        급성흉통으로 내원한 환자에서 심근경색증을 조기 발견하기 위한 TROP-T?Rapid Assay의 유용성

        Study Objectives : In the past, measurement of cardiac enzymes(i.e. CK and/or CK-MB, GOT, LDH)and myoglobin, cardiac specific serum troponin-T were widely used for the diagnosis of acute myocardial infarction, but it need a long time and have low specificity. Thus new diagnostic modality, TROP-T?? Rapid Assay, is very easy and rapid qualitative methods, is needed for the early detection of acute myocardial infarction. Methods : Total CK, GOT, LDH, myoglobin and serum troponin-T activities were measured in patients presenting with acute chest pain at the emergency department and at the same time TROP-T? Rapid Assay was done. Final diagnosis and diagnostic usefulness was evaluated. Results : Overall diagnotic sensitivity and specificity of initial results were as follows; TROP-T? Rapid Assay, 77.3, 100.0; myoglobin, 63.6, 70.4; CK, 95.5, 96.3; GOT, 86.4, 96.3; LDH, 90.9, 92.6; serum troponin-T, 86.4, 96.3, respectively. Conclusion : The TROP-T?? Rapid Assay is simple method and has a short time and nearly same sensitivity and specificity compared to other cardiac enzymes. Thus this is considered useful method in the early detection of acute myocardial infarction at emergency department.

        A systematic construction and re-use of technology related to the product development and production has been the most important for the semiconductor industry dependent on process and equipment Therefore, numerous outputs in the form of paper has been produced in the process of information management ranging from the creation to recycling and disposal of technologies. In this research, the technology and documents necessary for the business management in the field of semiconductor manufacturing were classified in an effort to solve problems while the modeling of document management architecture at the enterprise level was performed by properly setting up the security system to prevent the unauthorized disclosure of the product development technology to the third parties. Especially, the product and process specification are designed in such a way as to ensure a real-time response in interface with the production system in order to shorten the development lead-time and improve the productivity. This paper is to discuss the modeling approach, the strategy to construct the system and its results.

        외상성 간손상과 비장손상시 CT분류의 의의

        The Organ Injury Scaling Committee of the American Association for the Surgery of Trauma recently published a consensus classification of hepatic and splenic injuries. The hepatic and splenic injury scales, based on parenchymal laceration and intrahepatic hematoma, include grade 1 to 6 and grade 1 to 5 respectively, representing the least to most severe injury. Forty-eight patients with hepatic and splenic injury were evaluate by abdominal CT from January 1995 to May 1996 and we compared abdominal CT grading with operative grading. Four CT grades did not correlate with operative finding. CT grading showed a sensitivity of 80%, specificity 80% in regard to operative grading. We conclude that 1) CT is an accurate technique to determine the extent of hepatic and splenic injury. 2) CT grading of hepatic and splenic trauma has a high correlation with operative grading.

        2인과 3인의 기본 심페소생술 비교

        Study objectives: To compare the quality of CPR provided by medical students and pre-EMTs perfoming three-rescuer CPR with that achieved by same students trained to provide standard two-rescuer CPR and to find the most effective method of resuscitation and to educate this methods to students. Material and methods: From July 1 to September 30, 1996, we had trained 60 groups consisted of the medical students, students of EMS technology and pre-EMT. Each group practiced two-rescuer CPR and three-rescuer CPR for 120 minutes with Resusci? Anne and perfomed CPR for at least 3 minutes without feedback. To eliminate any visual cues of perfomance the mannequin's recorder was hidden. Each actions were recorded by Resusci? skillmeter and Laerdal printer. To compare the quality of two-rescuer CPR and three-rescuer CPR, we analyzed the data by SPSS with paired t-test. Results: Three-rescuer teams delivered a mean minute ventilation and a mean minute cardiac compression substantially greater than that produced by two-rescuer teams(11.30±1.85 vs 9.09±2.22, p<0.001; 99.22±12.42 vs 95.12±15.75, p<0.001). Three-rescuer teams produce the more correct actions in chest compression and ventilation(93.84±7.56% vs 83.49±16.94%, 90.90±9.33% vs 77.00±17.06%). Conclusion: Three-rescuers can produce better CPR than two rescuers when a bag-valve-mask devices is used and the technique is easily learned and readily retrained, so we think that repeated education and training of this CPR methods to students is needed.

