      • Bobath개념을 이용한 마미손상 치료 1례

        The Bobath concept is problem solving approach to the assesment and treatment of individuals with disturbances of tone, movement, and function. The goal of treatment is to optimize function by improving postural control and selective movement through facilitation. Incomplete spinal cord injury has been increased recently due to developing of medicine and transportation techniques. Most of the treatment methods for muscle strength are not sufficient for functional activities. A patient with cauda equina injury had below poor grade muscle power of both hip extensors and abductors. The goal of treatment for the patient was to facilitate the abductors and extensors of both hip. After two treatment sessions, the patient needed no more seat cushion while seated, increased muscle power of both hip extensors and abductors, and extended the span of ambulation more than three times after four treatment sessions.

      • Ink-foot Print를 이용한 성인편마비의 보행특성에 관한 연구

        The purpose of this study was to survey the characteristics of gait in adult hemiplegia. All 40 subjects were tested for differences in 6 aspects of the gait. Velocity, cadence, step length, stride length, foot angle and base of support were recorded for 40 subjects with adult hemiplegia. Each subject was tested twice. This study used the ink foot-print, one of several temporal distance measures of gait performance. The results of the study were that 1) all 3 walking levels showed lower than normal values in all gait characteristics, 2) the 3 variables studied all showed better scores(velocity: 25.0 cm/sec faster ; stride in the non-paretic leg: 14.5cm longer ; foot angle in the paretic leg: 2.9˚ smaller) the independent ambulation group rather than the patient group with mono cane, 3) the subject with mono cane had higher values than the subject with quad cane in the mean gait velocity and mean gait cadence were 15.3 cm/sec faster and 21.4 step/min more. These results show that the patient with independent ambulation had highest values among the all groups and also provide some guidance for identifying appropriate targets for treatment intervention with adult hemiplegia.

      • 편마비 환자의 안정성 한계와 신체 요동의 특성

        The purpose of this study was to determine differences between limits of stability(LOS) and body sway(BS) of the hemiplegic patients and of the healthy subjects. The subjects were 18 hemiplegic patients and age-matched 18 healthy men. LOS of the sagittal and coronal plane, and modified CTSIB were measured using Balance Master. The span of LOS of the hemiplegic patients was significantly shorter than that of the healthy subjects. In all four measurement conditions of the modified CTSIB, the hemiparetic patients had larger sways than the control subjects. Patients' sway on the paretic side was larger than on the nonparetic side, whereas no side differences were detected in the control subjects. Results suggests that a rehabilitation program for balance and walking considers the characteristics of LOS and body sway in the patients with hemiplegia.

      • 보행에서 Central Pattern Generator의 이해 : Understanding the Central Pattern Generator in Human Gait

        An essential feature of human gait is the ability to move from one place to another. The gait, rhythmic and alternating movements of the two legs, is automatic and less cognitive neuronal activity. A central pattern generator(CPG) is a neuronal network capable of generating a rhythmic pattern of motor activity in the absence of phasic sensory input from peripheral receptors. CPG have been identified and analyzed in more than 50 rhythmic motor systems, including those controlling such diverse behaviors as walking, swimming, feeding, respiration, and flying. The basic pattern produced by a CPG is usually modified by sensory information from peripheral receptors and signals from other regions of the central nervous system.

      • 편마비환자의 의자차 자가추진

        Physical therapy for the early hemiplegic patient attempts to restore as near a normal level of motor function as possible. Some therapists recommend that early self-propelling wheelchair to achieve independent activities of daily living. On the other hand, some therapists are against to use it early, because it causes to increase spasticity and associated reaction. Therefore, the patients should be carefully selected before being taught to self-propel their wheelchair. It is dependent on the goal of treatment and the status of patients.

      • 만성 뇌졸중 환자의 운동기능과 일상생활동작 수행 능력의 상관관계

        The purpose of this study was to determine the correlation between the motor function and ADL in the person with chronic stroke. In this study, 50 individuals who are living in Ui - wang city, home visiting physical therapy was performed once a week for 8 weeks and scored the Motor Assessment Scale (MAS), the Functional Independence Measure (FIM) and the Modified Barthel Index (MBL). After home visiting physical therapy correlation between the Motor FIM and the MBI was very strong and there were a strongly correlation between the MAS and the FIM, and between the FIM and the MIB (p <0.01). Results shown here indicates that the home visiting physical therapy can result in changed the motor function and the ADL in the person with chronic stroke. Also, the results of this study can provide a reference for the successful therapeutic program in the persons with chronic stroke.

