        Moxibustion for the treatment of osteoarthritis: a systematic review and meta-analysis.

        <P>The aim of this review was to summarise and critically evaluate the evidence from randomised clinical trials (RCTs) of moxibustion as a treatment for patients with osteoarthritis (OA). Twelve databases were searched from their inception through July 2011. RCTs were considered whether they assessed any type of clinical outcome from moxibustion therapy for patients with OA localised to any joints. Two reviewers independently performed the selection of studies, data abstraction and validations. The risk of bias was assessed using the Cochrane criteria. Eight RCTs met our inclusion criteria, and most of them had significant methodological weaknesses. Six RCTs tested the effects of moxibustion against conventional oral drug therapies in patients with knee OA (KOA). Meta-analysis showed favourable effects of moxibustion on the response rate (n = 540; RR, 1.09; 95 % CI 1.03-1.17; P = 0.005; heterogeneity: χ(2) = 5.48, P = 0.36, I (2) = 9 %). Two RCTs tested the effects of moxibustion on response rate after 2 months. The meta-analysis failed to show favourable effects of moxibustion (n = 180; RR, 1.10; 95 % CI 0.97-1.24; P = 0.13; heterogeneity: χ(2) = 0.03, P = 0.87, I (2) = 0 %). In conclusion, consistent results show that moxibustion may be effective in symptom management in patients with KOA. However, because of the number of eligible RCTs and the high risk of bias in the assessment of the available RCTs, the evidence supporting this conclusion is limited.</P>

      • Preferred Reporting Items for the Development of Evidence-based Clinical Practice Guidelines in Traditional Medicine (PRIDE-CPG-TM): Explanation and elaboration

        <P><B>Abstract</B></P> <P><B>Introduction</B></P> <P>The development of evidence-based clinical practice guidelines (CPGs) in traditional medicine (TM) is an ongoing challenge as it is unique area. This study describes a unified platform with Preferred Reporting Items for the Development of Evidence-based CPGs in TM (PRIDE-CPG-TM), which has been successfully used by the evidence-based CPGs committee for TM.</P> <P><B>Methods</B></P> <P>Initially we searched the literature and CPG handbooks, collected items from methodology references and drew on experiences gained from Korean medical physicians and methodology experts. A group of experts subsequently edited drafts of the items, identified one or more examples of good reporting for each item, and developed text that explained the rationale and discussed relevant evidence. Face to face meetings were held with experts to finalize the items with the most extensive elaboration.</P> <P><B>Results</B></P> <P>The PRIDE-CPG-TM, in the form of a checklist and description of items with TM approach and TM examples, were designed to improve the reporting of CPG in TM and thereby facilitate their interpretation and replication. The PRIDE-CPG-TM included 5 domains and 40 items. The items pertain to the development methodology (22 items), Overview of diseases and symptoms (6 items), Recommendations (4 items), Implementation and dissemination (5 items) and others (3 items).</P> <P><B>Conclusions</B></P> <P>The completeness of CPG descriptions in TM is very poor. Therefore, a complete description of the recommendations for TM in CPGs is necessary for physicians to implement the recommendations in clinical practice areas. The PRIDE-CPG-TM will provide useful guidance for TM developers in the development of evidence-based CPGs.</P>

      • Korean medicine clinical practice guideline on acupuncture for acute ankle sprains in adults: Evidence-based approach

        <P><B>Abstract</B></P> <P><B>Introduction</B></P> <P>Based on a structured, rigorous methodology developed for creating evidence-based clinical practice guidelines (CPGs), the Korean Institute of Oriental Medicine and Korean Acupuncture and Moxibustion Medicine Society developed a guideline for treating acute ankle sprain using acupuncture which is part of Korean medicine (KM).</P> <P><B>Methods</B></P> <P>Using a revised Grading of Recommendations Assessment, Development, and Evaluation (GRADE) approach, the committee based these recommendations on a systematic review of randomized controlled trials on any form of acupuncture used for ankle sprains published up to July 2015. Accessible sources relating to ankle sprain were collected, and a systematic review and meta-analysis were applied to the collected data. The quality of evidence was graded, and the recommendations were drafted and then finalized after official approval.</P> <P><B>Results</B></P> <P>For this CPG, six clinical questions and recommendations that were primarily related to manual acupuncture methods were addressed. Four were related to manual acupuncture alone, of which one focused on electroacupuncture and another on pharmacopuncture. Clinical outcomes evaluated were pain reduction using a visual analogue scale (VAS) or the numeric rating scale and improved cure rates, joint position sense, kinesthesia, and range of motion. Manual acupuncture was thus recommended as grade A treatment for alleviating VAS-graded pain.</P> <P><B>Conclusions</B></P> <P>The guideline for treating ankle sprain establishes a standard procedure to ensure high-quality medical care for ankle sprain that could be applied globally to patients seeking KM. It also supports international traditional medicine research methodology. When further clinical research is conducted, this CPG can be updated, adding value to its content.</P>

