        Effect of Highly Pressurized Hydrogen Gas Charging on the Hydrogen Embrittlement of API X70 Steel

        During the use of API X70 steel as a pipeline structural material for the transportation of natural gas,hydrogen embrittlement can occur due to the hydrogen contained in natural gas. The aim of this study is toinvestigate the effects of the hydrogen content under high-pressure hydrogen gas conditions on the hydrogenembrittlement of air-cooled API X70 steel. The air-cooled API X70 steel was manufactured by hot rollingand was then air-cooled to room temperature. Tensile test specimens were held for 0 h, 1000 h, and 2000 hwithin a pressure vessel filled with 100% hydrogen gas at a gas pressure of 10 MPa, with the tensile teststhen performed at room temperature. The microstructure of the API X70 steel consists of coarse polygonalferrite, coarse pearlite, and fine acicular ferrite. The yield and tensile strength increased and elongationdecreased considerably after a holding time of 2000 h compared to those of 0 h and 1000 h within the pressurevessel. The morphology of the fracture surface changed from ductile to brittle upon hydrogen gas charging. Secondary cracks were observed in both of the hydrogen-gas-charged specimens. No external cracks wereformed on the surface of the tensile-tested specimen with a 0 h holding time; however, many external crackswere observed on the specimen surface subjected to hydrogen gas charging.

        Recent advances in the management of immune-mediated thrombotic thrombocytopenic purpura

        Immune-mediated thrombotic thrombocytopenic purpura (iTTP) is a potentially life-threatening thrombotic microangiopathy caused by autoantibody-mediated severe ADAMTS13 deficiency. TTP should be suspected in patients with microangiopathic hemolytic anemia and thrombocytopenia without a definite cause. Early detection of iTTP and prompt treatment with plasma exchange and corticosteroids are essential. Rituximab administration should be considered for refractory or relapsed iTTP, and can be used as a first-line adjuvant or preemptive therapy. Treatment with caplacizumab, a novel anti-von Willebrand factor nanobody, resulted in a faster time to platelet count response, significant reduction in iTTP-related deaths, and reduced incidence of refractory iTTP. TTP survivors showed a higher rate of chronic morbidities, including cardiovascular disease and neurocognitive impairment, which can lead to a poor quality of life and higher mortality rate. Meticulous long-term follow-up of TTP survivors is crucial.

        Ileocolonoscopic findings in patients with anky-losing spondylitis: a single center retrospective study

        Background/Aims: In some Western countries, up to 50% of patients with anky-losing spondylitis (AS) have subclinical gut inflammation. This study was con-ducted to evaluate the prevalence and severity of gut inflammation and to deter-mine clinical factors associated with colonic inflammation in Korean AS patients who performed ileocolonoscopy without evidence of established inflammatory bowel diseases before. Methods: One hundred and eight AS patients who underwent ileocolonoscopy were included in this study. Patients were divided into two groups based on gross ileocolonoscopic findings; patients with inflammatory lesions, and patients with-out inflammatory lesions. Results: Inflammatory lesions in ileocolonoscopic findings were found in 40 patients. The Ankylosing Spondylitis Disease Activity Score C-reactive protein was higher in the group with inflammatory lesions and gut lesions were found often in the terminal ileum. The risk of inflammatory lesions was higher for AS patients whose symptoms required ileocolonoscopy than for AS patients who un-derwent routine ileocolonoscopy screening (odds ratio, 3.96). However, abnormal lesions were detected also in 17.6% of the patients who underwent ileocolonoscopy for routine screening and most of them were erosion and ulcer. Among patients with inflammatory lesions (n = 40), 23 showed subclinical gut inflammation asso-ciated with AS and 17 were diagnosed finally as Crohn`s disease (n = 12), intestinal tuberculosis (n = 4), and ulcerative colitis (n = 1). Conclusions: Our findings suggest that ileocolonoscopy might be recommended regularly in AS patients even without gastrointestinal symptoms, especially in the patients with high AS activity.

      • Benefits and harms of 308-nm excimer laser treatment for alopecia areata: A systematic review and meta-analysis of randomized controlled trials

        Background: Although it has been reported that the excimer laser (EL) treatment is effective for alopecia areata (AA), there has been no comprehensive systematic review of its efficacy and safety. Objectives: This study aimed to systematically review the benefits and harms of EL treatment in AA patients. Methods: A comprehensive database search of MEDLINE, EMBASE, Web of Science and the Cochrane library databases from inception to December 31, 2018, was performed for all prospective studies. Of 22 studies initially identified, the full texts of 11 studies were assessed for eligibility, and 5 were finally included in the analysis. The primary outcome was the proportion of treatment success, defined as cosmetically acceptable hair regrowth or at least 75% regrowth of each designated patch or whole lesions in a patient. Results: We analyzed 5 randomized controlled trials comprising a total of 87 AA patches/patients. The EL treatment showed a significantly higher treatment success rate than the untreated control group (risk difference [RD] 50.3%; 95% confidence interval [CI] 21.1-79.5%; number needed to treat 2). No special harm was found related to the use of EL. In a subgroup analysis excluding cases with alopecia totalis or universalis, the EL treatment had a superior result (RD 59.1%; 95% CI 25.1-93.1%) as well. Conclusion: This systematic review revealed the significant benefit of EL in the treatment of AA. The use of EL should be encouraged for AA patients.

