        Effects of vanadate on the isometric contraction, membrane potential and its maximal rate of depolarization of rabbit ventricular papillary muscles were recorded in normal Tyrode solution and 19mM K^+ Tyrode solution. Following results were obtained: 1. Vanadate increased the isometric contractions in normal Tyrode and 19mM K^+-Tyrode solution in a dosedependent manner. 2. Vanadate increased the duration and Vmax of slow action potential. 3. Vanadate induced slight hyperpolarization of unstimulated quit preparations in normal and high K^+-Tyrode solution. 4. Effects of vanadate which were observed in K^+-depolarized preparations were reversed by diltiazem, a calcium channel blocker. It could be conciuded that vanadate has positive intropic effect by increasing Ca-current in the papillary mucles of the rabbit.

        Today human needs for health promotion and maintenance are increasing. The nursing progression has a great responsibility for helping clients to practice good health behavior. self efficacy was found to be potent predictor in initiating and maintaining the health-related behavior. Therefore, the concept is important in nursig intervention for change of health behavior. The purpose of this paper was to analyze and clarify the meaning of the concept, self efficacy. This study used Walder & Avant's process of concept analysis. Critical attributes of self efficacy were : 1) positive perception of his won capability; 2) intrapersonal strength; 3) possibility of being learned; 4) individuality; 5) specificity; 6) generalizability. Antecedents of self efficacy consist of those facts that 1) new situation occurs; 2) individuals have needs to change the new situation; 3) there are some required actions in new situation. Consequences occurring as a result of self efficacy consist of those facts that 1) individual initiates the of failure; 5) cope with the situation; 6) controls the situation; 7) the level of self efficacy is enhanced; 8) the level of self-esteem is enhanced.

      • 호흡기 감염에서 Sparfloxacin 의 임상적 유용성에 대한 Enoxacin과의 비교 검토

        목적:폐렴, 기관지염, 만성기관지염에서 fluoroquinolone제제로서 항균력이 강하고, 혈중 반감기가 길고, 객담과 조직내 농도가 높은 sparfloxacin의 안전성과 효과를 enoxacin과 비교하기 위해 본 연구를 시행 하였다. 방법:1994년 10월부터 1995년 4월까지 충남대 부속병원과 대전 성모병원에 내원한 폐렴, 급성기관지염, 만성기관지염이 급성으로 악화된 환자 63명을 대조군(32명)(enoxacin 100 mg 1일 3회 경구투여), 실험군(31명)(sparfloxacin 200mg 1일 1회 경구투여)으로 나누어 임상증상, 세균학적 객담 검사, 혈액검사, 실험실내 항균력검사로 그 치료율 및 안전성을 비교하였다. 결과: 1) 임상종합 평가, 세균학적 균소실, 안전성, 유용성에서 두군간에 의미 있는 차이는 없었다. 2) 시험관내 항균력검사상 MIC가 실험군이 대조군보다 현저하게 낮았다.(p<0.05). 3) 경미한 두통, 식욕감퇴, 상복부 불쾌감, 설사등이 대조군 9예, 실험군 7예에서 발생하였고, 검사실 소견상 SGOT 혹은 SGPT 의 일시적 경미한 상승이 대조군 2명, 실험군 3명에서, 혈중 creatinine 상승이 실험군 2명에서 있었으나 경미하였다. 결론:이상의 결과로 sparfloxacin은 폐렴, 급성기관지염, 만성기관지염의 악화에서 하루 한번 투여로 enoxacin 하루 3번 투여와 유사하게 효과적이고 안전하며, 시험관내 항균력이 enoxacin보다 월등히 우수하여 임상에서도 유용하게 사용될수 있을것으로 생각된다. Background: Sparfloxacin, a new oral quinolone, has a broad spectrum, potent antimicrobial activity, prolonged half life, high concentration in sputum. The clinical efficacy and safety of sparfloxacin have been reported by some previous clinical studies performed in Japan. The objectives of this study were to evaluate the clinical and antimicrobial efficacy of this drug in respiratory tract infections compared to those of enoxacin. Methods:We performed an open controlled, randomized study in 63 patients with acute bronchitis, pneumonia, and exacerbation of chronic bronchitis at Chung Nam University hospital and Dae Jeon St. Mary's hospital from October, 1994 to April, 1995. In the control group(n=32), 100 mg enoxacin was administered orally 3 times a day and in the test group(n=31), 200 mg sparfloxacin once a day. Results: The results were as follows; 1) There were no significant differences between the two groups in clinical efficacy, bacteriological finding, and usefulness. 2) Means and SD of MICs were 0.44±0.89 ㎍/ml in the control group, 0.13±0.24 ㎍/ml in the test group. There was a significant difference in MICs between the two groups(p<0.05). 3) Headache, epigastric discomfort, and loss of appetite were observed in 7 cases of the control group and in 9 cases of the test group, and transient elevations of hepatic enzyme in 2 cases of the control group, and in 3 cases of the test group. There was mild elevation of serum creatinine in 2 cases of the test group. Those laboratory abnormalities by therapy might be associated with their underlying diseases. There were no significant adverse effects caused by the drugs Conclusion:In conclusion, sparfloxacin of 200 mg once a day was as effective and well tolerated as enoxacin of 100 mg three times per day in the treatment of respiratory tract infections. Sparfloxacin is more potent antibiotics because MICs of sparfloxacin were very lower than those of enoxacin.

