      • Uterine Perforation after High-Intensity focused Ultrasound Treatment for Adenomyosis

        High intensity focused ultrasound (HIFU) treatment of uterine myoma is a non-invasive procedure that uses high intensity, focused ultrasound to heat and destroy the tissue of uterine myoma. The history of using therapeutic ultrasound dates back to the early 20th century, and since, the technique has been continuously improved to an extent that the breadth of its clinical applications, both diagnostic and therapeutic, justify the integral role it plays in medicine today. HIFU treatment requires that ultrasound beams be focused on targeted uterine myoma. Due to significant energy dissipation at the focus, temperatures within tissues increase to more than 65℃, and destroy diseased tissue. This technology can achieve precise "ablation" of myoma tissue, and because it destroys diseased tissue non-invasively, it is also known as "Non-invasive HIFU surgery". Development of this technology significantly broadened the range of treatment options for patients suffering from uterine myoma. Furthermore, in contrast to surgery, HIFU requires only a short period of hospitalization and has a low complication rate. Today, HIFU is being used more frequently to treat adenomyosis, but its increased use has led to the reporting of more complications. Here, the authors report a case of uterine perforation after HIFU ablation for adenomyosis.

        Comparative Study on Hysteroscopic and Histologic Examinations of the Endometrium in Postmenopausal Women Taking Tamoxifen

        Objectives: To evaluate the histologic effects of tamoxifen on the endometrium using hysteroscopy in postmenopausal women with breast cancer. Methods: The study included 46 postmenopausal patients who were referred from another clinic due to thickening or bleeding of the endometrium after taking tamoxifen for breast cancer. All patients underwent transvaginal sonography (TVS) and hysteroscopic endometrial biopsy with a 5-mm, continuous-flow, operating hysteroscope. Results: The incidence of malignancy was high (20%) in cases of abnormal uterine bleeding (AUB) after taking tamoxifen. However, in the non-AUB group with thick endometrium after taking tamoxifen, the incidence of adenocarcinoma was 3.2%. Conclusions: Our findings confirm the estrogen-like effect of tamoxifen on the endometrium. Endometrial evaluation with TVS suggests further diagnostic procedures; moreover, histologic examination is necessary under hysteroscopy, especially in cases of endometrial bleeding after taking tamoxifen.         Uterine arteriovenous vascular malformation (UAVM) is a little known condition which is a potentially life - threatening disease due to excessive bleeding. This case report describes a case of a 37-year-old woman who experienced with repeated episodes of vaginal bleeding after she had dilation and evacuation done owing to missed abortion at 9 weeks of gestational age. Doppler ultrasound indicated increased vascularity and prominent vessels on the fundus of uterus. She also had retained products of conception which made the diagnosis complicated. UAVM was confirmed on Computed tomography angiography with 3 dimensional rendering. Uterine artery embolization was performed on her and dilation and curettage was done safely and successfully.

        Typhoon-generated microseisms observed from the short-period KSRS array

        The seismic-noise data recorded on 19 vertical-component short-period seismometers of the KSRS seismic array are analyzed (1) to determine whether typhoons in the Pacific Ocean can be tracked accurately, and (2) to explore the seismic phases comprising the noise field recorded at the array. For our tests, two super typhoons, Sinlaku and Rammasun of 2008, were selected on the basis of their strength and wide azimuthal coverage from the seismic array. To track the source of DF microseisms, f-k analysis was applied to the KSRS data to estimate the back azimuth of the 0.2–0.7 Hz noise field (DF microseisms). These computed back azimuths show good agreement with the known values to the centers of the NW Pacific typhoons. This clearly indicates that these typhoons were the main source of microseisms during their passing. The seismic phases in our DF microseism band are investigated with the phase velocities from our f-k analysis. The estimated horizontal phase velocities range from 3.2 to 3.8 km/s, with an average of about 3.5 km/s. This indicates that the major phases of the observed DF microseisms are surface waves consisting of mostly P-SV Mode 1 and Mode 2—in amplitude ratio A1/A2=1/3—and possibly some Mode 3.
