      • 建設工事 監理制度의 改善方案에 관한 硏究

        海外工事에서는 技術用役業者에 의한 工事監理制度가 一般的으로 運營되고 있으며 우리나라에서는 最近에 정부工事에 監理制度가 導入되어 部分的으로 運營되고는 있으나 建設工事의 運營에 問題點이 있음을 알 수 있다. 즉 監理豫算의 確保 및 執行에 따른 問題 資格監理要員의 配置. 監理要員의 業務上의 倫理 監理業務의 運營등이다. 보다 나은 監理制度가 되기 위해서는 建設工事 監理制度가 적절한 자격을 갖추도록 하는 중요한 정책이 있어야 한다.建設工事의 진보적인 기능과 개선된 품질관리를 위하여 監理制度는 제도화 하는것이 필요로 한다. 보통 정부난 지방자치단체에서 지시한 일부 建設工事는 품질관리와 특성이 요구된다. 監理制度의 執行은 확립되지 않은 監理用役회사로 원치않은 현상을 초래 할 수도 있다. 그런 환경때문에 대부분의 用役회서는 아마도 그 특성이라는 결핍을 가져올 수 있다. 왜소한 확립성이 없는 用役회사 역시 다른 문제점을 초래한다. 따라서 정부는 재정적으로 그리고 제도적으로 監理用役회사를 보조해서 촉진 시켜야 한다. 契約制度의 고용도입과 적절한 지불은 시급히 요망된다. 적절한 지불규정은 법률화 하는것이 감리제도 개선의 선행방안이다. Technology consultant service agents normally manage to control construction supervision in overseas constructions. Recently in domestic constructions the supervision system is partially introduced to government-operated building. But there are still a lot of problems in the management of constructions. That is, for normal supervision the procurement and execution of budget, the replacement and functional ethics of qualified supervisors are difficult problems. For a better perspective of supervision the following are important policies that should be designed to help construction supervision get its proper status. To institutionalize the supervision system for both advanced function and improved quality control of constructions is necessary. Some constructions above average ordered by central or provincial governments need such quality control and speciality. The execution of supervision system may bring unwatned phenomena of irregular circumstances most consultant companies. Under such circumstances most consultant companies probably fall into the lack of speciality. The smallness of such irregular consultant companies is also another problem. Thus the government should support and promote supervision consultant companies financially and institutionally. The introduction of employment of contract system and appropriate payment is urgently needed. To legislate suitable payment regulations is prerequisite to the improvement of supervision system.

      • Work Sampling 기법에 의한 철근콘크리트 작업의 작업분석

        The present study was intended to find the method of improving productivity at construction work sites by employing and adapting the work measurment techniques. The study includes analysis of individuals worker's activities and work time distribution, and evaluation of work time-rates. The study adapted the work sampling method which is known to be simple and easy to use for the intended purposes and applied the method to the most typical construction operation of reinfored concrete structure works at an on-going building construction site. The operation comprises the work processes of form erection, steel bar installation and concrete placing. The study reveal that the actual working time rates, contributed to the effective work contents range from 22 to 42 percent of the total work time which are quite below the generally accepted standard of 55 percent. Assuming that the results of the study are typical in the construction industry of KOREA today, it is suggested that the improvement of productivity in construction work site requires better preplanning of work processes in such a way as to eliminate or reduce ineffective time contents to reasonable levels.

