        Otitis media with effusion (OME) is one of the most common disease in children but its pathogenesis is not clear. In particular the role of allergy has revealed conflicting support with widely varing opinions as to its importance. Within recent years, striking biochemical and immunochemical analytic techniques have opened a new approach the study of otitis media with effusion. So we focused on distributions of IgE in adenoid tissue and serum IgE levels. Authors compared the serum IgE and the distribution of IgE in adenoid tissue between the 20 patients underwent tonsillectomy, adenoidectomy and ventilating tube insertion and control group of 11 patients underwent tonsillectomy and adenoidectomy. The results were as follows; 1. In the distribution of IgE in adenoid tissue, there was a significant increase in the group of patients with OME compared to the control group (P<0.01). 2. In the analysis of serum IgE levels, the levels of IgE in the OME patients an control group were higher than normal level, although the level of IgE in the OME patients were less than that of control group, there was no significance P>0.05).

        유전자 영상용 HSV1-TK 기질의 합성

        목적: 유전자 치료에서, 종양세포에 herpes simplex virus thymidine kinase 유전자를 발현시키고 이에 민감한 prodrug을 전신투여하여 발현된 종양세포에 특이적으로 집적되는 방법은 매우 효육적인 바업이다. 대표적인 prodrug인, IVDU, IVFRU는 herpes simplex virus type 1의 비침습적 영상화 방법에 응용되는 방사옥소 표지가능 화합물이다. 재료 및 방법: Trans-1-trimethylsilyl-2-tri-n-butylstannylethylene과 uridine 변형체상의 iodo-uracil을 Pd촉매하에서 반응시켜 표지전구체인 5-trimethylvinyl-deoxy-uridine과 5-trimethylsilylvinyl-deoxy-fluororibofuranosyl uracil을 합성하였다. 이것을 column chromatography의 방법으로 분리 정제하였다. 합성한 각 전구체를 ICl 담체를 이용하여 높은 수율로 표지하였고, HPLC를 사용하여 분리하였다. Radiohalogen exchange 방법은 비방사능을 낮게 할수록 표지에 효율적으로 알려져 있다. 이 방법으로 ICl 방법을 사용하여 담체를 포함하는 높은 방사능의 화합물을 얻을 수 있다. 결과: IVDU와 IVFRU 표지를 위한 전구체 합성수율은 각각 43, 72% 였다. ICl로 담체를 이용한 표지방법을 사용하여 IVDU와 IVFRU 표지수율은 98% 이상이었다. 결론: HSV1-tk를 이용한 유전자 영상용 기질의 전구체를 성공적으로 합성하였고, 95% 이상의 높은 표지수율의 유전자 영상용 방사성 추적자를 성공적으로 제조하였다. Purpose: In gene therapy, tumor cells expressing the herpes simplex virus thymidine kinase are sensitive to prodrugs. Potential prodrugs IVDU and IVFRU were synthesized and radiolabeled with radiodine for noninvasive imaging of herpes simplex virus type 1 gene expression. Material and Methods: 5-(2-trimethysilyl)vinyl-2`-deoxyuridine and 5-t(2-trimethylsilyl-2`-fluoro-2`-deoxyuridine, precursors of 5-(2-iodo)viny 1-2`-deoxy uridine(IVDU) and 5-(2-iodo)-2`-vinyl-2`-deoxy-2`-fluororibofuranosyl uracil(IVFRU), were synthesized from reaction of trans-1-trimethylsillyl-2-tri-n-butylstannylethylene with 5-iodo-2`-deoxyuridine and 5-iodo-2`-fluoro-2`-deoxyuridine, respectively, on the condition of Pd catalyst. These precursors were separated from reaction mixture by silica gel column chromatography method. Each precursor was radioiodinated with radioiodine by mixing with ICl oxidizing agent. These radioiodinated compounds were purified with HPKC. Radiohalogen exchange has been shown to be effective for the synthesis of products with lower specific activity. Similarly, carrier-added and high specific activity products have been isolated in respectable radiochemical yields using ICl method. Results: Synthetic yield of precursors, IVDU and IVFRU were 43% and 18%, respectively. Radiochemical purity of both compunds was over 98%. Conclusion: We synthesized precursors of IVDU and IVFRU for monitoring of HSV1-tk gene expression. Radiotracers were radioiodinated with high radiolabeling yield by ICl method.         복어 중독의 임상적 고찰

