A two-dimensional Darrieus-type hydroturbine system, installed with a wear for flow streams such as small rivers and waterways, has been developed for hydropower utilization of extra-low head less than 2m. There are several problems such as flow rate change and flowing wastes to be solved for its practical use in natural flow streams. In the present study, at first, a design guideline in the case of overflow or bypass flow is shown by using simple flow model. Next, in order to avoid the unexpected obstacles flowing into the hydroturbine, an installation of waste screening system is examined. It is confirmed that the screen is effective with some amount of bypass flow rate, however the output power is remarkably deteriorated.

Background/Aims: The anastomotic site after distal gastrectomy is the area most affected by duodenogastric reflux. Differentreconstruction methods may affect the lesion characteristics and treatment outcomes of remnant gastric cancers at the anastomoticsite. We retrospectively investigated the clinicopathologic and endoscopic submucosal dissection outcomes of remnant gastriccancers at the anastomotic site. Methods: We recruited 34 consecutive patients who underwent endoscopic submucosal dissection for remnant gastric cancer at theanastomotic site after distal gastrectomy. Clinicopathology and treatment outcomes were compared between the Billroth II and non-Billroth II groups. Results: The tumor size in the Billroth II group was significantly larger than that in the non-Billroth II group (22 vs. 19 mm;p=0.048). More severe gastritis was detected endoscopically in the Billroth II group (2 vs. 1.33; p=0.0075). Moreover, operation timewas longer (238 vs. 121 min; p=0.004) and the frequency of bleeding episodes was higher (7.5 vs. 3.1; p=0.014) in the Billroth IIgroup. Conclusions: Compared to remnant gastric cancers in non-Billroth II patients, those in the Billroth II group had larger lesions with abackground of severe remnant gastritis. Endoscopic submucosal dissection for remnant gastric cancers in Billroth II patients involvedlonger operative times and more frequent bleeding episodes than that in patients without Billroth II.

Fetal cardiac malformations are the most common congenital malformations with an incidence of 8:1000 among live births. Furthermore, 10% of neonatal deaths and up to 50% of infant deaths were attributed to congenital anomalies. In 2006 the International Society of Ultrasound in Obstetrics and Gynecology (ISUOG) published practice guidelines for the sonographic screening of congenital heart disease (CHD) at some time between 18 and 22 weeks` gestation. Two levels for screening low-risk fetuses for heart anomalies are recommended. Firstly, a basic scan should be performed by analyzing a four-chamber view of the fetal heart. Secondly, an extended-basic scan further examines the size and relationships of both arterial outflow tracts. Obstetricians, midwives, and perinatalogists are able to evaluate a thorough examination of the four-chamber view, both arterial outflow tracts, three vessels and trachea view, and an assessment of pulmonary venous return. These anatomical features are usually evaluated using transverse views, although sagittal scanning planes are also used as necessary. Color Doppler ultrasonography is an important component of the fetal echocardiogram. Occasionally, advanced techniques such as Velocity Vector Imaging (VVI) may be required to evaluate fetal cardiac function using measurements of ventricular ejection fraction, stroke volume, and ventricular strain parameters. An accurate prenatal diagnosis of cardiac defects, especially ductal dependent anomaly, is extremely important for healthcare professionals who will be counseling parents about the nature, severity, clinical management and prognosis of their unborn child.

Low head hydropower is one of realistic renewable energies. The Darrieus-type hydro turbine with an inlet nozzle is available for such low head conditions because of its simple structure with easy maintenance. Experimental and numerical studies are carried out in order to examine the effects of gap distances between the runner pitch circle and two edges of inlet nozzle on turbine performances. By selecting narrower gaps of left and right edges, the performance could be improved. From the results of two dimensional numerical simulations, the relation between the performance and flow behaviors around the Darrieus blade are discussed to obtain the guideline of appropriate inlet nozzle design.

Study Design: Retrospective study. Purpose: To examine the relationship between postoperative bowel bladder disorder (BBD) and the efficacy of needle electrodes for the external anal sphincter (EAS) in intraoperative spinal cord monitoring with transcranial muscle action potentials (Tc-MsEP). Overview of Literature: Spinal surgery for spina bifida, spinal cord tumor, and spinal tethered cord syndrome has a high rate of postoperative BBD. Monitoring of the EAS with Tc-MsEP is frequently performed during spinal surgery. We initially used plug-surface electrodes for this purpose, but have more recently switched to needle electrodes for the monitoring of the EAS. To date, there has been no comparison between the utility of these electrodes. Methods: Waveform derivation, exacerbation of postoperative BBD, and sensitivity and specificity for prediction of BBD by 70% amplitude reduction of EAS activity using needle and plug-surface electrodes were investigated in 239 spine surgeries. The cut-off for the % drop in amplitude for BBD prediction was determined for EAS monitoring using a needle electrode. Results: The overall rate of postoperative BBD aggravation was 7.1% (17/239 cases), with the individual rates using needle and plug-surface electrodes being 6.9% (8/116) and 7.3% (9/123), respectively. The waveform derivation rate was significantly higher using needle electrodes (91.3% [106/116] vs. 76.4% [94/123], p <0.01). In patients with baseline waveform detection, the sensitivity and specificity for postoperative BBD were similar in the two groups. With needle electrodes, a cutoff amplitude of Tc-MsEP for the EAS at the end of surgery of 25% of the baseline amplitude had a sensitivity of 89% and specificity of 82% for the prediction of postoperative BBD aggravation. Conclusions: The significantly higher waveform derivation rate using needle electrodes suggests that these electrodes are effective for monitoring the EAS in spinal surgery in cases with preoperative BBD.
