        A study on the relationship between ‘Lock, Door and Axis’ with the principal of following to the ‘Tip, Root and Middle qi’Lee Sang-Hyup1 *1Dept. of Korean Medicine, Graduate School, Dong Eui University Objective: I would like to study the relationship between the theory of ‘Lock, Door and Axis’ with the principal of following to the ‘Tip, Root and Middle qi’ in Huang-Di-Nei-Jing. It can be analysed from the relativeness among the sentences to appearing in Shang-Han-Lun. And consider the theoretical relationship of the two books. Method: First, I search out the exact meaning of ‘Lock, Door and Axis’ theory. Second, The principal of following to the ‘Tip, Root and Middle qi’ is closely associated with ‘Lock, Door and Axis’ theory. Third, I would like to prove that the several sentence in the Shang-Han-Lun are closely related to the Huang-Di-Nei-Jing. Result: Zhang Zhong-jing`s Shang-Han-Lun quote a theoretical basis in the Huang-Di-Nei-Jing. Furthermore, it extensively quote to explan a disease and treatment, and so on. Conclusion: It is necessary to study hard the Huang-Di-Nei-Jing for understand clearly the sentences of Shang-Han-Lun. I think that Huang-Di-Nei-Jing have a decisive effect on the Zhang Zhong-jing`s thinking to the disease and his medical theory.       • Effects of Ovariectomy and Corticosteroid-Induced Osteoporosis on the Osteoinductivity of rhBMP-2 in a Segmental Long-Bone Defect Model

        <P>This study used the segmental long-bone defect model to assess the effects of osteoporosis on the formation of new bones and the osteoinductivity of recombinant human bone morphogenetic protein-2 (rhBMP-2). Seventy-two female Sprague-Dawley rats were divided into two groups: an osteoporosis group with ovariectomies and dexamathasone intramuscular injections and a sham group. When they reached 22 weeks in age, each group was further divided into two groups and a 5-mm defect was made in both fibular mid-shafts of each rat. One fibula in each rat was picked randomly and was injected with 0.05?mL of hydrogel carrier; the opposite fibula was injected with the same carrier mixed with rhBMP-2 (10?μg). After rearing for a further 5 and 9 weeks, the ratios of the lengths of the newly formed bones in the fibular defects were determined using micro-CT and undecalcified histology. The sham rhBMP-2-injected group-in all of the 5- and 9-week-kept groups-showed a significantly higher bridging bone formation ratio than the other three groups. The osteoporosis rhBMP-2-injected group showed a significantly higher ratio than both the non-rhBMP-2-injected sham hydrogel and the osteoporosis hydrogel groups. The comparison of the micro-CT parameters of the newly formed bones showed that the sham rhBMP-2 group at both 5 and 9 weeks compared with the osteoporosis rhBMP-2 group had significantly higher percentage bone volumes, trabecular thicknesses, and trabecular numbers, in addition to significantly lower specific surfaces, trabecular pattern factors, and structural model indices. The histology results showed that the sham-rhBMP-2 group began forming bridging bones in the defect areas at 5 weeks, and at 9 weeks, trabeculae and marrow spaces were observed. However, the osteoporosis rhBMP-2 group exhibited a relatively minor level of new bone and trabecula formation. Consequently, the rhBMP-2 group showed significantly increased bone formation in the osteoporosis rat fibular defect model compared with the hydrogel group, whereas the new bone quantities, qualities, and remodeling in the osteoporosis rhBMP-2 group were less effective than those in the sham-rhBMP-2 group, signaling that ovariectomy and corticosteroid-induced osteoporosis significantly undermines rhBMP-2 osteoinductivity.</P>

      • A new fabrication method for IPMC actuators and application to artificial fingers

        <P>IPMC (ionic polymer metal composite), a kind of ionic electroactive polymer (EAP), has been used for various applications because it has light weight and can make large bending deformation under low driving voltage. In the present work, thick IPMC films were fabricated by hot-pressing several thin IPMC films and the actuating performance was evaluated. Displacement and maximum load with applied voltage were measured using a displacement measuring system, a load cell and a multimeter. Several cycles of Pt electroless-plating were performed on the IPMC films to improve the actuating performance. Then, SEM (scanning electron microscopy) micrographs and EDS (energy dispersive spectrometer) profiles of the IPMC specimen were examined. To demonstrate the feasibility of IPMC films for medical or robotic applications, the developed IPMC actuators were applied to artificial fingers and tested. </P>

        Clinicopathological Characteristics of Colon Cancer Diagnosed at Primary Health Care Institutions

        Background/Aims: The purpose of this study was to evaluate the clinicopathologic characteristics of colon cancers detectedat the SOK Sokpeynhan Internal Medical Network, a nationwide system of primary health care institutions. Methods: Weanalyzed 579 colon cancer patients diagnosed using colonoscopy at the SOK network from January 2011 through December2012. Cancers from the rectum to the splenic flexure were classified as left colon cancer. Patients over 65 were classified assenior. Results: The mean age (±SD) of subjects was 60.9±10.5 years and 61.1% were men. More than one quarter (28.2%) ofpatients were asymptomatic. The prevalence of left colon cancer was higher (77.9%) than that for right colon cancer. The mostfrequent macroscopic and histologic types were depressed (58.9%) and moderately differentiated adenocarcinoma (52.2%),respectively. Asymptomatic subjects displayed protruding or well differentiated adenocarcinoma, while symptomatic patientswere more likely to display depressed or moderately differentiated adenocarcinoma (P <0.05). The mean age of the right coloncancer group was higher than that for the left colon cancer group (P <0.05). Among symptomatic patients, the most frequentsymptoms were bloody stool for patients with left colon cancer and abdominal discomfort for patients with right colon cancer(P <0.05). The prevalence of depressed cancer was higher in older subjects as compared to younger subjects (P <0.05). Theprevalence of right colon cancer tended to increase with age, although this difference did not achieve statistical significance(P >0.05). Conclusions: Study results indicated an increase of colon cancer amongst younger demographics in recent years.The effectiveness of colonoscopy screening was also evident, as asymptomatic patients demonstrated frequent findings of welldifferentiated adenocarcinomas. Study results also suggested a need for closer examination of older patients, as right coloncancer tended to increase with age. (Intest Res 2014;12:131-138)

