        ‘융복합교육’ 기반 수업 설계 사례 연구 : 예비 교사를 중심으로

        본 사례 연구는 융복합교육의 현장 실천과 적용가능성 및 교사의 융복합교육 전문성 제고에 기여하고자, 한 중학교 현장에서 교생실습의 일환으로 자생적으로 행해진 예비교사들의 융복합교육 기반 수업 설계 프로젝트를 분석한 것이다. 한 교사의 주도 하에 21명의 예비교사들이 다섯 개 모둠으로 나누어 협업함으로써 융복합수업을 설계하였는데, 본 논문에서는 그 교사의 서면 인터뷰와 예비교사들의 수업지도안, 발표, 전문가 컨설팅, 성찰지를 자료로 취합하여 정성적으로 분석하였다. 특히 융복합수업 지도안에 대해서는 이전에 체계화되어 있는 융복합교육 프로그램 구성 틀을 바탕으로 분석하였다. 연구 결과, 수업지도안은 각각 창의적으로 다양하게 융복합교육적 목표와 방식 및 맥락을 충분히 잘 반영하였으며 주제, 내용, 활동요소 등도 융복합에 부합하게 유기적으로 연결된 것이 발견되었다. 단, 자율적 실천 역량을 더욱 촉진하고 개인에서 세계로까지 맥락을 확대하며 이를 다채로운 활동들을 혼합하여 구성할 것이 요구되었다. 한편, 성찰지에 의하면, 이번 사례에 대해 예비교사들은 1) 융복합교육을 ‘과목 간 연계성’, ‘수업의 맥락화’, ‘학습자 중심’으로 차별화하였으며, 2) 융복합수업 설계를 하면서 얻은 ‘새로운 경험’을 통해 자기 과목에 대한 새로운 측면, 학생의 관점, 동료의 전문성, 그리고 협업을 가치 있는 것으로 성찰하였다. 3) 또한, 융복합교육의 절대적 필요성을 인식하고 교사 전문성 신장과 연구자 자세 함양을 되새기며 향후 융복합교육을 실천하겠다고 진술하였다. 이에 따라 본 연구는 현장의 교육 개선을 위해 교사의 재량권 보장과 행정적 지원이 필요함과 융복합교육과 관련하여 (예비)교사교육의 체계적이고 실증적인 개선이 필요함을 시사한다. To explore the implementability of, and the teacher expertise in “Yungbokhap education”, which embraces “consilient”, “convergent”, “integrative”, and “interdisciplinary” approaches to curricular integration, this case study analyzed a self-motivated project for pre-service teachers that was initiated as part of a practicum by a middle-school teacher who supervised the practicum. In the project, the pre-service teachers formed five groups and collaborated to create five integrative instructional unit plans based on Yungbokhap education. Their products positively impressed the school principal, mentor teachers, and other experts. They wrapped up their projects by answering reflective questions. This study analyzed their instructional unit designs and reflections qualitatively, in light of the instruction design framework of Yungbokhap education. The results show that the unit plans sufficiently reflect the goals, approaches and contexts of Yungbokhap education with appropriate creativity and variety, and tightly connect lesson themes, contents and activities, except for the fact that they could have further promoted the competency for autonomous action and expanded the context from an individual context to a local/global one employing more varied activities. According to their reflective responses, 1) they seem to characterize Yungbokhap education with features like “inter-subject connectivity”, “contextualization” and “learner-centeredness”, 2) their novel experiences in the lesson planning have led them to discover fresh values in their own subjects, students’ perspectives, peers’ expertises, and fruitful collaboration, and 3) their realization of the necessity of Yungbokhap education has positively challenged them to be more firmly oriented towards professional development and teacher-researcher attitude. These results imply that for autonomous and creative improvement of education, in-service teachers’ initiatives should be carefully respected and administratively supported, and that teacher preparation courses should provide pre-service teachers with more empirical chances to deal with curriculum integration.

