        Nocturia is the Lower Urinary Tract Symptom With Greatest Impact on Quality of Life of Men From a Community Setting

        Purpose: Lower urinary tract symptoms are numerous, but the specific impact of each of these symptoms on the quality of life (QoL) has not been evaluated in community-dwelling men. An assessment of these symptoms and their effects on QoL was the focus of this study. Methods: We performed a cross-sectional study with 373 men aged >50 years from a community setting. Patients completed the International Prostate Symptom Score questionnaire, which includes questions on each of the specific urinary symptoms and a question addressing health-related QoL that are graded from 0 to 5. We used the Pearson correlation test to assess the impact of each symptom on QoL. Results: Nocturia (58.9%) was the most prevalent urinary symptom. The mean score was 0.9±1.4 for incomplete emptying, 1.0±1.5 for frequency, 0.9±1.3 for intermittency, 0.8±1.3 for urgency, 1.0±1.5 for weak stream, 0.5±1.0 for straining, and 2.0±1.6 for nocturia. Nocturia and frequency were the only symptoms associated with poorer QoL, with nocturia showing a stronger association. Conclusions: Nocturia affects 50% of community dwelling men aged >50 years, and is the lower urinary tract symptom with the greatest negative impact on QoL.

      • Processing, structure and thermal conductivity correlation in carbon fibre reinforced aluminium metal matrix composites

        <P><B>Abstract</B></P> <P>Al matrix composites reinforced with Cu-coated pitch-based carbon fibres (Al/Cu-CFs) were fabricated, using a novel combination of rheocasting and equal channel angular extrusion (ECAE) techniques, in order to exploit the thermal conductivity (<I>K</I>) of the material. Rheocasting allowed the introduction and dispersion of Cu-CFs within the Al<SUB>3</SUB>Mg matrix. The subsequent ECAE processing reduced the porosity of the composites from 3 to 0.03% and induced a high degree of fibre alignment within the matrix, although considerable damage to the fibres occurred during this processing step. After 6 ECAE passes, in which the billet orientation remained constant, the composite with the highest degree of fibre alignment show a thermal conductivity (<I>K</I>) improvement of ~20% with respect to the rheocast composite. The improvement is due to porosity reduction, improved fibre alignment and forced intimate contact of clean CF surfaces with the matrix.</P> <P><B>Highlights</B></P> <P> <UL> <LI> Al/CFs composites TC increased by 19.6% using a novel combination of processing techniques (Rheocasting + ECAE). </LI> <LI> ECAE can be used to align fibres within the Al matrix and maximise the TC of the Al/CF composites. </LI> <LI> It is the first time that this combination of processing has been demonstrated to improve TC. </LI> <LI> Theoretical models are in agreement with experimental findings. </LI> </UL> </P> <P><B>Graphical abstract</B></P> <P>[DISPLAY OMISSION]</P>

        Factors influencing decision-making around opportunistic salpingectomy: a nationwide survey

        Objective: To explore current practice and influencing factors on adoption of the opportunistic salpingectomy (OS), particularly regarding the decision making, to eventually enhance the development and implementation of clear guidelines. Methods: This nationwide cross-sectional survey study was conducted in all hospitals in the Netherlands. An anonymous online survey was sent to gynecologists with special interest in gynecological oncology, gynecological endoscopy or urogynecology and all Dutch gynecology trainees. The survey mainly focused on current practice regarding OS and identification of influencing factors on the level of innovation, organization, healthcare professional and individual patient. Results: The response rate was 348 out of 597 gynecologists (58.3%) and 142 out of 340 trainees (41.8%). Current practice of discussing and performing the OS varied widely, with ovarian cancer (OC) risk reduction as most important supportive factor on innovation level. Supportive factors on the level of organization and healthcare provider were; working in a non-training hospital, knowledge of current literature and extensive work experience (in years and annual number of hysterectomies). On individual patient level, a vaginal approach of hysterectomy, negative family history for OC and the presence of firm adhesions were suppressive factors for the OS. Conclusion: In this study we evaluated the current practice regarding the opportunistic salpingectomy in the Netherlands and identified influencing factors on different levels to raise awareness and attribute to development of a targeted implementation strategy, on both national and international level.

