        알레르기비염 환자에서 집먼지진드기와 쑥화분에 대한 MAST, 피부단자시험 및 비유발시험 간의 연관성에 관한 연구

        Objective : Allergic rhinitis is one of the most common diseases in the field of otorhinolaryngology. Correct diagnosis for allergic rhintis is important for its optimal management. Many stuides have been reported for the diagnosis with allergens. This study was performed to identify the relationship between MAST. the skin test, and the nasal provocation test. We attempted to find an appropriate diagnostic method for finding causative allergens. Method : We reviewed the medical records of patients with allergic rhinitis symptoms and physical findings from August 2001 through July 2002 at Inha University Hospital. We selected 197 patients who were examined with MAST, the skin prick test, and the nasal provocation test. The relationship between these tests was detremined with SPSS (Ver 10.0). Results : There was a significant correlation between MAST and the skin prick test (F<.05). There were also significant correlations between MAST and the nasal provocation test, and between the skin prick test and the nasal provocation test(F<.05). Conclusion : MAST might be a useful diagnostic method for finding causative allergens, especially in children and patients with skin hypersensitivity and other skin disorders.         단순 외상 환자에서 퇴원시 설명문이 환자의 이해도에 미치는 영향

        The many previous publications have shown the advantages of written discharge instruction over verbal discharge instruction on the basis of comparing the functional illiteracy among emergency department patients. Therefore, authors have conducted a study on the efficacy of written discharge instruction over the verbal discharge instruction of two groups: minor laceration and sprain patients. For the purpose of this study, minor laceration is defined as less than 3 ㎝ laceration on the face or head, or less than 5 ㎝ laceration on extremities. And simple sprains were limited to the extremities. A total of 416 patients who visited Yong Dong Severance Hospital Emergency Care Center were the subject of this study during the period of 2 months from Feb. 1. to Mar. 31. 1996. The study was done prospectively, and patients who entered this study were randomly selected. Simple discharge instruction were made prior to the study and patients were given or verbally told the one of two instruction sets by the emergency medicine resident. After reading or hearing the instructions, each patient was asked to answer four specific questions about them. Statistical significance was examined by t-test and p value of 0.05 or less was considered statistically significant. Mean age for verbal and written instructed group were 26.2 and 25.6 years old. There were 262 and 154 patients in verbal and written instructed group, and the mean score was 2.24±1.20 and 3.42±0.52, respectively. The type of injury was divided into laceration and sprain. Laceration group showed mean score of 2.15±1.23, 3.32±0.51, and sprain group of 2.34±1.16, 3.59±0.50 in verbal and written instruction group, respectively. For the patient younger than 13 years old, discharge instruction was given or described to a guardian. Age less than 13 years old group showed mean score of 2.75±1.11, 3.31±0.48 and more than 13 years old group of 2.10±0.19, 3.34±0.53 in verbal and written instruction group, respectively. The result showed statistical significance between verbal and written instruction group of type of injury and age difference. For the future, simplified written discharge instruction may help and improve the patient`s understanding of the proper management of injury.

      • 관광도시의 관광자원·상품연계 개발에 관한 연구 : 경상북도 3개 관광도시를 중심으로 article connection in tourist city

        As central cities of the provinces north of Gyongsang, Mungkyeong, Andong, Yeongju have to set up a strategic plan of attracting more international and domestic tourists for development of central districts broadly. In particular, Mungyeong city puts spurs to develop after foundation of Mungyeong Tourism and Leisure Town Inc., which the Ministry of Commerce, Industry and Energy, Kangwon land, Mungyeong city joint-found on April 2003. All citizens put their effort on developing the Mungyeong Tourism and Leisure Town Inc., which is invested of national (government) and private expenditure about 11hundred million won. There are several alternative plans that enabled balance-development of promoting tourism locality. However, secure of investment capital is more important in tourist attractions-article connection. The stable secure of financial support for tourism development is primarily based on local economic activity. Unfortunately, almost all Korea local authorities foundations have radical limitation of promoting to tourism development project, this is mainly due to weak economic situation with fragility in local financial. Therefore, it is significant to get the central government and a megalopolis governments intervention regarding tourism development project. At present, the tendency is toward increasing of participating in private sectors and coming under private management in all kinds of development project. In addition, there found some limitation of promoting relative management only in the public organization, thus it is important to induce active private departments participation. Basically, it is essential to find out key factors of overcoming the difficulties on attracting private sectors investment and their participation. At the same time, there need to seek solution of the difficulties and execute offering the incentives hence attract private investment. According to the results from confidence-analysis and factor-analysis, the infrastructure and the expansion of social indirect capital are urgent through active supporting of a central government, megalopolis government and connection of tourism system. Moreover, it is suggested that there need to promote establishing of effective information-system, and correctness in tourist information, which affect in decision of a tourism policy. Finally, it is vital to promote development of tourist attractions-article connection.

