mesh reinforcement ratio was in the range from 0.36 to 0.45%. On the other hand, a 3D non-linear finite element model was constructed using ABAQUS/standard program and verified against the experimental program. The numerical results agreed well with the experimental records. The validated numerical model was used to study effect of concrete compressive strength; longitudinal reinforcement bars ratio and stirrups concentration along one or two directions on the ultimate load, deflection, stiffness and failure patterns of RC isolated footings. Results concluded that adding longitudinal reinforcement bars did not significantly affect the punching response of RC isolated footings even high steel ratios were used. On the contrary, as the stirrups ratio increased, the ultimate load of RC isolated footings increased. Footing with stirrups ratio of 1.5% had ultimate load equal to 1331 kN, 19.6% higher than the bare footing. Moreover, adding stirrups along two directions with lower ratio (0.5 and 0.7%) significantly enhanced the ultimate load of RC isolated footings compared to their counterparts with higher stirrups ratio (1.0 and 1.5%).

        Damage Profile of HDPE Polymer using Laser-Induced Plasma

        In this paper we studied the laser-induced crater depth, mass, and emission spectra of laser-ablated high-density polyethylene (HDPE) polymer using the laser-induced plasma spectroscopy (LIPS) technique. This study was performed using a Nd:YAG laser with 100 mJ energy and 7 ns pulse width, focused normal to the surface of the sample. The nanoscale change in ablated depth versus number of laser pulses was studied. By using scanning electron microscope (SEM) images, the crater depth and ablated mass were estimated. The LIPS spectral intensities were observed for major and minor elements with depth. The comparison between the LIPS results and SEM images showed that LIPS could be used to estimate the crater depth, which is of interest for some applications such as thin-film lithography measurements and online measurements of thickness in film deposition techniques.

        Effectiveness of Active Rehabilitation Program on Sports Hernia: Randomized Control Trial

        Objective To determine whether an active rehabilitation program that involves repetitive effortful muscle contractions, including core stability, balancing exercises, progressive resistance exercises, and running activities, after a sports hernia, is effective. Methods Forty soccer players with sports hernias were randomly divided into two equal groups: group A (active rehabilitation program) and group B (conventional treatment). The methods of assessment included a visual analog scale (VAS) and hip internal and external range of motion assessments. Group A received conventional treatment (heat, massage, transcutaneous electrical nerve stimulation, and mobilization) plus an active rehabilitation program, while group B received only conventional treatment. Three treatment sessions were given each week for 2 months. Evaluations were performed pre- and post-treatment. Results A decrease in VAS was seen in both groups at the end of treatment, 80.25% in group A and 41.93% in group B. The difference between the two groups was statistically significant (p=0.0001), whereas there were no statistical differences in internal and external rotation between the groups at the end of treatment (p>0.05). After treatment, an improvement in outcome measures of group A compared to group B (p=0.01) was seen. Thirteen patients in group A and only three patients in group B returned to sports activities without groin pain. Conclusion Active rehabilitation was effective for sports hernia management measured by a decrease in pain and the return to sports.

        Appendiceal Neuroendocrine, Goblet and Signet-Ring Cell Tumors: A Spectrum of Diseases with Different Patterns of Presentation and Outcome

        Purpose Appendiceal tumors are a heterogeneous group of diseases that include typical neuroendocrine tumors (TNET), goblet cell carcinoids (GCC), and atypical GCC. Atypical GCC are classified into signet-ring cell cancers (SRCC) and poorly differentiated appendiceal adenocarcinoids. The prognosis and management of these diseases is unclear because there are no prospective studies. The aim of this study is to assess the characteristics and outcome of appendiceal TNET, GCC, and SRCC patients. Materials and Methods Appendiceal TNET, GCC, and SRCC patients diagnosed between 1973 and 2011 were identified in the Surveillance Epidemiology and End Results (SEER) database. Demographics, type of surgery, and clinicopathologic characteristics were collected. Survival functions were estimated by the Kaplan-Meier method, and log-rank test was used to assess the difference in overall survival (OS) among the three histologies. Results The SEER database yielded 1,021 TNET patients, 1,582 with GCC, and 534 SRCC patients. TNET presented at a younger age (p < 0.001). Patients with SRCC presented with advanced stage disease (p < 0.001). The median OS (mOS) for GCC and TNET patients was not reached; mOS for SRCC was 24 months. Multivariate analysis stratified for stage revealed significantly longer survival for TNET and GCC than SRCC (p < 0.001). Conclusion This is the largest report to date for appendiceal neuroendocrine tumor patients, suggesting a spectrum of diseases with different characteristics and outcomes. In this report, we present a treatment approach for this complex spectrum of disease, based on the experience of Ohio State and Emory Universities investigators.