      • Prognostic value of volumetric metabolic parameters measured by [ <sup>18</sup> F]Fluorodeoxyglucose-positron emission tomography/computed tomography in patients with small cell lung cancer

        <P><B>Background</B></P><P>We evaluated the prognostic value of volume-based metabolic positron emission tomography (PET) parameters in patients with small cell lung cancer (SCLC) compared with other factors.</P><P><B>Methods</B></P><P>The subjects were 202 patients with pathologically proven SCLC who underwent pretreatment <SUP>18</SUP>F-fluorodeoxyglucose (FDG) PET/computed tomography (CT). Volumetric metabolic parameters of intrathoracic malignant hypermetabolic lesions, including maximum and average standardized uptake value, sum of metabolic tumor volume (MTV), and sum of total lesion glycolysis (TLG) were measured.</P><P><B>Results</B></P><P>164 patients had died during follow-up (median 17.4 months) and median overall survival was 14 months. On univariate survival analysis, age, stage, treatment modality, sum of MTV (cutoff = 100 cm<SUP>3</SUP>), and sum of TLG (cutoff = 555) were significant predictors of survival. There was a very high correlation between the sum of MTV and the sum of TLG (r = 0.963, <I>P</I> < 0.001). On multivariate survival analysis, age (HR = 1.04, <I>P</I> < 0.001), stage (HR = 2.442, <I>P</I> < 0.001), and sum of MTV (HR = 1.662, <I>P</I> = 0.002) were independent prognostic factors. On subgroup analysis based on limited disease (LD) and extensive disease (ED), sum of MTV and sum of TLG were significant prognostic factors only in LD.</P><P><B>Conclusion</B></P><P>Both sum of MTV and sum of TLG of intrathoracic malignant hypermetabolic lesions are important independent prognostic factors for survival in patients with SCLC, in addition to age and clinical stage. However, it may be more useful in limited disease rather than in extensive disease.</P>

        Risk of Hepatitis B Virus (HBV) Reactivation in Patients with Immune-Mediated Inflammatory Diseases Receiving Biologics: Focus on the Timing of Biologics after Anti-HBV Treatment

        Background/Aims: Anti-hepatitis B virus (HBV) therapy is required for patients with HBV infection receiving biologics because of the high risk of HBV reactivation. However, it is unclear when to start biologics after anti-HBV treatment. We investigated the risk of HBV reactivation according to the timing of biologics initiation after anti-HBV treatment in immune-mediated inflammatory disease (IMID) patients with HBV infection. Methods: We retrospectively evaluated the incidence of HBV reactivation in IMID patients who received biologics between July 2005 and April 2020. The patients were divided into two groups (within 1-week and after 1-week) according to the timing of biologics initiation after anti-HBV treatment. The cumulative probabilities and factors associated with HBV reactivation were evaluated. Results: A total of 60 hepatitis B surface antigen-positive patients with IMID received biologics (within 1-week group, n=23 [38%]; after 1-week group, n=37 [62%]). During a median follow-up of 34 months (interquartile range, 20 to 74 months), three patients (5%) developed HBV reactivation. In univariate analysis, the timing of biologics after anti-HBV treatment was not significantly associated with the risk of HBV reactivation (hazard ratio, 0.657; 95% confidence interval, 0.059 to 7.327; p=0.733). The cumulative probabilities of HBV reactivation did not significantly differ according to the timing of biologics (p=0.731). Conclusions: The risk of HBV reactivation was not significantly associated with the timing of biologics administration after anti-HBV treatment. Thus, biologics may be initiated early in patients with IMID undergoing treatment for HBV.

        Predictive Factors for Rheumatoid Arthritis Flare After Switching From Intravenous to Subcutaneous Formulation of Tocilizumab in Real-World Practice

        Background: To evaluate the incidence and related factors of rheumatoid arthritis (RA) flares after switching from intravenous tocilizumab (TCZ-IV) to subcutaneous tocilizumab (TCZSC) injection in stable RA patients. Methods: We retrospectively evaluated the medical records of stable RA patients who used TCZ-IV for more than 6 months and switched to TCZ-SC between January 2013 and April 2020. RA flare was defined as an increase of more than 1.2 in the RA disease activity as assessed by the disease activity score in 28 joints. The factors associated with RA flare were evaluated by logistic regression analysis. Results: Among 106 patients treated with TCZ-IV for > 6 months, 37 patients were switched to TCZ-SC after the acquisition of remission or low disease activity. RA flares occurred in 11 (29.7%) of patients who switched TCZ-SC. Results from the multivariable logistic analysis revealed that the dose of TCZ-IV per weight at switching (odds ratio [OR], 20.70; 95% confidence interval [CI], 2.22–192.84; P = 0.008) and methotrexate (MTX) non-use (OR, 8.53; 95% CI, 1.21–60.40; P = 0.032) were associated with the risk of RA flares after switching to TCZ-SC. Interestingly, most patients who switched back to TCZ-IV had their RA activity controlled again. Conclusion: MTX non-use and high dose of TCZ-IV per weight were associated with a risk of RA flare after switching to TCZ-SC. RA patients with these factors need to be carefully observed for flare after switching from TCZ-IV to TCZ-SC.