      • PE-160: Robotic ALPPS in a Patient with Cecal Cancer and Multiple Liver Metastasis (with Video)

        Purpose: The Associating Liver Partition and Portal Vein Ligation Staged Hepatectomy (ALPPS) procedure is a new innovative surgical strategy aiming to potentially overcome the liver failure, thus permitting extended hepatectomy. Methods: A 41 year old female with cecal adenocarcinoma with synchronous numerous liver metastases in near all segments (S1, S4, S5, S6, S7 and S8) underwent Avastin and FOLFOX neoadjuvant therapy. S2 was free from metastasis. One small metastastic lesion was detected in S3 and S1, respectively. To increase a resectability in this patient, we performed ALPPS using daVinci Surgical System. Results: In the first operation, robotic right hemicolectomy was first performed. After that, wedge resection of small metastatic lesions in S3 and S1 was done. The parechymal was transected along the right side of the umbilical ligament. The middle hepatic vein was completely isolated and encircled with a vessel loop. After the parenchymal transection was completed, the hilum started to be dissected. The right hepatic artery was isolated and encircled with a vessel loop. The right portal vein was dissected and sectioned with hem-o-lock clips. To prevent adhesion, anti-adhesive solution was irrigated within the surgical field. The second stage was performed totally robotic on 11th postoperative days. Postoperative adhesion was dissected. After the right liver was mobilized, the right hepatic artery was sectioned. The remaining liver parenchyma was completed transected. Middle hepatic vein and right hepatic vein were ligated with Endo-GIA. Total operation time was 332 minutes and estimated blood loss was 70ml. The patient recovered well and was discharged 20 days after the second stage. Conclusion: Robotic ALPPS procedure can be a safe and feasible technique in experienced centers with advanced robotic skills.

        반지도 학습 토픽 모델 기반 우울 증상 발화 자동 라벨링 기법

        우울증을 호소하는 사람들은 빠른 속도로 증가하고 있지만, 치료와 사후 관리가 효과적으로 이루어지지 않고 있다는 문제가 있다. 이러한 면에서 비대면 심리상담 서비스의 중요성이 대두되고 있으며, 내담자의 발화문에서 DSM-5 기준 우울 증상을 탐지하는 기능이 중요해졌다. 우울 증상을 탐지하기 위해서는 지도 학습에 사용할 다량의 DSM-5 우울 증상 라벨링 데이터가 필요하다. 본 연구에서는 발화문에 나타난 DSM-5 기준 우울 증상들을 탐지하고, 각 발화에 대해 우울 증상 라벨을 자동으로 부여하는 반지도 학습 토픽 모델 기반 자동 라벨링 기법을 제안한다. 이는 특정 그룹의 발화 문서들이 가진 특징을 반영하는 라벨링 결과를 얻어낸다는 점에서 의의가 있다. Although the number of people complaining of depression is increasing rapidly, there is a problem that treatment and follow-up care are not effective due to social gaze and characteristics of depression. In this respect, the importance of online psychological counseling service is emerging, and the function of detecting depressive symptoms based on the DSM-5 in the text of clients has become important. In order to detect depressive symptoms, a large amount of DSM-5 depressive symptom labeling data to be used for supervised learning is required. In this study, we propose a semi-supervised learning topic model-based automatic labeling technique that detects DSM-5 standard depressive symptoms appearing in utterances and automatically assigns a depressive symptom label to each utterance. This is significant in that an attempt was made to obtain a labeling result that reflects the characteristics of a specific group of speech documents.

        <P><B>Abstract</B></P><P><B>Introduction:</B></P><P>Women with infertility who have a poor ovarian responder (POR), characterized by a low number of retrieved oocytes after ovulation induction, often have a significantly reduced pregnancy rate after in vitro fertilization-embryo transfer (IVF-ET), due to the few transferred embryos. Acupuncture is a form of Korean Traditional Medicine. It involves the insertion of a microscopic needle at a specific point in the body, known as an acupuncture point or an acupoint. In this study, our purpose is to investigate how acupuncture affects the retrieval of mature oocytes after ovulation induction in patients with POR.</P><P><B>Methods and analysis:</B></P><P>This study will be a randomized clinical trial comprising an IVF-ET trial and an IVF-ET trial after acupuncture. Seventy patients will by enrolled and randomly assigned to either of the 2 groups. The study subjects will be required to be diagnosed as having POR. Participants will be divided into 2 groups: IVF-ET single treatment group, and acupuncture and IVF-ET combined treatment group. The study subjects will be required to participate in a 15-week trial involving 16 acupuncture treatments over a period of approximately 2 months before ovulation induction for oocyte retrieval. The primary assessment of all participants will be comparing the number of oocytes.</P><P><B>Result:</B></P><P>This treatment will be a therapeutic model for POR.</P><P><B>Discussion:</B></P><P>Our results will provide patients with POR as well as complementary and alternative medicine professionals, such as Korean medicine doctors, about the potential role of acupuncture in the treatment of POR. This will improve the quality of life in women with infertility and provide an important treatment option for patients with POR. Further studies can be performed to determine the optimal treatment for POR.</P>

