        Age, diabetes mellitus, and dialysis modality are associated with risk of poor muscle strength and physical function in hemodialysis and peritoneal dialysis patients

        Background: Due to the poor outcomes associated with the impairment of physical function and muscle strength in patients on maintenance dialysis, it is important to understand the factors that may influence physical function and muscle strength. The aim of this study was to explore the factors associated with physical function in hemodialysis and peritoneal dialysis patients. Methods: Patients with chronic kidney disease on dialysis for at least 3 months, aged 18 years old or above, were enrolled. Physical function was assessed by handgrip strength, gait and sit-to-stand tests, and the Short Physical Performance Battery (SPPB). Clinical and laboratory data were collected to verify the association with physical function parameters through binary logistic regression. Results: One-hundred ninety patients on maintenance dialysis were included; 140 patients (73.7%) on hemodialysis and 50 (26.3%) on peritoneal dialysis. The mean age was 57.3 ± 14.9 years, 109 (57.4%) were male, and 87 (45.8%) were older than 60 years. The median SPPB was 8.0 points (6.0-10.0 points) and the mean ± standard deviation of handgrip strength was 24.7 ± 12.2 kg. Binary logistic regression showed that age, type of renal replacement therapy, diabetes mellitus, and serum creatinine were significantly associated with both higher 4-meter gait test times and lower SPPB scores. Only age and diabetes mellitus were associated with higher sit-to-stand test times, while age and ferritin were associated with lower handgrip strength. Conclusion: Age, diabetes mellitus, serum creatinine, and hemodialysis modality are factors related to physical function in dialysis patients.

        Effective Christian Leadership In An Ever-Changing World

        사람들이 특히 자발적으로 욕구하고 의도하는 것은 자주 그들이 실제로 일으킬 수 있는 일의 반명제가 되고는 한다. 어른들은 서로 경쟁하는 책무들에 연루되며 때로는 모순적인 의도와 행위를 보이는데, 이런 일들은 인격적으로 그리고 인간관계에 있어서, 자기패배적이고 파괴적일 수 있다. 신경생리학자들과 발달이론 연구자들이 논하는 바에 의하면, 영화문화는, 요즘 쓰이는 사회망 시스템들과 더불어, 의식을 채우고 제약하는데, 이것은 우리 두뇌에 큰 영향을 미쳐서 제대로 된 정보에 기초한 판단을 내리는 것을 어렵게 만든다. 이 논문에서는 우리 삶에서 상상력의 중요성을 되살림에 의해서 개인화가 일어나고 의식이 변형될 수 있다고 주장한다. Effective Christian leadership in a changing world requires an educational approach that is open, adaptive and directed towards the common good of humankind and of the planet. Mission statements of universities and corporations are usually written in terms that embody such an inclusive leadership vision and model. This paper argues that in order to achieve the goals described in such statements, transforming rather than transactional leadership is needed. Developmental researchers Robert Kegan and Lisa Laskow Lahey proposes that it is resistance to change rather than openness that characterizes western individuals and organizations. Kegan’s three plateaus of mindfulness are presented and the paper argues that for effective leadership in contemporary institutions that can be open, directive and directed towards the common good there is a fourth plateau, one which is Spirit-inspired, that needs to be considered. This is a plateau which takes the spiritual life into account and which is characterized by empathy and nurtured by contemplation. Some consequences of leadership exercised on these plateaus are discussed. Finally the paper argues that for effective Christian leadership in an ever changing and polarized world a self-transcending exercise of Christian leadership is needed.

