        한국 주요정신장애의 유병률 및 관련요인 : 2006 전국정신질환역학조사

        Objectives The aims of this study are to estimate the prevalence of the DSM-IV psychiatric disorders in the Korean population using the Korean version of Composite International Diagnostic Interview (K-CIDI), and to compare those with previous studies. Methods The Korean Epidemiologic Catchment Area study Replication (KECA-R) was Conducted between August 2006 and April 2007. The sampling of the subjects was carried out across 12 catchment areas. A multistage, cluster sampling design was adopted. The target Population included all eligible residents aged 181o 64 years. Face-to-face interviews were conducted with the Korean version of Composite International Diagnostic Interview (K-CIDI) based on the DSM-IV (N=6,510, response rate=81.7%). Results A total of 6,510 participants completed the interview. The lifetime and 12-month prevalence rates for all types of DSM-IV disorders were 30.0% and 17.3%, respectively. Those of Specific disorders were as follows : 1) alcohol use disorder, 16.2% and 5.6% ; 2) nicotine use disorder, 9.0% and 6.0% ; 3) specific phobia, 3.8% and 3.4% ; 4) major depressive disorder, 5.6% and 2.5% ; and 5) generalized anxiety disorder, 1.6% and 0.8%. Data relating to nicotine and alcohol use disorder revealed a very high male/female ratio. Mood disorder and anxiety disorder were more prevalent among females than males. Conclusion The prevalence of psychiatric disorders was high. In comparison with other studies, remarkable differences in the distribution of psychiatric disorders across the country and times were observed.

      • The Usefulness of Periareolar Zigzag Incision in Breast Conserving-Surgery in Breast Cancer Patients: Experience in a Single Institution

        Purpose: Various incision techniques have been used in breast-conserving surgery (BCS) to reduce scar formation, including the frequently used periareolar incision technique. However, its applicability in removing large-size tumors or those distant from the nipple has remained limited. We performed periareolar zigzag (P-Z) incision to address these problems in BCS and to improve cosmetic outcomes. Methods: Patients who underwent P-Z incision in BCS between January 2016 and November 2017 were retrospectively analyzed in terms of clinicopathological features and surgical findings. Factors affecting the positive margin were analyzed. Results: A total of 305 patients were reviewed. The patients presented with a median age of 51 years, mean tumor size 1.9 cm, and mean tumor distance of 3.3 cm from the nipple. Intraoperative frozen biopsy and final pathologic findings showed 43 (14.1%) and 7 (2.3%) tumor-positive margins. There were no major complications associated with the surgery, and no re-operations were required due to bleeding or other reasons. Conclusion: The P-Z incision technique was used to achieve sufficient operative field during BCS, and large tumors or those distant from the nipple could be removed without any complications. This technique is considered an oncologically safe surgical technique resulting in good cosmetic outcomes.

        외상성 간손상의 선택적 치료

        With the increment of the role of CT scan, nonoperative treatment is the recent trend in hemodynamically stable patients in the management of traumatic liver injuries. To investigate the trend of treatment and to suggest the possible criteria for nonoperative treatment, we reviewed 50 patients with traumatic liver injuries who were treated in the Department of Surgery, Dong-Guk University Hospital from January 1989 to June 1994. Of 50 patients, 35 patients were treated with nonoperative management and 15 patients were treated with operative management. Based on CT findings, we classified those 50 patients into each grade of liver injuries ranging from grade 1 to 5 and we classified t,he degree of hemoperitoneum ranging from grade 0 to gracle 3+, and we compared those results with the clinical outcome. The operation rate according to liver injury grade didn't increase with the increment of the grade, and the degree of hemoperitoneum corresponds well to the rate of operation. In the nonoperative cases, there was only one complication and there was no mortality. In conclustion, even major liver injuries including grade 4 by CT scan can be treated with nonoperative rnanagement if the patients are hemodynamically stable and they do not have combinded intraabdominal organ injury.

      • CDK7 is a reliable prognostic factor and novel therapeutic target in epithelial ovarian cancer