        ENVIRONMENTAL COOPERATION IN NORTHEAST ASIA

        The future quality of the environment in Northeast Asia will depend not only on the level of national attention paid to pollution control and environmental protection, but also to the effectiveness of cooperation and policy coordination among states at the regional level. It will be very difficult for Northeast Asia to develop quickly effective regional regimes for environmental protection because of the differences that exist among the states in the region in terms of economic levels, policy priorities, and political system. Still, as environmental protection becomes more important in the region, regional environmental cooperation is emerging. It is a loose form of environmental cooperation that is enhancing dialogue among states of the region.

        스페인 이주여성의 한국 사회 적응에 관한 사례 연구

        이 연구는 그간 한국의 이주자 논의에서 주목받지 못했던 서구권 출신의 이주자 중 비영어권인 스페인 이주여성의 한국사회 적응과 전략을 심층인터뷰를 통해 분석하였다. 연구 결과 이들도 이주를 통해 보다 나을 삶을 추구하는 전략적 행위자로서의 역할을 확인했다. 또한 한국사회 적응과 관련하여 이들도 갈등을 겪고 있었으나, 이는 한국사회에 내재하고 있는 사회문제와 결부된 것이었다. 한국 사회에서 다문화주의가 실현되기 위해서는 이주자를 한국사회의 구성원으로 인정하고 공유하는 사회문제 해결을 위해 협력하는 것이 현실적인 대안이 될 것이다. This study conducts in-depth interview to analyze the adaptation process and strategies of Spanish migrant women to Korea, a sub-group of a migrant population from western countries that has often been excluded from migration debate in Korea. The results confirm the role of Spanish migrant women as strategic actors who try to pursue a better life through migration. They also experience conflicts in adapting to Korean society, however, these are related to social problems inherent in the Korean society. Accepting migrants as members of Korean society and cooperating together to solve shared social problems would become a realistic alternative to achieve a true multiculturalism in Korean society.

        Genetic Reassortment of Rice stripe virus RNA Segments Detected by RT-PCR Restriction Enzyme Analysis-based Method

        Our previous sequence and phylogenetic analyses of the Korean Rice stripe virus (RSV) suggested possible genetic reassortment of RNA segments, but whether this RNA variation contributed to the recent RSV outbreaks in Korea is yet unclear. To further clarify these RSVRNA segment variations, we developed a reverse transcription-polymerase reaction/restriction enzyme (RTPCR/RE) analysis-based method. We identified five REs, including DraI, EcoR1, NdeI/AseI, and SpeI, that could differentiate RSV RNA 1–4 subtypes, respectively. Our RT-PCR/RE results provided a clear pattern of RNA reassortment, i.e., different groups of isolates having their RNA segments derived from two to three different RSV ancestors, such as from Eastern and Southwestern Chinese or Japanese M and T isolates. We also found that the migratory small brown planthopper from Eastern China caught by aerial net traps that possesses RSVRNA3genotypes corresponds mainly to Eastern China,with a few for Southwestern China based on RT-PCR/RE, sequence and phylogenetic analyses, indicating that RSV populations in Eastern China may also have strong RNA variation. The development of an RE analysisbased method proved a useful epidemiological tool for rapid genotyping and identification of mixed infections by RSV strain and by different subtype.

        Scar Revision Surgery: The Patient's Perspective

        Background Insufficient satisfaction outcome literature exists to assist consultations for scar revision surgery; such outcomes should reflect the patient's perspective. The aim of this study was to prospectively investigate scar revision patient satisfaction outcomes, according to specified patient-selection criteria. Methods Patients (250) were randomly selected for telephone contacting regarding scar revisions undertaken between 2007-2011. Visual analogue scores were obtained for scars pre- and post-revision surgery. Surgery selection criteria were; 'presence' of sufficient time for scar maturation prior to revision, technical issues during or wound complications from the initial procedure that contributed to poor scarring, and 'absence' of site-specific or patient factors that negatively influence outcomes. Patient demographics, scar pathogenesis (elective vs. trauma), underlying issue (functional/symptomatic vs. cosmetic) and revision surgery details were also collected with the added use of a real-time, hospital database. Results Telephone contacting was achieved for 211 patients (214 scar revisions). Satisfaction outcomes were '2% worse, 16% no change, and 82% better'; a distribution maintained between body sites and despite whether surgery was functional/symptomatic vs. cosmetic. Better outcomes were reported by patients who sustained traumatic scars vs. those who sustained scars by elective procedures (91.80% vs. 77.78%, P=0.016) and by females vs. males (85.52% vs. 75.36%, P<0.05), particularly in the elective group where males (36.17%) were more likely to report no change or worse outcomes versus females (16.04%) (P<0.01). Conclusions Successful scar revision outcomes may be achieved using careful patient selection. This study provides useful information for referring general practitioners, and patient-surgeon consultations, when planning scar revision.

