        제왕절개 분만율의 지역간 변이와 관련요인에 대한 연구

        The purpose of this study is to estimate cesarean section rate in Korea and analyze the socioeconomic variables and health resources which affect regional variation in the rate. Samples were drawn from the record of vaginal and cesarean section deliveries based upon insurance claim bills which have been submitted to the National Federation of Medical Insurance for the first three months, January through March, 1991. The results are obtained as follows : It was found that. cesarean section rate was increasing rapidly up to 23.1% in 1991. Cesarean section per 10 thousand insured people was 4.8 and the number of cesarean section per 10 thousand insured eligible($15{\sim}49$ years old) female was 7.6. The fee for normal delivery was 109,489 won and that for cesarean section was 390,024 won. The average days of hospitalization in normal delivery was 2.3 days, and those in cesarean section was 7.6 days. On the average cesarean section has a longer of stay as much as by 4.3 days and cost 3.6 times more than normal deliveries. Cesarean section rates vary among medical facilities 19.8% at clinics 37.6% in small-scale hospitals, and 29.1% in general hospitals. The regional variation of cesarean section rates was also fairly prominent. The South Cheju Gun has the highest rate of cesarean section, 56.2%. Meanwhile no cesarean section cases has been reported in Sunchang Gun during the period of this study. The variation is noted among provinces. The rate for Cheju province has been 3.4 times higher than that for Chunnam. The number of cesarean section per 10 thousand insured people vary greatly among regions, too. This study has found that there exists significant regional variations among various geographic units in terms of average length of stay, average cost, number of obsretricians and number of beds. Multiple regression analysis was done to identify factors explaining the regional variance of various cesarean section rates : In the urban areas, no significant explaining variables were noted except the number of beds for the dependent variable of cesarean section cases per 10 thousand insured eligible females. The smaller the number of bed, the more cases of cesarean section was noted for an urban area. The is mostly because the rate of cesarean section is higher in medium-size hospitals than in large general hospitals. In the rural areas, the factor of education has been found significant for all three deplendent variables. The higher the educational level, the rate of cesarean section is most likely to rise. An income variable measured by the amount of monthly insurance contribution has been identified a powerful predictor in explaining the valiance of cesarean section rates. The same has been noted for the number of obstetricians. Similar findings are observed for the country as a whole. The income level has veen found as the most powerful explaining factor in the regional variance of cesarean section rates. In general the rate is higher in the urban areas, and lower in the area with more small hospitals. As this is the initial attempt to identify the factors relevant to the regional difference in the rates of cesarean section, more elaborated study is urgently required.

        Effect of calcination temperature on the association between free NiO species and catalytic activity of Ni−Ce<sub>0.6</sub>Zr<sub>0.4</sub>O<sub>2</sub> deoxygenation catalysts for biodiesel production

        <P><B>Abstract</B></P> <P>In this study, a series of Ni−Ce<SUB>0.6</SUB>Zr<SUB>0.4</SUB>O<SUB>2</SUB> catalysts, which were synthesized by co-precipitation followed by calcination at different temperatures, were applied for the deoxygenation of oleic acid. The physicochemical properties of the catalysts were characterized by N<SUB>2</SUB> adsorption-desorption, X-ray diffraction (XRD), H<SUB>2</SUB> chemisorption, H<SUB>2</SUB> temperature-programmed reduction (H<SUB>2</SUB>-TPR), NH<SUB>3</SUB> temperature-programmed desorption (NH<SUB>3</SUB>-TPD), and X-ray photoelectron spectroscopy (XPS). The Ni−Ce<SUB>0.6</SUB>Zr<SUB>0.4</SUB>O<SUB>2</SUB> catalyst calcined at 300 °C exhibited the highest conversion for oleic acid as well as selectivity for diesel-range compounds. It is predominantly related to the highest amount of free NiO species.         Synthesis and Magnetic Relaxation of [Mn<sub>12</sub>O<sub>12</sub>(O<sub>2</sub>CCH<sub>2</sub>CH<sub>2</sub>CH<sub>2</sub>Cl)<sub>16</sub>(H<sub>2</sub>O)<sub>4</sub>] Complex

        $Mn_{12}O_{12}(O_2CCH_2CH_2CH_2Cl)_{16}(H_2O)_4]$ (noted as $Mn_{12}$-BuCl), a new polynuclear complex of manganese chlorobutyrate has been successfully prepared by substitution of acetate with 4-chlorobutyric acid. The $Mn_{12}-BuCl$ crystallizes into triclinic space group P-1 with a = 14.5560(11) ${\AA}$, b = 14.5819(11) ${\AA}$, c = 27.265(2) ${\AA}$, ${\alpha}\;=\;84.1140(10)^{\circ}\;,\;{\beta}\;=\;88.805(2)^{\circ},\;{\gamma}\;=\;89.8820(10)^{\circ}$, and Z = 2. The local environments of manganese 3+ and 4+ ions of the title compound are close to those of other $Mn_{12}$ compounds. The electrochemical data for $Mn_{12}-BuCl$ involve reversible reactions of two-electron reductions. The $Mn_{12}-BuCl$ also presents magnetic relaxation below 10 K implying that each molecule behaves as a single molecule magnet.

