        교실평가 실제 유형에 따른 중학생의 평가인식과 학업적 자기조절 및 피드백 추구 행동 간의 구조적 관계 분석

        본 연구의 주요 목적은 교실평가 실제의 유형에 따른 중학생의 학습을 위한 평가(Assessment for Learning:AFL) 인식이 학업적 자기조절과 피드백 추구 행동에 미치는 영향을 분석하는 것이다. 이를 위해 교실평가 장면에서 중학생이 인식하는 평가 유형을 교사주도 평가와 상호작용적 형성평가로 구분하였다. 교실평가 실제의 유형과 AFL 인식, 학업적 자기조절, 피드백 추구 행동 간의 관계를 파악하기 위해 구조방정식 모형 분석을 실시하여 변인 간 직・간접 효과를 확인하였다. 연구대상은 D와 K지역에 소재한 4개 중학교에 재학 중인 2, 3학년 학생 450명이다. 분석 결과, 성적 산출이나 등급부여를 목적으로 하는 교사주도 평가는 AFL 인식에 영향을 미치지 못한 반면, 학습 상태를 관찰하고 의견을 나누는 교사-학생 간 상호작용적 형성평가는 AFL 인식에 정적인 영향을 미치는 것으로 나타났다. 다음으로 교실평가 실제가 AFL 인식과 학업적 자기조절을 매개하여 피드백 추구 행동에 미치는 영향에 대한 간접효과를 분석한 결과, 상호작용적 형성평가는 AFL 인식을 매개하여 학업적 자기조절과 피드백 추구 행동에 통계적으로 유의한 정적 영향을 미치는 것으로 나타났다. 교사주도 평가는 AFL 인식과 학업적 자기조절을 매개하여 피드백 추구 행동에 미치는 간접효과가 유의하지 않았다. 마지막으로 학생이 인식하는 교실평가 실제와 AFL 인식의 중요성 및 교실 평가 개선과 학습자의 성장과 발전을 돕기 위한 상호작용적 형성평가의 교육적 시사점에 대하여 논의하였다. The main purpose of this study was to analyze the structural relationships of classroom assessment practice with middle school students’ conceptions of assessment for learning (AFL), their academic self-regulation, and feedback seeking behavior. The instrument for Students' Conceptions of Assessment (SCoA-IV) was completed by 450 middle school students attending to four different schools in D or K region of Korea. A structural equation modeling analysis was used to identify the direct and indirect effects among the variables such as two types of classroom assessment practices, AFL conception, academic self-regulation, and feedback seeking behavior. As a result of the analysis, it was found that the effect of teacher controlled assessment on AFL conception was not significant, whereas the interactive formative assessment had a positive effect on AFL conception. In the analysis of indirect effects, the teacher controlled assessment had no significant indirect effects on students' academic self-regulation and feedback-seeking behavior, whereas the interactive formative assessment had significant indirect effects on feedback seeking behavior through the AFL conception and academic self-regulation. Lastly, the educational implications of interactive formative assessment for students’ learning and teachers’ instruction improvement, and the importance of students’ AFL conception were discussed.

        F-18 Sodium Fluoride Positron Emission Tomography/Computed Tomography for Detection of Thyroid Cancer Bone Metastasis Compared with Bone Scintigraphy

        <P>Objective: The aim of the study was to compare the diagnostic performances of F-18 sodium fluoride positron emission tomography/computed tomography (bone PET/CT) and bone scintigraphy (BS) for the detection of thyroid cancer bone metastasis. Materials and Methods: We retrospectively enrolled 6 thyroid cancer patients (age = 44.7 +/- 9.8 years, M:F = 1:5, papillary: follicular = 2: 4) with suspected bone metastatic lesions in the whole body iodine scintigraphy or BS, who subsequently underwent bone PET/CT. Pathologic diagnosis was conducted for 4 lesions of 4 patients. Results: Of the 17 suspected bone lesions, 10 were metastatic and 7 benign. Compared to BS, bone PET/CT exhibited superior sensitivity (10/10 = 100% vs. 2/10 = 20%, p = 0.008), and accuracy (14/17 = 82.4% vs. 7/17 = 41.2%, p < 0.025). The specificity (4/7 = 57.1%) of bone PET/CT was not significantly different from that of BS (5/7 = 71.4%, p > 0.05). Conclusion: Bone PET/CT may be more sensitive and accurate than BS for the detection of thyroid cancer bone metastasis.</P>

