      • Impact of Interferon-Based Treatment on Quality of Life and Work Related Productivity from the Korean Cohort in the MOSAIC Study

        Aims: Chronic Hepatitis C Virus (HCV) infection increases the risk for progressive liver disease, hepatocellular carcinoma and negatively impacts the patient’s quality of life. HCV treatment is evolving with direct acting antivirals but IFN based therapy has been the standard of care for many years and remains available in some countries. The MOSAIC study aims to characterize patients with chronic HCV infection and assess the impact of IFN-containing treatment on health-related quality of life, work related productivity and health care utilization. Methods: MOSAIC is an international prospective multicenter observational study that has been conducted in 20 countries. Consecutive patients with chronic HCV infection were enrolled and those who initiated an IFN based regimen were prospectively followed for 48 weeks. We report results from the Korean cohort Results: 100 patients were enrolled: 86 were treatment naïve and 14 were treatment experienced. 33 patients initiated an IFN based regimen: 6 patients started IFN + RBV, 26 patients started Peg-IFN + RBV, none started Peg-IFN + RBV + DAA and 1 patient received other treatment. Among the treated cohort, demographic and disease characteristics were the following: the mean age was 54.5 years; 14 patients were male. 14 had minimal or no fibrosis, 2 portal fibrosis, 3 bridging fibrosis and 6 patients suffered from cirrhosis. HCV Genotype distribution was as follows: genotype 1: 11; genotype 2: 19 and genotype 3: 3. Table 1 describes the results at baseline and changes over 4, 12 and 48 weeks and end-of-treatment (EOT) for the quality of life and work productivity outcome measures (EQ-5D-5L, HCV-PRO and WPAI). Conclusions: Results from the Korean cohort of the MOSAIC study show a moderate trend for deterioration of health-related quality of life and work productivity associated with IFN based treatment for patients with chronic HCV infection during treatment period.       • 세균성 키토산 가수분해효소의 부분적 특성

        chitosan은 D-glucosamine이 β-(1→4) 결합으로 중합된 homopolymer로 chitin을 탈아세틸화(deacetylation)하여 제조된다. chitin은 갑각류의 껍질이나 곤충의 외피, 사상균체의 세포벽을 구성하고 있다. chitin, chitosan 또는 그 분해산물이나 유도체들의 산업적 효율성이 밝혀지면서, 이에 대한 연구가 활발히 이루어지고 있다. 그 중의 한 분야가 효소 분해법을 이용한 chitosan oligomer(chitosanoligosaccharides)의 생산이다. 본 연구에서는 chitosan을 분해하는 미동정 세균의 배양액으로부터 chitosan 가수분해효소(chitosanase)를 분리·정제하여, 주요 특성을 조사하였다. 세균의 무세포 배양액을 한외여과기(NMWL, 1K)로 여과하여 저분자량의 화합물을 제거한 다음 황산암모늄을 가하여 90% 포화시키고, 침전된 단백질을 회수하여 gel permeation chromatography(sephadex G-10과 G-100)로 chitosanase를 정제하였다. 정제전 효소의 비활성은 5.8U/mg 이었으나 정제과정을 통하여 9.7U/mg으로 증대되었다. 정제 전후를 통하여 SDS-PAGE 법에 의한 단백질의 분석과 효소활성 염색으로 한 종류의 chitosanase가 확인되었으며, 그 분자량은 약 30,000dalton 이었고, subunit는 한 종류인 것으로 추정되었다. chitosanase에 의한 기질의 분해산물을 한외여과와 TLC로 분석한 결과, 이 효소는 이량체에서 육량체까지의 chitosanoligosaccharides를 생산하였으며, 단량체를 생산하지 않는 특징이 있었다. 효소반응 최적온도는 40℃이었고, 50℃ 이상에서는 효소의 활성이 급격히 저하되어, 비교적 열안전성이 낮았다. 또한, 이 효소는 pH 5.0∼9.0 범위에서 1시간 방치해도 활성변화가 미미하여, 비교적 광범위한 중성영역에서 안정하였다. Chitosan is a polymer of D-glucosamine linked by β-(1→4) covalent bonds. It is obtained through deacetylation of chitin that occurs mainly in the shells of crustaceans and the exoskeletons of insects. It is also found in fungal cell walls. Having relatively rich natural resources, chitin and their derivatives are widely utilized in industries. Among those are chitosanoligosaccharides that are reported to have several biological functions. The production of chitosanoligosaccharides requires hydrolysis of chitosan by an acid or an enzyme. A chitosanase, the chitosan-depolymerizing enzyme, was purified from unidentified bacterial culture and its characteristics were determined. Cell-free culture of the unidentified bacterium was filtered with ultramembrane(NMWL, 1K) to remove low molecular weight compounds. Ammonium sulfate was added to the filtrate at the level of 90% saturation to induce precipitation of proteins. The precipitated proteins were desalted with sephadex G-10 column and fractionated through sephadex G-100 gel permeation chromatography. Specific activity of the enzyme before purification was 5.8 units/mg and was increased up to 9.7 units/mg through purification procedures. SDS-PAGE analysis of proteins and enzyme activity staining revealed only one kind of chitosanase in the bacterial culture. Its molecular weight was about 30,000 dalton with one kind of subunit. The catalytic unit could highly be a monomeric protein. TLC-analysis of enzymatic hydrolysis products of chitosan showed that the enzyme generated dimeric through hexameric oligomers of D-glucosamine without producing monomeric D-glucosamine. Its optimum temperature for catalytic activity was 40℃ and the catalytic activity dropped rapidly at the temperatures higher than 50℃, which reflected its low heat stability. The enzyme was stable for at least one hour at neutral pH ranges from 5.0∼9.0.

