In this paper, the 1/f noise characteristics of n-channel MOSFET (NMOSFET) and p-channel MOSFET (PMOSFET) are analyzed in depth as a function of body bias. The normalized drain current noise, SID/ID 2 showed strong dependence on the body bias in the sub-threshold region for both NMOSFET and PMOSFET, and NMOSFET showed stronger dependence than PMOSFET on the body bias. On the contrary, both of NMOSFET and PMOSFET do not exhibit the dependence of SID/ID 2 on body bias in strong inversion region, although the noise mechanisms of two MOSFETs are different from each other.

This study was conducted to evaluate the effects of duck-meat consumption on adult disease risk factors, including body compositions, hematological variables, and serum metabolic and lipid profiles in adult human males. To obtain results, 20 adult males aged 20 to 25 were subjected to a diet of 600 g/day of duck-meat for 4 wk, after which body composition, hematological variables, and serum metabolic and lipid profiles were investigated to determine if there was a relationship between duck-meat consumption and adult disease risk factors. The results revealed that high amounts of duck-meat intake did not negatively alter body indices such as body weight, fat mass, body mass index (BMI), % body fat or waste-to-hip ratio (WHR). Furthermore, there were statistically insignificant changes in the number of blood cells, although this number did increase significantly following intake of duck-meat. Moreover, general decreases in serum metabolic parameters were observed, but none of these changes were significant with the exception of the concentration of blood urea nitrogen (BUN). The serum concentrations of LDL-cholesterol also showed a statistically significant decrease by 5.86%. Therefore, this study suggests that the ingestion of duck-meat not only significantly increased the RBC count but also decreased BUN and LDL-cholesterol concentrations in adult males.
Postoperative chemoradiotherapy in high risk locally advanced gastric cancer
Song, Sanghyuk,Chie, Eui Kyu,Kim, Kyubo,Lee, Hyuk-Joon,Yang, Han-Kwang,Han, Sae-Won,Oh, Do-Youn,Im, Seock-Ah,Bang, Yung-Jue,Ha, Sung W. The Korean Society for Radiation Oncology 2012 Radiation Oncology Journal Vol.30 No.4
Purpose: To evaluate treatment outcome of patients with high risk locally advanced gastric cancer after postoperative chemoradiotherapy. Materials and Methods: Between May 2003 and May 2012, thirteen patients who underwent postoperative chemoradiotherapy for gastric cancer with resection margin involvement or adjacent structure invasion were retrospectively analyzed. Concurrent chemotherapy was administered in 10 patients. Median dose of radiation was 50.4 Gy (range, 45 to 55.8 Gy). Results: The median follow-up duration for surviving patients was 48 months (range, 5 to 108 months). The 5-year overall survival rate was 42% and the 5-year disease-free survival rate was 28%. Major pattern of failure was peritoneal seeding with 46%. Locoregional recurrence was reported in only one patient. Grade 2 or higher gastrointestinal toxicity occurred in 54% of the patients. However, there was only one patient with higher than grade 3 toxicity. Conclusion: Despite reported suggested role of adjuvant radiotherapy with combination chemotherapy in gastric cancer, only very small portion of the patients underwent the treatment. Results from this study show that postoperative chemoradiotherapy provided excellent locoregional control with acceptable and manageable treatment related toxicity in patients with high risk locally advanced gastric cancer. Thus, postoperative chemoradiotherapy may improve treatment result in terms of locoregional control in these high risk patients. However, as these findings are based on small series, validation with larger cohort is suggested.
Cardiovascular beriberi: rare cause of reversible pulmonary hypertension
Song, Joon Hyuk,Cheon, Sang Soo,Bae, Myung Hwan,Lee, Jang Hoon,Yang, Dong Heon,Park, Hun Sik,Cho, Yongkeun,Chae, Shung Chull Yeungnam University College of Medicine 2014 Yeungnam University Journal of Medicine Vol.31 No.1
Cardiovascular beriberi is caused by thiamine deficiency and usually presents as high cardiac output failure associated with predominantly right-sided heart failure and rapid recovery after treatment with thiamine. Because of its rarity in developed countries, the diagnosis can often be delayed and missed. We recently experienced a case of cardiovascular beriberi with pulmonary hypertension which successfully treated with thiamine infusion. A 50-year-old man with chronic heavy alcoholics was refered to our department for dyspnea with mental change. Echocardiography showed marked right ventricular (RV) dilatation and flattening of the interventricular septum with a D-shaped deformation of the left ventricle. Moderate tricuspid valve regurgitation was found and estimated RV systolic pressure was 52 mm Hg. Because of his confused mentality and history of chronic alcohol intake, neurological disorder due to thiamine deficiency was suspected and intravenous thiamine was administered and he continuously received a daily dose of 100 mg of thiamine. Follow up echocardiography showed marked reduction of RV dilatation and improvement of a D-shaped deformation of the left ventricle. He finally diagnosed as cardiovascular beriberi on the basis of dramatic response to intravenous thiamine. Thiamine deficiency can cause reversible pulmonary hypertension, and can still be encountered in the clinical setting. Thus high index of suspicion is critically needed for diagnosis.

