        Exposure to ambient fine particles and neuropsychiatric symptoms in cognitive disorder: A repeated measure analysis from the CREDOS (Clinical Research Center for Dementia of South Korea) study

        <P><B>Abstract</B></P> <P>There is a growing concern that air pollution, especially those particles <2.5 μm (PM<SUB>2.5</SUB>), could increase the risk of cognitive impairment and mental disorders. However, the relationship between ambient PM<SUB>2.5</SUB> and neuropsychiatric symptoms in people with cognitive impairment is still undetermined. This longitudinal study included 645 pairs of cognitively impaired subjects, who had not changed residence within Seoul, and their caregivers from the Clinical Research Center for Dementia of South Korea study cohort between September 2005 and June 2010 (1763 days). Neuropsychiatric symptoms were measured by the Korean version of the Neuropsychiatry Inventory, and caregiver burden was examined by the Neuropsychiatry Inventory Caregiver Distress Scale at the first and second visits at the outpatient clinic. District-specific PM<SUB>2.5</SUB> concentrations were constructed over 1 month to 1 year prior to each visit. A log-linear regression using generalized estimating equations to account for repeated measures was used to assess the relationship between PM<SUB>2.5</SUB> exposure and neuropsychiatric symptoms or caregiver burden. Aggravated neuropsychiatric symptoms were associated with exposure to high PM<SUB>2.5</SUB> levels (adjusted percent change: 16.7% [95% confidence interval (CI), 5.0–29.7] per 8.3 μg/m<SUP>3</SUP> increase in 1-month moving averages). Increased caregiver burden was associated with high PM<SUB>2.5</SUB> exposures only in caregivers for patients with Alzheimer's disease (adjusted percent change: 29.0% [95% CI, 8.1–53.9] per 8.3 μg/m<SUP>3</SUP> increase in 1-month moving averages). The present results indicate that PM<SUB>2.5</SUB> exposure is associated with aggravated neuropsychiatric symptoms and increased caregiver burden in subjects with cognitive impairment.       • Deep Q-Network 알고리즘 기반 자동 두부계측점 검출

        2D 두부계측 분석에서 두부계측점의 정확한 검출은 환자 진단과 수술 계획을 위해 필수적인 요소이다. 2D 두부방사선사진(cephalogram)은 해부학적 좌우 구조물이 하나로 겹쳐서 찍히고, 환자의 위치, 방향에 따라 차이가 나는 결과가 도출되어 환자의 계측점 검출이 쉽지 않다. 두부계측 분석을 위한 자동 계측점 검출 연구가 많이 이루어졌으나 임상 허용 수준까지 도달하지는 못했다. 본 논문에서는 최초로 심층 강화학습인 deep Q-network (DQN) 기법을 적용해서 자동 두부계측점을 검출하였다. ISBI Challenge 2015 데이터셋을 통해 DQN 을 적용하여 기존의 방법과 성능을 비교평가하였다. ISBI Challenge 데이터셋 평가에서 19개 계측점의 평균 반지름 오차(MRE)는 두 가지 테스트에서 각각 1.49 ㎜, 1.6 ㎜를 보여줬으며, 테스트 데이터 중 2 ㎜ 이내의 MRE를 갖는 비율인 성공 검출 확률(SDR)은 74.06%로 기존의 알고리즘보다 향상된 성능을 보였다. 큰 해상도의 의료영상 이미지를 이용하기 위해 적용한 dilated, multi-scale 전략의 계측점 별 최적화와 추가적인 DQN의 hyperparameter 최적화를 통해 성능을 더욱 향상시킨 후 임상 연구를 진행하려 한다.

      • 바이오 분야의 정보 추출을 위한 BioLUKE 모델 개발

        현존하는 자연어 처리 모델은 포괄적인 자연어 처리 데이터에 최적화되어 있고 생소한 단어들에 대한 모델링의 정밀도가 낮아 바이오 연구 분야와 같이 국부적인 분야의 정보 추출에서 뛰어난 성능을 보이지 못하고 있다. 본 연구진은 바이오 분야의 데이터를 이용한 사전 학습과 Entity 기반의 임베딩 방식을 접목한 BioLUKE 모델을 생성하여 이러한 한계점을 극복하고자 하였다. 기학습된 LUKE 모델에 대해 생명과학 문헌 데이터베이스에서 확보한 자연어 데이터셋을 활용한 사전 학습이 이루어졌으며, BioASQ 챌린지 데이터셋을 이용한 검증이 이루어졌다. 기존 LUKE 모델과 대비하여 바이오 연구 분야에 적용한 모델링의 정밀도가 상승하였음을 다양한 척도에서 확인할 수 있었고, 각 챌린지에서 상위의 점수를 기록한 모델들 이상의 정확도를 보였다. 이러한 바이오 연구 분야의 사전 학습과 Entity 기반의 임베딩 방식을 접목한 접근은 아직까지 시도된 바가 없으며, 타 분야로의 확장성이 높다.