      • 마라톤 동호인들의 참여동기와 활동만족도에 관한 연구

        The purpose of this study is to analyze the motivation and degree of satisfaction of marathon runners. The subjects are composed of 295 members of marathon clubs in Kwang Won Kung Kee provinces. The result of this study is as follows. First, as a result of analyzing the involvement motivation of marathon runners among demographic characteristics, self-esteem sub variable shoved statistically significant difference according to sex, and the sub variable of psychological coping·self-esteem·social recognition showed statistically significant difference according to age. In addition, the sub variabie of life-meaning·psychological coring·self-esteem showed statistically significant difference according to the final education years, and the sub variable of life meaning·psychological coping·self-esteem·striving·social recognition showed statistically significant difference according to job, and the sub variabie of life meaning·psychological coping·self-esteem·competition·health orientation·social recognition showed statistically significant difference according to monthly income. Second, as a result of analyzing the degree of satisfaction among demographic characteristics in marathon runners, physical satisfaction sub variabie showed statistically significant difference according to sex, and the sub variable of educational·social·psychological satisfaction showed statistically significant difference according to the final education years, job, monthly income.

      • 와우 갑각 전기자극 뇌간유발반응용 원추형 전극의 개발

        This paper introduces a new zinc coated copper wire electrode with coiled cone shape which has low surface resistance and tolerance to the motion artifact for promontory stimulation electrically auditory brainstem responses (PSEABR). Auditory brainstem responses (ABR) can be used to predict hearing threshold level with a great deal of accuracy particularly for a young child who cannot cooperate mechanically and some hearing impaired who are exaggerating a hearing loss for economic compensation. While severe profound sensorineural hearing losses may not be implemented by auditory potentials, PSEABR is proven as a useful tool even for some sensorineural related hearing impaired. It was shown that PSEABR gives the electrical stimuli to promontory of the cochlear instead of giving acoustic stimuli. For this reason, PSEABR can be used as an alternative for cochlear implantation, and can also be used as an optimal device selection and neural information for MAP. It was found that the role of electrode is very important in PSEABR. Even though this cone-shaped electrode was applied in animal experiments, waveforms are well produced by PSEABR. Thus, it was concluded that cone-shaped electrode turned out to be a useful preoperative audiological evaluation tool in deciding time for cochlear implantation surgery.

        The aim of this study was investigated with the behavior of parathion adsorption and degradation efficiency of parathion in various samples loaded with earthworm casting. Adsorption capacity of parathion in loam soil was better than clay loam. Degradation of parathion was 70 percent in loam and clay loam soil to time but samples was loaded 5% casting, L0C5 and CLC5 had shown 90 percent degradation. We found that parathion was leachers pesticide as groundwater ubiquity score (GUS) was more than 4.0.