      • 석탄폐석을 이용한 폴리머 콘크리트의 물리 역학적 특성

        최근의 급속한 경제성장과 건설물량의 증가로 인한 수요가 급격히 증대되고 있으나, 양질의 콘크리트용 골재 부족현상을 초래하고 있어 골재의 양적인 문제와 함께 질적인 문제의 해결이 건설재료 분야의 중요한 당면과제로 제기되고 있다. 따라서 본 연구에서는 강원도 탄광지역에 다량으로 산재되어 있는 석탄폐석을 콘크리트용 골재로서 활용하여 고강도이며 내구성이 우수한 폴리머 콘크리트를 제조하고 이에 대한 물리·역학적 특성을 실험적으로 구명하여 공장제품 재료로서의 이용을 위한 기초자료로서 제공코저 한다. Lately, rapid expansion of construction industry and following increment of demand for concrete in the construction created shortage of aggregates in the nation. Supplement of god quality aggregate is an immediate issue for the construction industry to solve. Therefore, this study evaluated a possibility of using coal mine waste collected from Kwangwon-do region as a source of aggregate in manufacturing polymer concretes which have high strength and high durability. First, aggregates were obtained by crushing coal mine waste and polymer concrete was manufactured using these aggregate. Mechanical property test results for the polymer concrete showed that the coal mine waste aggregates were acceptable to use as a replacement of the aggregate in polymer concrete manufacture.

        골반골절 환자에서 출혈의 정도를 반영하는 분류

        The causes of death of pelvic bone fracture are hemorrhage, associated internal organ injuries, sepsis, and multi-organ failure, among which hemorrhage still remains as the leading cause of death. While previous classification for pelvic bone fracture was focused on orthopedic treatment, Pennel & Sutherland suggested a new classification according to the pelvis AP film took in the emergency room. Since this classification supposedly evaluates hemorrhage from pelvic bone fracture, we are reporting on the efficiency of this classification. Out of all the 150 patients admitted to Yong Dong Severance Hospital Emergency Room from 1992, Jan. through 1994, Dec. due to pelvic bone fracture, 18 people with incomplete medical record and X-ray film excluded from the study. The study was done Cohort method and t-test was used. According to the new classification, the unstable fracture had a much more bleeding amount regardless of the type, and type I stable fracture bled more than all the other stable fracture. These were statistically meaningful. Also, there was no differences in the type of fracture when ISS(Injury Severity Scale), GCS(Glasgow Coma Scale), and associated injury were compared. In conclusion, previous classification was done according to orthopedic treatment and this classification could be done only after pelvis X-ray series and pelvic bone CT was performed. Therefore much more time is consumed, more stress is put upon the patient and no comparisons can be made in performing emergency care. On the other hand, modified Pennel &-Sutherl`s classification can be simply classified in the emergency room by taking pelvis AP film only and the bleeding amount can be measured according to the type. Therefore, this classification can be helpful in performing emergency care.

        2.5-헥산디온에 노출된 근로자에게서 발생한 말초 신경병증

        목적: 영상음향장비 제조업체에서 2,5-헥산디온을 취급한 근로자에게서 발생한 다발성 말초 신경병증 환례를 보고하고, 동일 공정근로자들의 말초 신경기능을 조사하고 자 한다. 방법: 환례에 대해 방사선학적 검사, 소변 및 혈액 검사, 신경전달속도 검사, 작업환경 측정을 하였으며, 2,5-헥산디온의 노출을 중단시킨 후 5개월 후에 다시 신경전달속도 검사를 하였다. 동일공정 근로자들의 영향평가에서는 2,5-헥산디온에 노출된 13명의 남자 근로자를 노출 군으로, 해당 물질에 노출된 적이 없는 5명의 남자 근로자를 비노출군으로 선정하여 검사를 하였다. 결과: 환례의 신경전도 검사결과 양측 척골신경과 정중 신경에 이상소견을 보였으며, 호전여부를 관찰하기 위하여 5개월 후 추적검사를 실시한 결과 증상 및 신경전달속도 검사결과가 호전되었다. 동일공정 근로자들의 영향평가에서 노출군의 정중 신경과 척골신경이 비노출군에 비하여 유의하게 감소되어 있었다. 감각 신경의 진폭의 경우, 노출군의 정중 신경과 비복 신경이 감소되었으며, 운동 신경 전도속도에서는 노출군의 비골신경이 유의하게 감소되어 있었다. 말단 잠복기에서는 노출군의 정중신경, 척골신경, 비골신경, 경골신경이 비노출군에 비해서 유의하게 연장되어 있었다. 결론: 환례의 증상과 신경전달속도 검사의 이상소견은 2,5-헥산디온에 의한 말초신경병증에 부합되었다. 동일공정 근로자 건강영향평가에서 신경전달속도 검사결과 노출 군과 비노출군간의 유의한 차이가 있었는데, 이를 통해 2,5-헥산디온이 근로자들의 신경독성에 영향을 줄 수 있다는 것을 확인할 수가 있었다. Objectives: This study is a case report of 2,5-hexanedione induced occupational peripheral polyneuropathy. We also investigated the peripheral nerve function of all workers who had been exposed to 2,5-hexanedione in the same process. Methods: In June, 2006, a 2,5-hexanedione exposed worker complained of both hand numbness. He received neurologic, radiologic, laboratorial and electrophysiologic evaluation, including measurements of workplace environment. Five months after cessation of exposure to 2,5-hexanedione, a follow-up electrophysiologic examination was done. We evaluated the peripheral nerve function of 2,5-hexanedione exposed workers by comparing 13 male 2,5-hexanedione exposed workers who were in same company with the patient and 5 male workers who had not been exposed to 2,5-hexanedione. Results: Under electrophysiologic examination, there were abnormalities in sensory and motor nerve velocity, terminal latency, and F-latency of both median nerve and ulnar nerve. After 5 months, the patient symptoms and the results of follow-up electrophysiologic examinations were improved. Comparing the 2,5-hexanedione exposed group with the unexposed group, the sensory nerve velocity of the median and ulnar nerves in the exposed group was decreased. The motor nerve velocity of the peroneal nerve, and sensory nerve velocity of the median and sural nerves were decreased. Terminal latency of median, ulnar, peroneal, and tibial nerves in the exposed group were increased compared with the unexposed group(<0.05). Conclusions: 2.5-hexandione can induce peripheral polyneuropathy in male workers.

