        The Study of Taiwan‘s New Immigrant Children‘s Mathematics Achievement

        Introduction: According the 2011 Taiwanes Government Statistics, the lower secondary shool enrollment number of the new-immigrant-children is about 200,000. As known, most of the new immigrants are from the Southeast Asian countries, such as Vietnam, China, Indonesia, Thailand, the Philippines and Cambodia. In order to satisfy the increasing needs and demands on education of the children of new immigrant (CNI,henceforth), Taiwanese government not only develops, but also puts the after-school learning assistance policy into pracice from 2006. Therefore, the main purpose of this study is to explore the mathematics achievement of the CNI after the implementation of the after school learning assistance policy (AsLA plicy, henceforth). Purposes: Firstly, to compare the mathematics achievenment of the CNI by coutries. Secondly, to compare the mathematics performance among the CNI, the children from high-risk family (CHRF, henceforth)and the children of general families. Samples:The 2,452 samples, selccted from two junior high schools located in central Taiwan, include 157 CNI, 522 CHRF. Methosd:The main method used in this study is interval fuzzy number(IFN, henceforth) in order to compare the mathematics achievement of the children after the implementation of the AsLA policy from different type of families. Results: To reach the two purposes of this study. We can find the effectiveness of mathematics performance from three group‘s children of new immigrants, high-risk, general family. Therefore, the results provide one of the ways to review the new immigrant‘s education policy of after-school learning assistance in Taiwan.

      • Daclatasvir plus Asunaprevir in Interferon (± Ribavirin)- Ineligible/Intolerant Asian Patients with Chronic HCV Genotype-1b Infection

        Aims: The efficacy/safety of daclatasvir (pan-genotypic NS5A inhibitor) plus asunaprevir (NS3 protease inhibitor) in interferon (± ribavirin)- ineligible/intolerant patients with chronic HCV genotype-1b infection from mainland China, Korea and Taiwan was investigated in a phase 3, open-label study. Methods: Patients received daclatasvir 60 mg (tablet) once daily plus asunaprevir 100 mg (soft capsule) twice daily for 24 weeks. The primary endpoint was sustained virologic response at post-treatment Week 24 (SVR24). Results: This study treated 159 patients from mainland China (80%), Korea (11%) and Taiwan (9%), including patients with cirrhosis (33%), IL28B non-CC genotypes (40%), and aged ≥70 years (4%). SVR24 was achieved by 91% of patients (100% concordance with SVR12) and was similarly high in all subgroups, e.g. cirrhotic patients (90%), and in patients from mainland China (91%), Korea (94%) and Taiwan (87%). SVR24 was higher in patients without baseline NS5A (L31M/Y93H) resistance-associated variants (RAVs) (n=137/139 [99%]), regardless of the presence (98%) or absence (99%) of cirrhosis, and lower in patients with baseline NS5A RAVs (n=8/19 [42%]). All serious adverse events (AEs) (n=5/159 [3.1%]), grade 4 laboratory abnormalities (n=3/159 [1.9%]) and deaths (n=1/159 [0.6%]) that occurred on-treatment were unrelated to the study drugs; two patients discontinued due to AEs. Treatment was generally well tolerated regardless of cirrhosis status. Conclusions: Daclatasvir plus asunaprevir achieved a high SVR24 rate of 91%, rising to 99% in patients without baseline NS5A RAVs, and was generally well tolerated in cirrhotic and non-cirrhotic interferon (± ribavirin)-ineligible/intolerant patients with HCV genotype-1b infection from mainland China, Korea and Taiwan.

        Maillard Reaction Products of Stir Fried Hordei Fructus Germinatus Are Important for Its Efficacy in Treating Functional Dyspepsia