      • 유두상 갑상선 암에서 p21, p53 및 증식세포 핵항원(PCNA)의 예후인자로서의 유용성

        There were many studies t o show that p21. p53 and PCNA overexpression were known to be predictive factors for human cancer. The purpose of this study was to evaluate whether these molecular oncogenes were useful as prognostic factors in patient with papillary thyroid carcinoma. Using the monoclonal antibodies MAb Y13-259, PAb 1801 and PClO against each oncogene protein studied, the avidin-biotin peroxidase complex method provided immunohisto-chemical staining of bound oncogene protein for p21-ras, p53 and PCNA, respectively. Analysis were made on paraffin-embedded pathology tissues of 58 patients with papillary thyroid cancer who underwent thyroidectomy in Korea University Anam Hospital from January 1984 to December 1992. Forty-six patients were women (79.3%) and 12 were men (21.7%), with mean age 42.0±19.2 years. Patients were divided into groups based on postoperative tumor recurrence. Eleven patients had recurrence of disease ; 47 remained free of loco-regional or distant metastasis at mean follow-up of 74.3 months(range 36 142 months). The positive rates of p21. p53 and PCNA were 22.2% (13 of 56), 45.6% (26 of 57) and 70.9% (39 of 55) respectively. No significant differences were observed in the age and sex of patients with p21, p53 and PCX4 positive and negative tumors. Using stastical analysis, there was no significant difference in p21, p53 and PCNA stains between recurrent and non-recurrent groups (p>0.05). while AMES score (p=0.042). the tumor size (p=0.005). capsular invasion(p=0.003) and lymph node metastasis(p=0.002) were significantly correlated with tumor recurrence. Conclusion : This study shows that expression of p21, p53, PCNA are not useful as prognostic factors in papillary thyroid cancer.

      • Prospective Study for Korean Red Ginseng Extract as an Immune Modulator following a Curative Surgery in Patients with Advanced Colon Cancer