        Correlation of FDG PET/CT Findings with Long-Term Growth and Clinical Course of Abdominal Aortic Aneurysm

        Purpose Herein, we report characteristics of $^{18}F$-fluorodeoxyglucose (FDG) uptake in abdominal aortic aneurysms (AAAs) during a long-term follow-up. In addition, we investigated the association between FDG uptake and the physician decision to perform an intervention. Methods We performed a retrospective review of 42 patients with AAAs who underwent FDG positron emission tomography (PET)/computed tomography (CT). The size of the AAA was measured in serial CT or PET/CT images. The long-term growth rate of AAAs was calculated by linear regression of the size change. Maximal SUV of the AAA ($SUV_{AAA}$) and mean SUV of the blood pool ($SUV_{Blood}$) were measured in PET/CT fusion images. To assess the FDG uptake of AAAs, the target-to-background ratio (TBR) was defined as the ratio of $SUV_{AAA}$ to $SUV_{Blood}$. We compared FDG uptake of AAAs with the long-term growth rate of AAAs and clinical data. Results TBR was not significantly different between patients with and without significant growth ($1.55{\pm}0.20$ vs. $1.57{\pm}0.14$; P = 0.5599). However, in patients with significant growth, TBR exhibited a significant positive correlation with the growth rate ($r^2=0.2601$, P = 0.0306). TBR also exhibited a significant difference between patients with and without intervention (P = 0.0228). Conclusion FDG uptake of AAA is associated with long-term growth of AAAs in a specified group that exhibits growth. FDG PET/CT may only be effective in predicting the long-term growth of AAAs in specific subgroups of patients. It is also suggested that FDG PET is potentially related to the clinical conditions of AAA patients who need surgical or interventional treatment.

        Quantitative single-photon emission computed tomography/computed tomography for technetium pertechnetate thyroid uptake measurement

        <▼1><P>Supplemental Digital Content is available in the text</P></▼1><▼2><P><B>Abstract</B></P><P><B>Objectives:</B></P><P>Technetium pertechnetate (<SUP>99m</SUP>TcO<SUB>4</SUB>) is a radioactive tracer used to assess thyroid function by thyroid uptake system (TUS). However, the TUS often fails to deliver accurate measurements of the percent of thyroid uptake (%thyroid uptake) of <SUP>99m</SUP>TcO<SUB>4</SUB>. Here, we investigated the usefulness of quantitative single-photon emission computed tomography/computed tomography (SPECT/CT) after injection of <SUP>99m</SUP>TcO<SUB>4</SUB> in detecting thyroid function abnormalities.</P><P><B>Materials and methods:</B></P><P>We retrospectively reviewed data from 50 patients (male:female = 15:35; age, 46.2 ± 16.3 years; 17 Graves disease, 13 thyroiditis, and 20 euthyroid). All patients underwent <SUP>99m</SUP>TcO<SUB>4</SUB> quantitative SPECT/CT (185 MBq = 5 mCi), which yielded %thyroid uptake and standardized uptake value (SUV). Twenty-one (10 Graves disease and 11 thyroiditis) of the 50 patients also underwent conventional %thyroid uptake measurements using a TUS.</P><P><B>Results:</B></P><P>Quantitative SPECT/CT parameters (%thyroid uptake, SUVmean, and SUVmax) were the highest in Graves disease, second highest in euthyroid, and lowest in thyroiditis (<I>P</I> < 0.0001, Kruskal–Wallis test). TUS significantly overestimated the %thyroid uptake compared with SPECT/CT (<I>P</I> < 0.0001, paired <I>t</I> test) because other <SUP>99m</SUP>TcO<SUB>4</SUB> sources in addition to thyroid, such as salivary glands and saliva, contributed to the %thyroid uptake result by TUS, whereas %thyroid uptake, SUVmean and SUVmax from the SPECT/CT were associated with the functional status of thyroid.</P><P><B>Conclusions:</B></P><P>Quantitative SPECT/CT is more accurate than conventional TUS for measuring <SUP>99m</SUP>TcO<SUB>4</SUB> %thyroid uptake. Quantitative measurements using SPECT/CT may facilitate more accurate assessment of thyroid tracer uptake.</P></▼2>