      • Nourishing yin method using herbal medicine for skin disease: A systematic review and meta-analysis

        <P><B>Abstract</B></P> <P><B>Introduction</B></P> <P>Nourishing yin is one of the most important treatment methods for skin diseases. The main conditions for which herbal prescriptions are used for nourishing yin are: skin pruritus, psoriasis, eczema, atopy, acne and alopecia. The aim of this systematic review was to identify and analyse trial data on the efficacy and safety of the nourishing yin method using herbal medicine for skin disease.</P> <P><B>Methods</B></P> <P>Eleven electronic databases were searched from their inception through until December 2016, with no language limitation. Randomised controlled trials for nourishing yin using herbal prescriptions used to treat skin disease were identified and their risk of bias of assessed.</P> <P><B>Results</B></P> <P>Of 725 of the total trials identified, 29 studies were included. Most studies had a high risk of bias. Twenty-five RCTs tested nourishing yin herbal medicines for various skin diseases compared with conventional medicines. Most showed superior response rates compared with conventional medicine, and 7 trials reported equivalent effects. The meta-analysis showed favourable effects of the nourishing yin method using herbal medicine for various skin diseases (n=2813, RR: 1.20, 95% CI: 1.13 to 1.26, P<0.00001, I2=62%). Four studies assessed the nourishing yin herbal medicine plus conventional medicine for various skin diseases compared to conventional medicine alone. All of these 4 trials showed positive effects and the meta-analysis also showed favourable effects (n=497, RR: 1.14, 95% CI: 1.05 to 1.23, P=0.0009, I2=37%).</P> <P><B>Conclusion</B></P> <P>This review provides superior or equivalent effects of the nourishing yin method using herbal prescriptions for treating skin diseases compared with conventional medicines.</P>

        Additional increased effects of mannitol-temozolomide combined treatment on blood-brain barrier permeability

        <P><B>Abstract</B></P> <P>The blood−brain barrier (BBB) is major obstacle in drug or stem cell treatment in chronic stroke. We hypothesized that adding mannitol to temozolomide (TMZ) is a practically applicable method for resolving the low efficacy of intravenous mannitol therapy. In this study, we investigated whether BBB permeability could be increased by this combined treatment. First, we established a chronic ischemic stroke rat model and examined changes in leakage of Evans blue dye within a lesion site, and in expression of tight junction proteins (TJPs), by this combined treatment. Additionally, in an in vitro BBB model using <I>trans</I>-wells, we analyzed changes in diffusion of a fluorescent tracer and in expression of TJPs. Mannitol-TMZ combined treatment not only increased the amount of Evans blue dye within the stroke lesion site, but also reduced occludin expression in rat brain microvessels. The in vitro study also showed that combined treatment increased the permeability for two different-sized fluorescent tracers, especially large size, and decreased expression of TJPs, such as occludin and ZO-1. Increased BBB permeability effects were more prominent with combined than with single treatments. Mannitol-TMZ combined treatment induced a decrease of TJPs with a consequent increase in BBB permeability. This combined treatment is clinically useful and might provide new therapeutic options by enabling efficient intracerebral delivery of various drugs that could not otherwise be used to treat many CNS diseases due to their inability to penetrate the BBB.</P> <P><B>Highlights</B></P> <P> <UL> <LI> The blood−brain barrier (BBB) poses a major obstacle to therapeutic drug delivery. </LI> <LI> Mannitol-temozolomide (TMZ) combined treatment induced decreased tight junction protein expression. </LI> <LI> Mannitol-TMZ combined treatment increased BBB permeability. </LI> <LI> This combined treatment may improve intracerebral delivery of various drugs. </LI> </UL> </P>

        COVID-19 outbreak in a religious village community in Republic of Korea and risk factors for transmission

        Objectives: This study aimed to assess the scale and transmission patterns of coronavirus disease 2019 (COVID-19) in a religious village community in South Korea, to determine the risk factors of transmission, and to evaluate vaccine effectiveness.Methods: An epidemiological survey was conducted, and data were collected and analyzed from 602 villagers in the religious village community. Multivariate logistic regression analysis was used to identify the risk factors for COVID-19 transmission and to evaluate vaccine effectiveness.Results: The outbreak attack rate was 72.1% (434/602). The attack rate was high among women in their 60s, the unemployed, residents living near religious facility (<500 m), and the unvaccinated. Age, the distance between religious facility and residences, and the absence of vaccination were identified as risk factors for transmission. Vaccine effectiveness was 49.0%, and the highest effectiveness was seen in the age group of 59 years or younger (65.8%).Conclusion: This village community was isolated, with little communication with the outside world. However, the frequency of close contact between residents was relatively high, contributing to the spread of COVID-19 in the village even with relatively short exposure. Vaccination rates in the village community were also lower than those in the general public. Public health authorities should consider the potential impact of cultural factors, including religion, that could lead to the exponential spread of COVID-19 in closed village communities.
