Adiponectin Concentrations: A Genome-wide Association Study

<P>Adiponectin is associated with obesity and insulin resistance. To date, there has been no genome-wide association study (GWAS) of adiponectin levels in Asians. Here we present a GWAS of a cohort of Korean volunteers. A total of 4,001 subjects were genotyped by using a genome-wide marker panel in a two-stage design (979 subjects initially and 3,022 in a second stage). Another 2,304 subjects were used for follow-up replication studies with selected markers. In the discovery phase, the top SNP associated with mean log adiponectin was rs3865188 in <I>CDH13</I> on chromosome 16 (p = 1.69 × 10<SUP>−15</SUP> in the initial sample, p = 6.58 × 10<SUP>−39</SUP> in the second genome-wide sample, and p = 2.12 × 10<SUP>−32</SUP> in the replication sample). The meta-analysis p value for rs3865188 in all 6,305 individuals was 2.82 × 10<SUP>−83</SUP>. The association of rs3865188 with high-molecular-weight adiponectin (p = 7.36 × 10<SUP>−58</SUP>) was even stronger in the third sample. A reporter assay that evaluated the effects of a <I>CDH13</I> promoter SNP in complete linkage disequilibrium with rs3865188 revealed that the major allele increased expression 2.2-fold. This study clearly shows that genetic variants in <I>CDH13</I> influence adiponectin levels in Korean adults.</P>

Objective: To define the impact of administration of Bupivacaine (100mg/20ml) at incisional site in postoperative pain after Laparo-Endoscopic Single (LESS) adnexal surgery. Methods: This was a prospective study of periumbilical infiltration of Bupivacaine at incisional site in LESS Adnexal Surgery. Study was performed at Yonsei Cancer Center during February 2017 to March 2018. The study group was randomly selected preoperatively and divided into two groups to receive either bupivacaine or no analgesic medicine at incision site. Total 111 patients participated in bupivacaine group, and 110 patients were in control group. Bupivacaine group received 1 vial (100mg/20mL) of bupivacaine administered at periumbilical surgical site. While control group did not receive any medicine during the closure of incisional site. Postoperative pain assessment was performed by using Numerical Rating Scale (NRS) system after 6 hours, 24 hours, 48 hours from the end of surgery. Two groups were compared for age, body mass index, and number of previous abdominal surgeries. Both groups received intravenous, oral analgesia for controlling abrupt postoperative pain. Results: Bupivacaine group had significantly lower postoperative pain scores after 6 hours (2.7±0.7 vs 3.4±1.2, P= 0.0001) and after 24 hours (2.3±0.7 vs 2.8±1.0, P=0.0001) from operation. There was no difference in pain score after 48 hours from surgery (2.0±0.7 vs 2.1±0.8, P= 0.135). Bupivacaine group used significantly less PCA (9 (8.1%) vs 73(66.4%)) and used less number of analgesia after operation (3.2+0.9 vs 3.8+1.1, P= 0.0001). There was no difference in hospital stay in two groups. Conclusion: Periumbilical infiltration of Bupivacaine at incision site was useful in reducing postoperative pain scores after 6hours and 24 hours after operation. Also, bupivacaine injection reduced frequency of postoperative analgesic use. Bupivacaine injection could be an option to improve postoperative pain and faster recovery after LESS adnexal surgery.
High prevalence of HBV and HCV infection among intravenous drug users in Korea

<P>Although intravenous drug users are a well-known route of hepatitis C virus (HCV) and hepatitis B virus (HBV) transmission, there is no data on the prevalence of HBV and HCV infection among intravenous drug users in Korea. In order to describe the prevalence of HBV and HCV infection, and to determine HCV genotypes in the population, serum samples were collected from 107 intravenous drug users during 2005–2006. Fifty-seven percent (n = 61) were HCV RNA positive and 51% (n = 55) were HBV DNA positive. Co-infection of HBV and HCV were found in 23% (n = 25). HCV genotypes 1b, 2a/2c, 2, 2b, and 3a were found in 38% (n = 23), 44% (n = 27), 8% (n = 5), 2% (n = 1), and 3% (n = 2), respectively. Moreover, mixed infections of genotypes 1b and 2a/2c were found in 5% (n = 3). When the number of patients with HCV genotype 1b compared with that of patients with genotype 2a/2c, HBV DNA titer was not significantly different by independent t-test (t = −0.881, P = 0.392 > 0.05) between the two patient groups. These results suggest that the prevalence of HBV and HCV infection among intravenous drug users is high showing over 50% in Korea and a strategic prevention program should be performed in this group to prevent further infection into local community. Electrochemical nucleation of SiOx nanoparticles into the pore bottoms of an anodic aluminum oxide.

<P>Utilizing the wafer-scale anodization of a thermally evaporated Ti layer onto a Si substrate, SiOx nanoparticles could be electrochemically nucleated into the pore bottoms of an anodic aluminum oxide. The formation of a Si-containing Ti layer (Ti(1-x)Six, x < 0.1) was identified between the Al and the silicon substrate by thermal diffusion of Si during the evaporation. Upon prolonged anodization of approximately 1 h after alumina barrier layer touched the Si-containing Ti layer, pyramid-shaped TiOx nanopillars formed underneath the pore bottoms as a result of a curvature inversion of the barrier oxide with Ti migration. These TiOx nanopillars were observed to act as a diffusion route of silicon from the Si-containing Ti layer. Only one SiOx nanoparticle (approximately 8 +/- 5 nm) for each pore was generally precipitated without Ti contamination. This finding suggests a new route which can make SiOx nanoparticles confined within an anodic aluminum oxide template.</P>