        Incidence of rheumatic diseases during the COVID-19 pandemic in South Korea

        Background/Aims: The recent coronavirus disease 2019 (COVID-19) pandemic has been associated with changes in the epidemiology of not only infectious diseases but also several non-infectious conditions. This study investigated changes in the recorded incidence of various rheumatic diseases during the COVID-19 pandemic. Methods: The number of patients for each disease from January 2016 to December 2020 was obtained from the Korean Health Insurance Review and Assessment Service database. We compared the incidence of nine rheumatic diseases (seropositive rheumatoid arthritis, systemic lupus erythematosus [SLE], idiopathic inflammatory myositis [IIM], ankylosing spondylitis [AS], systemic sclerosis, Sjogren’s syndrome, Behcet’s disease [BD], polymyalgia rheumatica, and gout) and hypertensive diseases to control for changes in healthcare utilisation before and after the COVID-19 outbreak. The disease incidence before and after the COVID-19 outbreak was compared using the autoregressive integrated moving average (ARIMA) and quasi- Poisson analyses. Results: Compared with the predicted incidence in 2020 using the ARIMA model, the monthly incidence of SLE, BD, AS, and gout temporarily significantly decreased, whereas other rheumatic diseases and hypertensive diseases were within the 95% confidence interval (CI) of the predicted values in the first half of 2020. In age- and sex-adjusted quasi-Poisson regression analysis, the annual incidences of IIM (rate ratio [RR], 0.473; 95% CI, 0.307 to 0.697), SLE (RR, 0.845; 95% CI, 0.798 to 0.895), and BD (RR, 0.850; 95% CI, 0.796 to 0.906) were significantly decreased compared with those in the previous 4 years. Conclusions: The recorded annual incidence of some rheumatic diseases, including IIM, SLE, and BD, decreased during the COVID-19 pandemic.

        Ileocolonoscopic findings in patients with anky-losing spondylitis: a single center retrospective study

        Background/Aims: In some Western countries, up to 50% of patients with anky-losing spondylitis (AS) have subclinical gut inflammation. This study was con-ducted to evaluate the prevalence and severity of gut inflammation and to deter-mine clinical factors associated with colonic inflammation in Korean AS patients who performed ileocolonoscopy without evidence of established inflammatory bowel diseases before. Methods: One hundred and eight AS patients who underwent ileocolonoscopy were included in this study. Patients were divided into two groups based on gross ileocolonoscopic findings; patients with inflammatory lesions, and patients with-out inflammatory lesions. Results: Inflammatory lesions in ileocolonoscopic findings were found in 40 patients. The Ankylosing Spondylitis Disease Activity Score C-reactive protein was higher in the group with inflammatory lesions and gut lesions were found often in the terminal ileum. The risk of inflammatory lesions was higher for AS patients whose symptoms required ileocolonoscopy than for AS patients who un-derwent routine ileocolonoscopy screening (odds ratio, 3.96). However, abnormal lesions were detected also in 17.6% of the patients who underwent ileocolonoscopy for routine screening and most of them were erosion and ulcer. Among patients with inflammatory lesions (n = 40), 23 showed subclinical gut inflammation asso-ciated with AS and 17 were diagnosed finally as Crohn`s disease (n = 12), intestinal tuberculosis (n = 4), and ulcerative colitis (n = 1). Conclusions: Our findings suggest that ileocolonoscopy might be recommended regularly in AS patients even without gastrointestinal symptoms, especially in the patients with high AS activity.

        Biochemical Characterization of Oligomerization of Escherichia coli GTP Cyclohydrolase 1

        GTP cyclohydrolase I (E.C. 3.5.4.16) is a homodecameric protein that catalyzes the conversion of GTP to 7,8-dihydroneopterin triphosphate (H2NTP), the initial step in the biosynthesis of pteridines. It was proposed that the enzyme complex could be composed of a dimer of two pentamers, or a pentamer of tightly associated dimers; then the active site of the enzyme was located at the interface of three monomers (Nar et al. 1995a, b). Using mutant enzymes that were made by site-directed mutagenesis, we showed that a decamer of GTP cyclohydrolase I should be composed of a pentamer of five dimers, and that the active site is located between dimers, as analyzed by a series of size exclusion chromatography and the reconstitution experiment. We also show that the residues Lys 136, Arg139, and Glu152 are of particular importance for the oligomerization of the enzyme complex from five dimers to a decamer.

