Novel D- and L-4´-thioadenosine derivatives lacking the 4´-hydroxymethyl moiety were synthesized, starting from D-mannose and D-gulonic γ-lactone, respectively, as potent and selective species-independent A₃ adenosine receptor (AR) antagonists. Among the novel 4´-truncated 2-H nucleosides tested, a N^(6)-(3-chlorobenzyl) derivative 7c was the most potent at the human A₃ AR (K_(i) = 1.5 nM), but a N^(6)-(3-bromobenzyl) derivative 7d showed the optimal species-independent binding affinity.

In vivo imaging of small animals offers several possibilities for studying normal and disease biology, but visualizing organs with single-cell resolution is challenging. We describe rotational side-view confocal endomicroscopy, which enables cellular imaging of gastrointestinal and respiratory tracts in mice and may be extensible to imaging organ parenchyma such as cerebral cortex. We monitored cell infiltration, vascular changes and tumor progression during inflammation and tumorigenesis in colon over several months.

<P>Recent reports have associated <I>NCF2</I>, encoding a core component of the multi-protein NADPH oxidase (NADPHO), with systemic lupus erythematosus (SLE) susceptibility in individuals of European ancestry. To identify ethnicity-specific and -robust variants within <I>NCF2</I>, we assessed 145 SNPs in and around the <I>NCF2</I> gene in 5325 cases and 21 866 controls of European-American (EA), African-American (AA), Hispanic (HS) and Korean (KR) ancestry. Subsequent imputation, conditional, haplotype and bioinformatic analyses identified seven potentially functional SLE-predisposing variants. Association with non-synonymous rs17849502, previously reported in EA, was detected in EA, HS and AA (<I>P</I><SUB>EA</SUB> = 1.01 × 10<SUP>−54</SUP>, <I>P</I><SUB>HS</SUB> = 3.68 × 10<SUP>−10</SUP>, <I>P</I><SUB>AA</SUB> = 0.03); synonymous rs17849501 was similarly significant. These SNPs were monomorphic in KR. Novel associations were detected with coding variants at rs35937854 in AA (<I>P</I><SUB>AA</SUB> = 1.49 × 10<SUP>−9</SUP>), and rs13306575 in HS and KR (<I>P</I><SUB>HS</SUB> = 7.04 × 10<SUP>−7</SUP>, <I>P</I><SUB>KR</SUB> = 3.30 × 10<SUP>−3</SUP>). In KR, a 3-SNP haplotype was significantly associated (<I>P</I> = 4.20 × 10<SUP>−7</SUP>), implying that SLE predisposing variants were tagged. Significant SNP–SNP interaction (<I>P</I> = 0.02) was detected between rs13306575 and rs17849502 in HS, and a dramatically increased risk (OR = 6.55) with a risk allele at each locus. Molecular modeling predicts that these non-synonymous mutations could disrupt NADPHO complex assembly. The risk allele of rs17849501, located in a conserved transcriptional regulatory region, increased reporter gene activity, suggesting <I>in vivo</I> enhancer function. Our results not only establish allelic heterogeneity within <I>NCF2</I> associated with SLE, but also emphasize the utility of multi-ethnic cohorts to identify predisposing variants explaining additional phenotypic variance (‘missing heritability’) of complex diseases like SLE.</P>

Objective: To evaluate the utility of a society-based robotic surgery training program forfellows in gynecologic oncology. Methods: All participants underwent a 2-day robotic surgery training course between2015–2017. The course included interactive didactic sessions with video, dry labs, androbotic cadaver labs. The labs encompassed a wide range of subject matter includingtroubleshooting, instrument variation, radical hysterectomies, and lymph node dissections. Participants completed a pre- and post-course survey using a 5-point Likert scale rangingfrom “not confident” to “extremely confident” on various measures. Statistical analysis wasperformed using SPSS Statistics v. 24. Results: The response rate was high with 86% of the 70 participants completing the survey. Sixteen (26.7%) of these individuals were attending physicians and 44 (73.3%) were fellows. In general, there was a significant increase in confidence in more complex proceduresand concepts such as radical hysterectomy (p=0.01), lymph node dissection (p=0.01),troubleshooting (p=0.001), and managing complications (p=0.004). Faculty comfort andpractice patterns were cited as the primary reason (58.9%) for limitations during roboticprocedures followed secondarily by surgical resources (34.0%). Conclusion: In both gynecologic oncology fellows and attendings, this educational theory based curriculum significantly improved confidence in the majority of procedures andconcepts taught, emphasizing the value of hands-on skill labs.
Cases of Mild Ptosis Correction with Suture-Method

There are many methods to correct mild blepharoptosis and create double eyelid creases. However, all prior methods have the disadvantages of prolonged swelling, increased recovery time,and the need for an incision when the ptosis correction is simultaneously performed with double eyelid surgery. The authors have devised a new minimally invasive technique to correct mild ptosis during double eyelid surgery. Our technique utilizes stronger suture methods to create double eyelid folds and simultaneously perform the ptosis repair. The correction of mild blepharoptosis is achieved through the transconjunctival Müller’s muscle tucking technique during a non-incisional suture method of double eyelid surgery.