        한국 내 라틴아메리카에서 온 결혼이주여성의 모성 경험에 관한 연구

        Marriage migrant women residing in Korea face various difficulties (i.e. struggles with the Korean language) throughout motherhood experiences including pregnancy, childbirth, and parenting, in social and culturally unfamiliar environments. A large portion of the research conducted upon motherhood experiences in Korea focuses on marriage migrant women from Asian countries. However, socio- cultural factors, in both the countries of origin and settlement, influence the experience of motherhood. Therefore, it is necessary to extend the study to marriage migrant women from non-Asian regions to comprehensively identify the diversity of the perceptions and responses to motherhood conflicts. As ethnographic research, this study uses in-depth interviews and participant observation to examine the motherhood experiences of seven Latin American marriage migrant women living in Korea. By analyzing the collected data on marriage and migration process, pregnancy, childbirth, and childcare, the primary purpose of the study is to identify the conflicts that arise from their experiences of motherhood and how they deal with them through resistance and negotiation. The study analyzes motherhood experiences of Latin American women in Korea, within the context of migration, by focusing on significant topics such as differences in childbirth customs and experiences, difficulties due to lack of proficiency in Korean language, lack of support from native family; transnational lifestyle, the desire to preserve their native language and culture, or conflict with the Korean education system. Unlike marriage migrant women from Asian countries, Latin American marriage migrants emphasize preserving their native languages and cultures, and they show a strong desire to pass it down to their children. Furthermore, their active attitude in raising their children might be related to the fact that most of the women in the study come from large cities and have received education at the tertiary level. 국내 결혼이주여성들은 한국어 구사의 어려움 등 낯선 사회·문화적 환경 속에서 임신과 출산 및 육아 과정을 의미하는 모성 경험 과정에서 다양한 어려움에 직면한다. 결혼이주여성의 자녀 양육을 비롯한 모성 경험에 관한 국내 기존 연구의 대부분은 아시아 국가 출신 결혼이주여성을 대상으로 수행되었다. 그러나 모성 경험이 여성의 모국과 정착국가의 사회·문화적 환경의 영향을 받으며, 이 과정에서 야기되는 모성갈등의 인식과 대응 방식의 다양성을 포괄적으로 규명하기 위해서는 비아시아 지역의 결혼이주여성에 관한 연구도 필요하다. 이 연구는 국내 일부 라틴아메리카 결혼이주여성의 모성 경험을 규명하기 위해 이들에 대한 심층 인터뷰와 참여 관찰을 수행한 ‘민족지적 연구’(ethnographic research)에 해당한다. 이들 여성의 결혼과 이주 과정 및 임신·출산·육아에 대해 수집된 자료의 분석을 통해, 이들의 어머니 되는 경험에서 야기되는 모성갈등과 이의 해결을 위한 협상과 경합 등의 대응 방식의 규명이 이 연구의 주요 목적이다. 이 글은 출산 경험과 관습의 차이, 한국어 능력의 부족에서 오는 어려움, 본국 가족의 지지 부족, 초국적 생활방식, 모국어와 모국문화 보존 욕구, 한국 교육시스템과의 충돌 등의 주요 주제를 중심으로 이주라는 맥락 속에서 국내 라틴아메리카 여성들의 모성 경험을 분석하고 있다. 아시아 국가 출신 결혼이주여성들과는 달리, 라틴아메리카 결혼이주여성들은 자국 언어와 문화 보존을 강조하고 이를 자녀에게 계승시키고자 하는 강한 욕구를 보인다. 더 나아가 이들 여성의 대부분이 고학력에 대도시 출신이라는 점에서 자녀 양육 등에 적극적인 태도를 보이는 것으로 보인다.