        Clinical features and prognoses of acute transverse myelitis in patients with systemic lupus erythematosus

        Background/Aims: Acute transverse myelitis (ATM) is a severe complication of systemic lupus erythematosus (SLE). This study evaluated the clinical factors related to outcome in patients with SLE-associated ATM. Methods: The medical records of patients diagnosed with SLE-associated ATM between January 1995 and January 2015 were reviewed. The patients were divided into two groups based on improvement of neurological deficits after treatment: favorable response group and unfavorable response group. During follow-up, the recurrence of ATM was also analyzed. Results: ATM was identified in 16 patients with SLE. All of the patients were treated with high doses of methylprednisolone (≥ 1 mg/kg daily). Although 12 patients (75%) recovered (favorable response group), four (25%) had persistent neurologic deficits (unfavorable response group) after the treatment. Compared to the favorable response group, significantly higher Systemic Lupus Erythematosus Disease Activity Index-2000, lower complement levels and initial severe neurologic deficits were found in the unfavorable response group. Among the 12 favorable response patients, five (41.7%) experienced recurrence of ATM during the follow-up. Patients (n = 5) who experienced relapse had a shorter duration of high-dose corticosteroid treatment (13.2 days vs. 32.9 days, p = 0.01) compared to patients who did not relapse. The mean duration of tapering-off the corticosteroid until 10 mg per day was significantly longer in non-relapse group (151.3 ± 60.8 days) than in relapse group (63.6 ± 39.4 days, p = 0.013). Conclusions: Higher disease activity in SLE and initial severe neurologic deficits might be associated with the poor outcome of ATM. Corticosteroid slowly tapering-off therapy might be helpful in preventing the recurrence of ATM.

        Background: The prevalence of concurrent Ankylosing spondylitis (AS) and Crohn`s disease (CD) or Ulcerative colitis (UC) is estimated at 10%. Up to 50% of patients with AS have subclinical gut infi ammation seen. This study was purposed to evaluate the association between reasons for ileocolonoscopy and the ileocolonoscopic fi ndings in the patients with AS. Methods: The retrospective study has included 108 AS patients who had ileocolonoscopy from January 2000 to April 2014. Patients with a history of CD, UC, intestinal tuberculosis, or colon cancer were excluded. Patients were divided into 2 groups based on ileocolonoscopic results; 1) negative infi ammatory lesions and 2) positive infi ammatory lesions. The clinical features including HLA-B27 profi les, and reasons for ileocolonoscopy including regular health checkup without symptoms, abdominal pain, rectal bleeding, diarrhea, constipation, anemia and positive stool occult blood were evaluated. Results: As shown in Table, infi ammatory lesions in ileocolonoscopic fi ndings were found in 40 patients out of the 108 patients. Mean ESR and mean CRP were signifi - cantly higher in the group with infi ammatory lesions than the group without infi ammatory lesions. Presence of symptoms or signs (n=34) was associated with risk of infi ammatory ileocolonoscopic fi ndings compared to for screening (n=6) (OR=3. 96). Among the patient`s symptoms or signs, abdominal pain (n=13) was associated with infi ammatory ileocolonoscopic fi ndings most importantly (vs. negative abdominal pain (n=20); OR =2. 47). Among 40 patients with infi ammatory lesions, 12 as CD, 1 as UC,and 4 as intestinal tuberculosis were diagnosed fi nally. However, most of them, 23 patients were considered as subclinical gut infi ammation. Conclusions: Considerable proportion of AS patients showed infi ammatory gut lesions, even in the patients without gastrointestinal symptoms. Moreover, abdominal pain increased a possibility of infi ammatory gut lesions in AS patients. Therefore, regular checkup with ileocolonoscopy could be recommended in AS patients.

      • Synthesis of AlN Particles by Microwave-Assisted Urea Route

        <P>Novel simple route to prepare AlN nanoparticles was proposed in this study. Aluminum nitride powder was synthesized by microwave-assisted urea route. Aluminum chloride was dissolved in ethanol and urea was added to this solution considering molar concentration with Aluminum chloride. The solution was heated under microwave (MW) irradiation from 60°C to 80°C for 5-10 minutes. And then heat treatment was performed in N2 atmosphere with various temperatures and time to obtain AlN particles. While microwave was irradiated to the mixed solution with Aluminum chloride, urea and ethanol, the solvent was eliminated and polymerization reaction was accelerated to formmetal-organic complex. AlN particles were successfully synthesized after heat treatment above 1600°C under N2 or N2-H2 mixed gas atmosphere in the molar ratio (urea/Aluminum chloride) of 6.</P>