        Hordei Fructus Germinatus (HFG) has been used as a traditional medicine to treat functional dyspepsia (FD) in China. Stir fried HFG (F-HFG) containing Maillard reaction products (MRPs) is used more widely than the raw HFG (R-HFG). However, the exact mechanisms in its functionality remain unclear. This article investigated the effect of R-HFG, F-HFG, and MRPs on brain-gut peptides, gut microbiota, and digestive enzymes using an FD animal mode. After administration of R-HFG, F-HFG, and MRPs, higher mRNA expression level of gastrin (GAS) and lower mRNA expression level of vasoactive intestinal peptide (VIP) were exhibited in F-HFG and MRPs rats than R-HFG rats (P < .05). Furthermore, compared with the R-HFG group, the contents of motilin (MTL) and GAS showed an upward tendency, whereas the contents of VIP and chokcystokinin (CCK) showed a downward tendency in the F-HFG group. In addition, bacterial communities in the control, F-HFG, and MRPs groups clustered closely to one another, and bacterial communities in the model and recovery groups clustered together, whereas the bacterial communities in the R-HFG group were clustered into a category. Moreover, there were no apparent differences in brain-gut peptides and gut microbiota between the F-HFG and MRPs groups. However, after the oral administration of R-HFG, F-HFG, and MRPs, the level of digestive enzyme did not show a significant change as compared with the recovery group. These results indicated that the stronger effect of F-HFG could be attributed to the MRPs produced during stir frying, and MRPs possessed the effect of regulating brain-gut peptides and gut microbiota.

      • Preparation and Characterization of Anti-GP73 Monoclonal Antibodies and Development of Double-antibody Sandwich ELISA

        Background: Serum Golgi protein 73 (GP73) as a novel and potential marker for diagnosing hepatocellular carcinoma (HCC) have been found to be elevated in HCC patients and associated with clinical variables representing tumor growth and invasiveness. The aim of this study was to prepare a pair of monoclonal antibodys (mAbs) against GP73 and develop a newly designed double-antibody sandwich enzyme-linked immunosorbent assay (s-ELISA), which would be used in the detection of serum GP73 (sGP73) as well as in the diagnosis of HCC. Materials and Methods: Produced by prokaryotic expression, the purified recombinant GP73 (rGP73), produced by prokaryotic expression, was used to immunize the Balb/c mice. Two hybridoma cell lines against GP73 were obtained by fusing mouse Sp2/0 myeloma cells with spleen cells from the immunized mice. The titers of anti-GP73 mAb reached 1:243,000. Western blotting analysis and Immunohistochemistry staining revealed that anti-GP73 mAb could recognize GP73 protein. The double-antibody s-ELISA was successfully established and validated by 119 HCC and 103 normal serum samples. Results: showed that the detection limit of this method could reach 1.56 ng/ml, and sGP73 levels in HCC group (mean=190.6 ng/ml) were much higher than those of in healthy controls (mean=70.92 ng/ml). Conclusions: Results of our study not only showed that sGP73 levels of HCC patients were significantly higher than those of healthy controls, but also indicated that the laboratory homemade anti-GP73 mAbs could be the optimal tool used in evaluating sGP73 levels, which would provide a solid foundation for subsequent clinical applications.

      • Selecting and Identifying Lactic Acid Bacteria in Sourdough and Their Effect on Sourdough Physical and Chemical Properties

        Sourdough is traditionally used as the leavening agent for bread making and a potential source of probiotics. In this study, sourdoughs from seven restaurants in Taiwan were collected. The lactic acid bacteria strains were isolated and identified by 16S rDNA sequencing from these sourdoughs. The following lactic acid bacteria species were identified: Lactobacillus pentosus, Lactobacillus paracasei, Lactobacillus casei, Lactobacillus plantarum, Lactobacillus sanfranciscensis. Five lactic acid bacteria strains, Lactobacillus paracasei LYC1242, Lactobacillus casei LYC1244, Lactobacillus plantarum LYC1245 and Lactobacillus casei LYC1246, were selected and singly used to ferment wheat flour. The pH value of various sourdoughs decreased to pH3 from pH5 and kelp at pH3. The titratable acidity and viscosity of various sourdoughs increased significantly compared to spontaneous fermented flour. However, there is no significant difference in titratable acidity between various sourdoughs. The sourdough fermented with L. casei LYC1244 had highest cell count. These studies provided preliminary results for choosing the best lactic acid bacteria starter strain to develop probiotic sourdough bread.