<P>This paper presents a biomimetic turtle flipper actuator consisting of a shape memory alloy composite structure for implementation in a turtle-inspired autonomous underwater vehicle. Based on the analysis of the Chelonia mydas, the flipper actuator was divided into three segments containing a scaffold structure fabricated using a 3D printer. According to the filament stacking sequence of the scaffold structure in the actuator, different actuating motions can be realized and three different types of scaffold structures were proposed to replicate the motion of the different segments of the flipper of the Chelonia mydas. This flipper actuator can mimic the continuous deformation of the forelimb of Chelonia mydas which could not be realized in previous motor based robot. This actuator can also produce two distinct motions that correspond to the two different swimming gaits of the Chelonia mydas, which are the routine and vigorous swimming gaits, by changing the applied current sequence of the SMA wires embedded in the flipper actuator. The generated thrust and the swimming efficiency in each swimming gait of the flipper actuator were measured and the results show that the vigorous gait has a higher thrust but a relatively lower swimming efficiency than the routine gait. The flipper actuator was implemented in a biomimetic turtle robot, and its average swimming speed in the routine and vigorous gaits were measured with the vigorous gait being capable of reaching a maximum speed of 11.5 mm s(-1).</P>

<P>X-linked hypophosphatemic rickets (XLH) results from mutations in the <I>PHEX</I> gene. Mutational analysis of the <I>PHEX</I> gene in 15 unrelated Korean patients with hypophosphatemic rickets revealed eight mutations, including five novel mutations, in nine patients: two nonsense mutations, two missense mutations, one insertion, and three splicing acceptor/donor site mutations. Of these, c.64G>T, c.1699C>T, c.466_467 insAC, c.1174-1G>A, and c.1768+5G>A were novel mutations. To analyze the correlation between genotype and phenotype, phenotypes were compared between groups with and without a mutation, in terms of mutation location, mutation type, and sex. Skeletal disease tended to be more severe in the group with a mutation in the C-terminal half of the <I>PHEX</I> gene, but no genotype-phenotype correlation was detected in other comparisons. Further extensive studies of the <I>PHEX</I> gene mutations and analyses of the genotype-phenotype relationships are required to understand PHEX function and the pathogenesis of XLH.</P>

