        Polyostotic Spinal Paget’s With Lumbar Kyphotic Deformity: A Case Report And Literature Review

        Study Design: Case report Objectives: Paget’s disease is a metabolic disorder of the skeletal system involving the spine commonly. Prevalence of Paget’s is uncommon in Southern Asia when compared to the West. The disease can be divided into three major phases namely lytic, mixed and a sclerotic phase. They have 2 types of clinical presentation- polyostotic or monostotic type. Blood and radiological investigations help in diagnosis. The mainstay of management is pharmacotherapy, rarely requiring surgery. Materials and Methods: A 45 years old male, presented with back pain and claudication, initially mistreated to be a case of Pott’s spine. After investigating, he was operated because of worsening symptoms and conformation of diagnosis. Retrospective analysis of operation theatre and out patient records was done, compiled and presented. Results: Confirmatory diagnosis of Paget’s disease was done based on histopathology. A whole body radionucleotide scan was done to rule out any other skeletal lesion. He was started on medical management for Paget’s. He improved symptomatically, during the last follow up he was asymptomatic with no clinical disability and radiological improvement of lesion. Conclusions: Atypical presentation of Paget’s with a polyostotic spinal involvement in a South Asian is rare. High suspicion and prompt investigations help in early diagnosis and treatment. Pharmacotherapy is the mainstay of treatment with surgery when indicated.

        Comparison of oropharyngeal leak pressure of I-gelTM and BlockbusterTM laryngeal mask airway in anaesthetized pediatric patients

        Background: Supraglottic airways (SGA) are increasing used in pediatric anesthesia. Among SGA i-gelTM is commonly used device in pediatric patients. The BlockbusterTM laryngeal mask airway (LMA) is latest addition in pediatric airway armamentarium. This study was conducted to compare the clinical performance of i-gelTM and BlockbusterTM LMA in pediatric patients. Methods: A total of 140 children aged 1–5 years, who were undergoing elective surgery, were randomized into two groups either i-gelTM (Group I) or BlockbusterTM LMA (Group B). Airway was secured with appropriate-sized LMA according to group allocation under general anesthesia. The primary objective of study was oropharyngeal leak pressures (OPLP), and secondary objectives were number of attempts of device insertion, success rate, ease of LMA insertion, hemodynamic parameters, and postoperative pharyngolaryngeal morbidities. Results: The mean OPLP was significantly higher for i-gelTM compared to BlockbusterTM LMA (27.97 ± 1.65 vs. 26.04 ± 2.12; P < 0.000). The devices were successfully inserted on the first attempt in 97.14% and 90% of the i-gelTM group and BlockbusterTM LMA groups respectively. The insertion time, ease of insertion, hemodynamic parameters and postoperative complications were comparable between groups. Conclusions: The i-gelTM was more efficacious device in term of OPLP than BlockbusterTM LMA for positive pressure ventilation in pediatric patients undergoing short surgical procedures under general anesthesia.

        Modified “Lift up” Laminoplasty – A Novel Surgical Technique For Long Segment Epi/Intradural Tuberculoma: A Case Series

        Study Design: Orginal article. Objective: Spinal tuberculosis presents mostly as a spondylodiscitis or a discitis, rarely as non-osseous tuberculoma. They can be either extradural, intradural extramedullary, or intramedullary lesions. Rarely they present as an intradural extramedullary (IDEM) tuberculomas or spinal epidural abscess (SEA). We would like to describe the surgical technique called the modified lift-up laminoplasty, for management of long segment organised IDEM/SEA tuberculomas. Summary of Literature Review: ATT continues to be the gold standard treatment for tuberculosis while surgery is indicated in a select few. Combination has given successful outcomes and fusion in about 97% of cases. Material and Methods: Retrospective review of the records for lift up laminoplasty in the past two years was done. All the relevant records and imaging of the patients were retrieved and briefed. Results: Four patients were included in the study. Two patients were diagnosed with IDEM tuberculomas, one case of tubercular SEA with and without osseous involvement each. All were managed with modified lift up laminoplasty technique. Three patients had good clinical improvement at the final follow up. Conclusions: An early MRI helps in early diagnosis of atypical spinal tuberculosis. The technique of modified lift up laminoplasty gives dual advantage of adequate working space, preserves the posterior tension band ligaments and avoids long segment instrumented fixation. It is an efficient method for long segment epi/intradural organised granulation tissue.

