        Port-Site Metastases and Chimney Effect of B-Ultrasound-Guided and Laparoscopically-Assisted Hyperthermic Intraperitoneal Perfusion Chemotherapy

        Purpose: CO2 leakage along the trocar (chimney effect) has been proposed to be an important factor underlying port-site metastasisafter laparoscopic surgery. This study aimed to test this hypothesis by comparing the incidence of port-site metastasis betweenB-ultrasound-guided and laparoscopically-assisted hyperthermic intraperitoneal perfusion chemotherapy (HIPPC). Materials and Methods: Sixty-two patients with malignant ascites induced by gastrointestinal or ovarian cancer were divided into two groups to receive either B-ultrasound-guided or laparoscopically-assisted HIPPC. Clinical efficacy was assessed from the objective remission rate (ORR), the Karnofsky Performance Status (KPS) score, and overall survival. The incidence of port-site metastasis was compared between the two groups. Results: Patients in the B-ultrasound (n=32) and laparoscopy (n=30) groups were comparable in terms of age, sex, primary diseasetype, volume of ascites, and free cancer cell (FCC)-positive ascites. After HIPPC, there were no significant differences between the B-ultrasound and laparoscopy groups in the KPS score change, ORR, and median survival time. The incidence of port-site metastasis after HIPPC was not significantly different between the B-ultrasound (3 of 32, 9.36%) and laparoscopy (3 of 30, 10%) groups, but significantly different among pancreatic, gastric, ovarian, and colorectal cancer (33.33, 15.79, 10.00, and 0.00%, p<0.001). Conclusion: The chimney effect may not be the key reason for port-site metastasis after laparoscopy. Other factors may play a role, including the local microenvironment at the trocar site and the delivery of viable FCCs (from the tumor or malignant ascites) to the trauma site during laparoscopic surgery.

      • Evaluate VTS benefits: A case study of Zhoushan Port

        It has been well acknowledged that Vessel Traffic Services (VTS) has played a growing important role to ensure the safety of navigation in the busy ports and waterways. However, the benefits produced by VTS are usually ignored by the public and private sectors. Besides, the previous evaluations generally exist following problems: (1) It is difficult to collect the data for the parameters in the evaluation models and/or the parameters are designed illogically; (2) Those models did not take the following factors into consideration such as reducing the frequency of coastal vessel patrolling and saving human and material resources; (3) It is difficult to clearly discriminate the benefits derived from VTS and non-VTS. In this paper, a framework is presented to calculate the benefits of VTS in China. Four key indicators (safety, traffic efficiency, environmental protection and reducing supervising cost) and quantitative methods have been introduced into the framework. When calculating the benefits quantitatively, the traffic condition before the construction (expansion) of the VTS has acted as a benchmark. For a case study, the project of the expansion of VTS in Zhoushan Port, East China was evaluated with 10-year data. According to the results, the largest contribution is from the benefit of environmental protection. Via Cost-benefit analysis the benefit cost ratio (B/C) of the VTS is up to 5.248, which shows the benefits produced by VTS are considerable. The research could provide references for VTS benefits evaluation and investment optimizing.

        A DFT study of the geometrical structures and electronic properties of CumMn (m + n = 6, M = Fe, Co, Ni) clusters

        In this work, we investigated bimetallic CumMn (m + n = 6, M = Fe, Co, Ni) clusters on the basis of the density functional theory at the M06L level. While changing the structure and composition, our team studied the geometrical structures, relative stabilities and electronic magnetic moments of these clusters. We found that the average atomic binding energies vary linearly with the number of doping atoms in the CumFen and the CumNin clusters. The values of electron affinity (EA) and the ionization potential (IP) for the CumNin clusters are closer to those of pure Cu clusters compared with the others. A negative value of the EA is only seen in CumFen clusters. The electronic magnetic moments of these clusters indicate that most of them exhibit a local minimum when doped with three atoms. Moreover, we further analyzed the HOMO–LUMO gaps, the natural population, and the geometrical structures.

        Current state of research about acupuncture for the treatment of COVID-19: A scoping review

        Background Since the outbreak of coronavirus disease (COVID-19), acupuncture has been widely used in the treatment of COVID-19. The research community has responded rapidly and has already published many research articles about this topic. Methods We searched PubMed, Embase, Cochrane Library as well as CNKI, Wanfang and VIP from January 1, 2020 to July 31, 2021. The dates of publication, language of publication, methodological characteristics and the key findings were analyzed separately. The data are presented as bar graphs, structured tables and figures. Results In this scoping review, 16 research articles were included: 7 case reports, 6 observational studies, 1 review, 1 RCT and 1 nonrandomized clinical trial. The majority of the articles (81.3%) were published by Chinese scholars, 12.5% articles were by scholars in the United States, and 6.3% articles were by scholars in Iran. The included studies reported that acupuncture could alleviate the symptoms of COVID-19 patients, shorten their hospitalization days, and is effective for the elderly. There were no side effects reported. The most frequent acupoints used were LI4, PC6, ST36 and KI3. They reported many obstacles in implementing acupuncture therapy for treating COVID-19 patients. Conclusion Acupuncture has a good effect for the treatment of COVID-19, but high-quality evidence support is still lacking. Coupled with the difficulties that acupuncturists experienced during the process of treatment, the promotion of acupuncture treatment for COVID-19 faces many obstacles.