        Radiotherapy combined with chemotherapy increases the risk of herpes zoster in patients with gynecological cancers: a nationwide cohort study

        Objective: This study aimed to determine the effect of radiotherapy (RT) on the risk of herpeszoster (HZ) in patients with gynecological cancers via a nationwide population-based study. Methods: Based on patient data obtained from the National Health Insurance ResearchDatabase, 1928 gynecological cancer patients were identified with 1:1 matching for RT andnon-RT cohorts by age, index date, and cancer type. Another cohort consisting of 964 non cancer individuals matched was used as normal control. The incidence of HZ was comparedbetween cancer patients with and without RT. Age, comorbidities, cancer-related surgery andchemotherapy (CT), and cancer type were adjusted as confounders. Results: The risk of HZ in cancer patients was higher than that of non-cancer individuals(14.23 versus 8.34 per 1,000 person-years [PY], the adjusted hazard ratio [aHR]=1.38,p=0.044). In the cancer population, the incidence of HZ for the RT and non-RT cohorts was20.55 versus 10.23 per 1,000 PY, respectively (aHR=1.68, p=0.009). Age >50 years was anindependent factor for developing HZ. The 5-year actuarial incidence for patients receivingneither RT nor CT, RT alone, CT alone, and combined modalities was 5.4%, 6.9%, 3.7%,and 9.9%, respectively (p<0.001). In the RT cohort, the risk rose rapidly in the first year,becoming steady thereafter. Conclusion: This population-based study showed that gynecological cancer patientsreceiving RT combined with CT had the highest cumulative risk of HZ. Health careprofessionals should be aware of the potential toxicities.

        Mapping out the surgical anatomy of the lingual nerve: a systematic review and meta-analysis

        Objectives: Understanding the lingual nerve’s precise location is crucial to prevent iatrogenic injury. This systematic review seeks to determine the lingual nerve’s most probable topographical location in the posterior mandible. Materials and Methods: Two electronic databases were searched, identifying studies reporting the lingual nerve’s position in the posterior mandible. Anatomical data in the vertical and horizontal dimensions at the retromolar and molar regions were collected for meta-analyses. Results: Of the 2,700 unique records identified, 18 studies were included in this review. In the vertical plane, 8.8% (95% confidence interval [CI], 1.0%-21.7%) and 6.3% (95% CI, 1.9%-12.5%) of the lingual nerves coursed above the alveolar crest at the retromolar and third molar regions. The mean vertical distance between the nerve and the alveolar crest ranged from 12.10 to 4.32 mm at the first to third molar regions. In the horizontal plane, 19.9% (95% CI, 0.0%-62.7%) and 35.2% (95% CI, 13.0%-61.1%) of the lingual nerves were in contact with the lingual plate at the retromolar and third molar regions. Conclusion: This systematic review mapped out the anatomical location of the lingual nerve in the posterior mandible, highlighting regions that warrant additional caution during surgeries to avoid iatrogenic lingual nerve injuries.

      • B7-H4 Expression is Associated with Cancer Progression and Predicts Patient Survival in Human Thyroid Cancer

        Objective: This study aimed to investigate the expression of B7-H4 in human thyroid cancer and determine any association with patient clinicopathological parameters and survival. Methods: B7-H4 expression in 64 clinical thyroid cancer specimens was assessed with immunohistochemistry. Moreover, B7-H4 mRNA expression in 10 fresh resected specimens were evaluated by the reverse transcription-polymerase chain reaction (RT-PCR). Immunohistochemical staining of CD3 was performed to assess the number of tumor infiltrating T lymphocytes (TILs) in thyroid cancers. Results: Positive B7-H4 immunohistochemical staining was observed in 61 out of 64 (95.3%) specimens of thyroid cancer tissues. Significantly more B7-H4 mRNA copies were found in thyroid cancer tissue than that adjacent normal tissue. Moreover, B7-H4 expression in human thyroid cancer tissues was significantly correlated with patient TNM stages and extrathyroidal extension (P<0.05), being inversely correlated with the number of TILs (P<0.05). The overall survival rate of the patients with higher B7-H4 expression was significantly worse than that of the patients with lower B7-H4 expression. Conclusions: This present study suggests that high B7-H4 expression is associated with cancer progression, reduced tumor immunosurveillance and worse patient outcomes in human thyroid cancer.
