      • ATYPICAL MESOBLASTIC NEPHROMA 1례

        Atypical mesoblastic nephroma is a rare infantile renal tumor that may behave aggressively in older infants. The features that distinguish atypical mesoblastic nephroma from congenital mesoblastic nephrorma are as follows; (1) atypical gross features consisting of one or more of the follwing: flesh areas, foci of hemorrhage, necrosis, involvement of adjacent structures; and (2) high cellularity and mitotic index. Treatment is not established whether additional modes of therapy are indicated in completely resected atypical mesoblastic nephroma. A close and regular periodic follow-up is indicated in all cases of atypical mesoblastic nephroma. A case of atypical mesoblastic nephroma in 45-day-old-male is presented with brief review of literatures.

        치과용 디지털 색상 분석용 기기의 정확성과 재현 능력에 대한 평가

        본 연구에서는 치과용 디지털 색상 분석기기 중 하나인 ShadeScan (CYNOVAD, Montreal, Canada)의 색상 분석의 정확성과 재현 능력을 평가하였다. 균일한 치아 배열을 가진 20대 성인 남자 8명과 여자 8명의 상악 인상을 고무인 상제 (Exaflex, GC, Japan)를 이용하여 채득하고, 인상에 A2 색상의 임시 크라운 제작용 자가중합형 레진을 주입하여 치아 모델을 완성하고, ShadeScan을 이용해 각 치아의 영상을 얻어 컴퓨터의 ShadeScan 프로그램으로 분석하였다. 그리고 측정 결과의 재현 능력를 평가하기 위해 2개의 치아 모델을 임의로 선택하여 각 치아마다 10회의 색상 측정을 시행하여 분석 결과를 비교하였다. 또한 분석된 영상의 동일 shade 간의 색차를 확인하기 위해 광중합 복합레진인 Gradia Direct (GC, Tokyo, Japan)와 shade guide의 CIE L^(*)a^(*)b^(*) 값을 백색과 흑색 배경판 상에서 spectrophotometer (Spectrolino, GretagMacbeth, USA)로 측정하고, ShadeScan에 의해 동일한 shade로 분석된 shade guide간의 색차를 계산하여 다음의 결과를 얻었다. 모든 치아에서 모델 제작 재료와 동일한 A2 색상므로 분석된 경우는 없었으며, 개개의 치아에서 전체 순면이 동일한 색상으로 분석된 예는 없었다. 또한 동일한 치아군에서 치아의 크기 , 형태 , 순면의 굴곡 등에 따라 서로 다른 색상 분포를 보였다. 재현성 평가에서 각 치아의 중앙부는 비교적 일정한 재현성출 보였으나, 절단부와 주변 부위는 재현성이 낮았으며, 동일한 shade로 분석된 shade guide 간에는 3 이상의 색차 (ΔE^(*))를 보였다. 이상의 결과는 치과용 디지털 색상 분석기기는 비록 높은 재현 능력은 가지고 있지만 정확성에 오류가 있음을 시사하며 , 따라서 이러한 기기를 이용한 색상 분석 시 좀 더 세밀한 주의와 다른 방법의 추가 사용이 필요하리라 사료된다. This study was done to evaluate the reliability of the digital color analysis system (ShadeScan, CYNOVAD, Montreal, Canada) for dentistry. Sixteen tooth models were made by injecting the A2 shade chemical cured resin for temporary crown into the impression acquired from 16 adults. Surfaces of the model teeth were polished with resin polishing cloth. The window of the ShadeScan handpiece was placed on the labial surface of tooth and tooth images were captured, and each tooth shade was analyzed with the ShadeScan software. Captured images were selected in groups, and compared one another. Two models were selected to evaluate repeatability of ShadeScan, and shade analysis was performed 10 times for each tooth. And, to ascertain the color difference of same shade code analyzed by ShadeScan, CIE L^(*)a^(*)b^(*)values of shade guide of Gradia Direct (GC, Tokyo, Japan) were measured on the white and black background using the Spectrolino (GretagMacbeth, USA), and Shade map of each shade guide was captured using the ShadeScan. There were no teeth that were analyzed as A2 shade and unique shade. And shade mapping analyses of the same tooth revealed similar shade and distribution except incisal third. Color difference (ΔE^(*)) among the Shade map which analyzed as same shade by ShadeScan were above 3. Within the limits of this study, digital color analysis instrument for dentistry has relatively high repeatability, but has controversial in accuracy.

      • 비용함수 최대화에 의한 에지추출

        에지추출은 영상인식 시스템에서 매우 중요한 단계이다. 지금까지 많은 에지추출 방법들이 연구되어 왔음에도 불구하고 에지추출 문제는 여전히 어려운 문제로 남아있다. 기존의 기울기 연산자나 표면접합에 의한 에지추출과 달리 본 논문에서는 에지추출을 에지평가 비용함수의 최대화 문제로 다루었다. 에지평가 비용함수는 에지크기,에지연속성,에지굵기 그리고 에지길이를 기준으로 하여 구성하였으며,이와같은 에지평가 비용함수를 최대화 시키는데 유전자 알고리즘을 사용하였다. 그리고 유전자 알고리즘의 탐색성능을 향상시키기 위하여 Crossover연산의 3요소를 중심으로 유전자 알고리즘(MCPC)을 구성하였으며,다른 에지추출 연산자보다 좋은 에지추출 성능을 보여주었다. Eddge detection is the first step and very important step in image analysis. We cast edge detection as a problem in cost maximization. This is acheived by the formulation of a cost function that evaluates the quality of edge conFigurations. The cost function can be used as a basis for comparing the performances of different detectors. We used a Genetic Algorithm for maximizing cost function. Genetic algorithms are a class of adaptive search techniques that have been intensively studied in recent years and have been prome to converge prematurely before the best solution has been found. This paper shows that carefully chosen modifications(three factors of the crossover operator) are implenented can be effective in alleviating this problem.

