        Role of dexmedetomidine in pediatric dental sedation

        Dexmedetomidine is a highly selective ${\alpha}2$-adrenoceptor agonist with a vast array of properties, making it suitable for sedation in numerous clinical scenarios. Its use was previously restricted to the sedation of intensive care unit patients. However, its use in pediatric dental sedation has been gaining momentum, owing to its high suitability when compared with conventional pediatric sedatives. Its properties range from sedation to anxiolysis to analgesia, due to its sympatholytic properties and minimal respiratory depression ability. Because dexmedetomidine is an efficacious and safe drug, it is gaining importance in pediatric sedation. Thus, the aim of this review is to highlight the properties of dexmedetomidine, its administration routes, its advantages over the commonly used pediatric sedatives, and especially its role as an alternative pediatric sedative.

        Higher Prevalence of Metabolic Syndrome in Child-adolescent Patients with Bipolar Disorder

        Objective: Previous studies have indicated a convergent and bidirectional relationship between metabolic syndrome (MetS) and bipolar disorder (BD). As most of these studies focused mainly on adults diagnosed with BD, our study aims to investigate and characterize metabolic disturbances in child-adolescents diagnosed with BD. Methods: We retrospectively examined the medical records of psychiatric hospitalizations with admitting diagnosis of BD in child-adolescents (age < 18 years). Body mass index (BMI), lipid profile, fasting blood glucose, and blood pressure were primary variables. National Cholesterol Education Program criteria were used to define MetS. Reference group data was obtained from the National Health and Nutrition Examination Survey study. Statistical analyses included t tests, chi-square tests, and Fisher’s exact tests. Results: We identified 140 child-adolescent patients with BD (mean age = 15.12 ± 1.70 years, 53% male). MetS was significantly more common in BD compared to the reference group: 14% (95% confidence interval [95% CI] 8−20) vs. 6.7% (95% CI 4.1−9.2), p = 0.001 with no significant difference by sex. MetS components were higher in the BD group, particularly BMI ≥ 95% (25% vs. 11.8%, p < 0.001) and high blood pressure (17% vs. 8%, p = 0.05). Moreover, female patients had lower odds of high blood pressure (odds ratio = 0.24 [95% CI 0.08−0.69], p = 0.005). Conclusion: Compared with the general child-adolescent population, the prevalence of MetS was significantly higher in patients with BD of same age. This reiterates the notion of an increased risk of MetS in patients diagnosed with BD; and thus, further exploration is warranted.

        Non-Surgical Management of Cord Compression in Tuberculosis: A Series of Surprises

        Study Design: Prospective study. Purpose: We present a series of 50 patients with tuberculous cord compression who were offered systematic non-surgical treatment, and thereby, the author proposes that clinico-radiological soft tissue cord compression is not an emergency indication for surgery. Overview of Literature: Spinal cord compression whether clinical or radiological has usually been believed to be an indication for emergency surgery in spinal tuberculosis. Methods: Fifty adults were prospectively studied at our clinic for spinal cord compression due to tuberculous spondylitis, between May 1993 and July 2002. The inclusion criteria were cases with clinical and/or radiological evidence of cord compression (documented soft tissue effacement of the cord with complete obliteration of the thecal sac at that level on magnetic resonance imaging scan). Exclusion criteria were lesions below the conus level, presence of bony compression, severe or progressive neurological deficit (<than Frankel grade C) and children below the age of maturity. All patients were treated with a fixed, methodically applied non-surgical protocol including hospital admission, antitubercular medications, baseline somatosensory evoked potentials and a regular clinicoradiological follow-up. Results: At the time of presentation, 10 patients had a motor deficit, 18 had clinically detectable hyper-reflexia and 22 had normal neurology. Forty-seven of the 50 patients responded completely to non-operative treatment and healed with no residual neurological deficit. Three patients with progressive neurological deficit while on treatment were operated on with eventual excellent recovery. Conclusions: Radiological evidence of cord compression and early neurological signs need not be an emergency surgical indication in the management of spinal tuberculosis.

        Effect of solution concentration on physicochemical and gas sensing properties of sprayed WO3 thin films

        Sub-stoichiometric tungsten trioxide (WO3) thin films are deposited onto the glass substrates by spray pyrolysis technique using ammonium metatungstate. Effect of solution concentration on structural, morphological, optical, electrical and NO2 sensing properties of WO3 thin films is studied. Films are polycrystalline with monoclinic crystal structure and sub-stoichiometric as observed form the XRD and XPS studies, respectively. The SEM and AFM images show micro grained structure and surface roughness increases with increase in solution concentration. The PL studies revealed that the majority of the defects are the oxygen vacancies. From XPS and PL studies it is observed that, oxygen vacancies decrease with increase in solution concentration. The dielectric constant of the films as a function of frequency is in concurrence with resistivity measurements. Films show reproducible and reversible gas response at various operating temperatures and gas concentrations. Highest sensor response (38%) towards 200 ppm NO2 concentration is observed for the film with 15 mM solution concentration at moderate operating temperature (200 ℃). Pd sensitization enhanced gas response to 68% and improved kinetics of the sensor. Films are highly selective towards NO2 as compared with the various gases such as SO2, LPG, NH3 and H2S.