        Multi-dimensional Task Offloading using Deep Learning for Vehicular Edge Computing Networks: A Survey

        Vehicular networks must support connection ubiquity and high levels of services for a large number of vehicles. In vehicular networks, mobile edge computing (MEC) is considered a viable technique, utilizing computing resources at the edge of wireless access networks. This survey examines effectual task offloading systems forvehicular edge computing (VEC) networks. Several deep learning methods have been suggested to enable task offloading schemes. Therefore, task offloading optimality is a main research topic in fog computing. The main aim of this work is to offer readers an overview of the journey from the task offloading concept to its mathematic problem formulation. This survey introduces fog computing and the process of task offloading that emulates numerous aspects of optimization. Subsequently, a number of machine learning and deep learning methodologies are employed in task offloading,and the challenge of fog computing is discussed. This paper provides a detailed statistical analysis from papers published between 2019 and 2022.

        Flavonoids as Novel Therapeutic Agents Against Chikungunya Virus Capsid Protein: A Molecular Docking Approach

        Chikungunya fever has a high morbidity rate in humans and is caused by chikungunya virus. There are no treatments available until now for this particular viral disease. The present study was carried out by selecting 19 flavonoids, which are available naturally in fruits, vegetables, tea, red wine and medicinal plants. The molecular docking of selected 19 flavonoids was carried out against the Chikungunya virus capsid protein using the Autodock4.2 software. Binding affinity analysis based on the Intermolecular interactions such as Hydrogen bonding and hydrophobic interactions and drug-likeness properties for all the 19 flavonoids have been carried out and it is found that the top four molecules are Chrysin, Fisetin, Naringenin and Biochanin A as they fit to the chikungunya protein and have binding energy of -8.09, -8.01, -7.6, and 7.3 kcal/mol respectively. This result opens up the possibility of applying these compounds in the inhibition of chikungunya viral protein.

        Liver Enzymes and Risk of Stroke: The Atherosclerosis Risk in Communities (ARIC) Study

        Background and Purpose Liver enzymes (aspartate aminotransferase [AST], alanine aminotransferase [ALT], and gamma-glutamyl transpeptidase [GGT]) are glutamate-regulatory enzymes, and higher glutamate levels correlated with worse prognosis of patients with neurotrauma. However, less is known about the association between liver enzymes and incidence of stroke. We evaluated the association between serum levels of AST, ALT, and GGT and incidence of stroke in the Atherosclerosis Risk in Communities (ARIC) study cohort from 1990 to 1992 through December 31, 2016. Methods We included 12,588 ARIC participants without prevalent stroke and with data on liver enzymes ALT, AST, and GGT at baseline. We used multivariable Cox regression models to examine the associations between liver enzymes levels at baseline and stroke risk (overall, ischemic stroke, and intracerebral hemorrhage [ICH]) through December 31, 2016, adjusting for potential confounders. Results During a median follow-up time of 24.2 years, we observed 1,012 incident strokes (922 ischemic strokes and 90 ICH). In age, sex, and race-center adjusted models, the hazard ratios (HRs; 95% confidence intervals [CIs]) for the highest compared to lowest GGT quartile were 1.94 (95% CI, 1.64 to 2.30) for all incident stroke and 2.01 (95% CI, 1.68 to 2.41) for ischemic stroke, with the results supporting a dose-response association (P for linear trend <0.001). Levels of AST were associated with increased risk of ICH, but the association was significant only when comparing the third quartile with the lowest quartile (adjusted HR, 1.82; 95% CI, 1.06 to 3.13). Conclusions Elevated levels of GGT (within normal levels), independent of liver disease, are associated with higher risk of incident stroke overall and ischemic stroke, but not ICH.