        Background : Cord blood(CB), which has no HLA restriction, is an alternative to bone marrow for hematopoietic stem cell transplantation. The use of cord blood, however, is limited by the number of progenitor/stem cells necessary to reconstitute the older child or adult. Therefore, ex vivo expansion of CB could have tremendous impact on diverse clinical settings. We studied the ex vivo expansion of isolated population of CD34_(+) cells from cryopreserved CB cells. Methods : CD34 cells were isolated from cryopreserved CB mononuclear cells. Purified cells were cultured with various combinations of hematopoietic growth factors including erythropoietin(EPO), stem cell factor(SCF), granulocyte-colony-stimulating factor(G-CSF), gra-nulocyte, macrophage-colony-stimulating factor(GM-CSF), interleukin-1β(IL-1β), 1L-3, and IL-6. After 7, 10 or 14 days of culture, the fold increases of colony-forming unit- granu-locyte, macrophage(CFU-GM), burst-forming unit-erythroid(BFU-E), colony-forming unit-mix (CFU-Mix), and high proliferative potential colony-forming cell(HPP-CFC) were evaluated. Results : Ten-day culture with the combination of EPO, SCF, G-CSF, IL-1β, and IL-3 resulted in a median of 60-fold increase of CFU-GM, which was greater than those with the combinations of less than 5 growth factors. The addition of IL-6 or GM-CSF to this combination did not enhance CFU-GM expansion. Ten-day culture was significantly superior to 7-day culture for CFU-GM expansion. Prolongation of culture to 14 days, however, revealed decreased expansion of CFU-GM compared to 10 days. BFU-E and CFU-Mix were expanded to 2~5 folds in 7-day culture with the combination of EPO, SCF, and G-CSF. Further expansion was not achieved in 10-day culture and colonies disappeared in 14-day culture. HPP-CFC was expanded to a median of 7.5 folds in 7-day culture with the combination of EPO, SCF, G-CSF, IL-1β, IL-3, and IL-6. Neither 10-day or 14 day-culture enhanced expansion of HPP-CFU. Conclusion : Cryopreserved cord blood cells maintain ex vivo expansion potential. In our system, 10-day culture with the combination consisting of EPO, SCF, G-CSF, IL-1β, and IL-3 seems to be adequate for hematopoietic progenitor/stem cell expansion from cryopreserved cord blood cells.

        응급센터 입원환자의 통증처치에 대한 응급의학적 고찰

        Study objective: To recognize importance of pain management in ED and properly manage the ED patients complaining pain by analyzing the intervals between arrival time in ED and pain medicine starting time, the types and the administered routes of pain medicine. Design: Retrospective review of ED charts Setting: University teaching hospital Participants: The patients had admitted in ED of Yeungnam University Hospital with a chief complaint of acute abdominal pain or pain due to bone fracture between June 1, 1995 and December 31, 1995. Results: The study group consisted 256 patients, 161 of them female. ED administrations of analgesics were 103 cases, 17 of them administered more than two times. The average time interval between arrival time and medication time was 169.02±120.25 minutes. Each types of perceived analgesics were antispasmodics 55 cases(53.40%), NSAID 26 cases(25.24%) and opioid 19 cases(18.45%). Every analgesics were administered through intramuscular routes, except only 3 cases. Violences of patients or relatives due to delayed pain control were occured 32 cases, 5 cases of them self-discharged and looked for other hospitals. Conclusion: Emergency physicians including interns and residents delayed management so long and ordered inappropriate administration routes and types of analgesics. So we thought proper protocols about pain management in ED were established.