        <P><B>Abstract</B></P> <P><B>Objective</B></P> <P>Cyclin-dependent kinase 7 (CDK7) engages tumor growth by acting as a direct link between the regulation of transcription and the cell cycle. Here, we investigated the clinical significance of CDK7 expression and its potential as a therapeutic target in epithelial ovarian cancer (EOC).</P> <P><B>Methods</B></P> <P>CDK7 expression was examined in 436 ovarian tissues including normal to metastatic ovarian tumors using immunohistochemistry, and its clinical implications were analyzed. Furthermore, we performed <I>in vitro</I> and <I>in vivo</I> experiments using CDK7 siRNA or a covalent CDK7 inhibitor (THZ1) to elucidate the effect of CDK7 inhibition on tumorigenesis in EOC cells.</P> <P><B>Results</B></P> <P>The patient incidence of high CDK7 expression (CDK7<SUP>High</SUP>) gradually increased from normal ovarian epithelium to EOC (<I>P</I> < 0.001). Moreover, CDK7<SUP>High</SUP> was associated with an advanced stage and high-grade histology (<I>P</I> = 0.035 and <I>P</I> = 0.011, respectively) in EOC patients and had an independent prognostic significance in EOC recurrence (<I>P</I> = 0.034). CDK7 inhibition with siRNA or THZ1 decreased cell proliferation and migration, and increased apoptosis in EOC cells, and this anti-cancer mechanism is caused by G0/G1 cell cycle arrest. In <I>in vivo</I> therapeutic experiments using cell-line xenograft and PDX models, CDK7 inhibition significantly decreased the tumor weight, which was mediated by cell proliferation and apoptosis.</P> <P><B>Conclusion</B></P> <P>Mechanistic interrogation of CDK7 revealed that it is significantly associated with an aggressive phenotype of EOC, and it has independent prognostic power for EOC recurrence. Furthermore, CDK7 may be a potential therapeutic target for patients with EOC, whether platinum sensitive or resistant.</P> <P><B>Highlights</B></P> <P> <UL> <LI> CDK7 inhibition has an anti-cancer effect on platinum-sensitive EOC. </LI> <LI> CDK7 inhibition could induce responsiveness to platinum chemotherapy in platinum-resistant EOC. </LI> <LI> CDK7 overexpression had independent negative prognostic value for disease recurrence of EOC. </LI> <LI> CDK7 might play a critical role in EOC tumorigenesis, and it also serves as a possible therapeutic target in EOC. </LI> </UL> </P>

      • Adenovirus-Mediated p53 Treatment Enhances Photodynamic Antitumor Response

        <P>Photodynamic therapy (PDT) has been reported to be effective for treating various tumors and to induce apoptosis in many tumor cells. In this study, we evaluated the ability of PDT combined with a tumor suppressor factor, recombinant adenovirus p53 (AdCMVp53), to induce apoptosis as well as cell growth inhibition in CaSki human cervical cancer cells and in nude mice with implanted CaSki cells. To examine levels of apoptosis, CaSki cells were treated with PDT and/or AdCMVp53, and an annexin V-staining assay was then conducted. In addition, Western blot analysis was done to identify p53 induction at the cellular and tumor tissue levels. PDT+AdCMVp53 cotreatment caused remarkable inhibition of CaSki cell proliferation, as compared with the individual treatments. In parallel with the inhibition of cell proliferation, the cotreatment caused a significantly greater increase in the annexin V-stained cell population compared with the individual treatments, as determined by fluorescence-activated cell-sorting analysis. The Western blotting assay also showed significantly more cellular p53 expressed after PDT+AdCMVp53 cotreatment than after each separate treatment. This was consistent with observations of tumor tissue in the mouse system. However, apoptosis- related protein, p21, was significantly suppressed by PDT+AdCMVp53 cotreatment, contrary to treatment with AdCMVp53 alone. Taken together, these findings suggest that PDT plus AdCMVp53 gene therapy exerts more potent antitumor effects on human cervical cancer cells, with induction of apoptosis at least through activation in p53 protein at the cellular and tumor tissue levels.</P>

        소아 안와골절 환자의 임상양상

        Purpose: We sought to evaluate the clinical aspects associated with a preoperative and postoperative state in cases of blow-out fracture. Methods: We retrospectively assessed the cause, location, type, and ocular motility restriction in 25 eyes of 25 pediatric patients with blow-out fracture, which were repaired by orbital reconstruction between January 2001 and June 2004. Results: The mean age of patients was 14.96 years, the most common cause of fracture was assault (13 cases, 52%); there were 15 case (60%) of inferior wall fracture, 14 cases (45%) of comminuted type fracture, and 11 cases (35.4%) of trapdoor type fracture. The mean time to intervention was 14 days (range, 3 to 56 days). Ocular motility restriction improved most in the early postoperative period, and then decreased gradually. At last follow-up, the majority of patients had improved. Their diplopia improved almost completely, except in one patient who had late orbital reconstruction. Conclusions: Most cases of blow-out fracture were resolved with orbital reconstruction. Even when performed late, orbital reconstruction is helpful in improving the symptoms associated with blow-out fracture.

        Phase I trial of neoadjuvant concurrent chemoradiotherapy with S-1 and weekly irinotecan in locally advanced rectal cancer

        <P><B>Abstract</B></P><P>S-1 is a novel, oral fluoropyrimidine and a known radiosensitizer. We conducted a phase I trial to establish a schedule of S-1/irinotecan with standard pelvic radiotherapy as a preoperative treatment of locally advanced rectal cancer. Our findings suggest that this new combination is feasible and well tolerable.</P>
