        치근단 병소에서 면역적격세포의 분포에 관한 연구

        Periapical lesions are developed as a result of inflammatory response to irritants from root canal system. Clinicians remove these irritants from root canal system and seal the root canal space to induce healing of the periapical lesions. Immunopathologic responses may play an impostant role in development ans progression of periapical lesions ans periapical lesions contain immunocompetent cells. The purposes of the present study were to analys and to compare the distrebution of the immunocompetent cells in the human periapical lesions caaording to the stage of endodontic treatment using indrect immunoperoxdase technique. Obtained 94 human periapical lesions were devided into four groups : Group 1 : no endodontic treatment(28 samples) Group 2 : root canal enlarged and irrigated(28 samples) Group 3 : root canal filied(29 samples) Group 4 : unknown(9 samples) Monoclonal antibodies to examine target cells were UCHL-1 for T lymphocytes(1:200, Dakopatt, Denmark), L26 for B lymphocytes(1:200, Dakopatt, Denmark), OPD4 for heper T lymphocytes(1:200, Dakopatt, Denmark) and alpha-1-antichymotrypsin for macrophages (1:200, Dakopatt, Denmark). The following results were obtained : 1.All the periapical lesions studied were infiltrated by T lymphocytes, plasma cell, B lymphocytes, and macrophages, T lymphocytes were more infiltrated than B lymphocytes, and B lymphocytes and macrophages were less infiltrated than T lymphocytes and plasma cell(P<0.05 : Oneway ANOVA test). 2. In untreated group and canal irrigated and enlarged group of all the periapical lesions, helper T lymphocytes were predominently infiltrated(P>0.05 : Oneway ANOVA test). 3. In canal filled groups of all lesions except periapical cyst, plasma cells were predominently infiltrated. But, in canal filled group of periapical cyst, helper T lymphocytes were the predominent cells(P>0.05 : Oneway ANOVA test). The above results shows that the immunologic responses play important role in pathogensis of periapical lesions and the immunologic response involved undergoes certain changes after endodontic therapy.

        공초점 레이저 주사 현미경을 이용한 법랑질 초기 우식 재광화의 정량적 분석

        Dental Caries which has high prevalence rate, accounts for majority of dental diseases. Many treatment and preventive treatment has been developed, thereby reducing the prevalence rate, but in our country, flouridization has not spread widely yet, so prevention has not been done satisfactorily. When dental caries progresses, irreversible damage of tooth structure occurs. In initial dental caries. Demiteralized tooth structure can be remineralized, so restorative treatment is unnecessary. In the study, 20 teeth restored with composite resin without fluoride release were used and divided into two groups. Incipient dental caries were artificially made and remineralization procedure was done for 1 and 2 weeks, for each group. Changes in mineral contents aroud the margins were analysed with confocal laser scanning microscope. The results were as follow. 1.Both total fluorescence of the lesion and average fluorescence of the lesion of remineralized samples decreased compared to demineralized state. (p<0.01) 2.Confocal laser scanning microscopy can be used in quantitative analysis of mineral change. In result, confocal laser scanning microscopy can be used in quantitative analysis of mineral change and it could be used in many different fields of dentistry in the future.         만곡근관에서 수동식 기구와 엔진 구동형 기구의 근간성형 효과에 대한 비교 연구

        The purpose of this study was to compare the effects of various techniques on the quality and quantity of instrumentation in curved canals. Instrumentation time was evaluated too. Forty eight canals of resin blocks(35°) were divided into three groups and filed. In group A, 16 blocks were instrumented using a step-back technique with stainless steel(SS) K-file(Mani, Matsutani Seisakusho Co., Japan). In group B, canals were prepared with SS K-files using the Endo-Angle(Nakanishi Dental MFG.CO.,LTD.,Japan). Group C was prepared using Ni-Ti engine-driven instruments(Quantec Series 2000^TM system). Group A and B were filed from #15 to #25 at the apex followed by a 1mm stepback technique, and the coronal half of the canals were flared with Gates Glidden burs(#2,3,4). Group C was instrumented from #1 to #8. After the instrumentation time was measured, canals were filled with India ink, and photographed, which to magnify the canal images 8 times. Using these photographs the apical portions were evaluated And scored from 1 to 4 according to the severity of ledging and zipping. On the same photographs, the area of the coronal two thirds of the canals were measured using a personal computer with the computer program Brain and Digitizer(SummaSketch Ⅲ). The following results were obtained. 1. Instrumentation time was significantly shorter for group C, as opposed to group A and B(ANOVA, P<0.05). 2. The qualitative evaluation of the apical portions of the canals showed no significant difference between the groups(Kruscal-Wallis, p>0.05). 3. The area removed by group C was significantly smaller than group A and B(ANOVA, P<0.05).

