Background/Aims: L-carnitine not only alleviates hyperammonemia and reduces muscle cramps in patients with liver cirrhosis, but also improves anemia in patients with chronic hepatitis and renal dysfunction. This study prospectively evaluated the preventative efficacy of L-carnitine supplementation against hemolytic anemia during antiviral treatment using ribavirin in patients with hepatitis C virus (HCV)-related chronic liver disease. Methods: A total of 41 patients with chronic hepatitis were consecutively enrolled in this study. Group A (n=22) received sofosbuvir plus ribavirin for 3 months, whereas group B (n=19) was treated with sofosbuvir, ribavirin, and L-carnitine. Hemoglobin concentration changes, the effects of antiviral treatment, and the health status of patients were analyzed using short form-8 questionnaires. Results: A significantly smaller decrease in hemoglobin concentration was observed in group B compared to group A at every time point. Moreover, the prescribed dose intensity of ribavirin in group B was higher than that of group A, resulting in a higher ratio of sustained virological response (SVR) 24 in group B compared with group A. The physical function of patients in group B was also significantly improved compared to group A at the end of antiviral treatment. Conclusions: L-carnitine supplementation alleviates ribavirin-induced hemolytic anemia in patients with HCV and helps relieve the physical burden of treatment with ribavirin-containing regimens. These advantages significantly increase the likelihood of achieving SVR.

Background/Aims: To compare the diagnostic yield of 20-gauge forward-bevel core biopsy needle (CBN) and 22-gauge needle for endoscopic ultrasound (EUS)-guided fine needle aspiration (FNA) of solid pancreatic masses. Methods: The use of 20-gauge CBN was prospectively evaluated for 50 patients who underwent EUS-FNA from June 2016 to December 2016. Data were compared with those obtained by a retrospective study of 50 consecutive patients who underwent EUS-FNA using standard 22-gauge needles between December 2016 and April 2017. At least two punctures were performed for each patient; the sample from the first pass was used for cytology with or without histology and that from the second pass was used for histology. Sample quantity was evaluated using the sample obtained from the second pass. Results: There was no significant difference in the diagnostic accuracy rate between the first and second passes (20-gauge CBN: 96% [48/50]; standard 22-gauge needle: 88% [44/50]). Samples >10× power fields in length were obtained from 90% (43/48) and 60% (30/50) of patients using the 20-gauge CBN and standard 22-gauge needle, respectively (p=0.01). Technical failure occurred for two patients with the 20-gauge CBN. Conclusions: Diagnostic accuracy of the 20-gauge CBN was comparable to that of the 22-gauge needle. However, two passes with the 20-gauge CBN yielded a correct diagnosis for 100% of patients when technically feasible. Moreover, the 20-gauge CBN yielded core tissue for 90% patients, which was a performance superior to that of the 22-gauge needle. (Gut Liver 2019;13:349-355)

In this study, an elastic property of EuZn₂Ge₂ with the tetragonal ThCr₂Si₂-type structure has been investigated by ultrasonic measurements. Elastic constants C<SUB>33</SUB> and C<SUB>44</SUB> were measured as a function of temperature and magnetic _eld applied along the [001] direction. A previous study reported that EuZn₂Ge₂ exhibited three magnetic phase transitions at T<SUB>N1</SUB> = 14.9 K, T<SUB>N2</SUB> = 8.4 K, and T<SUB>t</SUB> = 5 K. We found that an elastic anomaly appeared in the temperature dependence of both of C<SUB>33</SUB> and C<SUB>44</SUB> at T<SUB>N2</SUB>. On the other hand, the corresponding anomaly was hardly visible at T<SUB>N1</SUB> and T<SUB>t</SUB>. These results suggest that a magneto-elastic coupling at T<SUB>N2</SUB> may be relatively large compared with those at T<SUB>N1</SUB> and T<SUB>t</SUB>.
Prototype Development of On-Board Vision Processing Micro Air Vehicle for Wide-Range Monocular SLAM

This study was undertaken to create an indoor environmental map using a monocular camera based on structure from motion (SfM). Numerous methods on simultaneous localization and mapping (SLAM) have been proposed for 3D map construction used for various types of mobile robots. We used MonoSLAM proposed by Davidson et al., which is a fundamental method using extended Kalman filters (EKFs) for feature tracking and updating with camera position estimation. For this study, we developed our original micro air vehicle (MAV) prototype to obtain time-series images in a wide-range of indoor environments. The MAV prototype has four monocular cameras to actualize rapid omnidirectional sensing and has computer systems for online vision processing. We applied the method to a benchmark dataset and our original dataset, which was obtained using a single monocular camera. Using the benchmark dataset, we confirmed the appearance tendencies and their meaning of three color patches: red, blue, and yellow patches that respectively correspond to correct tracking, failure tracking, and temporary waiting local features. The experimentally obtained results obtained using our dataset reveal detection of numerous yellow patches, a few blue patches, and a few red patches. Moreover, we obtained 3D trajectories of a camera on the MAV as 3D maps.
Parent Artery Complex Coil Protection for Side-Branched Wide-Neck Aneurysms