Background: There have been a few reports suggesting that N-acetylcysteine (NAC) which is mucolytic and antioxidant could destroy biofi lm formed by Helicobacter pylori ( H. pylori). The aim of this study was to identify whether the addition of NAC to fi rstline sequential therapy for H. pylori infection could improve the eradication rate. Methods: Patients with H. pylori infection were randomly assigned to receive either sequential therapy with (SQT+NAC group, n = 49) or without (SQT-only group, n = 50) NAC. Sequential therapy consisted of rabeprazole 20mg and amoxicillin 1g for the fi rst 5 days, followed by rabeprazole 20mg, clarithromycin 500mg and metronidazole 500mg for the remaining 5 days; all drugs are given twice daily. For SQT+NAC group, NAC 400mg b.i.d. was added for the fi rst 5 days of sequential therapy. H. pylori eradication was evaluated 4 weeks after completing therapy. Results: The eradication rates by intention-to-treat analysis were 58.0% in SQT-only group and 67.3% in SQT+NAC group, respectively (P = 0.336). The eradication rates by per-protocol analysis were 70.0% in SQT-only group and 80.5% in SQT+NAC group, respectively (P = 0.274). Compliance was very good in the both groups (SQT-only/ SQT+NAC group: 95.2%/100%, P = 0.494). There was no signifi cant difference in the adverse event rates between the groups (SQT-only/SQT+NAC group: 26.2%/26.8%, P = 0.947). Conclusions: The addition of NAC to first-line sequential therapy for H. pylori infection resulted in an approximately 10% increase in eradication rate. However, this feature did not reach statistical signifi cance, indicating that larger trials are needed

<P>The treatment of choice for remnant gastric cancer is resection by open conventional method, but due to adhesion and deformed anatomic structure, reoperation is one of the most complicated surgeries. We therefore introduce 2 cases of laparoscopy-assisted completion total gastrectomy. One was 67 yrs old male who had radical subtotal gastrectomy with gastrojejunostomy due to stomach cancer 30 years ago. Gastric polyp was found in routine EGD. Biopsy results showed focal adenocarcinoma and laparoscopy-assisted total gastrectomy with Roux-en-Y esophagojejunostomy was decided and performed. The other case was 65 yrs old male who went through radical subtotal gastrectomy with gastroduodenostomy 8 years ago due to stomach cancer. Recur was diagnosed by routine EGD, and laparoscopic assisted total gastrectomy with Roux-en-Y esophagojejunostomy was done. As can be seen in this study, laparoscopy-assisted gastrectomy could be safely applied in remnant gastric cancer.</P>

Background: To identify potential molecular prognostic markers in core binding factor (CBF) AML, we analyzed incidences and prognostic impacts of mutations in c-KIT, WT1, CEBPA, CBL, and a number of epigenetic genes in CBF AML. Methods: Seventy one and 21 AML patients with t(8;21) and inv(16) were enrolled in this study, respectively. NPM1, CEBPA, c-KIT, IDH1/2, DNMT3A, EZH2, WT1, and CBL mutations were analyzed by direct sequencing. Patients were categorized with respect to c-KIT and WT1 mutation status, and both clinical features and prognoses were compared. Results: The incidences of FLT3 internal tandem duplication (ITD), NPM1, CEBPA, IDH1/2, DNMT3A, EZH2, and CBL mutations were low (≤5%) in CBF AML patients. However, c-KIT and WT1 mutations occurred frequently (10.9% and 13.8%, respectively). t(8;21) patients with c-KIT mutations showed significantly shorter overall survival (OS) and disease free survival (DFS) periods than those without mutations (P<0.001, for both); however, although the limited number of t(8;21) patients were analyzed, WT1 mutation status did not affect prognosis significantly. Relapse or death during follow-up occurred more frequently in t(8;21) patients carrying c-KIT mutations than in those without the mutation, although the difference was significant only in a specific patient subgroup with no WT1 mutations (P=0.014). Conclusions: The incidences of mutations in epigenetic genes are very low in CBF AML; however, c-KIT and WT1 mutations occur more frequently than others. The poor prognostic impact of c-KIT mutation in t(8;21) AML patients only applies in a specific patient subgroup without WT1 mutations. The prognostic impact of WT1 mutation in CBF AML is not evident and further investigation is required.