        Bent Babies and Closed Borders: Paid Maternity Leave, Ideal Families and the Australian Population Project

        This article examines recent public debate in Australia around the question of paid maternity leave with specific reference to the way this policy has been mooted as a possible key to reversing or stabilizing Australia’s declining birth-rate. The paid maternity leave debate is read against a set of debates that have unfolded concurrently but generally have been treated separately: those concerning access to assisted reproductive technologies, gay marriage, border protection, and mandatory detention. What unites these debates is tension over who constitutes “proper” families, “correct” mothers and the “right” (white) babies. We are interested in the way these debates not only give expression to shared anxieties about race, (reproductive) biology and nation, but in fact depend upon one another in their efforts to re-constitute familiar hierarchies of meaning and merit in the realms of motherhood and family, which are then materialised in the current Australian federal government’s policies on family, welfare, work and immigration.

        Culture Shocks : Feminism and Difference in the Classroom

        Across the past two decades feminist movements world-wide have been responding to the question of cultural difference. Comfortable notions of universal sisterhood have increasingly given way to contingent alliances shaped by new understandings of difference, power, and pluralism. In extending some of the contemporary debates in cross-cultural feminism into the domain of teaching, this essay will explore how some of these debates are influencing the methods and ideals underpinning feminist pedagogy and will consider how responsive the "feminist classroom" has been to a range of cross-cultural dilemmas. As feminist pedagogic practice is inevitably informed by shifting cultural and power politics at play in individual contexts, the essay will seek to address some of the specific issues arising in different cultural contexts, including the politics of "political correctness" and multiculturalism in Australia and North America and the politics of colonialism and postcolonialism in Hong Kong. How do these issues affect the meaning(s) of feminism in the classroom? How do they redefine our understanding of "radical pedagogy"?

        An Empirical Study of Qualities of Association Rules from a Statistical View Point

        Minimum support and confidence have been used as criteria for generating association rules in all association rule mining algorithms. These criteria have their natural appeals, such as simplicity; few researchers have suspected the quality of generated rules. In this paper, we examine the rules from a more rigorous point of view by conducting statistical tests. Specifically, we use contingency tables and chi-square test to analyze the data. Experimental results show that one third of the association rules derived based on the support and confidence criteria are not significant, that is, the antecedent and consequent of the rules are not correlated. It indicates that minimum support and minimum confidence do not provide adequate discovery of meaningful associations. The chi-square test can be considered as an enhancement or an alternative solution.

        Effects of additional electrical stimulation and pre-rigor conditioning temperature on the ageing potential of hot-boned bovine muscles

        Objective: The aim of this study is to characterize the impact of additional electrical stimulation (AES) and various pre-rigor holding temperatures (for 3 h) on the ageing-potential of hot boned bovine M. longissimus lumborum (LL). Methods: Paired LL loins from 12 bulls were hot-boned within 40 min of slaughter, immediate AES applied and subjected to various holding temperatures (5°C, 15°C, 25°C, and 35°C) for 3 h. Results: AES did not accelerate the rate of rigor attainment, but the 3 h pre-rigor holding temperature did. Shear force values decreased as the pre-rigor holding temperatures increased. AES and holding for 3 h (at 25°C) resulted in higher water-holding capacity. Conclusion: Data confirmed that AES did not influence the various meat quality parameters in the present study, but pre-rigor holding temperature (25°C) alone or in combination with AES resulted in superior meat quality.

        A Clinical Approach to an Unidentified Aerosolized Bioterrorism Agent; A Narrative Review for Emergency Providers

        The current heightened international political climate is accompanied by increased risk of chemical or biological agent weaponization. Historical accounts of biochemical warfare are extensive, and considering the recent use of such agents for targeted attacks, clinicians need to recognize and manage these cases. However, agent properties such as the color, odor, ability to be aerosolized, and long incubation period can introduce difficulties in the diagnostic and management approach. We searched PubMed and Scopus for a colorless, odorless, aerosolized substance with an incubation period of at least 4 hours. Data from articles were summarized and reported by agent. Based on data from the available literature, we included agents such as nerve agents, ricin, botulinum toxin, anthrax, tularemia, and psittacosis in this review. We also highlighted potential chemical and biological agents that could be weaponized and the optimal strategies for the diagnosis and treatment of patients exposed to an unknown aerosolized biological or chemical bioterrorism agent.