        Synthesis and characterization of Pt-, Pd-, and Ru-promoted Ni–Ce<sub>0.6</sub>Zr<sub>0.4</sub>O<sub>2</sub> catalysts for efficient biodiesel production by deoxygenation of oleic acid

        <P><B>Abstract</B></P> <P>Herein, we prepared Pt-, Pd-, and Ru-promoted Ni–Ce<SUB>0.6</SUB>Zr<SUB>0.4</SUB>O<SUB>2</SUB> catalysts with improved oleic acid deoxygenation performance and investigated their physicochemical properties by N<SUB>2</SUB> adsorption-desorption, X-ray diffraction (XRD), NH<SUB>3</SUB> temperature-programmed desorption (NH<SUB>3</SUB>-TPD), and X-ray photoelectron spectroscopy (XPS).       • Recombinant human epidermal growth factor for the treatment of chronic wound in recessive dystrophic epidermolysis bullosa patients: a prospective, placebo- controlled pilot study

        Background: Recessive dystrophic epidermolysis bullosa (RDEB) is an inherited skin disease with a gene defect in type VII collagen メ chains, which results in widespread bullae and erosive skin wounds. Epidermal growth factor (EGF) interacts with epidermis and particularly in the basal layer, promoting epithelial growth. Local EGF application has been used in several clinical situations, but there has been no report of using EGF for RDEB chronic wounds. Objectives: A prospective pilot clinical trial to evaluate the use of EGF in patients with RDEB Methods: A Pilot study was performed in 10 cases for 6 patients with RDEB and full thickness ulcer with more than 3 months of evolution. Two different chronic wounds are selected and patients were educated to apply the topical 0.005% rh-EGF (Easyef三; DaeWoong Pharm., Seoul, Korea) and placebo (normal saline) daily, respectively, for 4 weeks. Wound sizes are measured before, after 2 and 4 weeks of treatment and compared. Dermatology life quality index (DLQI) is done for each patient. Results: After 4-week treatment, all wounds healed more rapidly with EGF comparing with placebo, with a mean area decrease of 81.3% (range: 36.5% - 100%), although it was statistically not significant (p=0.063). DLQIs also improved (Mean score decreased from 27 to 19.2). Conclusion: Topical EGF treatment accelerates wound healing without any evidence of safety limitations for chronic wounds in RDEB patients. A large controlled study is needed to confirm these results.

        Two Methods of Setting Positive End-expiratory Pressure in Acute Lung Injury: An Experimental Computed Tomography Volumetric Study

        <P>This study was conducted to observe effects of two methods of setting positive end-expiratory pressure (PEEP) based on the pressure-volume (PV) curve. After lung injury was induced by oleic acid in six mongrel adult dogs, the inflation PV curve was traced and the lower inflection point (LIP) was measured. The 'PEEP<SUB>INF</SUB>' was defined as LIP+2 cmH<SUB>2</SUB>O. After recruitment maneuver to move the lung physiology to the deflation limb of PV curve, decremental PEEP was applied. The lowest level of PEEP that did not result in a significant drop in PaO<SUB>2</SUB> was defined as the 'PEEP<SUB>DEF</SUB>'. Arterial blood gases, lung mechanics, hemodynamics, and lung volumes (measured on computed tomography during end-expiratory pause) were measured at PEEP of 0 cmH<SUB>2</SUB>O, PEEP<SUB>INF</SUB> and PEEP<SUB>DEF</SUB> sequentially. The median PEEP<SUB>INF</SUB> was 13.4 cm H<SUB>2</SUB>O (interquartile range, 12.5-14.3) and median PEEP<SUB>DEF</SUB> was 12.0 cm H<SUB>2</SUB>O (10.0-16.5) (<I>p</I>=0.813). PEEP<SUB>DEF</SUB> was associated with significantly higher PaO<SUB>2</SUB> and lung volumes, and significantly lower shunt fraction and cardiac index when compared to PEEP<SUB>INF</SUB> (<I>p</I><0.05). Setting the PEEP based on the deflation limb of the PV curve was useful in improving oxygenation and lung volumes in a canine lung injury model.</P>

        To evaluate immunomodulators from herbal medicines that affect the expression of IL-10 and TNF-α, we developed a cell-based high throughput screening system and tested 17 kinds of herbal medicine. We found that extracts of Coptidis Rhizoma can be used to stimulate TNF-α and IL-10 expression.

