Dermoscopic features of large cell acanthoma

Background: Large cell acanthoma (LCA) is a sharply demarcated epidermal lesion composed of large keratinocytes. There is a lack of consensus regarding whether it represents a distinct benign entity or a variant of other diseases. LCA commonly mimics other lesions such as solar lentigo and seborrheic keratosis. Dermoscopy is a noninvasive method of diagnosis which allows the visualization of pigmented and vascular structures. There was no previous study focusing on dermoscopic features of LCA. Objectives: To investigate characteristic dermoscopic patterns of LCA, and to find distinctive features that can differentiate them from other lesions. Methods: Clinical features and dermoscopic patterns were evaluated in 13 patients, histologically diagnosed as LCA. Results: The patients were between 48 and 94 years of age with mean age of 68.1 years. The common site of occurrence was leg. The dermoscopic features showed yellow opaque homogeneous area, gray and brown dots and globules, moth-eaten border, short white streaks, and pseudonetwork (100%, 69.2%, 46.2%, 38.5%, and 30.8%, respectively). Otherwise, milia-like cyst and white-to-yellow surface scale (23.1% and 7.7%, respectively) were uncommon findings. Conclusion: Dermoscopy provides valuable information for the diagnosis of LCA and can be useful in the differential diagnosis of other pigmented skin lesions.
Late Type III Endoleak after Loss of Component Overlap after EVAR with AFX2 Device: A Case Report

Addressing the high incidence of late type III endoleaks in previous AFX models, Endologix upgraded the device material and updated its recommendation regarding component overlap. However, whether upgraded AFX2 models are safe for endoleaks remains controversial. Here we report a case of a 67-year-old male with an AFX2-implanted abdominal aortic aneurysm experiencing a delayed type IIIa endoleak. Aneurysmal sac enlargement occurred 36 months post-endovascular aneurysm repair (EVAR), with a computed tomography scan at 52 months revealing component overlap loss and a significant type IIIa endoleak. We performed endograft explantation and endoaneurysmal aorto-bi-iliac interposition grafting. Our findings suggest that sufficient component overlap is necessary when using an AFX2 endograft outside the manufacturer’s instructions for use to prevent late type IIIa endoleaks. Moreover, patients who undergo EVAR with AFX2 for tortuous large aortic aneurysms should be carefully monitored for conformational changes.

The Electro-membrane process has advantages in sustainability, scalability, and eco-friendliness, so it is used in various fields such as electrodialysis, reverse electrodialysis, and PEMFC. In this study, the effect of the homogeneity and heterogeneity of the IEM (ion exchange membrane) on electroconvection and flow is investigated by visualizing the flow in micro channels. Nafion211 used in PEMFC was used as a homogeneous IEM, and Ralex Membrane was used as a heterogeneous IEM. As a result of the experiment, in the case of a heterogeneous IEM, electroconvection occurs quickly due to the heterogeneity of the surface, and the number of electroconvection is larger and the size is larger. In the experiment to which a non-conductive pattern of the surface using ultra-fine laser processing is given, the size of the electroconvection is formed smaller as the pattern size decreases.