        Optimization, Production, and Partial Characterization of an Alkalophilic Amylase Produced by Sponge Associated Marine Bacterium Halobacterium salinarum MMD047

        An endosymbiont Halobacterium salinarum MMD047, which could produce high yields of amylase, was isolated from marine sponge Fasciospongia cavernosa, collected from the peninsular coast of India. Maximum production of enzyme was obtained in minimal medium supplemented with 1% sucrose. The enzyme was found to be produced constitutively even in the absence of starch. The optimum temperature and pH for the enzyme production was 40℃ and 8.0, respectively. The enzyme exhibited maximum activity in pH range of 6~10 with an optimum pH of 9.0. The enzyme was stable at 40℃ and the enzyme activity decreased dramatically above 50℃. Based on the present findings, the enzyme was characterized as relatively heat sensitive and alkalophilic amylase which can be developed for extensive industrial applications.

        Performance Enhancement of PVDF/LiCIO4 Based Nanocomposite Solid Polymer Electrolytes via Incorporation of Li0.5La0.5TiO3 Nano Filler for All-Solid-State Batteries

        Experimental and computational techniques have been applied to investigate the influence of Li0.5La0.5TiO3 nanoparticles on the ionic conductivity of the poly(vinylidene fluoride) (PVDF)/LiClO4 nanocomposite solid polymer electrolyte. The theoretical evidence facilitated to suggest a plausible mechanism for Li-ion conduction across the PVDF/LiClO4/Li0.5La0.5TiO3 based solid polymer electrolytes. The solid composite polymer electrolyte with 30wt% of Li0.5La0.5TiO3 (LLTO) nanofiller exhibited an unprecedented ionic conductivity of 2.3687 × 10-3 S cm-1 at room temperature. The addition of LLTO nanoparticles to the polymer matrix enhanced its ionic conductivity by two orders of magnitude. The activation energy (Ea) and total transference number (t) were estimated to be 0.29 eV and 0.853, respectively. The interaction between the filler and polymer matrix has been inferred by the density functional theory (DFT)-IR analysis. The DFT calculations have been performed on the above system using the basis set of B3LYP-LANL2DZ. The calculated IR spectra were compared with the experimental FTIR data, which allowed us to propose accurate vibrational assignments and to clarify the complex IR vibration of the samples. All-solid-state Li2FeSiO4/C|PVDF/LiClO4/LLTO| graphite lithium cell has been fabricated using the highest Li-ion conducting PVDF/LiClO4/LLTO composite polymer electrolyte. The all-solid-state cell exhibits an excellent initial specific capacity of 87.13 and 73.24 mAh g-1 after 30 cycles, demonstrating higher capacity retention. The findings provide an avenue for exploring the simple all-solid-state lithium batteries, which are potential candidates for next-generation energy storage technology.

        Background: Our objective was to evaluate the accuracy of cardiovascular disease (CVD) risk score classification by direct LDL cholesterol (dLDL-C), calculated LDL cholesterol (cLDL-C), and non–HDL cholesterol (non–HDL-C) compared to classification by reference measurement procedures (RMPs) performed at the CDC. Methods: Weexamined 175 individuals, including 138 with CVD or conditions that may affect LDL-C measurement. dLDL-C measurements were performed using Denka, Kyowa, Sekisui, Serotec, Sysmex, UMA, and Wako reagents. cLDL-C was calculated by the Friedewald equation, using each manufacturer’s direct HDL-C assay measurements, and total cholesterol and triglyceride measurements by Roche and Siemens (Advia) assays,respectively. Results: For participants with triglycerides <2.26 mmol/L (<200 mg/dL), the overall misclassification rate for the CVD risk score ranged from 5% to 17% for cLDL-C methods and 8% to 26% for dLDL-C methods when compared to the RMP. Only Wako dLDL-C had fewer misclassifications than its corresponding cLDL-C method (8% vs 17%; P <0.05). Non–HDL-C assays misclassified fewer patients than dLDL-C for 4 of 8 methods (P <0.05). For participants with triglycerides ≥2.26 mmol/L (≥200 mg/dL) and <4.52 mmol/L (<400 mg/dL), dLDL-C methods, in general, performed better than cLDL-C methods, and non–HDL-C methods showed better correspondence to the RMP for CVD risk score than either dLDL-C or cLDL-C methods. Conclusions: Except for hypertriglyceridemic individuals, 7 of 8 dLDL-C methods failed to show improved CVD risk score classification over the corresponding cLDL-C methods. Non–HDL-C showed overall the best concordance with the RMP for CVD risk score classification of both normal and hypertriglyceridemic individuals.