        Anti-hyperglycaemic and lipid lowering potential of Adenanthera pavonina Linn. in streptozotocin induced diabetic rats

        In India, Adenanthera pavonina is traditionally used in the treatment of diabetes mellitus and lipid disorders. In the present study, the antihyperglycaemic and lipid lowering effect of Adenanthera pavonina seed aqueous extract (APSAE) was evaluated using streptozotocin induced diabetes in rats. Streptozotocin was given at the dose of 55 mg/kg, i.p. After induction of diabetes, APSAE was administered for 30 days p. o. and simultaneously different biochemical parameters like plasma glucose, HbA1c, serum triglyceride, cholesterol, LDL-cholesterol and HDL-cholesterol were estimated. Diabetic control showed significant increase (P<0.01) in plasma glucose, serum triglyceride, cholesterol, LDL-cholesterol and significant decrease (P<0.01) in serum HDL-cholesterol and HbA1c. Treatment with APSAE showed significant reduction (P<0.01) in plasma glucose when compared with diabetic control. The elevated levels of serum triglyceride and cholesterol levels were significantly reduced (P<0.01) by APSAE. APSAE treatment for 30 days showed significant decrease in serum LDL-cholesterol (P<0.01) and significant increase in serum HDL cholesterol level (P<0.01). Moreover, diabetic control there was significant decrease in HbA1c which was significantly increased (P<0.05) by treatment with APSAE. Hence, from the result obtained in the present study it can be confirmed that Adenanthera pavonina has the potential to treat diabetes condition and associated lipid disorders.

        Development of Ag/WO3/ITO Thin Film Memristor Using Spray Pyrolysis Method

        The unique nonlinear relationship between charge and magnetic flux along with the pinched hysteresis loop in I-V plane provide memory with resistance combinations of attribute to Memristor which lead to their novel applications in non volatile memory, nonlinear dynamics, analog computations and neuromorphic biological systems etc. The present paper reports development of Ag/WO3/ITO thin film memristor device using spray pyrolysis method. The structural, morphological and electrical properties of the thin film memristor device are further characterized using x-ray diffraction (XRD), Scanning Electron Microscopy (SEM), and semiconductor device analyzer. The memristor is simulated using linear dopent drift model to ascertain the theoretical and experimental conformations. For the simulation purpose, the width of doped region (w) limited to the interval [0, D] is considered as a state variable along with the window function characterized by the equation f (x) = w (1 − w). The reported memristor device exhibits the symmetric pinched hysteresis loop in I-V plane within the low operating voltage (±1 V).

      • Role of Organic Counterion in Lead- and Tin-Based Two-Dimensional Semiconducting Iodide Perovskites and Application in Planar Solar Cells

        <P>Hybrid halide perovskites are emerging semiconducting materials, with a diverse set of remarkable optoelectronic properties. Besides the widely studied three-dimensional (3D) perovskites, two-dimensional (2D) perovskites show significant potential as photovoltaic (PV) active layers while exhibiting high moisture resistance. Here, we report two series of new 2D halide perovskite solid solutions: (HA)Pb1-xSnxI4 and (BZA)(2)Pb1-xSnxI4 (x = 1, 0.75, 0.5, 0.25, 0), where HA stands for the organic spacer histammonium and BZA stands for benzylammonium cations. These compounds are assembled by corner-sharing octahedral [MI6](4-) units stabilizing single-layered, anionic, inorganic perovskite sheets with organic cations filled in between. The optical band gaps are heavily affected by the M-I-M perovksite angles with the band gap steadily decreasing when the angle approaches 180 degrees, ranging from 2.18 eV for (BZA)(2)PbI4 to 2.05 eV for (HA)PbI4. We find an anomalous trend in electronic band gap in the mixed compositions (HA)Pb1-xSnxI4 and (BZA)(2)Pb1-xSnxI4. When Sn substitutes for Pb to form a solid solution, the band gap further decreases to 1.67 eV for (HA)SnI4. The minimum band gap is at x = 0.75 at 1.74 eV. For BZA, the irregular trend is more intense, as all the intermediate compounds (BZA)(2)Pb(1-x)SnxI(4) (x = 0.75, 0.5, 0.25) have even slightly lower band gaps than (BZA)(2)SnI4 (1.89 eV). DFT calculations confirm the pure Pb and Sn compounds are direct band gap semiconductors. Relatively shorter photoluminescence (PL) lifetime in (BZA)2PbI4 than (HA)PbI4 is observed, suggesting faster recombination rates of the carriers. Solution deposited thin films of (HA)PbI4 and (BZA)2PbI4 show drastically different orientations with (HA)PbI4 displaying a perpendicular rather than parallel growth orientation with respect to the substrate, which is more favorable for PV devices. The higher potential in PV applications of the HA system is indicated by device performance, as the champion air stable planar device with the structure ITO/PEDOT:PSS/2D-perovskite/PCBM/Al of (HA)PbI4 achieves a preliminary power conversion efficiency (PCE) of 1.13%, featuring an open-circuit voltage (VOC) of 0.91 V.</P>