      • 심전도의 디지털 기록화 모듈과 개인용컴퓨터 저장/분석시스템의 개발

        연구목적: 현재 우리나라의 의료분야에서는 정보기술의 발전으로 처방 전달 시스템과 전자차트의 활용이 상용화 되고 있으나 병원내에서 가장 많이 처방되고 검사되는 12-유도 심전도의 디지털화와 이에 연계된 심전도 결과의 개인용 컴퓨터로의 저장, 인터넷을 이용한 병원으로의 전송이나 타 기관에서의 심전도 결과의 동시 관찰 등에 관한 기술 등은 개발되지 못한 상태이다. 이에 본 연구에서는 심전도를 디지털화하며 저장이 가능한 PC 모듈을 개발하고 심전도 결과를 병원 전산 시스템에 포함시켜 병원내외에서 편리하게 조회, 검색할 수 있는 디지털 인터페이스를 개발하려 하였다. 재료 및 방법 : 낮은 전력에서 잡음특성이 우수한 대역여파기와 증폭기를 설계하였으며 각종기능이 내장된 고신뢰성 프로세서를 채택하였으며 전원 잡음이 고려된 필터와 전원 회로 설계로 하드웨어를 개발하였다. MSC 6.0과 Visual Basic 6.0을 바탕으로 하여 active X 모듈화를 유도하고 조회용 PC에서의 복원과 PACS로의 연동을 통해 순환기내과용 PACS 개발이 가능하도록 소프트웨어를 준비하였다. 결과: 심전도 검사자가 쉽게 장착, 검사가 가능한 소형화된 측정 모듈을 개발하였고 극저전류에서 동작이 가능하였으며 우수한 데이터의 압축 및 복원률을 구현하였다. 결론: 이번 심전도 디지털 모듈의 개발로 기존의 감광지에 의존하던 심전도 결과의 보존과 보관공간에 의한 경제적인 손실을 상당 부분 해결할 수 있으며 OCS에 연계된 결과로 인해 원내에서의 다면적 진료가 가능하다. 이번 연구에서 개발된 기술은 향후 디지털화된 심전도 기록장치와 같은 고부가 의료장치의 국산화와 광파일 의무기록 시스템이나 검사물의 무선 전송에 채용되어 의료 전달 체계나 응급체계의 발전에 기여하리라 사료된다. Purpose : The progressed communication technology commercialized the order communication system ( OCS) and the electric medical records( EMR) in the field of medicine in Korea. However, technologies of digitalization such as recording to personal computer, transfer to internet and simultaneous observation of electrocardiogram ( ECG) recorded at the different center were not realized yet. This study tried to invent personal computer module that can digitalize and record of the ECG and design the digital interface that can approach and search digitalized ECG in various part of hospital simultaneously. Materials and Methods : We had designed high-fidelity digital filter and amplifier which can work in very low voltage, had selected multifunctioned microprocessor. Activation X modulation was conducted by MSC 6.0 and Visual Basic 6.0. Softwares were prepared for the transfer of ECG informations from terminal PC to PACS and cardiologic PACS was designed for future application. Results : We have developed the small, portable, very low voltage managed ECG module and high quality digitalized information compression software in OCS and PACS. Conclusion : This invented ECG digital archive module can save hospital cost by substitution of ECG papers for digital storage or on-line storage, therefore ECG data can be handled in multisector of the hospital and through telemedicine. This technology can be applied to domestic ECG digitalizing device development and effective medical transfer system by adoption cordless medical information delivery.