        인천 인현동 호프집 화재 피해자 분석

        Purpose : This study was conducted to develop field triage, transportation, distribution, and prehospital care at a fire disaster by analyzing the victims of the fore that broke out at a bar in Incheon. Method : We analyzed the cases of the victims of a fire in Incheon in Oct. 1999. We determined the primary care hospital, the arrival time, the burn size, the outcome, and the injury type from the medical records, the concerned organ records, and interviews with concerned persons. Result : The total number of victims was 137: 56 prehospital deaths, 1 hospital death, and 80 survivals. The Pearson correlation coefficient between the burn size and the severity was -0.175. There were 121(89.6%) cases of inhalation injury, 59 (43.7%) cases of flame burns, 66 (48.9%) cases of hypoxic brain damage, and 16 (11.9%) cases involving other types of injury. Conclusion : The causes of death of the fire victims were inhalation injury and hypoxic brain damage due to CO poisoning and other toxic inhalants. We propose the use of a simple triage and rapid treatment(START) system and a reassessment the delayed category in fire disasters.

      • A 1.0-ns/1.0-V Delay-Locked Loop With Racing Mode and Countered CAS Latency Controller for DRAM Interfaces

        <P>The digital delay-locked loop (DLL) with racing mode and the countered column address strobe (CAS) latency controller are proposed in this paper. The dual-DLL architecture with racing operation is adopted to achieve low power consumption, low jitter, fast locking, wide range of locking, and stuck-free control. The merged dual coarse delay line (MDCDL) reduces the dynamic power consumption of a variable delay line by 30% by sharing a part of the delay line path in DLL. In addition, jitter is reduced by 45 ps in the 1066-DDR3 operating mode by MDCDL. The proposed DLL utilizes an or-and functioned duty cycle corrector (or-and DCC), which consumes 15% of DLL's power, 0.915 pJ/Hz at tCK=1.5 ns and VDD=1.575 V. The countered CAS latency controller (CCLC) saves IDD3N current because it does not need a DLL clock and does not need to be activated for IDD3N (active non-power down) state. The DLL clock is enabled and CCLC is activated only when the read command is issued. This operation condition saves the IDD3N current by 60% with the proposed DLL. The proposed DLL is employed in 128 M×8 DDR3 SDRAM and 64 M×16 DDR3 SDRAM. The former and the latter are fabricated by 5×nm and by 4× nm DRAM process technology, respectively. Experimental results show that ±10% duty error of the external clock can be corrected to within ±2% duty error in less than 512 cycles of locking time under 1.5 ns of tCK. The proposed DLL and CCLC can operate above 1.0-GHz operating frequency at 1.2 V in 5× nm DDR3 SDRAM and at 1.0 V in 4× nm DDR3 SDRAM, respectively. The proposed DLL fabricated with 4× nm technology consumes 6.1 pJ/Hz at 1.575 V.</P>

        Control of Three-Phase Inverter for AC Motor Drive With Small DC-Link Capacitor Fed by Single-Phase AC Source

        <P>If a small dc-link capacitor is used in the dc link fed by a single-phase ac source, then the dc-link voltage severely fluctuates at twice of the source frequency. To handle this fluctuation, the concept of “average voltage constraint” is proposed in this study. On the basis of this concept, a flux-weakening scheme, generating the d-axis current reference (i<SUB>ds</SUB><SUP>r</SUP>*) for the interior permanent-magnet synchronous machine, is devised. The q-axis current reference (i<SUB>qs</SUB><SUP>r</SUP>*) is modified for the unity power factor operation in the viewpoint of an ac source without an additional sensor. The proposed scheme has been applied to the inverter-driven 1-kW compressor of an air conditioner. From the experimental results, it has been verified that the compressor operates well at the required operating condition, regardless of the severe fluctuation of dc-link voltage, because of the reduced dc-link capacitance. The frequency spectrum of the ac source current reveals that the harmonics of the source current meet the regulation of IEC 61000-3-2 Class A and that the overall power factor is above 96% without any additional circuit, such as an input filter and a power factor correction circuit.</P>