        Appropriate margin thresholds for isocontour metabolic volumetry of fluorine-18 fluorodeoxyglucose PET in sarcoma: a hybrid PET/MRI study

        <P>ObjectivesMetabolic volumetry and heterogeneity indexes are increasingly being used in fluorine-18 fluorodeoxyglucose (F-18-FDG) PET, mostly using isocontour methods. However, there is no standard method to determine optimal margin thresholds for isocontour volume of interest (VOIiso). Here, VOIiso with various margin thresholds were compared with exact anatomical tumor volumes in sarcoma using a hybrid PET/MRI scanner.Patients and methods(18)F-FDG PET/MRI images were retrospectively analyzed for 36 lesions of soft tissue or bone sarcoma. VOIiso of tumors was drawn with varying margin thresholds; standardized uptake value (SUV) 1.0-4.0 with an increment of 0.5 and 30-70% of maximal SUV (SUVmax) with an increment of 10%. Exact anatomical tumor volumes (VOIMRI) were drawn manually on the basis of MRI. In each VOI, metabolic tumor volume, SUVmean, total lesion glycolysis, and SD were measured and compared between VOIiso and VOIMRI.ResultsSignificant correlations were observed between the indexes from VOIiso and VOIMRI. For metabolic tumor volume, no significant difference existed between VOIiso and VOIMRI with margin thresholds of 30 and 40% of SUVmax, and SUV 1.5. For SUVmean, no significant difference existed with 30% of SUVmax, and SUV 1.5. For total lesion glycolysis, no significant difference existed with 40% of SUVmax, SUV 1.5, and 2.0. For SD, no significant difference existed with 30% of SUVmax, SUV 1.5, and 2.0.ConclusionMargin thresholds of 30 or 40% of SUVmax and SUV 1.5 or 2.0 may be used for the VOIiso method in F-18-FDG PET images of sarcoma.         Review of two immunosuppressants: tacrolimus and cyclosporine

        Immunosuppressants are vital in organ transplantation including facial transplantation (FT) but are associated with persistent side effects. This review article was prepared to compare the two most used immunosuppressants, cyclosporine and tacrolimus, in terms of mechanism of action, efficacy, and safety and to assess recent trials to mitigate their side effects. PubMed and Google Scholar queries were conducted using combinations of the following search terms: “transplantation immunosuppressant,” “cyclosporine,” “tacrolimus,” “calcineurin inhibitor (CNI),” “efficacy,” “safety,” “induction therapy,” “maintenance therapy,” and “conversion therapy.” Both immunosuppressants inhibit calcineurin and effectively down-regulate cytokines. Tacrolimus may be more advantageous since it lowers the likelihood of acute rejection, has the ability to reverse allograft rejection following cyclosporine treatment, and has the potential to reinnervate nerves. Meanwhile, graft survival rates seem to be comparable for the CNIs. To avoid nephrotoxicity, various immunosuppressants other than CNIs have been studied. Despite averting nephrotoxicity, these medications show increases in acute rejection or other types of adverse effects compared to CNIs. FT has been a topic of interest for oral and maxillofacial surgeons, and the postoperative usage of immunosuppressants is crucial for the long-term prognosis of FT. As contemporary transplantation regimens incorporate novel medications along with CNIs, further research is required.