나노크기의 입자를 수용액속에 분산시킨 나노잉크와 이를 이용한 직접인쇄방식 (Direct Write Technology)은 시간과 비용을 절약할 수 있고, 그 응용의 다양성과 소량다품종을 생산할 수 있는 장점으로 21세기에 들어서 주목받고 있는 기술이다. 직접인쇄방식은 RFID-Tag, Flexible-Electronics, OLED display, e-paper, 안테나, Flip-Chip용 범퍼(bumper), 언더필링(underfiling), Frit, 소형저항 및 생화학, 인공치아 등 그 응용부분이 광대하다. 이러한 다양한 응용분야로의 적용 가능성에도 불구하고, 직접인쇄방식은 재현성, 나노잉크의 점도, 입자크기, 재료 종류, 기판과의 접합성, 잉크 전기전도도, 화학적 안정성, 패터닝된 선의 분해능과 같은 물리화학적 특성과 적용 분야의 복잡성으로 인해 산업계에서 아직 활발하게 적용되어 지지 않고 있다. 본 연구에서는 직접인쇄방식 중 잉크 및 재료의 적용성에서 가장 호환성이 우수한 nScrypt사의 icro-Dispensing Direct Write(MDDW)기술의 한계와 가능성에 대해 조사하였다. MDDW기술은 나노잉크의 점도가 1~백만 cps까지 넓은 영역에서 사용 가능하여 여러 종류의 물질을 3차원 형상화 할 수 있으며, 직접 접촉식 방식으로 곡면 및 기판의 종류에 크게 영향을 받지 않는다. 그러나 다른 인쇄방식과 비교하여 미세한 선폭 10마이크론(um) 이하의 정확도로 패터닝하는 분야로는 적용에는 어려움이 있다. 또한 모든 직접인쇄방식이 갖고 있는 다양한 잉크와의 호환성은 개선되었으나, 잉크자체의 전기전도도 및 재현성에 문제가 있다. 본 논문에서는 현재의 MDDW로 가능한 시장분야와 새로운 나노잉크의 개발로 가능한 분야와 MDDW와 비교하여 기술적으로 상용화에 근접한 에어로-졸(Aerosol) 분사방식인 Maskless Mesoscale Materials Deposition(M3D)의 기술개발한계를 비교ㆍ검토하였다.
Transareolar-Perinipple (Areolar Omega) Zigzag Incision for Augmentation Mammaplasty

BACKGROUND:: Optimal augmentation mammaplasty results not only from proper implant size and breast shape, but also from the minimization of postoperative scarring, especially in patients prone to hypertrophic scars. In this context, the authors present a transareolar-perinipple (areolar omega) zigzag approach. METHODS:: Between March of 2003 and June of 2012, a total of 613 patients underwent augmentation mammaplasty using a transareolar-perinipple incision. Among them, 45 patients received a classic (straight line) transareolar-perinipple incision, whereas 568 patients received a modified zigzag transareolar-perinipple incision. RESULTS:: Patients’ ages ranged from 21 to 60 years. Areola size varied from 2.3 to 4.5 cm in diameter. Follow-up duration ranged from 1 to 10 years, with an average of 2 years 7 months. Postoperative complications included capsular contracture, which occurred in 16 patients (2.6 percent). Nine patients (1.5 percent) had Baker class II and seven patients (1.1 percent) had Baker class III capsular contracture. Mild inferior displacement of the implant occurred in four patients (0.6 percent). The prevalence of areolar distortion was 3.4 percent. Nipple hypesthesia was found in approximately 70 percent of the patients, which returned to normal after 2 to 3 months. Based on third-party observers, 74.7 percent of patients who received zigzag transareolar-perinipple incision had excellent to good scarring results. CONCLUSIONS:: The transareolar-perinipple (areolar omega) zigzag incision resulted in satisfactory postoperative scarring and surgical results in Asian patients. This method increases the opening of the areolar incision and can be performed in patients with small (<3.5 cm) areolas. This approach can be an alternative in patients who are prone to hypertrophic scarring. CLINICAL QUESTION/LEVEL OF EVIDENCE:: Therapeutic, III.