        A Study on the Creatively Gifted

        최근들어 창의성의 연구에서도 절적연구의 필요성이 제기되고 있다. 이에 본 연구에서는 질적.양적 연구 방법을 병행하여 7명을 대상으로 소규모의 복합 사례 연구를 시도했다. 본 연구의 목적은 종합적인 창의성(언어, 도형, 신체)은 뛰어나지만 도형적인 창의성이 저조한 유아들의 창의성을 함양하기 위해 연구자들이 개발하여 적용한 프로그램이 창의성 개발에 효과가 있는지 알아보기 위한 것이다. 또한 도형적인 창의성이 저조한 원인을 알아보기 위해 부모와 교사 설문조사와 인터뷰를 실시한 결과를 분석해 보고자 했다. 연구대상은 유아용 종합 창의성 검사를 실시했던 유아교육 기관 6곳의 유아 367명 중에서 종합 창의성 총 점수가 80 백분위 이상이지만 도형적인 창의성의 백분위는 총점의 백분위 점수에 비해 20 백분위 이상 차이가 나는 만4-6세 유아 7명으로 선정하였다. 창의성을 측정하기 위한 검사도구지는 전경원이 개발한 유아용 종합검사(CCTYC)와 송인섭이 개발한 성격검사(HTP)를 사용하였다. 이 연구는 단일그룹 실험연구(single group research design)로써 1그룹을 대상으로 9주간 동안 1주일에 2회씩 30-40분 가량 도형적인 창의성을 강조한 미술 프로그램을 작용했다. 이 프로그램의 내용은 연구자에 의해 개발된 것으로 ‘나’라는 주제를 중심으로 고정적인 시각에서 벗어나 다양한 범주의 아이디어를 생각해 내는 융통성과 여러가지의 아이디어를 생각할 수 있는 유창성 및 기발한 생각을 할 수 있는 독창성을 강조하고 있다. 주변에서 흔히 볼 수 있는 부분적인 도형(예컨대 신체의 일부분인 눈, 손, 귀 등)을 제시하고 고정관념에서 벗어날 수 있도록 교사가 자세하게 안내하도록 구성하였다. 본 연구 결과 도형적인 창의성을 저조하지만 창의성 총점수가 비교적 상위권인 창의적인 영재/재능 유아의 창의성 총점은 물론 특히 도형적인 창의성의 점수가 많이 향상되었다. 또한 창의성의 하위 요인 중에서 상상력을 제외한 유창성, 융통성, 독창성의 점수도 향상하였다. 교사들과의 면접에서도 사례들의 창의성이 증가한 것으로 나타났고, 미술활동에 대한 흥미와 자신감이 향상된 것으로 나타났다. 그러나 자기 이해 능력은 통계적으로 유의하지 않았다. 부모와의 면담결과 7명 중에서 6명의 가정에서는 미술과 관련된 경험제공이 부족했고 미술 활동을 할 수 있는 풍요로운 가정 환경이 갖춰지지 않았고, 가정에서 미술 활동에 대한 격려와 지원이 부족한 것으로 밝혀졌다. 본 연구 결과 도형적 창의성이 저조한 유아에게 도형적 창의성 프로그램을 적용하면, 유아들의 창의성 함양은 물론 그림을 통하여 자기의 느낌과 생각을 표현할 수 있는 정서발달에도 많은 도움을 줄 것이라고 보여진다. 그러므로 무엇보다도 체계적인 창의성 교육프로그램을 잘 계획하여 적용하는 것이 중요하며, 가정에서도 미술활동과 관련된 다양한 활동을 제공하여 적극적으로 지원하는 것이 바람직하다고 보여진다. 앞으로 창의성 연구에서는 이와 같이 사례연구가 많이 도입되어 질적 연구에 초점을 맞춰야 하는 것과 또한 최근 들어 영역별 창의성 프로그램이 강조되고 있다는 점을 고려하여 영역별 창의성 연구에 관심을 기울여 부족한 창의성 영역에 중점적으로 프로그램을 적용해야 한다는 점을 제안하고자 한다. The purpose of this study was to find out the effectiveness of the creative thinking program through creative drawing over 9 weeks. The subjects were 7 four-to-six year old creatively gifted children from 367 children. Using a combination of qualitative and quantitative research method. the data of each of the 7 children were analyzed individually and collectively. In order to foster the children’s figural creativity and their self-understanding, a creative drawing program with the assigned topic of ‘myself’ was designed by the investigators. The creativity test and personality test as the pretests(CCTYC & HPT) were administered. The findings of this study revealed that the creative drawing program with the assigned topic of ‘myself’ could enhance the creatively gifted children’s figural creativity. The results also revealed the importance of home environmental factors for the children’s creative drawing. However, there was no statistically significant difference on the self-understanding score.