        A patient with uncontrollable postoperative renal hemorrhage was successfully treated by selective injection of gelfoam into the anterior inferior segmental branch of right renal artery. Follow up arteriography was done 16 days later. It revealed persistent occlusion of the bleeding vessels. This method is a simple effective procedure in cases of renal hemorrhage to diverse etiology without responsive management.

      • 차류에 의한 찻물중 중금속(Cd, Pb)제거 효과

        차재료를 찻물에 담근후 차건데기를 건저내고 마시는 차류의 중금속 제거능을 조사하기 위하여 Cd, Pb 용액에서 보리차, 옥수수차, 쟈스민차, 현미녹차, 홍차, 결명자차, 감껍질차, 귤껍질차, 누룽지의 입자별, 농도별, 온도별 제거정도를 조사한 결과는 다음과 같다. 입자의 크기가 작아질수록 모든 차에 있어서 중금속 제거력은 증가 되었고, 금속별 제거량은 Cd는 결명자차(3.253-6.136mg/g)가, Pb는 쟈스민차(6.784-7.557mg/g)가 높았다. 농도별 중금속 제거량은 중금속의 농도가 증가할 수록 증가하였는데 Cd와 Pb는 쟈스민차(1.209-4.815mg/g)가 제거력이 높았다. 농도별 중금속의 제거작용은 모든 차류에서 증가되었으나 옥수수차는 Pb에서 0.497-0.647mg/g로 제일 낮았다. 온도별 중금속 제거량은 온도가 높아질수록 증가하여 Cd은 결명자차가 4.080-7.799mg/g, Pb는 쟈스민차가 7.003-7.763mg/g으로 증가하는 경향을 보였다. 이상의 결과로 부터 맹물로 끓여마시는 것보다는 각종 차를 이용하여 찻물을 만들어 마시면 음용수에 함유된 중금속을 제거하는데 도움이 된다. To investigate heavy metals(Cd and Pb) removablity by the tea materials (barley-tea, corn-tea, jasmin-tea, brown rice-green tea, black-tea, cassia tora-tea, persimmon peel-tea, orange peel-tea and rice-tea) in the tea-water, heavy metal removability of nine kinds of tea was investigated in the various conditions; particle size of tea(20, 40, 70 mesh), concentration of heavy metal(25, 50, 100 ppm) and extraction temperature(20, 30, 50, 100℃). The removabilities by the tea solids were increased as the particle size decreased, concentration of heavy metal increased, and extracting temperature increased. Of the nine kinds of tea, Cd and Pb removability by jasmin-tea was highest. In addition, heavy metal removability by cassia tora tea was better than that of other tea as extraction temperature was increased.

      • 거대적아구성빈혈의 임상적 고찰

        Megaloblastic anemias are disorders caused by impaired DNA synthesis and chara-cterized by the presence of megaloblastic cells. Clinical data on these disorders in Korea are limited. The aim of this study was to analyze clinical characteristics of megaloblastic anemias caused by vitamin B12 deficiency. We identified 40 cases who were diagnosed as megaloblastic anemia from January 1993 to December 2002 in a retrospective study on the basis of chart review. We investigated the annual distribution of diagnosed patients, clinical manifestations and response to treatment. 19 cases were complicated with a previous total gastrectomy(gastrectomy group) and 21 cases had no history of surgical operation(non-gastrectomy group). The most common presenting complaint was generalized weakness. The median age at diagnosis was 7(range: 38-79) years in non-gastrectomy group. The mean pos-toperative duration was 7(range: 4-25)years in group of total gastrectomy and mean level of serum vitamin B12 was lower than 100 pg/mL in both group. Peripheral blood revealed thrombocy-topenia, macrocytic anemia and hypersegmented neutrophil in all cases. After treatment of pareliteral vitamin B12, Their clinical symptoms and hematologic findings improved except two patients who had bone marrow with lower cellularity. Cobalamin replacement after total gastrectomy is still missing. Diagnoses are made too late in a population of patients, resulting in irreversible neurological sequale, and we suggest that Pernicious anemia seems not to be very rare in Korea.

      • Geographic Genetic Contour of A Leaf Beetle, Chrysolina aurichalcea (Coleoptera: Chysomelidae), on the Basis of Mitochondrial COI Gene and Nuclear ITS2 Sequences

        The leaf beetle, Chrysolina aurichalcea (Coleoptera: Chysomelidae), is a pest damaging plants of Compositae. In order to understand the genetic diversity and geographic variation of the species we sequenced a portion of mitochondrial COI gene (658 bp) and complete nuclear internal transcribed spacer 2 (ITS2) collected from seven Korean localities. A total of 18 haplotypes (BARCA01 ~ BARCA18), with the maximum sequence divergence of 3.04% (20 bp) were obtained from COI gene sequence, whereas 17 sequence types (ITS2CA01 ~ ITS2CA17), with the maximum sequence divergence of 2.013% (9 bp) were obtained from ITS2, indicating substantially larger sequence divergence in mitochondrial gene sequence. Phylogenetically, the mitochondrial DNA has shown several haplotypes formed independent groups with substantially high node support (≥ 90%), whereas no such grouping was evidenced for ITS2, indicating different behaviors of the two molecules. Such difference may reflect a diverse dynamics of the species such as biogeographic history, mating behaviors, and also possibly different mode of inheritance of the two molecules, but requires further scrutinized examination of the dataset. In terms of population genetic perspective, overall no population subdivision was detected from both molecules, except for locality 7 (Eocheong islet) from mitochondrial DNA. As more scrutinized analysis is performed, further fruitful inference on the geographic contour of the species might be available.