        농업의 비교적적 관심사항(NTC)에 대한 論議動向과 그 示唆點

        The purpose of this study is to analysis international discussion of non-trade concerns(NTCs) and find out some implications. As we know well, NTCs are important to most people living in the modern world for a number of reasons such as preserving the environment, rural viability, agricultural landscape and cultural heritage and food security. Therefore NTCs cannot be a tradable thing likewise general private goods in the market. For that reason, developing and food-importing countries have been stressed NTCs of agriculture. From this point of view, the following implications should be reflected in the Doha Development Agenda(DDA). Firstly, principle of NTCs should be kept and expanded to develop multifunctionality of agriculture. Secondly, the NTC countries have to build more cooperative relationship based on mutual confidence. Such cooperation will bring benefits to each country. Thirdly, in order to fully reflect NTCs of agriculture in the coming negotiations, we should be improved negotiation ability and make out cooperative connection with non-government organizations, producer organizations and national assembly.

        This study is an improvement effect with varying the factors, such as ultrasonic irradition time, sludge depth and ultrasonic intensity. The calculated result about the moisture content rate and the specific resistant coefficient, based on the outcome of the dewatering test by BFT, can be summarized as follows : 1. The specific resistant coefficient of the paper mill sludge was 4.34×10¹²m/㎏ at ultrasonic intensity of 0 watt/㎡ and ultrasonic irradiation time of 6 minutes. And also, the balue of a slope "a" was 0.024 and the moisture content rate of sludge was about 72% on condition that the depth o sludge was 60㎝. 2. In case that the dosing concentration of ultrasonic+a congulant was 100㎎/l, the moisture content rate of dewatered sludge was about 58%.

        클라이언트로부터 전송되는 인증 데이터를 실시간으로 서버에서 분석하여 처리하는 시스템의 경우에는 입력되는 데이터의 양에 비례하여 서버의 부담은 증가하게 된다. 이때 서버에서는 데이터의 병목현상이 발생하게 되고, 이는 바로 전체적인 시스템의 성능을 저하시키는 결과를 초래하게 된다. 본 논문에서는 데이터 처리의 병목현상을 해결하여 시스템의 성능을 높일 뿐만 아니라, 인터넷상의 유휴 서버들을 활용할 수 있게 하기 위한 방안으로 분산처리 기술을 이용한 전처리 작업 시스템과 그 성능을 향상시키기 위한 분산 객체 활성화 시스템을 제안한다.

        This paper introduces a development of an electrical stimulator for auditory stimulation. The electrical stimulator is useful in neurotological diagnosis, audiological evaluation, candidate selection for cochlear implantation, optimal device selection and decision making of MAP strategy for severe-to-profound hearing impaired persons. The development was based on sound parameters of auditory brainstem responses and auditory electrophysiological characteristic such as effective firing of auditory nerve and recording evoked potentials during refractory period of neuron. Besides pulse parameter could adjustable by programming for more varied electrical stimulation evoked response audiometry. Using the electrical stimulator, electrical square pulse was applied to promontory, and electrically evoked auditory brainstem response and electrically middle latency response were successfully recorded in cats.

      • 이명과 청각민감증을 동반한 편측 고음 급추형 감각신경성 난청의 청각 재활

        In case of the hearing impairment with tinnitus and increased sound sensitivity, it is known that the patients tend to appeal the psychologically oriented social handicap rather than communication disability. The audiologist who is responsible for such patients in aural rehabilitation should pay special attention to the counseling techniques inducting tinnitus retain therapy (TRT), ear protector, noise generator, or specific acoustic training based on dose cooperation and rapport. And then the audiologist should try to lessen their reaction to the tinnitus by using a hearing aid. This therapies tries to focus not a total approach but a treatment to lessen the severity of tinnitus. This paper as a case report that a unilateral sharply slopped sensorineural hearing impaired person with tinnitus and increased sound sensitivity by using four channel digital signal processing (DSP) hearing aid with programming increment at low level (PILL).