        Background - In this paper, we present evidence that the Korean red ginseng extract shows the immunomodulatory activities during postoperative chemotherapy after curative surgery in patients with advanced colon cancer. We measured the circulating interleukin-2(IL-2), interleukin-8(IL-8)and interleukin-10(IL-10) as a immune modulator to evaluate the effect of Korean red ginseng. Material & Method - From July, 2005 to February, 2006,total 47 of the advances colon cancer patient who underwent the curative surgery at Korea University Hospital were enrolled. 24 of colorectal carcinoma patients were assigned to non-red ginseng (non-RG) group that would not be treated with red ginseng extract. And 23 of colorectal carcinoma patients were assigned to red ginseng(RG) group. All of them was treated with 5-fluouracil +leukovorin chemotherapy after curative surgery for 6 months. We treated RG group patient with 3 g/day of red ginseng extract(Korea Ginseng Corporation, Seoul, Korea) for 3 months. IL-2, IL-8 and IL-10 were evaluated at preoperative day, postoperative months I and 3 from peripheral circulating blood draws. The value was analysed with ELISA test. Result - The mean preoperative value of IL-2 was similar in the non-RG group and the RG group(5.72 pg/㎖ versus 6.87 pg/㎖, p>0.05). The mean value of IL-2 was compared with IL-2 from healthy control group, there was no significant difference(14.89 pg/㎖ versus 14.22 pg/㎖, p>0.05). The mean preoperative value of IL-8 was higher in the non-RG group comparing with the RG group (30.92 pg/㎖ versus 36.25 pg/㎖, p<0.05). At postoperative 3 month, the mean values of IL-8 from non-RG and RG group down to 24.56 pg/㎖ and 21.46 pg/㎖ respectively. The IL-8 of RG group at 3 month showed no difference with that of HC group(21.46 pg/㎖ versus 16.31 pg/㎖, p>0.05). The preoperative mean value of IL-10 of non-RG, RG and HC group was 11.56 pg/㎖, 10.8 pg/㎖, and 3.68 pg/㎖ respectively. At postoperative 3 month, the mean values of IL-10 from non-RG and RG group down to 8.45 pg/㎖ and 5.04 pg/㎖ respectively. In spite of decreasing IL-10 levels of both cancer patients group with time, there was still significant difference with that of HC group (non-RG versus HC group, p=0.00, RG versus HC group, p=0.04). Conlusion - The results of this study suggest that the red ginseng extract may have some immunomodulatory properties associated with IL-2, IL-8 and IL-10 activity in patients with advanced colorectal cancer during postoperative chemotherapy. We think to need the further studies and a larger sample size to fully evaluate the antitumor effect of ginseng and need to establish this mechanism of action as well as identify the active components associated with antitumor activity and immunomodulation in patients with advanced colorectal cancer.

        진행성 위암 환자에서 수술 후 홍삼엑기스에 의한 면역 조절자 역할에 관한 전향적 연구

        The aim of this study was to evaluate prospectively the impact of the red ginseng extract on circulating interleukin (lL) 2 and l0 in advanced gastric cancer during chemotherapy after operative treatment. Analysis of circulating IL2 and 10 was performed in 50 patients with advanced gastric adenocarcinoma who underwent a curative surgery or with an unresectable gastric adenocarcinoma by using ELISA and monoclonal antibodies at preoperative day I, postoperative months 1 and 3. Twenty-five patients as the control group, twenty-six patients as the non-ginseng (NG) group, and twenty-four patients as the ginseng (G) group were eligible in this study. All plasma IL-2 of the NG and G groups was significantly lower than that of the control group on preoperative I day. These values of the G group were more increase than these of the NG group during the postoperative chemotherapy. The mean value of serum IL-1of the control group (0.608pg/ml) was signiticantly lower than that of the advanced gastric cancer patients including the NG (12.015 pg/ml) and G group (9.409 pg/ml) (p<0.001). These values of the G group were reduced progressively during the postoperative chemotherapy. The mean value of the G group were only close to that of the control group on postoperative months 3 (p=0.003). The number of patients who were enrolled in this study was relatively small to fully evaluate the immunologic effects of the red ginseng extract on circulating IL-2 and 10. Despite this limitation, these results suggest that the postoperative intake of the red ginseng extract have potential to improve earlier anti-cancer immunity with recovering IL-2 and reducing IL-10 from the depressed IL-2 and elevated IL-10 by gastric cancer during the postoperative chemotherapy. This study will be based on the future study to evaluate the anti-immunity of the red ginseng extract.