        Inhibitor of DNA binding 2 is a novel therapeutic target for stemness of head and neck squamous cell carcinoma

        <P><B>Background:</B></P><P>Head and neck squamous cell carcinomas (HNSCCs) are highly lethal epithelial tumours containing self-renewal cancer stem cells (CSCs). CSCs in HNSCCs are strongly associated with tumour initiation, invasion, and chemoradiation resistance. However, the important factors regulating stemness in HNSCCs remain unclear. Here, we investigated the molecular roles and clinical significance of inhibitor of DNA binding 2 (Id2) protein to determine if it constitutes a novel therapeutic target for ablating HNSCC cells with stemness.</P><P><B>Methods:</B></P><P>We performed <I>in vitro</I> and <I>in vivo</I> studies of Id2 function and its effects on stemness using HNSCC cells. We also examined whether Id2 expression could be used as a prognostic indicator through immunohistochemical staining of 119 human HNSCC tumours.</P><P><B>Results:</B></P><P>Expression of Id2 was higher in HNSCC cells with stemness compared with differentiated HNSCC cells. Overexpression of Id2 increased proliferation, self-renewal, and expression of the putative stemness marker CD44 in HNSCC cells <I>in vitro</I> and <I>in vivo</I>. In contrast, silencing of <I>Id2</I> using short hairpin RNA attenuated the stemness phenotype of HNSCC cells by reducing self-renewal, CD44 expression, cisplatin chemoresistance, and xenograft tumourigenicity. Most importantly, increased expression of Id2 was closely associated with poorer post-treatment survival rates in HNSCC patients.</P><P><B>Conclusions:</B></P><P>Inhibitor of DNA binding2 represents a novel and promising therapeutic target for treating and improving the clinical outcomes for patients with HNSCC.</P>

        Experience of treatment of patients with granulomatous lobular mastitis

        Purpose: To present the author’s experience with various treatment methods of granulomatous lobular mastitis (GLM) and to determine effective treatment methods of GLM. Methods: Fifty patients who were diagnosed with GLM were classified into five groups based on the initial treatment methods they underwent, which included observation (n = 8), antibiotics (n = 3), steroid (n = 13), drainage (n = 14), and surgical excision (n = 12). The treatment processes in each group were examined and their clinical characteristics, treatment processes, and results were analyzed respectively. Results: Success rates with each initial treatment were observation, 87.5%; antibiotics, 33.3%; steroids, 30.8%; drainage, 28.6%; and surgical excision, 91.7%. In most cases of observation, the lesions were small and the symptoms were mild. A total of 23 patients underwent surgical excision during treatment. Surgical excision showed particularly fast recovery, high success rate (90.3%) and low recurrence rate (8.7%). Conclusion: The clinical course of GLM is complex and the outcome of each treatment type are variable. Surgery may play an important role when a lesion is determined to be mass-forming or appears localized as an abscess pocket during breast examination or imaging study.

      • Pharmacokinetics and therapeutic effects of oltipraz after consecutive or intermittent oral administration in rats with liver cirrhosis induced by dimethylnitrosamine

        <P>Pharmacokinetics and therapeutic effects of oltipraz were evaluated after consecutive (once per day at 30 mg/kg/day for 7 and 14 days) or intermittent (once per week at 100 mg/kg/week for 1–3 weeks) oral administration to rats with liver cirrhosis induced by dimethylnitrosamine. The AUC of oltipraz was significantly greater in cirrhotic rats than controls (890 compared with 270 µg · min/mL) due to impaired liver function in cirrhotic rats. However, the AUC values after consecutive 7 (421 compared with 753 µg · min/mL) and 14 (309 compared with 821 µg · min/mL) days oral administration of oltipraz in cirrhotic rats were significantly smaller than those in respective vehicle-treated cirrhotic rats. Moreover, the AUC values after intermittent 2 and 3 weeks in cirrhotic rats were also significantly smaller than that in 1 week vehicle-treated cirrhotic rats (2370 and 1690 compared with 4760 µg · min/mL). This could be due to induction of CYP isozymes and considerably greater numbers of normal liver cells in cirrhotic rats by oral administration of oltipraz. Improved liver function by oltipraz in cirrhotic rats was proved by liver microscopy; livers are free of significant fibrosis, although evidence of bridging necrosis is still present in many rats. © 2006 Wiley-Liss, Inc. and the American Pharmacists Association J Pharm Sci 95:985–997, 2006</P>

        Transcranial Doppler Monitoring in Subarachnoid Hemorrhage

        Cerebral vasospasm and delayed cerebral ischemia are common complications associated with subarachnoid hemorrhage (SAH) outcomes. Various modalities can be used to evaluate and detect cerebral vasospasm after SAH. Transcranial Doppler (TCD) ultrasonography can be used as an appropriate bedside dynamic monitoring tool to screen for the presence and progression of cerebral vasospasm in patients with SAH. This article briefly reviewed the severity of SAH and pathophysiology of vasospasm. In addition, information regarding the use of TCD for monitoring cerebral vasospasm, several variables measured by TCD to monitor vasospasm detection, and differentiating vasospasm from physiological conditions in the clinical setting following SAH were presented in this article.