        Background : Puffer fish can be the source of lethal food poisoning in humans. Tetrodotoxin(TTX) poisonings are not infrequently seen in Korea, but there are few clinical reports. So we reviewed the patients of TTX poisoning and analyzed the clinical characteristics of patients. Methods : A retrospective study was performed of 40 patients who visited Chung Ang Gil Hospital from Jan. 1, 1995 to May. 31, 1998 with a diagnosis of TTX poisoning by a review of patients medical records and telephone inquiries. The diagnosis of TTX poisoning was made by causal links between consumption of puffer fish and the development of typical symptoms of tetrodotoxication. The clinical severity of the patients in this study was classified according to the classification of Fukuda. Results : Mean age of the patients was 40 years. The highest incidence was in the 4th decade in 21 patients(52.5%). The ratio of male to female was 3.44:1. Seasonal distribution excluding cases in 1998 was 12 patients in spring, 6 in summer, 5 in autumn and 12 in winter. The mean interval between consumption and symptom onset was 137 minutes. Common initial symptoms were circumoral numbness(32 patients) and paresthesia of extremities(24 patients). Various symptoms were developed after ingestion of puffer fish such as neuromuscular(39 patients), cardiovascular/pulmonary(23 patients) and gastrointestinal(16 patients) system. Mean recovery time from the onset of symptom was 22.00 hours. All were treated with symptomatic and supportive measures and recovered completely without sequelae. In 2 cases ventilatory supports were applied for 18.5 hours and 31.5 hours respectively.

        A nonvolatile memory technology utilizing reversible changes between fcc and hcp crystalline phases is proposed. In this new type of phase-change memory, data are stored in different forms of crystalline phases of (Ge<SUB>1</SUB>Sb<SUB>2</SUB>Te<SUB>4</SUB>)<SUB>0.8</SUB>(Sn<SUB>1</SUB>Bi<SUB>2</SUB>Te<SUB>4</SUB>)<SUB>0.2</SUB> chalcogenide alloy. RESET operation produces the less conductive metastable fcc phase via melt-quenching from the more conductive stable hcp phase and SET operation involves a phase change from fcc directly to hcp. Both RESET and SET operations can be completed as fast as 70 ns with large changes in cell resistance.

        Meaningful standard of reference for appendiceal perforation

        Purpose: This retrospective study was aimed to determine if appendiceal perforation identified pathologically but not surgically is clinically meaningful. Methods: The study consists of 2 parts. First, we reviewed 74 studies addressing appendiceal perforation published in 2012 and 2013. Second, in a cross-sectional study, we classified 1,438 adolescents and adults (mean age, 29.3 ± 8.4 years; 785 men) with confirmed appendicitis as “nonperforation” (n = 1,083, group 1), “pathologically-identified perforation” (n = 55, group 2), “surgically-identified perforation” (n = 202, group 3), or “pathologically- and surgically-identified perforation” (n = 98, group 4). The 4 groups were compared for the frequency of laparoscopic appendectomy and the length of hospital stay using multivariable logistic regression analyses. Results: The reference standard for appendiceal perforation was frequently missing or inconsistent in the previous studies. Laparoscopic appendectomies were less frequent in groups 3 (52.5%, P = 0.001) and 4 (65%, P = 0.040) than in group 1 (70.7%), while group 2 (73%, P = 0.125) did not significantly differ from group 1. Median hospital stays were 2.9, 3.0, 5.1, and 6.0 days for groups 1–4, respectively. Prolonged hospital stay (≥3.7 days) was more frequent in groups 3 (77.7%, P < 0.001) and 4 (89%, P < 0.001) than in group 1 (23.4%), while group 2 (35%, P = 0.070) did not significantly differ from group 1. Conclusion: We recommend using surgical rather than pathologic findings as the reference standard for the presence of appendiceal perforation in future investigations.

      • Impact of Nucleos(t)ide Analogue Treatment for Chronic Hepatitis B on Various Malignancies Development