This study aimed to validate the usefulness of parent artery complex coil protection for the treatment of wide-neck, side-branched, and ruptured aneurysms. A microcatheter was first introduced into the aneurysmal sac, and another microcatheter was introduced into the parent artery or near the orifice of the branch artery. A framing coil was deployed partially from the first microcatheter, and a protection coil was deployed from the second microcatheter to prevent protrusion of the first framing coil to the parent artery and side branches. After the first framing coil insertion, the protection coil was withdrawn to confirm the stability of the framing coil and blood flow. The procedures with this technique were successful for 3 patients. Parent artery complex coil protection can be an effective and safe coil embolization technique for the preservation of parent and side branch arteries and an alternative method for emergent ruptured cases.

This paper presents a semantic scene recognition method from indoor areal time-series images obtained using a micro air vehicle (MAV). Using category maps, topologies of image features are mapped into a low-dimensional space based on competitive and neighborhood learning. The proposed method comprises two phases: a codebook feature description phase and a recognition phase using category maps. For the former phase, codebooks are created automatically as visual words using self-organizing maps (SOMs) after extracting part-based local features using a part-based descriptor from time-series scene images. For the latter phase, category maps are created using counter propagation networks (CPNs) with extraction of category boundaries using a unified distance matrix (U-Matrix). With manual MAV operation, we obtained areal time-series image datasets of five sets for two flight routes: a round flight route and a zigzag flight route. The experimentally obtained results with leave-one-out cross-validation (LOOCV) for datasets divided with 10 zones revealed respective mean recognition accuracies for the round flight datasets and zigzag flight datasets of 71.7% and 65.5%. The created category maps addressed the complexity of scenes because of segmented categories in both flight datasets.

Objective: This study aimed to assess gynecologic oncologists (GOs)’ perceptions and attitudes toward cancer survivorship to help improve survivor care. Methods: We conducted a web-based questionnaire survey about survivorship issues for the GOs belonging to the Japan Gynecologic Oncology Group. We analyzed the proactiveness of the participants toward addressing 25 survivor issues. In addition, the practice patterns and barriers to care for survivors’ long-term health issues, such as second primary cancer (SPC) and lifestyle-related diseases (LSRD), and return-to-work (RTW) support were assessed. Results: We received 313 responses. The respondents had a mean of 22 years of physician experience. The ratio of men to women was approximately 7:3, and 84.7% worked at facilities for multidisciplinary cancer treatment. The respondents’ proactiveness for addressing psychosocial problems was significantly lower than physical and gynecological issues (p<0.01 by χ2 test). However, most GOs tried to contribute to such issues according to patients’ demands. Women GOs were more proactively involved in some survivorship issues than the men (p<0.05 by logistic regression analysis). The rates of the respondents who proactively discussed SPC, LSRD, and RTW were unexpectedly high (60.7%, 36.1%, and 52.4%, respectively). However, the GOs only provided verbal support for these issues in many cases. Conclusion: The Japanese GOs were enthusiastic about survivorship care. However, their tendency to deal with survivors’ problems through their own knowledge and judgments raises concerns about the quality of care. Therefore, creating survivorship care guidelines and enhancing multidisciplinary collaboration should be prioritized.

In this paper, we propose a screening test robot for prevention and early detection of functional decline of the shoulder joint. For this purpose, we used a three-dimensional reaction force display robot (3D robot) that executed a method for quantitatively evaluating shoulder joint functions. We focused on two shoulder joint functions: one is flexibility/stability, and the other is cooperativeness. In the screening test for functional decline of flexibility/stability, the 3D robot measured the maximum isometric force exerted on the brace of the 3D robot by a subject. In the screening test for functional decline in cooperativeness, the 3D robot measured the maximum isokinetic force exerted on the brace by the subject and the rotational angle of the upper limb during the testing motion. We propose procedures for implementing the two kinds of screening tests using the 3D robot, and we developed a control method. We verified the effectiveness of the proposed screening robot in executing these procedures, the control method, and the quantitative evaluation method.

A twin unidirectional impulse turbine for wave energy conversion has been suggested in our previous study, and the performance under unsteady flow has been investigated by quasi-steady analysis. In the present study, the performance of twin impulse turbine under unsteady flow condition has been investigated by unsteady analysis of Computational fluid dynamics. As a result, the mean efficiency of twin unidirectional impulse turbine under unsteady flow is lower than the maximum efficiency of unidirectional impulse turbine. Moreover, it is verified that airflow goes backward in the reverse turbine in low flow rates.
