Reactions are acute or subacute hypersensitivity states that complicated the existing active and, more often, the treated infection. A modern description of reactions in Hansen's disease will include 3 types such as Type 1 reaction, erythema nodosum leprosum, and Lucio's phenomenon. Type 1 reaction occurs in patients suffering from borderline leprosy during the first six months of treatment. Characteristic symptoms are nerve pain concomittently with muscle paralysis, rapid swelling & erythema of some or all of the skin lesions and edema of face, hands & feet. It is generally considered that this type of reaction is a delayed hypersesitivity phenomenon in which antigen from breaking down bacilli reacts with T lymphocytes. Our case was sixty five year old woman who had multiple erythematous, swollen lesions on the both hands, feet and face, and suffered from paresthesia, dyshydrosis and mild tenderness on that sites 1 year ago. The symptoms were more aggrevated 20 days ago. General symptoms were accompanied such as fever, malaise, fatigue, loss of appetite. Family and past history was non contributory. And we tried many tests, skin smear and skin biopsy and then concluded final diagnosis was borderline tuberculoid leprosy with type 1 reaction. We had treated this patient with 20-40㎎ of prednisolone, 100㎎ of dapsone, 200㎎ of clofazimine and 600㎎ of rifampicin daily for a week, and then, had transfered to special leprosy clinic in the mild improved state. Now we present a case of borderline tuberculoid leprosy with type 1 reaction with references.

This study investigated the optimum temperature and holding solutions for the storage of cut flowers of Arenaria juncea M. Bieb. to facilitate their introduction as a new ornamental plant. We examined the effects of different holding solutions on the water uptake, vase life, and fresh weight of five branches of A. juncea. After harvest, cut branches were stored at 4, 15, 20, and 25°C. To study the effect of the holding solutions, the cut branches were treated with a combination of sucrose (1% and 3%), 8-hydroxyqunoline sulfate (8-HQS: 10, 50, and 100 mg·L-1), and commercial preservative solutions Chrysal and Floralife. At a storage temperature of 4°C, the vase life was 29 days, which was significantly longer compared to room temperature, where the vase life was 7.8 days. Thus, a lower temperature was more effective at extending the vase life. A combination of 1% sucrose and 100 mg·L-1 8-HQS prolonged the vase life by approximately two weeks compared to the control. The effect of both Chrysal and Floralife was similar to the effect of the holding solutions composed of sucrose and 8-HQS. Moreover, rapid reduction of water uptake and fresh weight resulted in a shorter vase life.

In this study, we examined the effects of dietary 1,3-diacylglycerol (DG) compared to conventional triacylglycerol (TG) oil on the postprandial response of total and chylomicron TG, glucose, insulin, and free fatty acid (FFA). This study was conducted using a cross-over design. Ninety subjects participated in the high-fat meal tolerance test where they were randomly assigned to consume two experimental sandwiches containing mayonnaise with TG or DG oil with a seven-day interval. Blood samples were collected before ingestion and at 2, 3, 4 and 6 hr time point after ingestion and analyzed for total and chylomicron TG, glucose, insulin, FFA and phospholipid fatty acid composition. Both TG and DG ingestion had similar effects on postprandial TG response, but a different response from chylomicron TG. Compared with the TG group, TG levels were significantly lower only at 6 hr time point in the DG group. On the other hand, chylomicron TG rose steeply at 2 hr time point and decreased faster in this group. Also, the adjusted value to fasting levels was the same as the unadjusted level. Fasting levels and net differences in insulin were significantly lower at 3 hr time point where chylomicron TG levels were significantly lower in the DG group. But those of glucose and FFA in the TG and DG groups did not differ significantly. Fasting and postprandial levels of fatty acid composition in serum phospholipids in the two groups did not differ significantly. In conclusion, this study indicated that one could reduce the magnitude of postprandial lipemia without influencing glucose metabolism by consumning DG oil as a substitute for TG oil. Based on the correlation of coronary artery disease and postprandial lipemia, dietary DG ingestion might have a beneficial effect in treating such a disease. Further studies are required to clarify the long-tenn effects of dietary DG on blood lipid levels in humans.

OBJECTIVE

The aim of this study was to determine the effect of common adiponectin gene polymorphisms on dietary intervention-mediated changes in adiponectin levels and homeostasis model assessment of insulin resistance (HOMA-IR) indexes.

RESEARCH DESIGN AND METHODS

A total of 363 subjects with impaired fasting glucose (IFG) or newly diagnosed type 2 diabetes followed a dietary intervention (replacement of cooked refined rice with whole grains and an increase in vegetable intake) and regular walking for 12 weeks without any medication. Adiponectin gene single nucleotide polymorphisms (SNPs) (45, 276, and −11377) were examined in these subjects.

RESULTS

After this dietary intervention, fasting glucose levels decreased in all three SNP 45T>G genotype groups. Subjects with the SNP 45TT genotype showed increased adiponectin levels and decreased HOMA-IR indexes. Haplotype analysis revealed that homozygous carriers of the TG haplotype (45TT and 276GG) and heterozygous carriers of the TG haplotype (TG/X) showed a reduction in the HOMA-IR index after adjustment for baseline levels. Significant differences were observed in changes in HOMA-IR indexes and adiponectin concentrations according to the 45-276 TG haplotype in overweight-obese, but not in normal-weight subjects: the greatest decrease in HOMA-IR indexes and the greatest increase in adiponectin levels were shown in overweight-obese subjects with the TG/TG haplotype.

CONCLUSIONS

ADIPOQ genetic variants can affect circulating adiponectin levels and insulin resistance indexes in subjects with IFG or newly diagnosed type 2 diabetes in response to dietary intervention.