        Factors Associated with Caregiver Burden in Patients with Alzheimer’s Disease

        Objective-Caregivers for patients with Alzheimer’s disease (AD) suffer from psychological and financial burdens. However, the results of the relationship between burden and cognitive function, performance of activities of daily living, and depressive symptoms have remained inconsistent. Therefore, the aim of this study was to examine which factors are more significant predictors of heightened burden, cognitive impairment or functional decline, besides neuropsychiatric symptoms. Methods-A cross-sectional study was conducted in a sample comprised of 1,164 pairs of patients with AD and caregivers from the Clinical Research of Dementia of South Korea study cohorts. The cognitive function of each sub-domain, functional impairments, depressive symptoms, and caregiver burden were assessed using the dementia version of Seoul Neuropsychological Screening Battery (SNSB-D), Barthel Index for Daily Living Activities (ADL), Seoul-Instrumental Activities of Daily Living (S-IADL), the Clinical Dementia Rating Sum of Box (CDR-SB), the Global Deterioration Scale (GDS), the Korean version of the Neuropsychiatric Inventory (K-NPI), and the 15-item Geriatric Depression Scale. Results-We found that higher severity (higher CDR-SB and GDS scores) and more functional impairment (lower ADL and higher SIADL scores) were significantly associated with higher caregiver burden. In addition, depressive symptoms of patients (higher Geriatric Depression Scale scores) were associated with higher caregiver burden. Conclusion-Therefore, interventions to help maintain activities of daily living in patients with AD may alleviate caregiver burden and improve caregiver well-being.

        Damage detection on an aluminum plate from the cross-correlation of diffuse field using the support vector machine

        <P><B>Abstract</B></P> <P>In order to assess the damage detectability of the structural health monitoring method using diffuse fields, a laboratory experiment is conducted on an aluminum plate where two accelerometers are mounted as receivers. Two types of damages, a nonlinear material attachment and a punctuated hole, are considered. A hand-held impact hammer is used for the excitation, whereby the hammer is moved over grid points drawn on the aluminum plate, and thus the diffuse fields are generated by superposing the wave fields by many excitations randomly sampled. From the cross-correlation of diffuse fields between two receivers, we extract the coherent wave field in cases with and without damages. To detect the damage, a novel damage detection algorithm using a support vector machine is suggested based on the reduced features, transformed from several statistical parameters of damaged and undamaged noise cross-correlation functions, aided by the principal component analysis. The performance of the proposed algorithm is analyzed for the number of sources and damage types.</P> <P><B>Highlights</B></P> <P> <UL> <LI> A laboratory experiment was performed on an aluminum plate to confirm the damage detectability using diffuse fields. </LI> <LI> The Green's function was extracted from the averaged noise cross-correlation functions, and used for the damage detection. </LI> <LI> A damage detection algorithm using elastic diffused fields was suggested, and is robust even in incomplete diffused fields. </LI> </UL> </P>

        시변동이 있는 근거리 음향신호의 시간영역 지음향학적 역산

        2006년 여름 New Jersey 대륙붕 근해에서 일련의 해상실험(Shallow Water 2006, SW06)이 수행되었다. 이 때 수직선배열에서 계측된 근거리 chirp 신호(1100~2900 Hz)에서 직접도달파와 해수면 반사파의 강한 시변동이 관찰되었다. 본 논문은 시변동성이 있는 근거리 음향신호의 지음향학적 역산기법과 실험 데이터에 대한 역산결과를 제시한다. 불규칙한 해수면 반사파가 역산에 미치는 영향을 최소화하기 위해 수직선배열에서 음원의 위치로 직접도달 경로와 해저면 반사경로를 통해 역전파된 신호의 에너지로써 목적함수를 정의하였다. 또한 VFSR(Very Fast Simulated Reannealing) 최적화기법을 활용한 다단계 역산기법을 실험데이터에 적용하였다. 역산 결과 음원은 주기적인 수직운동을 한 것으로 파악되었고 그 주기는 수면파의 주기와 일치하였다. 해저면의 음속은 1645 m/s로 추정되었고 이는 동일 해역의 다른 연구결과와 유사한 것으로 파악되었다. A set of experiments (Shallow Water 2006, SW06) was carried out in shallow water near the New Jersey shelf break in summer 2006. Significant fluctuations in direct and surface reflected arrivals were observed from the chirp data (1100~2900 Hz) measured on a vertical line array. This paper presents a geoacoustic inverssion technique for short-range acoustic data with fluctuating arrivals and inversion results of experimental data. In order to reduce effects of random sea surface on the inversion, the acoustic energy back-propagated from the array to the source through direct and bottom-reflected paths is defined as the objective function. A multi-step inversion scheme is applied to the data using VFSR (Very Fast Simulated Reannealing) optimization technique. The inversion results show a source depth oscillation period equal to the measured ocean surface wave period. The inverted bottom sound speed is 1645 m/s and is similar to that estimated by other work at the same site.

      • Fluctuating arrivals of short-range acoustic data.

        <P>Geoacoustic inversion using fluctuating signal observations can be challenging. The origin of these fluctuations needs to be understood so the signals can be used appropriately. A set of experiments [Tang et al., Oceanogr. 20(4), 156-167 (2007)] was carried out in shallow water near the New Jersey shelf break in summer 2006. Significant fluctuations in the direct path and surface-reflected arrivals of short-range chirp transmissions (1.1-2.9 kHz) were observed on a vertical line array. This paper explains the origin of these signal fluctuations through analysis of the arrival amplitudes. It is shown that the strong thermocline combined with an oscillating source motion due to ocean surface waves results in the signal fluctuations.</P>