        수종의 엔진 구동형 니켈-타이타늄 합금파일의 절삭각에 따른 근관성형 효과에 관한 전자현미경적 연구

        The purpose of this in vitro study was to compare the effects of root canal debridement following rotary Ni-Ti instruments with positive versus negative rake angle. Seventy sound, extracted human anterior teeth & premolars were randomly divided into four groups. The used rotary instruments were Ni-Ti HERO642(Micro-Mega in France, 20 specimens), Ni-Ti ProFile(Maillefer, Ballaigues, Switzerland, 20 specimen), stainless steel engine reamer(Mani, Matutani Seisakusho Co.,Japan, 20 specimen) and negative control group(10 specimens) was only extirpated with barbed broach(Mani, Matsutani Seisakusho Co., Japan) Group 1 & 2 teeth were prepared to a #40 at the apex followed by 1 mm using crown-down technique. Group 3 teeth were instrumented from a #15 to a #40 in sequential order. After preparation and final irrigation, the roots split longitudinally into a bucco-lingual direction. Root halves were cross-sectioned in apical third portion again. all root specimens were prepared for SEM investigation & photographed. Separate evaluations were undertaken for smear layer on prepared walls with a five score-index for each using reference photograph in root halves. the penetration depth of smear layer into dentinal tubules was also estimated in the other halves. the following results were obtained : 1. Smear layer was observed on all the prepared walls with three experimental groups except negative control group 2. Smear layer characteristics1) HERO 642 groups showed snowy & dusty appearance & were observed only few some dentinal tubuli open on the prepared walls, and the penetration depth of it into dential tubules may be 1-2 ㎛ thick. 2) ProFile groups showed shiny & burnished appearance & complete root canal wall covered by a homogenous smear layer with no open dentinal tubuli and penetration depth of it into dentinal tubules may be 1-2㎛ thick. 3) Engine reamer groups showed obviously file's passed tracks on the prepared walls & were observed complete root canal wall covered by a homogenous smear layer with no open dintinal tubuli. The results revealed that a completely clean root canal could not be achieved regardless of positive & negative rake angle, which is in accordance with the majority of studies on root canal cleanliness In conclusion, throughout irrigation with antibacterial solutions or chelating agents is recommended to remove the smear layer on prepared canal walls.

      • PHENOMENA OF TEMPERATURE INVERSION IN THE EAST SEA OF KOREA

        Statical properties of temperature inversion in the East Sea were studied by using the BT data from 1965 to 1982. Temperature inversions occurred in winter about 7 times more than in summer. In winter, temperature inversions were found at every depth and in almost all areas of the East Sea. In summer, it usually occurred at the surface layer(0~10m) in the warm current region(35˚~41˚N), and at the intermediate layer(150~250m) in the cold current region(40˚~41˚N). Temperature inversion of winter can be explained by mixing due to a strong wind and convection due to surface cooling. Temperature inversion of summer in the surface layer results from the northward advection of the Tsushima Current. And temperature inversion of summer in the intermediate layer is affected by the mixing of the Tsushima warm water and the northern cold water.

        In order to observe hydrological characteristics in the Gulf of Yoeng-il, measurements of water temperature have been carried out from February to August 1983, and from August 1985 to December 1986. Surface water temperatures are closely linked to air temperature and solar-radiation for all littoral stations. Seasonal and annual variations of all horizontal stations are very similar to one another. Vertical variations of water temperatures for the stations located along the central-line of the Gulf are characteristic (and observed) as thermoclines and thermal stratification in the hottest summer-time (August). It is interesting to note that water column temperature of July and September are rather warmer than August, on account of a cold water mass coming from NE direction. For example, surface temperature in June 1982 was above 19℃, but the water below a depth of about 30m drastically drops to 4℃. It is indicative of invasion of water mass from cold current or deep sea.

        From September 1985 to August 1987, the horizontal and vertical variations of dissolved oxygen were studied at the Gulf of Yeongil on the coast of the East Sea of Korea. The concentration of values of dissolved oxygen and rate of oxygen saturation ranged from 6.0 to 8.0ml.?? and 80 to 120%, respectively. The dissolved oxygen oscillated between 0 and 13.17ml.?? with an average value of 5.55ml.??. The rate of oxygen saturation fluctuated from 0 to 233.8%. The characteristics of the water mass defined by the dissolved oxygen and the rate of saturation varied with the different environments. The annual viration of the these two factors fro northern coast area were 6.17ml.?? and 106.4%, those of central water area 6.18ml.?? and 104.6%, those of the old port coast area 2.33ml.?? and 38.1%, those of the POSCO coast area 6.10ml.?? and 102.8%, and those of the estuary of Hyungsan 6.72ml.?? and 118.5%, respectively.
