<P>Hematopoiesis involves a series of lineage differentiation programs initiated in hematopoietic stem cells (HSCs) found in bone marrow (BM). To ensure lifelong hematopoiesis, various molecular mechanisms are needed to maintain the HSC pool. CCCTC-binding factor (CTCF) is a DNA-binding, zinc-finger protein that regulates the expression of its target gene by organizing higher order chromatin structures. Currently, the role of CTCF in controlling HSC homeostasis is unknown. Using a tamoxifen-inducible CTCF conditional knockout mouse system, we aimed to determine whether CTCF regulates the homeostatic maintenance of HSCs. In adult mice, acute systemic CTCF ablation led to severe BM failure and the rapid shrinkage of multiple c-Kit<SUP>hi</SUP> progenitor populations, including Sca-1<SUP>+</SUP> HSCs. Similarly, hematopoietic system-confined CTCF depletion caused an acute loss of HSCs and highly increased mortality. Mixed BM chimeras reconstituted with supporting BM demonstrated that CTCF deficiency-mediated HSC depletion has both cell-extrinsic and cell-intrinsic effects. Although c-Kit<SUP>hi</SUP> myeloid progenitor cell populations were severely reduced after ablating <I>Ctcf</I>, c-Kit<SUP>int</SUP> common lymphoid progenitors and their progenies were less affected by the lack of CTCF. Whole-transcriptome microarray and cell cycle analyses indicated that CTCF deficiency results in the enhanced expression of the cell cycle-promoting program, and that CTCF-depleted HSCs express higher levels of reactive oxygen species (ROS). Importantly, <I>in vivo</I> treatment with an antioxidant partially rescued c-Kit<SUP>hi</SUP> cell populations and their quiescence. Altogether, our results suggest that CTCF is indispensable for maintaining adult HSC pools, likely by regulating ROS-dependent HSC quiescence.</P>

Endobronchial tuberculosis is defined as a tuberculous infection of the tracheobronchial tree and has a prevalence of up to 50% in active pulmonary tuberculosis cases. The most common complication of endobronchial tuberculosis is bronchial stenosis; benign fistula formation by endobronchial tuberculosis is rare, especially inter-bronchial fistula formation. We reported a rare case of a 73-year-old woman with a fistula between the right upper bronchus and bronchus intermedius. A diagnosis of inter-bronchial fistula caused by endobronchial tuberculosis was based on the results of chest computed tomography scans, bronchoscopy, and microbiological and pathological tests. The patient was treated with anti-tuberculous medication, and her symptoms gradually improved.

Economic reality supersedes political rhetoric, even in the hermit kingdom. North Korea is one of the most secluded countries in the world, and its self-imposed isolation and the resulting dearth of hard data make any systematic study of the nation a challenge. Despite the political and technical constraints, this paper nevertheless attempts to demystify North Korea’s claim of economic self-sufficiency. A longitudinal survey reveals the reasons why the Kim Il Sung regime had to make a major policy compromise by enacting the 1984 joint venture law. This paper shows the gradual evolution of North Korea’s economic stagnation that has since the mid 1980s led to economic openness towards the West. Ovarian reserve as a predicting factor for successful pregnancy after adenomyomectomy

Objective: To compile the fertility outcomes of adenomyomectomy and to evaluate which demographic factors predict a successful pregnancy in patients who have undergone adenomyomectomy. Methods: We retrospectively reviewed the patient data between January 2017 and April 2020. Possible causes for infertility were excluded by hysterosalpingography, blood tests, karyotyping, hormonal assay, and semen analysis before the surgery. Candidates were selected for surgery based on the following criteria: 1) patients aged ≤45 years, 2) patients who consented to proceed with the surgery after being informed about the possibility of recurrence of the disease and operative blood loss, and 3) patients desiring pregnancy in the future. Patients were followed up monthly for 3 months. The subsequent follow-ups were conducted every 3-6 months. Patients who wanted to get pregnant could try for pregnancy 3 months after the surgery. We defined pregnancy success as the presence of fetal heartbeat at 6-7 weeks of gestation. The number of pregnancy trials was defined as the number of embryos transferred. Results: Forty-three patients were divided into two groups: the pregnancy failure group (n=28) and the pregnancy success group (n=15). The age of patients was higher (39.0 [32.046.0] vs. 37.0 [33.042.0] years, P=0.006) and the level of anti-Mllerian hormone (AMH) (0.54 [0.018.54] vs. 2.91 [0.347.92] ng/mL, P=0.002) was lower in the pregnancy failure group than in the pregnancy success group. The operative time was longer (220.0 [68.0440.0] vs. 175.0 [65.0305.0] minutes, P=0.048) and the estimated blood loss was higher (750 [1002500] vs. 500 [502000] ml, P=0.016) in the pregnancy failure group than in the pregnancy success group. No significant difference was observed in body mass index, symptoms, cancer antigen 125, preoperative uterine volume, and type of adenomyosis. In the multivariate analysis, age and AMH were significant predictive factors for successful pregnancy. Conclusion: Ovarian reserve (age and AMH) and disease severity might be predictive factors for successful pregnancy in patients who have undergone adenomyomectomy. Adenomyomectomy should be positively considered for women desiring pregnancy and having appropriate ovarian reserve. This information will be beneficial for patients and clinicians before the decision about adenomyomectomy. Larger prospective studies are needed to confirm our findings.