        Histopathological Evaluation of Non-Neoplastic Renal Parenchyma Adjacent to Tumors in Partial Nephrectomy Specimens of Renal Cell Carcinoma

        Purpose: The optimal margin for partial nephrectomy of renal cell carcinoma (RCC) has not been clearly defined. We investigated the viability of glomeruli and tubules in non-neoplastic renal parenchyma adjacent to tumors and analyzed the predictive factors that affect them. Materials and Methods: Data were gathered retrospectively from 64 patients who underwent partial nephrectomy for RCC between January 2008 and December 2011. We evaluated glomerular viability of non-neoplastic renal parenchyma adjacent to tumors using H&E staining and tubular viability was assessed by Klotho stain. Each renal unit was assigned a RENAL nephrometry score using preoperative imaging. We also investigated the correlation between RENAL score, estimated glomerular filtration rate (GFR) and glomerular and tubular viability. Results: The percentage of non-viable glomeruli was higher in the region within 2-mm from the tumor compared with the region>2-mm from the tumor. The mean non-viable tubular distance from the tumor was 1.36±0.597mm. Only the correlation between glomerular non-viability (%) within 2-mm from the tumor and non-viable tubular distance (mm) was significant. Correlations between RENAL score and glomerular non-viability in the region ≤2- mm from the tumor and non-viable tubular distance were not significant for all RENAL factors. But, glomerular non-viability 5-10-mm from the tumor was inversely correlated with preoperative estimated GFR. Conclusions: Regardless of RENAL nephrometry scores, the viability of glomeruli and tubules within 2-mm from RCC was affected by tumor mass, and that >5-mm from the tumor was affected by subclinical comorbidities. These may suggest new guidelines of RCC management intraoperatively and postoperatively.

        Zirconium mesostructures immobilized in calcium alginate for phosphate removal

        Eutrophication caused by the excessive supply of phosphate to water bodies has been considered as one of the most important environmental problems. In this study, the powder of zirconium mesostructure (ZM), which was prepared with the template of surfactant, was immobilized in calcium alginate for practical application and the resulting material was tested to evaluate the phosphate removal efficiency. Sorption isotherms and kinetic parameters were obtained by using the entrapped ZM beads with 30 to 60% of ZM. The maximum sorption capacity increased with the higher ZM content. Qmax in Langmuir isotherm was 51.74 mg/g for 60% of ZM with 7 mm of size. The smaller the particle size of the ZM beads, the faster the rate of phosphate removal, because the phosphate ions had less distance to reach the internal pores of the immobilized ZM beads. Chemical and electrochemical regeneration techniques were compared. Phosphates adsorbed on the ZM beads were effectively desorbed with NaCl, NaOH, and Na2SO4 solutions. An electrochemical regeneration system consisting of an anion exchange membrane between two platinum-coated titanium electrodes was successfully used to desorb and regenerate the phosphate-saturated ZM beads. Complete regeneration was reached under optimal experimental conditions. Chemical and electrochemical regeneration proved the reusability of the bead form of the entrapped ZM, and will enhance the economical performance of the phosphate treatment process.

In a real ferroelectric thin‐film system, eight stable and reproducible polarization states are realized (i.e., 3‐bit data storage) by adjusting the displacement current. This approach can be used to triple or quadruple the memory density, even at existing feature scales.</P>

        Comparison of Intracardiac Echocardiography Versus Transesophageal Echocardiography for Guidance During Transcatheter Aortic Valve Replacement

        Background and Objectives: Evidence regarding the efficacy and safety of intracardiac echocardiography (ICE) for guidance during transcatheter aortic valve replacement (TAVR) is limited. This study aimed to compare the clinical efficacy and safety of ICE versus transesophageal echocardiography (TEE) for guiding TAVR. Methods: This prospective cohort study included patients who underwent TAVR from August 18, 2015, to June 31, 2021. Eligible patients were stratified by echocardiographic modality (ICE or TEE) and anesthesia mode (monitored anesthesia care [MAC] or general anesthesia [GA]). Primary outcome was the 1-year composite of all-cause mortality, rehospitalization for cardiovascular cause, or stroke, according to the Valve Academic Research Consortium-3 (VARC-3) definition. Propensity score matching was performed, and study outcomes were analyzed for the matched cohorts. Results: Of the 359 eligible patients, 120 patients were matched for the ICE-MAC and TEE-GA groups, respectively. The incidence of primary outcome was similar between matched groups (18.3% vs. 20.0%; adjusted hazard ratio, 0.94; 95% confidence interval [CI], 0.53–1.68; p=0.843). ICE-MAC and TEE-GA also had similar incidences of moderate-to-severe paravalvular regurgitation (PVR) (4.2% vs. 5.0%; adjusted odds ratio, 0.83; 95% CI, 0.23–2.82; p=0.758), new permanent pacemaker implantation, and VARC-3 types 2–4 bleeding. Conclusions: ICE was comparable to TEE for guidance during TAVR for the composite clinical efficacy outcome, with similar incidences of moderate-to-severe PVR, new permanent pacemaker implantation, and major bleeding. These results suggest that ICE could be a safe and effective alternative echocardiographic modality to TEE for guiding TAVR.