        Aims: It is unclear whether nucleos(t)ide analogues (NAs) against hepatitis B influence the risk of various malignancies except for hepatocellular carcinoma. We evaluated the association of NAs and the risk of various malignancies in patients with chronic hepatitis B. Methods: A total of 9,867 patients with chronic hepatitis B who were treated with NAs or not between December 1998 and June 2013 were followed up ≥ 12 months for the occurrence of any kind of malignancy at Ajou University Hospital, Suwon, South Korea. Patients who developed HCC or other malignancy within 12 months were excluded. Patients who received NAs ≥180 days were defined as NA group. Propensity score matching performed using adjustment for age, gender, diabetes mellitus and liver cirrhosis produced control (n = 2,220) and NA group (n = 2,220). National Health Insurance Service (NHIS) sample cohort dataset was used for external validation. Results: During the median follow-up period of 1,477 days, 1,943 (16.1%) patients developed any kind of malignancy. Multivariate analysis showed that only age over 50 was an independent risk factor for development of malignancies other than HCC (HR 3.17, 95% CI 1.71-5.88, P <0.001). Considering the specific cancer such as thyroid, breast, lung, stomach, colorectal, pancreas cancer and hematologic malignancy, there was no difference of incidence of each malignancy between two groups in both hospital based cohort and NHIS sample cohort. Conclusions: NA treatment was not associated with significantly increased incidence of extrahepatic malignancies in patients with chronic hepatitis B.

        톨루엔 디이소시아네이트 폭로 근로자들의 생물학적 모니터링

        Toluene diisocyanate(TDI) is widely used in the production of flexible polyurethane foams, as well as in the formulation of polyurethane paints and coatings. The commercial material is generally a mixture of 2,4- and 2, 6-TDI, the predominant mix being 80% 2,4 and 20% 2,6-TDI. The 2,4-isomer is considerably more reactive than the 2,6-TDI at ambient temperatures due to steric factors involving the positions of the isocyanate groups relative to the ring methyl group. Because of this difference in the reactivities of the isomers, it seemed probable that there might be an increase in the amount of 2,6-TDI offgased relative to the 2,4-isomer. Therefore a relative enrichment of the 2,6-TDI has been found in industrial atmospheres. Toluene diamines, which are metabolites of TDI, in urine have a linear relation with exposure to TDI, so that urianry TDA could be used as a biological index of the exposure to TDI. This study was conducted to investigate the distribution of TDI isomer in industrial atmospheres and to propose proper biological monitoring methods by identifying the relationships between the environmental TDI exposure and concentration of TDA in urine. Concentrations of 2, 4-TDI and 2, 6-TDI in air were 4.38㎍/㎥ and 25.43㎍/㎥, respectively. The Threshold Limited Value of 40㎍/㎥ was exceeded for the 2, 6-TDI in about 46.8%(22 samples) of the samples, while the 2, 4-TDI was not at all exceeded. The ratio between 2, 4-TDI and 2, 6-TDI varied in air samples in the range of 2.4%:97.6%-51.0%:49.0%. There was an enrichment of 2, 6-TDI in air relative to the 2, 4-TDI. Concentrations of 2, 4-TDA and 2, 6-TDA in urine were 1.31㎍/g creatinine and 4.16㎍/g creatinine, respectively. The ratio between 2, 4-TDA and 2, 6-TDA varied in urine samples in the range of 1.4%:98.6%-99.9%:0.1%. There was an enrichment of 2, 6-TDA in urine relative to the 2, 4-TDA. No relation between the concerations of TDA isomer in urine and concerations of TDI isomer in air was found. Above results of this study, workers were more exposed to the 2, 6-TDI relative to the 2, 4-TDI in industrial atmospheres. Therefore, the establishment of TLV for 2, 6-TDI should be considered. Also, the further studies on biological monitorigs of workers exposed to TDI should be continued.

        Structure-Activity Relationships of Polyphenols Inhibiting Lipopolysaccharide-induced NF-${\kappa}B$ activation

        Most plant-derived phenolic compounds are synthesized as secondary metabolites in plants due to their defense strategies. Previous studies have reported the inhibitory effect of chalchones, one of polyphenols, on nuclear factor kappa-light-chain-enhancer of activated B cell (NF-${\kappa}B$)-dependent transcriptional activities in various cancer cells. However, detailed relationship between the structure and biological activity on the inhibition of NF-${\kappa}B$ in the microglia is less defined. In this study, twenty-two polyphenols including sixteen hydroxy/methoxy chalcones, four 2H-chromenes, and two N-phenylacetamides were synthesized. Lipopolysaccharide (LPS) is a constituent of the outer membrane of the Gram-negative bacteria, which is widely employed as a proinflammatory stimulus in many experimental settings. The effects of polyphenol compounds synthesized on the inhibition of LPS-induced NF-${\kappa}B$ activity in BV2 microglial cells were investigated, and the structural conditions to show better inhibitory effects were evaluated. Western blot analysis was performed on compounds showing good inhibitory effects.