        최근 8년간 K지방대학병원 응급실로 내원한 치과 응급환자에 관한 임상적 연구

        The delivery of the proper care to the emergency dental patients is more important in the aspdect of community dental service. To attain such a purpose requires the sacred duty to the training of oral and maxillofacial surgeons. This is a retrospective study on the characteristics of dental injuries and diseases in emergency patients. Ths study was based on a series of 2,649 patients treated as dental emergency patients at K country hospital of medical college, from Jan. 1, 1993, to Dec. 31, 2000. The obtained results were as follows. The trauma(71.6%) was the most frequent cause in dental emergency patients, odontogenic infection and oral hemorrhage were next in order of frequency. The primary interdental wiring and transpalatal wiring were frequent proper methods with rubber drainage for prevention of hemorrhage and infection in the primary care of jaw fracture. The peak incidence(45.0%) about the infection group was occurred in the pulpitis of advanced dental caries, and common treatments were the medication of antibiotics & anagesics and I&D. In the dentoalveolar hemorrhage group, a major cause was postoperative hemorrhage(57.9%) and hemostasis was obtained by pressure dressing, suture and drainage of infected wound. About TMJ disorder group, the peak incidence was showed in the post-traumatic myofascial pain dysfunction syndrome and its primary care was medications(anagesics & sedatives). In the other groups, various irritable specific pains were complained, such as trigeminal neuralgia-form pain, acute stomatitis, chemical burn, terminal stage neuritis of head & nech cancer and their primary care methods were various individually. For the rapid and correct care of the emergency dental disease above, well-trained education should be presented in the intern and resident curriculum of oral & maxillofacial surgery.

      • 인공관절 치환술 후 발생한 대퇴 골절 치료에서 금속판 내고정의 문제점

        목 적: 최근 빈도가 증가하고 골절양상도 복잡해지는 인공관절 수술 후 발생한 대퇴 골절의 발생 빈도와 금속판을 이용한 치료 시의 문제점을 문헌 고찰과 함께 보고하고자 한다. 대상 및 방법: 1985년부터 1998년까지 시행된 1,275 (1,188 일차 치환, 97 재치환술)례의 고관절 치환술 시행 환자 중 52명에서 발생한 52ㄹㅖ의 대퇴부 골절을 대상으로 조사하였다. 이를 안정성 여부와 스템 끝 주위의 골절 위치에 따라서 근위부 (Ⅰ), 중심부 (스템 주위) (Ⅱ), 언위부(Ⅲ)로 구분하였고 이를 다시 비전위성 (A), 전위성 (B), 불안정성 삽입물 (C)으로 세부나였다. 치료의 방법으로 삽입물이 안정성을 가진 근위부 골절 (제 1형)에서는 부목 고정이나 환상 강선 고정을 시행하였으며, 스템 끝 (제 2형)이나 원위부 (제 3형) 골절에서는 금속판과 나사못 등을 사용하고 대퇴골 과상부인 슬관절 주변의 골절인 경우 외고정 장치 장착을 하였다. 금속판 고정을 한 경우는 골이식술을 시도하였다. 결 과: 인공관절 치환 후 발생한 골절의 빈도는 4.1%이었고, 이 중 술 중 골절은 1.5%, 술 후 골절은 2.6%이었다. 골절의 평균 유합기간은 3.1개월이었다. 금속판을 이용하여 치료한 경우 2례의 불유합과 3례의 재골절 중 1례는 골절 유합 후 보행 도중 낙상에 의하여, 1례는 골절부 유합 후 금속판 제거 후 발생하였으며, 1례는 체중 부하 도중 발생한 골절이었다. 제 Ⅲ형 골절의 경우 전례에서 스템의 이완 소견은 볼 수 없었다. 결 론: 저자들은 인공고관절 치환술 후 발생한 대퇴골 골절 치료에서 금속판 내고정 후 5례의 비교적 많은 합병증을 경험하였다. 이 중 2례는 부적절한 짧은 금속판의 선택과 조기 보행, 나머지 3례는 금속판 및 나사못과 관련된 골절로서 스트레스가 집중되는 부위인 대퇴 스템 원위단이나 금속판의 근위부는 재골절의 위험이 있으므로 골절의 치료에 있어서 골이식이나 긴 금속판 사용, 골유합시까지 외부충격에 보호 등으로 재골절의 위험 빈도를 해결해야 한다. Purpose: The purpose was to evaluate the postoperative periprosthetic femur fracture of hip arthroplasty and relative problems of management using plate fixation. Material and Method: We reviewed 37 cases of femoral fractures out of 1,270 (1,183 primary hips and 97 revision hips) hips which received hip arthroplasty during the observation period(1985~1998). We classified them according to the stability and the location of the fractures related to the stem tip: Proximal (Ⅰ), middle (around stem)(Ⅱ) and distal(Ⅲ). Then we subclassified the fractures as cortical perforation or a fissure, undisplaced (A), displaced(B) and unstable prosthesis (C). we managed periprosthetic fracture with splint applied or cerclage proximal fracture (type Ⅰ), plate fixation and bone grafts in the step tip(type Ⅱ) or distal fracture (type Ⅲ). Un unstable fractures, we revised them with long fluted stems. Result: The average union time is 3.1 months (range, 2~6.2 months). After management of fracture with plate showed 5 complications, two nonunion and three refractures. The causes of nonunion are weight bearing and a too short plate. The causes of refracture are screw fixation and empty hole just distal to stem tip. The other caused by the roles of stress riser in proximal screw of blade plate in management of supracondylar fracture. Conclusion: We recommend the use of cerclage band system of fixed in a more proximal site in long plate and bone graft. In unstable prosthesis, we had taken a best results after revised with distal fluted stem, in which we don't need further distal stability like a additional cerclage or locking screw and additional bone graft.