      • 평균대 운동이 청각장애아동의 균형에 미치는 영향

        The purpose of this study was to determine the effects of exercise on a balance beam for the children with hearing impaired. The subjects were recruited by 20 children with only auditory deficit Measurements of the balance were taken by One Legged Stance Test (OLST), Functional Arm Reach Test (FART), Backward Walking Test (BWT), and Timed Up Go Test (TUGT). There were significant differences in BWT with eyes open and closed, and in TUGT after the exercises on a balance beam. There were no statistical significant differences between static exercises and dynamic exercises of children with hearing impaired in OLST, BWT with eyes closed, and TCGT. From the results we conclude that exercises on a balance beam influenced on improving balance proficiency of children with hearing impaired, but there were no differences between static exercises and dynamic exercises on a balance beam.

      • 도금사업장의 작업환경의 유해인자 노출수준에 관한 연구

        Objective : For the purpose of preparing the fundamental data and assessing the status of exposure level of chemical substances and physical agents. Methods and Material : Such as noise, two types of dust, four types of metals, seventeen types of organic solvents, and four types of other materials classified by specific chemical substances in 21 metal product plating factories classified by 28 processes. The author determined exposure level on those hazardous materials from Jan. to Dec. 2001. Results : The results were as follows: 1. The exposure level for noise was exceeded to Threshold Limit Value(TLV) in the process of 6 cases of Machining, 5 and 4 cases of pre and post-buffing, and 6 cases of waterdrop-removing, the mean noise level of all of processes was not exceeded to compliance level. 2. The exposure level for dust was not exceeded to TLV at all. 3. The exposure level for Chromium(Cr) was exceeded to TLV in Chromium plating process, metal exposure level including Zinc(Zn), Manganese(Mn), and Lead(Pb) was not exceeded to TLV. 4. The exposure level of toluene in case of organic solvents was exceeded to TLV in 1 case of finishing/cleaning, coating/developing process, respectively. 5. The exposure level of four types of other materials classified by specific chemical substances were not exceeded to TLV. 6. The exposure level for dust and metal including Zn, Mn, and Pb was decreased in comparison with two results of air monitoring performed. There was no difference of the exposure level for Cr, noise, and four types of other materials classified by specific chemical substances, and the exposure level for organic solvents was slightly increased. 7. The exposure level for noise in factories employing more than 21 workers was higher than that for noise in foadtories employing less than 20 workers. The more workers, the lower exposure level in metal including Zn, Mn. The exposure level for Cr was increasing in factories employing more than 51 workers, and the more workers the lower exposure level in organic solvents. Conclusion : According to above results, this study suggests positive work environment managements in processes exceeding exposure limit value as well as continuously consecutive work environment management in processes not exceeding exposure limit value and revealing no chronological difference of exposure level should be needed.

      • THE INFLUENCE OF GRAVITY AND BASE OF SUPPORT WITH REFERENCE TO THE MOBILITY AND STABILITY OF THE PELVIS

        All our activities require and are dependent upon postural control that involves controlling the body's position in space for the dual propose of stability and mobility. In order to achieve this then there must be a complex interaction between the musculoskeletal and neural system. The pelvis transmits forces from the vertebral column to the lower limbs. It interacts directly with the hip joint and lumbar spine, but also indirectly with the head, shoulder girdle, knees and feet. The base of support and centre of gravity influence postural tone which in turn affects the mobility and stability of the pelvis. In supine lying the BOS is large and the COG low in relation to the supporting surface. Hence there is less tone required to maintain position and stability and the general influence on tone is of an extensor nature. In contrast, standing requires extensive antigravity activity to sustain a high COG over a relatively small BOS.

      • The Concepts of Bobath Approach for Adult Hemiplegia

        Bobath치료는 중추신경계질환으로 인한 근긴장도, 동작 그리고 기능장애를 동반한 개개인의 문제점을 평가와 치료를 통한 문제 해결 방법이다. 이 치료의 목적은 자세와 동작을 촉진시켜 기능을 최대화하는데 있다. 억제는 고등동물일수록 발달된 운동조절기구로 동작을 섬세하게 하는 역할을 하며, 수의운동과 자동운동은 서로 조화를 이를 때 경제적인 운동이 이루어진다. 그리고 운동조절점은 그 배열이 균형적으로 이루어져야 정상적인 운동의 형태가 나타나게 된다. 고유수용기 조절은 환경에 대하여 선택적으로 받아들이고 통합하며 반응하는 능력을 말하며, 중추자세조절기관에 절대적으로 영향을 미쳐 운동의 질을 결정한다. 공동운동은 중추신경계의 손상으로 인한 억제조절기능의 결함으로 자극의 수용이 낮아져 나타나는 현상이다. 그러므로 Bobath치료는 중추신경계의 손상으로 인한 여러가지 증상을 정상운동을 기초로 하여 공동작용을 조절하고, 저긴장을 재교육시키며 경직을 예방하고 수의동작과 자동동작을 통한 기능을 재교육시키는 방법이다.
