Purpose: The purpose of the study was to forecast manpower by building the nursing manpower-staffing model(surgical-medical nursing unite). Method: The system dynamics approach was adapted to build a simulation model for calculating medical-surgical nursing manpower-staffing. The model was built up on Venism 5.0b DSS. Result: The results in the basic circumstance are in following. The level of nursing service quality, patientsatisfaction, and nurse job satisfaction maintains higher at medical-surgical nursing unit of the tertiary hospitals with the ratio of bed to nurse 2.5:1-3.0:1 than at any other situations. The level of nursing service quality, patient satisfaction, and nurse job satisfaction maintains higher at madical nursing unit of the secontary hospotals with the ratio of bed to nurse 3.0:1 and at surgical nursing unit of the secondary hospitals with the ratio of bed to nurse 3.0:1-3.5:1 than at any other situations in the fundamental from. The results in the simulation circumstance are in following, As author raises the operating rate of bed into 95-100%, and cut skilled nursing staff down, the level of nursing-service quality, patient satisfaction, and nurse job satisfaction drops slightly. Conclusion: In terms of economic efficiency on tertiary hospotals, the ratio of bed to nurse for the best reasonable medical-surgical nursing manpower-staffing is 3.0:1. In terms of economic efficiency on secondary hospitals, the ratio of bed to nurse for the best reasonable medical nursing manpower-staffing is 3.0:1 and surgical nursing manpower-staffing is 3.5:1

Abstract

Cinnamaldehyde from the bark of Cinnamomum cassia has been reported to have antitumor activity mediated by the inhibition of farnesyl transferase. We assessed in vivo the chemo-preventive effect of cinnamaldehydes on H-ras12V-induced hepatocellular carcinoma formation. A mouse model of hepatocellular carcinoma was established by using the transgene of mutated H-ras12V under the regulation of albumin enhancer/promoter. When treated with cinnamaldehyde for 10 weeks, hepatic tumor development was delayed with 2′-benzoyloxycinnamaldehyde (BCA) compared with control hepatocellular carcinoma formation. The effect of 2′-hydroxycinnamaldehyde (HCA) was comparable. The number of lesions and the size of each lesion were significantly reduced by BCA. Cell proliferation in the lesion was detected by incorporation of 5-bromo-2′-deoxyuridine (BrdU). BCA increased the number of splenocytes, concanavalin A-stimulated splenocyte proliferation and the infiltration of lymphocytes into liver. Data suggest that the delayed hepatic tumor development observed with BCA could be mediated by a long-term immunostimulating effect on T cells.

Peak-to-average power control for multiple-antenna HIPERLAN/2 and IEEE802.11a systems

A combination of multi-input multi-output (MIMO) signal processing with orthogonal frequency division multiplexing (OFDM) is regarded as a promising solution for enhancing the performance of next generation wireless local area network (WLAN) systems. This paper evaluates the peak-to-average power ratio (PAR) performance in a MIMO OFDM-based WLAN system using a space-time block code (STBC). The investigated PAR control schemes, which are originally based on the selective mapping (SLM) and partial transmit sequence (PTS) approaches, select the transmitted sequence with the lowest average PAR over all transmit antennas and retrieve the side information (SI) very accurately at the expense of a slight degradation of the PAR performance. The low probability of false SI can improve the overall detection performance of the MIMO-WLAN system encountered with erroneous SI compared to the ordinary SLM and PTS approaches, respectively.

Three solvent systems, chlorobenzene (ink 1), chlorobenzene/o-dichlorobenzene (ink 2) and chlorobenzene/tetrahydronaphthalene (ink 3), were compared as printable inks for the fabrication of polymer light-emitting diodes (PLEDs) using poly[2-methoxy-5-(2-ethylhexyl-oxyl)-1,4-phenylenevinylene (MEH-PPV) as an emissive material and an inkjet printer (Fujifilm Dimatix DMP-2831). Ink 1 clogged the printer's nozzle and gave non-uniform film. Inks 2 and 3 were used to fabricate PLEDs with ITO/PEDOT:PSS/MEH-PPV/LiF/Al configurations. The best performance (turn-on voltage, 3.5 V; luminance efficiency, 0.17 cd/A; luminance, 1,800 cd/m) was obtained when ink 3 was used to form the emissive layer (thickness, 49 nm), attributable to the better morphology and suitable thickness of the MEH-PPV layer.

Moon, Hyoung-Joon,Song, DaeSub,Seon, Bo Hyeon,Kim, Hye-Kwon,Park, Seong-Jun,An, Dong-Jun,Kim, Jong-Man,Kang, Bo-Kyu,Park, Bong-Kyun American Society for Microbiology 2012 Journal of virology Vol.86 No.18

The complete genome sequence of porcine enterovirus B (PEV-B) from a Korean isolate was analyzed. The genome size was 7,393 bp. Previously, full genome sequences of PEV-B had been reported from the United Kingdom, Hungary, and China. The Korean PEV-B isolate presented polyprotein gene nucleotide sequence similarities of 77.9, 73.7, 78.9, and 80.3%, respectively, to PEV-B UKG/410/73, LP54, PEV15, and Chinese strains (Ch-ah-f1).

Dabigatran is the first oral direct thrombin inhibitor approved by the US Food and Drug Administration (FDA) for prevention of stroke and systemic embolism in patients with nonvalvular atrial fibrillation. Because dabigatran is excreted mainly by the kidneys, serum levels of dabigatran can be elevated to a supratherapeutic range in patients with renal failure, predisposing to emergent bleeding. We describe the case of a 66-year-old man taking dabigatran 150 mg twice daily for atrial fibrillation and cerebral infarction who presented with hematochezia and disseminated intravascular coagulation. Laboratory evaluation showed a hemoglobin level of 6.3 g/dL, platelets of 138,000/mm3, activated partial thromboplastin time (aPTT) of 10?s, and an international normalized ratio (INR) of 8.17. Colonoscopy showed a bleeding anal fissure. Hemostasis was provided by hemoclips and packed red blood cells and fresh frozen plasma were transfused. Since then, there was no further hematochezia, however, bleeding including oral mucosal bleeding, hematuria, and intravenous site bleeding persisted. At presentation, his serum creatinine was 4.96 mg/dL (baseline creatinine, 0.9 mg/dL). Dabigatran toxicity secondary to acute kidney injury was presumed. Because acute kidney injury of unknown cause was progressing after admission, he was treated with hemodialysis. Fresh frozen plasma transfusion was provided with hemodialysis. At 15 days from admission, there was no further bleeding, and laboratory values, including hemoglobin, partial thromboplastin time, and prothrombin time were normalized. He was discharged without bleeding. After 2 months, he undergoes dialysis three times per week and no recurrence of bleeding has been observed.