        Most patients with aplastic anemia who do not respond to immunosuppressive treatment or are not candidates for bone marrow transplantation die of infection or bleeding. The neutropenia in acute leukemia, aplastic anemia, or occurring subsequently to chemotherapy and bone marrow transplantation increases susceptibility to infection. In general, the number of infectious episodes correlate with the degree and duration of neutropenia. Global immunosuppression produced by conditioning for bone marrow transplantation or graft-versus-host disease, is associated with unusual bacterial and fungal pathogens, or serious viral and protozoan infections. In addition, repeated treatment with broad-spectrum antibiotics is associated with the emergence of resistant organisms and fungal diseases because of the altered microbial microenvironment of the host. The incidence of invasive fungal infection caused by Saccharomycetes eerevisiae in immunosuppressed patients is very rare, compared with that of infection by candida or aspcrgillus species. Cases of Saccharomycetes cerevisiae fungemia occurring in the course of treatment with broad-spectrum antibiotics are reported in patients with extensive burn or with prosthetic valve endocarditis. We experienced a case of urinary tract infection by Saccharomycetes cerevisiae in a 27-year old female patient with severe aplastic anemia. We report the case with a review of relevant literatures.

        Effects of introducing functional groups on the performance of phenoxazine-based dye-sensitized solar cells

        <P><B>Abstract</B></P> <P>Phenoxazine dyes with a hexyloxy chain and furan moiety were synthesized and used in dye-sensitized solar cells. The dyes were designed to investigate the effects of these substituents on the photophysical and electrochemical properties of the dyes and on cell photovoltaic performance. The introduced hexyloxy moiety, as an additional donor, improved the short-circuit current and open-circuit voltage of the cell by increasing light absorption and steric hindrance, respectively. The furan moiety introduced in the phenoxazine dye as a bridge unit led to a bathochromic shift of the absorption band and a large amount of dye adsorption, increasing the short-circuit current of the cell. Among the synthesized dyes, those with the furan moiety showed the best conversion efficiency of 6.34%.</P> <P><B>Highlights</B></P> <P> <UL> <LI> Effects on the cell performance by hexyloxy and furan group introduced on the phenoxazine dye. </LI> <LI> Increase of open-circuit voltage current by suppressing charge recombination. </LI> <LI> Increase of short-circuit current by bathochromic shift and a large amount of dye adsorption. </LI> <LI> The best efficiency was shown by a cell based on the dye with a furan unit. </LI> </UL> </P>

        A photosynthetic cell-based energy material for flow battery

        Photosynthesis has been applied in energy-related devices including photobioelectrochemical cells (PBCs). Although PBCs’ theoretical efficiency is high because they directly utilize photosynthesis, they are not suitable for commercial use owing to energy loss during photosynthetic electron harvest and cell degradation. To overcome these drawbacks, a novel photosynthetic cell-based energy material (PCEM), composed of photosynthetic cells, nanomaterials, and hydrogel, is developed. To characterize this PCEM, a PBC is built and electrochemically characterized by measuring the polarization curve, which exhibited 1.91-times improved power density compared to a PBC without the nanomaterials. A flow battery incorporating the PCEM showed 3.1-times improved power density, while maintaining longterm viability. Our results illustrate that the fabricated particles improved the energy efficiency by facilitating the transfer of photosynthetic electrons to the electrodes, while maintaining the viability of cells; therefore, these PBCs can be used for commercial energy applications, such as flow batteries.
